Nephroureterectomy is currently the criterion-standard treatment for high-grade upper tract urothelial carcinoma(UTUC).Current guidelines and expert opinions propose some exceptions to this approach based on patient characteristics,disease status,and function of the contralateral *** present a rare case of a patient with horseshoe kidney,bilateral large nephrolithiasis,high-grade UTUC in one moiety,and relative parenchymal thinning of the contralateral *** patient was treated with a percutaneous,minimally invasive,nephron sparing *** patient also had intracollecting system instillations of gemcitabine and *** invasive percutaneous resection of high-grade UTUC is a safe procedure in select *** guidelines may not apply to all patients;unique scenarios with UTUC may require personalized decision-making and treatment at specialized centers.

BACKGROUND Smartwatches have become readily accessible tools for detecting atrial fibrillation(AF).There remains limited data on how they affect psychosocial outcomes and engagement in older *** examine the health behavior outcomes of stroke survivors prescribed smartwatches for AF detection stratified by *** We analyzed data from the Pulsewatch study,a randomized controlled trial that enrolled patients(≥50 years)with a history of stroke or transient ischemic attack and CHA2DS2-VASc≥*** participants were equipped with a cardiac patch monitor and a smartwatch-app dyad,while control participants wore the cardiac patch monitor for up to 44 *** evaluated health behavior parameters using standardized tools,including the Consumer Health Activation Index,the Generalized Anxiety Disorder questionnaire,the 12-Item Short Form Health Survey,and wear time of participants categorized into three age groups:Group 1(ages 50-60),Group 2(ages 61-69),and Group 3(ages 70-87).We performed statistical analysis using a mixedeffects repeated measures linear regression model to examine differences amongst age *** Comparative analysis between Groups 1,2 and 3 revealed no significant differences in anxiety,patient activation,perception of physical health and wear *** use of smartwatch technology was associated with a decrease in perception of mental health for Group 2 compared to Group 1(β=-3.29,P=0.046).CONCLUSION Stroke survivors demonstrated a willingness to use smartwatches for AF ***,among these study participants,the majority did not experience negative health behavior outcomes or decreased engagement as age increased.

The real benefits of vitamins and dietary supplementation on health outcomes have always been mired in controversy,and to date,contradictory studies continue to be published to highlight their benefits when downplaying possible adverse effects.!This recently published National Health and Nutrition Examination Survey(NHANES)database study population investigating the role of niacin on erectile function reported that higher niacin intake,specifically in the highest tertile,was associated with a decreased risk of erectile dysfunction(ED)compared to the lowest tertile,showing an odds ratio(OR)of 0.56(95%confidence interval[CI]:0.37-0.85)based on 863 participants with ED from a pool of 3184 adults.2 Further analysis of dose-response curves and interaction analyses found a negative correlation between dietary niacin intake and the risk of ED,suggesting that a higher dietary niacin intake is associated with areducedriskof ED.

Background: Cosmetic formulations, and particularly solar products which contain mineral and chemical UV-filters, are often suspected of causing harmful effects on marine fauna and flora. After the publication of our work in 2019 concerning the ecotoxicological effects of such formulations on corals (Seriatopora hystrix), we here provide some new information about the biodegradability and the ecotoxicological effects of these products on marine zoo- and phytoplankton. Therefore, we choose to realize in silico and in vitro studies of the biodegradability of several solar products but also to evaluate the ecotoxicological effects of these products on one phytoplankton, i.e. Phaeodactylum tricornutum, and one zooplankton, i.e. Acartia tonsa, of a great importance for sea species survival (notably as sources of food). Materials and methods: Two different approaches were used to study the biodegradability of the tested products: One in silico method and an in vitro one. 2 solar products were involved in the in silico study which consisted in the determination of the degradation factor (DF) of each ingredient of the tested formulas in order to finally obtain their estimated biodegradability percentage. Already available data concerning each ingredient coupled to a computer model developed with one of our partners were used to achieve this study. The in vitro study involved 8 formulas containing UV-filters and was led by following the OECD 301 F guidelines. Ecotoxicological studies of 7 of the formulas containing UV-filters were for their part realized by following the ISO 10253 guidelines for the experiments led with Phaeodactylum tricornutum, and the ISO 14669 guidelines for the experiments led with Acartia tonsa. In these studies, the effect of each tested product on crustaceans’ mortality and algal growth inhibition was assessed. Stroke Patients’ Reintegration into Normal Living Post-Discharge from Inpatient Rehabilitation: An Integrative Review

Background: Stroke rehabilitation professionals have historically focused rehabilitation on physical functions and overlooked the concept of community reintegration after discharge from inpatient rehabilitation. The lack of focus on psychosocial functions post-stroke may lead to lower levels of satisfaction during community reintegration. Methods: This integrative review synthesized findings from research literature on stroke patients’ reintegration into the community after inpatient rehabilitation to address three research questions: a) What specific physical and psychosocial functions have been identified as predictors of successful reintegration into normal living after stroke?, b) How do physical and psychosocial functions promote successful reintegration into normal living after stroke?, and c) What factors have been identified that hinder stroke patients’ reintegration into normal living after stroke? Results: A systematic search of literature identified sixteen studies that provided significant context for the research questions. What physical and psychosocial functions of stroke patients included, for example, improved mobility, independence in daily activities, reduced disability, psychological well-being, self-efficacy, social support, and personal relationships. How physical and psychosocial functions promote reintegration included, for example, disability management, emotional well-being, self-care independence, sense of purpose, and employment influence. Factors that hinder stroke patients’ reintegration consisted of longer stride time, impaired balance/mobility, activities limitation, severe stroke, presence of comorbidity, depressive symptoms, speech and language challenges, inadequate self-efficacy, fear of falling, older age, low educational level, lack of social support, and social isolation. Background Predictive scales have been used to prognosticate long-term outcomes of traumatic brain injury(TBI),but gaps remain in predicting mortality using initial trauma resuscitation *** sought to evaluate the association of clinical variables collected during the initial resuscitation of intubated pediatric severe patients with TBI with in-hospital *** Intubated pediatric trauma patients<18 years with severe TBI(Glasgow coma scale(GCS)score≤8)from January 2011 to December 2020 were *** between initial trauma resuscitation variables(temperature,pulse,mean arterial blood pressure,GCS score,hemoglobin,international normalized ratio(INR),platelet count,oxygen saturation,end tidal carbon dioxide,blood glucose and pupillary response)and mortality were evaluated with multivariable logistic *** Among 314 patients,median age was 5.5 years(interquartile range(IQR):2.2-12.8),GCS score was 3(IQR:3-6),Head Abbreviated Injury Score(hAIS)was 4(IQR:3-5),and most had a severe(25-49)Injury Severity Score(ISS)(48.7%,153/314).Overall mortality was 26.8%.GCS score,hAIS,ISS,INR,platelet count,and blood glucose were associated with in-hospital mortality(all p<0.05).As age and GCS score increased,the odds of mortality *** 1-point increase in GCS score was associated with a 35%decrease in odds of *** hAIS,INR,and blood glucose increased,the odds of mortality *** each 1.0 unit increase in INR,the odds of mortality increased by 1427%.Conclusions Pediatric patients with severe TBI are at substantial risk for in-hospital *** are needed to examine whether earlier interventions targeting specific parameters of INR and blood glucose impact mortality.

Optical transport networks are now the basic infrastructure of modern communications systems, including the SDH and WDM backbone network of local network operators, in the case of Cameroon. Given the colossal investments required to deploy these networks, particularly related to the cost of equipment (optical fibers, transponders and multiplexers), the optimization of bandwidth and dynamic allocation of resources is essential to control operating costs and ensure continuity of service. Automatic switching technology for optical networks brings intelligence to the control plane to fully facilitate bandwidth utilization, traffic redirection, and automatic configuration of end-to-end services. This paper considers a local network operator’s WDM network without the implementation of the automatic switching technology, develops a network modeling software platform called Graphic Networks and using graph theory integrates a particularity of the automatic switching technology, which is the automatic rerouting of traffic in case of incident in the network. The incidents considered here are those links or route failures and node failures.

Magneto-ionics,an emerging approach to manipulate magnetism that relies on voltage-driven ion motion,holds the promise to boost energy efficiency in information technologies such as spintronic devices or future non-von Neumann computing *** this purpose,stability,reversibility,endurance,and ion motion rates need to be synergistically *** various ions,nitrogen has demonstrated superior magneto-ionic performance compared to classical species such as oxygen or ***,we show that ternary Co1−xFexN compound exhibits an unprecedented nitrogen magneto-ionic *** substitution of Co by Fe in binary CoN is shown to be favorable in terms of generated magnetization,cyclability and ion motion ***,the Co_(0.3)5Fe_(0.6)5N films exhibit an induced saturation magnetization of 1,500 emu/cm^(3),a magneto-ionic rate of 35.5 emu/(cm^(3)·s)and endurance exceeding 103 *** values significantly surpass those of other existing nitride and oxide *** improvement can be attributed to the larger saturation magnetization of Co_(0.35)Fe_(0.65) compared to individual Co and Fe,the nature and size of structural defects in as-grown films of different composition,and the dissimilar formation energies of Fe and Co with N in the various developed crystallographic structures.

Many species exhibit color polymorphisms which have distinct physiological and behavioral ***,the consistency of morph trait covariation patterns across species,time,and ecological contexts remains *** trait covariation is especially relevant in the context of invasion biology and urban ***,physiological traits pertaining to energy maintenance are crucial to fitness,given their immediate ties to individual reproduction,growth,and population *** investigated the physiological traits of Podarcis muralis,a versatile color polymorphic species that thrives in urban environments(including invasive populations in Ohio,USA).We measured five physiological traits(plasma corticosterone and triglycerides,hematocrit,body condition,and field body temperature),which compose an integrated multivariate *** then tested variation among co-occurring color morphs in the context of establishment in an urban *** found that the traits describing physiological status and strategy shifted across the active season in a morph-dependent manner—the white and yellow morphs exhibited clearly different multivariate physiological phenotypes,characterized primarily by differences in plasma *** suggests that morphs have different strategies in physiological regulation,the flexibility of which is crucial to urban *** white-yellow morph exhibited an intermediate phenotype,suggesting an intermediary energy maintenance *** morphs also exhibited distinct phenotypes,but the low prevalence of this morph in our study populations precludes clear *** work provides insight into how differences among stable polymorphisms exist across axes of the phenotype and how this variation may aid in establishment within novel environments.
