Differential diagnosis of diarrhoea in patients with neuroendocrine tumours: A systematic review

Translating new data to the daily practice in second line treatment of renal cell carcinoma:The role of tumor growth rate

The therapeutic options for patients with metastatic renal cell carcinoma(mRCC) have completely changed during the last ten years. With the sequential use of targeted therapies, median overall survival has increased in daily practice and now it is not uncommon to see patients surviving kidney cancer for more than four to five years. Once treatment fails with the first line targeted therapy, head to head comparisons have shown that cabozantinib, nivolumab and the combination of lenvatinib plus everolimus are more effective than everolimus alone and that axitinib is more active than sorafenib. Unfortunately, it is very unlikely that we will ever have prospective data comparing the activity of axitinib, cabozantinib, lenvatinib or nivolumab. It is frustrating to observe the lack of biomarkers that we have in this field, thus there is no firm recommendation about the optimal sequence of treatment in the second line. In the absence of reliable biomarkers, there are several clinical endpoints that can help physicians to make decisions for an individual patient, such as the tumor burden, the expected response rate and the time to achieve the response to each agent, the prior response to the agent administered, the toxicity profile of the different compounds and patient preference. Here, we propose the introduction of the tumor-growth rate(TGR) during first-line treatment as a new tool to be used to select the second line strategy in m RCC. The rapidness of TGR before the onset of the treatment reflects the variability between patients in terms of tumor growth kinetics and it could be a surrogate marker of tumor aggressiveness that may guide treatment decisions.

Sequential treatment in disseminated well-and intermediate-differentiated pancreatic neuroendocrine tumors:Common sense or low rationale?

Fortunately,the landscape of the systemic treatment for grade 1 and 2 pancreatic neuroendocrine tumors has changed in the last decade with at least four different alternatives approved in the ***,somatostatin analogues,sunitinib and everolimus remind valid options according to the most referenced international ***,and although this is something done in the routine practice,there is a lack of evidence for the use of any of these strategies after failure to the ***,further sequential alternatives in third or fourth line have never been tested *** need for a better understanding of the rationale to sequence different systemic options is even greater in non-pancreatic neuroendocrine tumors since available therapies are *** strategies in other solid tumors have led to a clear improvement in overall *** is also believed to occur in neuroendocrine tumors but no clear data on it has been delivered *** postulate that the different mode of action of the systemic options available for the treatment of neuroendocrine tumors may avoid the complete resistance of one option after the other and that sequential use of these agents will be translated into a longer overall survival of *** and randomized trials that seek for the activity of drugs after failure to another systemic alternatives are highly needed in this field of neuroendocrine tumors.

Probabilistic Safety Analysis of High Speed and Conventional Lines Using Bayesian Networks

A Bayesian network approach is presented for probabilistic safety analysis(PSA)of railway *** idea consists of identifying and reproducing all the elements that the train encounters when circulating along a railway line,such as light and speed limit signals,tunnel or viaduct entries or exits,cuttings and embankments,acoustic sounds received in the cabin,curves,*** addition,since the human error is very relevant for safety evaluation,the automatic train protection(ATP)systems and the driver behaviour and its time evolution are modelled and taken into account to determine the probabilities of human *** nodes of the Bayesian network,their links and the associated probability tables are automatically constructed based on the line data that need to be carefully *** conditional probability tables are reproduced by closed formulas,which facilitate the modelling and the sensitivity analysis.A sorted list of the most dangerous elements in the line is obtained,which permits making decisions about the line safety and programming maintenance operations in order to optimize them and reduce the maintenance costs *** proposed methodology is illustrated by its application to several cases that include real lines such as the Palencia-Santander and the Dublin-Belfast lines.

Cotton (Gossypium hirsutum L.) Boll Rot and Associated Microorganisms in South Texas Fields

A rise in cotton boll rot in south Texas has been generally associated with increased yield losses. Here, we measured boll rot incidence during two growing seasons (2011 and 2012) at a south Texas (Kleberg County) research farm Variety Trial and in producer fields. The Variety Trial was conducted to compare boll rot susceptibility between five current cultivars. The commercial fields surveyed were located along the Coastal Bend (Wharton County) and Rio grande Valley regions (Cameron and Willacy Counties). Bolls with evidence of external damage potentially inflicted by piercing-sucking insect vectors were dissected for disease detection and plated for microorganism isolation and characterization. Microbial isolates were putatively identified based on standard fatty acid methyl ester profile analysis. In the Variety Trial, the highest incidence of disease occurred in July for both growing seasons, and significant differences in susceptibility to boll rot between cultivars were determined (P Bacillus spp. as a potential and prevalent causative agent(s).

Energetic-protein supplementation in the last 60 days of gestation improves performance of beef cows grazing tropical pastures

Background： Nutrition is one of the most important factors that affect animal performance, and it therefore also impacts on financial results in beef systems. In this way, finding the best strategy for feeding supplements is of paramount importance. Aiming to evaluate the effect of supplement feeding strategies for beef cows in the last third of gestation, two experiments were conducted. In Experiment 1, 35 pregnant Nellore cows were assigned to a completely randomized design with four treatments： control, which received no supplement; supplementation for the last 30 d of gestation（30-d; 3.0 kg/d）; supplementation for the last 60 d of gestation（60-d; 1.5 kg/d）; or supplementation for the last 90 d of gestation（90-d; 1.0 kg/d）. All supplemented treatments received the same total amount of supplement throughout the experiment： 90 kg（20% of crude protein）. A second experiment（Experiment 2） was delineated to evaluate the effects of the amounts offered in Experiment 1 on intake and metabolism. Four multiparous pregnant Nellore cows were assigned to a 4 × 4 Latin square design, with periods of15 d ***： There was a linear effect of the number of days of supplementation on calving body weight（BW; P 〈 0.05）and a quadratic effect on BW change from parturition to d 31 post-calving（P 〈 0.05）, with cows on the 60-d strategy losing less BW post-calving. No difference was found in offspring birth BW（P 〉 0.10）. A significant linear effect on interval from parturition to conception（P 〈 0.05） was observed, with the highest calving to conception interval being observed in the 90-d strategy. The level of supplementation did not affect forage intake or neutral detergent fiber digestibility（P 〉 0.10）. Nitrogen excreted through urine tended to increase linearly with the level of supplementation（P 〈 0.10）.Conclusion： Providing 1.5 kg of supplement during the last 60 d of gestation improves cow performance after calving, reducing the magnitude of BW lost, and reduces the number of

Attention Detection System Based on the Variability of Heart Rate

Attention is a cognitive and psychobiological variable that lacks objective measures in natural contexts of formal learning. However, it can be objectively measured from the heart autonomic activity. In the present study, the validity and reliability of a system for recording heart rate variability are demonstrated. Twelve prototypes were created and paired with standard electrocardiogram. Long segments with cardiac pulse recordings were used within ten minutes. A highly significant correlation index (p < 0.01) and a significantly high Cronbach’s alpha were obtained. In the variance analysis, all data analyzed presented a value of p < 0.05. It is concluded that there is no significant variation between paired systems;therefore, the attention detection system based on heart rate variability is valid and reliable.

Physical and chemical characterization of sediments from an Andean river exposed to mining and agricultural activities: The Moquegua River, Peru

This study presents an analysis of sediments in an Andean river impacted by both natural conditions and anthropogenic activities. Fifty samples were collected from selected sites throughout the Moquegua River drainage basin, and Tambo River headwaters at Pasto grande, in Peru, and analysed with X-ray diffraction, energy dispersive X-ray spectroscopy, scanning electron microscopy, Fourier-transform infrared spectroscopy and thermogravimetric analysis. Si, Ca, Al, Fe, and O, common constituents of soils, were dominant, along with K, N, and P. The latter originating as primary macronutrients or chemical residuals from fertilizers used in agriculture. Higher concentrations of macronutrients and organic components were found in agricultural and urban areas, respectively. Arsenic minerals were also detected, which occur naturally, but possibly at levels augmented by anthropogenic activity. The application of cluster analysis revealed clustering between arsenic, arsenolite, and potassium but no significant geospatial correlation between sample sites in the drainage basin.

Nocardiosis diagnosis is a major challenge. The clinical features and radiological findings are nonspecific. Traditionally, Nocardia identification is based on colonial and microscopical morphology and biochemical tests. However, molecular biology techniques allow a better characterization of species and biotypes. PCR-RFLP of the 65-kDa heat shock protein (HSP) gene provides a rapid, sensitive, and time and labor-efficient method for this proposal. Using this technique, six of eight isolates tested were identified as Nocardia asteroides type VI. PCR-RFLP of the 65-kDa HSP gene could be very useful for determining the incidence of this pathogen in different population groups and its association with susceptibility/resistance profiles to the drugs of choice for treatment. This work is the first molecular detection of Nocardia species in Cuba.
