Effect of pathological myopia on biomechanical properties:a study by ocular response analyzer

AIM: To evaluate the ocular response analyzer(ORA)measurements of patients with pathological myopia in comparison with those of emmetropic control subjects,and to investigate the correlation between these ORA measurements and spherical equivalent(SE).METHODS: Measurements of 53 eyes of 53 subjects with pathological myopia(SE >-6.00 D) were compared with those of 60 eyes of 60 emmetropic controls. Corneal hysteresis(CH), corneal resistance factor(CRF),noncontact tonometer intraocular pressure(IOPg), and corneal-compensated IOP(IOPcc) were obtained for each subject. The refractive error value was determined as SE via a cycloplegic refraction ***: The mean age was 54.1±18.9y(ranging from5 to 88) in the pathological myopic group and 56.2±19.0y(ranging from 6 to 89) in the control group. There were no significant differences between the groups concerning age and sex. CH and CRF were significantly lower in the pathological myopic group than in the control group(P <0.001, P =0.005, respectively). IOPcc and IOPg were significantly higher in the pathological myopic group than in the control group(P <0.001, P =0.009,respectively). There were significantly positive correlations between CH and SE(r =0.565, P <0.001) and between CRF and SE(r =0.364, P =0.007). There were significantly negative correlations between IOPcc and SE(r =-0.432, P =0.001) and between IOPg and SE(r =-0.401,P =0.003).CONCLUSION: The present study displayed that pathological myopia affected biomechanical properties measured by ORA. The results of corneal biomechanicalproperties measured by ORA may need to be appreciated by taking refraction into account. Further, pathological myopia might be related with the increased IOP.

Serum neopterin levels in children with hepatitis-B-related chronic liver disease and its relationship to disease severity

AIM:To evaluate serum neopterin levels and their correlations with liver function tests and histological grade in children with hepatitis-B-related chronic liver disease. METHODS:The study population comprised 48 patients with chronic active hepatitis B,32 patients with hepatitis-B-related active liver cirrhosis and 40 normal controls. Serum neopterin was measured using an enzyme-linked immunosorbent assay. RESULTS:The mean ± SD serum neopterin levels were 14.2 ± 5.6 nmol/L in patients with chronic hepatitis,20.3 ± 7.9 nmol/L in patients with liver cirrhosis and 5.2 ± 1.4 nmol/L in control group. Serum neopterin levels were signif icantly higher in patients with chronic hepatitis (P = 0.005) and cirrhosis patients (P = 0.008),than in control subjects. Cirrhotic patients had signif icantly higher serum neopterin levels than patients with chronic hepatitis (P = 0.004). There was a positive correlation between serum neopterin levels and alanine aminotransferase levels in patients with chronic hepatitis (r = 0.41,P = 0.004) and cirrhotic patients (r = 0.39,P = 0.005). Positive correlations were detected between serum neopterin levels and inflammatory score in patients with chronic hepatitis (r = 0.51,P = 0.003) and cirrhotic patients (r = 0.49,P = 0.001). CONCLUSION:Our results suggest that serum neopterin levels can be considered as a marker of inflammatory activity and severity of disease in children with hepatitis-B-related chronic liver disease.

Violence toward emergency physicians: A prospective-descriptive study

BACKGROUND: The highest rate of workplace violence occurs in the health sector, although most cases remain unreported. Emergency services face the majority of these incidents for many reasons, such as the patient profile, long waiting time, and overcrowding. We aimed to determine the characteristics and causes of violence toward emergency physicians. METHODS: The acts of violence toward emergency physicians over a one-year period were prospectively recorded. After a violent incident took place, a third party separately interviewed the physician exposed to the violent behavior and the perpetrator who displayed this behavior. We examined the perpetrator's reasons for violence, their demographic characteristics, and the medical complaints of patients involved in such events to determine the characteristics and causes of ***: Of the violent acts investigated, 85.1% were verbal, and most were directed toward male doctors by the young male relatives of the patients. More than half of the violent acts occurred within the 15 minutes of presentation to emergency service(60.5%) and at off-hours(69.4%). Concerning the health insurance, 20.4% of the cases were covered by the free green card system, and a small number of the perpetrators of violence lived in rural areas(38.2%). The most common reason for violent behavior was the patients' or their relatives' dissatisfaction with the examination or treatment method(38.2%).CONCLUSIONS: Appropriate communication should be established with the patients, and they should be adequately informed about the treatments and interventions to be performed in order to prevent possible acts of violence.

Casting properties of ASTM A128 Gr.E1 steel modified with Mn-alloying and titanium ladle treatment

This work aims to produce a high manganese steel with more refined austenite grains and better wear resistance without sacrificing the toughness and tensile properties by Mn alloying and Ti ladle treatment in comparision to ASTM A128 Gr.E1 steel (1.0C-13Mn) that is mostly used in the mining *** 1.0C-17Mn-xTi alloys (x=0,0.05 and 0.1,in wt.%) were prepared.A relationship was established between the microstructures and mechanical properties of the as-cast and solution annealed *** Ti content increases the stable Ti(CN) phase on and beside the grain boundaries and decreases up to 37% the austenite grain size of the as-cast alloy with 0.10wt.% ***,after solution annealed,optimized titanium content (0.05wt.%) results in significant improvements in wear resistance,hardness,elongation,yield and tensile strengths by 44%,31%,30%,8% and 12%,respectively,except 9% decrease in impact toughness compared to ASTM A 128 Gr.E1 steel without *** results show that 1.0C-17Mn-0.05Ti alloy can be used for parts exposed to high load wear and applied in conditions where relatively high tensile properties with sufficent ductility is needed.

Influence of H pylori on plasma ghrelin in patients without atrophic gastritis

AIM:To determine the association between H pylori infection and serum ghrelin levels in patients without atrophic ***:Fifty consecutive patients(24 males and 26 females)with either H pylori-positive gastritis(n = 34)or H pylori-negative gastritis(n = 16)with normal gastric acid secretion determined by 24-h pHmetry and without atrophic gastritis in histopathology were enrolled in this ***-four H pylori-infected patients were treated with triple therapy consisting of a daily regimen of 30 mg lansoprazole bid,1 g amoxicillin bid and 500 mg clarithromycin bid for 14 d,followed by an additional 4 wk of 30 mg lansoprazol treatment.H pylori infection was eradicated in 23 of 34(67.6%)patients.H pylori-positive patients were given eradication *** acidity was determined via intragastric pH *** ghrelin was measured by radioimmunoassay(RIA).RESULTS:There was no signifficant difference in plasma ghrelin levels between H pylori-positive and H pylori-negative groups(81.10 ± 162.66 ng/L vs 76.51 ± 122.94 ng/L).In addition,there was no significant difference in plasma ghrelin levels and gastric acidity levels measured before and 3 mo after the eradication ***:H pylori infection does not influence ghrelin secretion in patients with chronic gastritis without atrophic gastritis.

Analysing Shakespeare＇s Emblem Language in Midsummer Night＇s Dream

Shakespeare has been studied from many perspectives so far. However, his language style used in his literary works is still worth studying. First of all, this study explores changing sense of emblem and emblem language comparing Shakespeare＇s era and today. Secondly, it puts stress on the effects of emblem traditions on Shakespeare. Lastly, this article aims to shed light on his emblematical language in his Midsummer Night＇s Dream, which is one of his most leading works in terms of having highly visible emblematical interpretations.

Are there fetal stem cells in the maternal brain?

Fetal cells can enter maternal blood during pregnancy but whether they can also cross the blood-brain barrier to enter the maternal brain remains poorly understood. Previous results suggest that fetal cells are summoned to repair damage to the mother＇s brain. If this is confirmed, it would open up new and safer avenues of treatment for brain damage caused by strokes and neural diseases. In this study, we aimed to investigate whether a baby＇s stem cells can enter the maternal brain during pregnancy. Deceased patients who had at least one male offspring and no history of abortion and blood transfusion were included in this study. DNA was extracted from brain tissue samples of deceased women using standard phenol-chloroform extraction and ethanol precipitation methods. Genomic DNA was screened by quantitative fluorescent-polymerase chain reaction amplification together with short tandem repeat markers specific to the Y chromosome, and 13, 18, 21 and X. Any foreign DNA residues that could be used to interpret the presence of fetal stem cells in the maternal brain were monitored. Results indicated that fetal stem cells can not cross the blood-brain barrier to enter the maternal brain.
