Launching a paradigm for first and redo-surgery in primary congenital glaucoma: institutional experience

AIm: To evaluate the outcome of the initial and the redo-surgeries for primary congenital glaucoma(PCG) correlated to its degree of severity. mETHODS: A retrospective study involved patients with PCG presented between 2010 and 2018. medical records were reviewed to assess the degree of the preoperative severity according to the intraocular pressure(IOP), corneal diameter and corneal edema. Success and failure rates were calculated for both first and redo-surgeries at 6 and 12 mo respectively then correlated to the severity of the cases. RESULTS: Complete records were retrieved for 272 eyes(153 patients) with PCG: 43 eyes were mild, 136 moderate and 93 severe. Combined trabeculotomy and trabeculectomy(CTT) had the highest success rate in moderate(96.4%) and severe cases(59.3%) while trabeculotomy had the highest success rate in mild cases(96.3%). medical records of 88 eyes(63 patients) with recurrent PCG were analyzed, most with severe presentation(59 eyes). Ahmed glaucoma valve(AGV) was used in 67(76%) eyes and augmented trabeculectomy in 21(24%) eyes. At 12 mo, there was no statistically significant difference between both surgeries in total success rate(P=0.256). For mild cases, success rate was 100% for both surgeries. Severe cases had higher success rates following AGV(87%) than augmented trabeculectomy(20%). Preoperative severity of the disease was an independent factor affecting the failure rate in secondary trabeculectomy but not in AGV. Patients younger than 24 mo had higher probabilities of failure following both redo-surgeries with hazard ratio =1.325 and 0.37 for augmented trabeculectomy and AGV respectively. CONCLUSION: Preoperative assessment of the severity of eyes with PCG helps in the selecting the optimal primary and secondary surgery. For first surgery, trabeculotomy is more effective in mild cases whereas; CTT and augmented subscleral trabeculectomy(SST) are appropriate for moderate and severe cases. Background We aimed to systematically review the clinical and laboratory features of patients with the multisystem inflam-matory syndrome in pediatrics diagnosed during the COVID-19 *** sources A literature search in Web of Science,Pubmed,Scopus,and Science Direct was made up to June 29,*** Analysis of 15 articles(318 COVID-19 patients)revealed that although many patients presented with the typical multisystem inflammatory syndrome in pediatrics,Kawasaki-like features as fever(82.4%),polymorphous maculopapular exanthema(63.7%),oral mucosal changes(58.1%),conjunctival injections(56.0%),edematous extremities(40.7%),and cervical lymphadenopathy(28.5%),atypical gastrointestinal(79.4%)and neurocognitive symptoms(31.8%)were also *** had elevated serum lactic acid dehydrogenase,D-dimer,C-reactive protein,procalcitonin,interleukin-6,troponin I levels,and *** 77.0%developed hypotension,and 68.1%went into shock,while 41.1%had acute kidney *** care was needed in 73.7%of cases;13.2%were intubated,and 37.9%required mechanical *** immunoglobulins and steroids were given in 87.7%and 56.9%of the patients,respectively,and anticoagulants were utilized in 67.0%.Pediatric patients were discharged after a hospital stay of 6.77 days on average(95%CI 4.93-8.6).Conclusions Recognizing the typical and atypical presentation of the multisystem inflammatory syndrome in pediatric COVID-19 patients has important implications in identifying children at *** cardiac and renal decompensation and early interventions in patients with multisystem inflammatory syndrome is critical to prevent further morbidity.

Fractional Order Generalized Thermoelastic Infinite medium with Cylindrical Cavity Subjected to Harmonically Varying Heat

In this work, a mathematical model of an elastic material with cylindrical cavity will be constructed. The governing equations will be taken into the context of the fractional order generalized thermoelasticity theory (Youssef 2010). Laplace transform and direct approach will be used to obtain the solution when the boundary of the cavity is exposed to harmonically heat with constant angular frequency of thermal vibration. The inverse of Laplace transforms will be computed numerically using a method based on Fourier expansion techniques. Some comparisons have been shown in figures to present the effect of the fractional order parameter and the angular frequency of thermal vibration on all the studied felids.

minimizing the Loss Probability in m/m/2/1 Queueing System with Ordered Entry

This study analyzed the m/m/2/1 queueing model with queue of length one (waiting room of capacity just one), heterogeneous servers and ordered entry using the method of semi-markov process. The customers who arrive in the system enter the free server;if the two servers are free, the customers enter the first server. If the two servers are busy, just one customer can wait at the waiting room. If the two servers are busy and the waiting room has a customer, the following customers will leave the system without receiving any service. Such a customer is called LOST COSTOmER. The probability of lost customers in the queueing system under examination was computed. Furthermore, by using inequality obtained from Jensen’s inequality, it was shown that the loss probability was minimum when inter-arrival times fit deterministic distribution [1] [2].

Diabetes mellitus(Dm)could negatively affect patients’health via inducing a lot of serious functional hazards in many tissues’cells at molecular ***,many scientists had proposed stem cell therapy being an appropriate alternative treatment protocol for numerous health threatening issues including ***,the current study was designed to investigate the antioxidant potentiality of two mSCs types in alleviating tissues’oxidative stress dramatic elevation resulting as a consequence of Type 1 Dm *** our 4 weeks study,animals were divided into four groups:control group,STZ-diabetic group(D),D+AD-mSCs group and D+Bm-mSCs *** reported that diabetic rats treated with either AD-mSCs or Bm-mSCs exhibited a marvelous body tissues(Pancreas,Liver and Kidney)enhancing capabilities in attenuating the oxidative stress status;as evidenced by XO,ROS,and mDA levels down-regulation;with a general concomitant elevation in the antioxidants’content;evidenced by many enzymatic and non-enzymatic antioxidants up-regulation;relative to the diabetic untreated ***,comparing both treatments with each other and to control group,most of the measured parameters were reverted back to near normal levels after AD-mSCs injection;which clearly point out their stunning health benefits and superiority as anti-diabetic agent in overcoming different tissues’complications;owing to their marked cytoprotective and regenerative potentialities.

Objective: The aim of the work was to compare the dosimetric results that were obtained by using two treatment planning systems (TPS) Siemens KonRad version 2.2.23, Elekta XiO version 4.4 to perform a simultaneous integrated boost (SIB) for head and neck and central nervous system (CNS) cases in paediatric patients. methods: The CT scan data for five paediatric patients, with head and neck and CNS tumors, were transferred into both of the TPSs. Clinical step-and-shoot intensity-modulated radiotherapy (ImRT) treatment plans were designed using 6 mV photon beam for delivery on a Siemens Oncor Accelerator with multileaf collimator mLC (82 leaf). Plans were optimized to achieve the same clinical objectives using the same beam energy, number and direction of beams. The analysis was based on isodose distributions, the dose volume histogram (DVH) for planning target volume (PTV) and the relevant organs at risk (OARs) as well as volume receiving 2 Gy and 5 Gy, also total number of segments, mU/segment, and the number of mU/cGy had been investigated. Treatment delivery time and conformation number were two other parameters in this study. Results: The segmentation using KonRad was more efficient, resulting in fewer segments (reduction between 13.2% and 48.3%), fewer mUs (reduction between 10.7% and 33%) and that reflected on treatment delivery times to be shorter by up to 8 min or 46%. In most of the cases KonRad had the highest volume receiving in excess of 2 and 5 Gy, and XiO showed the lowest. Also KonRad achieved slightly better conformality (0.76±0.054) than XiO (0.73±0.05) while XiO presented a higher modulation factor value (3.3 mU/cGy) than KonRad (2.4 mU/cGy). Conclusion: The KonRad treatment planning system was found to be superior to the XiO treatment planning system. This is true for the possible increase of radiation-induced secondary malignancies as well as for the local control.

FSH/LH Ratio as a Predictor of the IVF Outcome in Young Women

In Vitro Fertilization (IVF) is the treatment for many causes of infertility. many studies were done to investigate different factors that can affect the success rate. This study was conducted to evaluate if cycle day 3 (CD3) follicle-stimulating hormone (FSH)/luteinizing hormone (LH) ratio can be a predictor for the IVF outcome in young sub-fertile females ≤ 35 years with normal baseline FSH. This is a retrospective case control study conducted at the Centre of Fertility and Andrology Care (CFAC) in Egypt where 235 sub-fertile women underwent IVF. Patients were grouped based on CD3 FSH/LH ratio. Group A consisted of ≤35-year-old women with FSH/LH ratio ≤35-year-old women with FSH/LH ratio ≥ 2. The primary outcomes include the fertilization rate, implantation rate and the clinical pregnancy rate. The secondary outcomes include duration and the total dose of gonadotrophin used. We found that, there was no significant difference in the total dose of gonadotropin used during the IVF cycle. Also, there was no significant difference in the number of retrieved and fertilized oocytes and the number of good embryos. Clinical pregnancy rate was the same in both groups. In conclusion, in patients younger than 35 years, CD3 FSH/LH ratio is not a predictor for IVF outcome.

Vibration of Gold Nano-Beam with Variable Young’s modulus Due to Thermal Shock

In this paper, we will study the most important effects in the nano-scale resonator: the coupling effect of temperature and strain rate, and the non-Fourier effect in heat conduction. A solution for the generalized thermoelastic vibration of nano-resonator induced by thermal loading has been developed. The Young’s modulus is taken as a linear function of the reference temperature. The effects of the thermal loading and the reference temperature in all the studied fields have been studied and represented in graphs with some comparisons. The Young’s modulus makes significant effects on all the studied fields where the values of the temperature, the vibration of the deflection, stress, displacement, strain, stress-strain energy increase when the Young’s modulus has taken to be variable.

Ameliorative effect of morus alba leaves extract against developmental retinopathy in pups of diabetic and aluminum intoxicated pregnant albino rats

Objective: To investigate the possible ameliorative effect of crude water extract of morus alba(m. alba) leaves on retinopathy of rat pups maternally subjected to diabetes and/or Al ***: Both control and experimental groups were subjected to certain integrated approaches, namely, biochemical assessments, light microscopic investigation, transmission electron microscopic investigation, single cell gel electrophoresis(comet assay) and determination of DNA ***: The retina of pups of diabetic and/or Al-intoxicated mothers exhibited abnormal alterations in retinal cell layers including retinal pigmented epithelium, photoreceptor inner segment and ganglion cells. Increased incidence of DNA fragmentation and apoptosis were evident in pups of diabetic and/or Al-intoxicated mothers. However, retina of pups maternally received m. alba extract plus diabetes or Al-intoxicated alone or in combination showed marked amelioration. Less degree of ameliorations was seen in retina of pups maternally subjected to combined treatment. Furthermore, application of crude water extract of *** resulted in amelioration of the alterations of maternal serum glucose as well as Al ***: Based on the results of the present study, m. alba extract is effective against experimentally diabetic and Al-induced developmental retinopathy.
