CT Findings in Patients Affected by H1N1 Influenza Virus

Objectives: To evaluate the primary lesions, the complications and the evolution of disease in patients affected by H1N1 viral infection. Materials and Methods: 24 Patients affected by H1N1 infection, diagnosed by polymerase chain reaction (PCR) on throat swabs, underwent CT examination. Seven patients were hospitalized in intensive care unit (ICU). In five patients the evolution of disease was monitored. The following features were evaluated: primary lesions significant for viral infection, their possible complications and the evolution of disease in controlled patients. Results: Primary lesions variously associated with each other were found in 22 out of 24 patients: ground glass opacities (19/24, 79.2%), interstitial thickening (13/24, 54.2%), centrilobular nodules (3/24, 12.5%) and consolidation (8/24, 33.3%). The following complications were observed: 3 consolidations with air bronchogram, 9 pleural effusions, 7 ARDS and 1 barotrauma. In the 5 patients who underwent follow-up (including 3 admitted to ICU), complete resolution was demonstrated in 4 cases and focal fibrotic evolution in one case. 3 ICU patients affected by ARDS died. Conclusions: In case of H1N1 virus infection. CT is an important tool for staging the disease, recognize complications and to study disease

AIM： TO determine the DNA binding activity and protein levels of the Ku70/80 heterodimer, the functional mediator of the NHEJ activity, in human colorectal carcinogenesis. METHODS： The Ku70/80 DNA-binding activity was determined by electrophoretic mobility shift assays in 20 colon adenoma and 15 colorectal cancer samples as well as matched normal colonic tissues. Nuclear and cytoplasmic protein expression was determined by immunohistochemistry and Western blot analysis. RESULTS： A statistical found in both adenomas y significant difference was and carcinomas as compared to matched normal colonic mucosa （P〈0.00）. However, changes in binding activity were not homogenous with approximately 50% of the tumors showing a clear increase in the binding activity, 30% displaying a modest increase and 15% showing a decrease of the ***, with increased DNA-binding activity, also showed a statistically significant increase in Ku70 and Ku86 nuclear expression, as determined by Western blot and immunohistochemical analyses （P〈0.001）. Cytoplasmic protein expression was found in pathological samples, but not in normal tissues either from tumor patients or from healthy subjects. CONCLUSION： Overall, our DNA-binding activity and protein level are consistent with a substantial activation of the NHEJ pathway in colorectal tumors. Since the NHEJ is an error prone mechanism, its abnormal activation can result in chromosomal instability and ultimately lead to tumorigenesis.

Leukocytosis at Diagnosis in Patients with Essential Thrombocythemia Is a Risk Factor for Transformation into Myelofibrosis

Myelofibrosis (MF) represents the major long-term complication of essential thrombocythemia (ET). There is evidence that leukocytosis at diagnosis is associated with poorer survival in patients with ET. In this study, we retrospectively evaluated 143 patients with ET, diagnosed in agreement with WHO criteria, followed in a single centre over >10 years. Nine of them transformed into MF (post-essential thrombocythemia-myelofibrosis PET-MF). We compared PET-MF data at diagnosis with that of the remaining 134 patients (ET-1) and with a selected sub-group of ET-1 (ET-2, 19 pats) sex, age and follow-up duration matched to PET-MF. The PET-MF evolution rate was 4.6 per 1000 person-years;white blood cells count (WBC) count, haemoglobin levels and hematocrit were higher in PET-MF than in ET-1 (P = 0.01) while only WBC was higher than in ET-2 (P = 0.01). With multivariate analysis, only WBC count retained its signifi-cance. Our study highlights the prognostic relevance of leukocytosis on myelofibrotic transformation of ET.

TM6SF2 E167K variant predicts severe liver fibrosis for human immunodeficiency/hepatitis C virus co-infected patients, and severe steatosis only for a non-3 hepatitis C virus genotype

AIM To evaluate the impact of the Glu167Lys(E167K) transmembrane 6 superfamily member 2(TM6SF2) variant on the biochemical and morphologic expression of liver lesions in human immunodeficiency virus(HIV)/hepatitis C virus(HCV) co-infected *** The study comprised 167 consecutive patients with HIV/HCV coinfection and biopsy-proven chronic hepatitis. A pathologist graded liver fibrosis and necroinflammation using the Ishak scoring system, and steatosis using Kleiner's scoring system. Patients were genotyped for TM6SF2 E167K(rs58542926) by real-time Polymerase chain reaction. The 167 patients, 35 therapy-naive and 132 receiving ART, were prevalently males(73.6%), the median age was 40.7 years and the immunological condition good(median CD4+ cells/mm3 = 505.5).RESULTS The 17 patients with the TM6SF2 E167 K variant, compared with the 150 with TM6SF2-E/E, showed higher AST(P = 0.02) and alanine aminotransferase(P = 0.02) and higher fibrosis score(3.1 ± 2.0 vs 2.3 ± 1.5, P = 0.05). In a multivariate analysis, TM6SF2 E167 K was independently associated with severe fibrosis. The same analysis showed that HCV-genotype 3, present in 42.2% of patients was an independent predictor of severe steatosis. The association of TM6SF2 E167 K with severe steatosis, absent for the whole group of 167 patients, was re-evaluated separately for HCVgenotype 3 and non-3 patients: No factor was independently associated with severe steatosis in the HCV-genotype-3 subgroup, whereas an independent association was observed between severe steatosis and TM6SF2 E167 K in non-3 HCV genotypes. Sand Dune Species Distribution and Size Variations in Two Areas Inside a Natural Protected Area Subjected to Different Human Disturbance

Sand dune species were analyzed across two areas included in the “Roman Coastal State Nature Reserve” (Italy): Ostia (O) and Marina di Palidoro (P). Significant variations in species distribution, dry mass and size over the gradient from the water-edge toward the inland between O and P were observed. Species colonize O, on an average, at 79 m from the water-edge extended along the gradient toward the inland over a length of 26 m. PD is 40.8 ± 9.9 plants.m-2. Ononis variegata and Elymus farctus have the highest PD (15.4 ± 1.3 plants.m-2), followed by Silene colorata and Sporobulus pungens (4.3 ± 3.9 plants.m-2), by Cakile maritima, Echinophora spinosa, Eryngium maritimum, Ammophila arenaria, Anthemis maritima, Cyperus capitatus, Medicago marina and Calystegia soldanella (.m-2), with C. soldanella having the lowest PD (0.02 plants.m-2). On an average, in P, the species colonize the dune at 17 m from the water-edge for a length of 46 m toward the inland, PD is on an average 21.5 ± 16.1 plants.m-2. Ammonia Diffusion Phenomena through Nalophan<sup>TM</sup>Bags Used for Olfactometric Analyses

The ammonia loss through NalophanTM bags has been studied. Ammonia was chosen as target compound in order to be representative of odorous molecules of small dimensions. The losses observed for storage conditions and times as allowed by the reference standard for dynamic olfactometry (EN 13725:2003) indicate that odour concentration values due to the presence of small molecules may be significantly underestimated if samples are not analysed immediately after sampling. The diffusion coefficient of ammonia through the NalophanTM film was evaluated using the Fick’s law, and it turned out to be equal to 2.38E-12 (m2/s). The results and their theoretical interpretation indicate that concentration losses due to ammonia diffusion through the NalophanTM film can be decreased by using large bags and filling them up to their maximum capacity.

Left ventricular false tendons and electrocardiogram repolarization abnormalities in healthy young subjects

AIM To describe echocardiographically left ventricular false tendon characteristics and the correlation with ventricular repolarization abnormalities in young *** Three hundred and sixteen healthy young athletes from different sport disciplines were evaluated from 2009 to 2011 during routine screening for agonistic sports eligibility. All subjects, as part of standard preparticipation screening medical evaluation, underwent a basal and post step test 12-lead electrocardiogram(ECG). The athletes with abnormal T-wave flattening and/or inversion were considered for an echocardiogram evaluation and an incremental maximal exercise test on a cycle ergometer. Arterial blood pressure and heart rate, during and after exercise, were also *** Twenty-one of the 316 subjects(6.9%) showed false tendons in the left ventricle. The majority of false tendons(52.38%) were localized between the middle segments of the inferior septum and the lateral wall, 19.06% between the distal segments of the septum and the lateral wall, in 5 subjects between the middle segments of the anterior and inferior walls, and in one subject between the middle segments of the anterior septum and the posterior wall. ECG abnormalities, represented by alterations of ventricular repolarization, were found in 11 subjects(52.38%), 90% of these anomalies were T wave abnormalities from V1 to V3. These anomalies disappeared with an increasing heart rate following the three minute step test as well as during the execution of the maximal *** Left ventricular false tendons are frequently localized between the middle segments of the inferior septum and the lateral wall and are statistically associated with ventricular repolarization abnormalities.
