Advances in the management of infertility in men with spinal cord injury

Couples with a spinal cord injured male partner require assisted ejaculation techniques to collect semen that can then be further used in various assisted reproductive technology methods to achieve a pregnancy. The majority of men sustaining a spinal cord injury regardless of the cause or the level of injury cannot ejaculate during sexual intercourse. Only a small minority can ejaculate by masturbation. Penile vibratory stimulation and electroejaculation are the two most common methods used to retrieve sperm. Other techniques such as prostatic massage and the adjunct application of other medications can be used, but the results are inconsistent. Surgical sperm retrieval should be considered as a last resort if all other methods fail. Special attention must be paid to patients with T6 and rostral levels of injury due to the risk of autonomic dysreflexia resulting from stimulation below the level of injury. Bladder preparation should be performed before stimulation if retrograde ejaculation is anticipated. Erectile dysfunction is ubiquitous in the spinal cord injured population but is usually easily managed and does not pose a barrier to semen retrieval in these men. Semen analysis parameters of men with spinal cord injury are unique for this population regardless of the method of retrieval, generally presenting as normal sperm concentration but abnormally low sperm motility and viability. When sperm retrieval is desired in this population, emphasis should be placed on initially trying the simple methods of penile vibratory stimulation or electroejaculation before resorting to more advanced and invasive surgical procedures.

Proposed Simple, Efficient Free Hand Technique for Frontal External Ventricular Drain (EVD) in Case of Small Ventricle: A Report of New Technique

Background: EVD is a common procedure done in neurosurgery and the residents should master it and this is what exactly means that it should be simple, safe, fast and accurate. EVD can be done through many entry points to the different part of the lateral ventricle but the famous site is the anterior horn of the lateral ventricle which is commonly approached through Kocher’s point which is a famous point for all neurosurgeon by its measurements of allocation. The commonest problem with EVD insertion into the anterior horn is the navigation of the ventricular cavity with the right trajectory and so the location of the catheter tip in the right place near the foramen of monro to ensure CSF draining. Size of the ventricle plays significant role in the success of the procedure especially free hand technique. The more dilated ventricle, the more chance to hit the ventricular cavity especially from the first trial and vice versa. In case of small ventricle, the procedure with free hand technique seems more complicated and the chance to hit the ventricle from the first trial may reduce especially with non-expert surgeon but may succeed in the following trials with increased risk of complications with more trials. Purpose: Most EVD’s are placed with freehand technique which depends on the operator skills to navigate the ventricle with right trajectory and hit the ventricle from the first trial, because many trials may complicate the procedure and produce undesirable side effects. So the key in this free hand technique is how to ensure the success from the first trial. In case of small anterior horn of the lateral ventricle, the procedure is even more difficult with the free hand technique and the classic Kocher point needs an expert to get the right trajectory to navigate such small ventricle. Retrocaval Ureter: A Case Series of Three Cases Managed with Uretrouretrostomy

Retrocaval ureter is a very rare congenital malformation. We report a 10 years’ experience in the diagnosis and treatment of retrocaval ureter, a case series of 3 cases in two different countries of the Middle East. This is a retrospective study that included 3 cases of retrocaval ureters in Egypt and Saudi Arabia. Standard open ureteroureteric anastomosis was performed through a flank incision for each case. Patients’ symptoms were re-evaluated after two to four months. Complete recovery from symptoms occurred, and hydroureter and hydronephrosis regressed in all cases. Early diagnosis and treatment are the keys to prevent hydronephrosis and deterioration of renal functions.

Radiological Effects of Soil and Rock Samples of Different Libyan Regions

The activity concentration of natural radioactivity for soil samples collected from western and mid Libyan regions were measured using HPGe detector. The average activity concentration of 226Ra, 232Th and 40K for ten regions was found to be 51.86 ± 7.14, 75.56 ± 10.95 and 128.98 ± 6.88 Bq/kg respectively. The results obtained for the corresponding nuclides 226Ra and 232Th are above the worldwide average values (35 and 30 Bq/kg) while 40K was smaller than worldwide average (400 Bq/kg). The average outdoor absorbed dose and the annual effective dose rates due to 226Ra, 232Th and 40K were observed to be 21.51 ± 2.93 nGy/h and 0.297 ±.03 mSv/y respectively, which are lower than world average values (60 nGy/h and 0.8 mSv/y). The radium equivalent activity and external hazard indices were found less than the world wide average values.

Management of Post-Circumcision Trapped Penis with Glanular Amputation

A phenomenon known as trapped penis has numerous origins, including injudicious circumcision. Materials and Methods: Surgery for trapped penis with glanular amputation was performed for 5 children. The surgical policy in these cases included complete penile degloving, excising the scarring due to circumcission, fixing the penile skin at the penopubic and penoscrotal angles and reconstructing the penile skin with full thickness skin graft. Results: Improvement was noted in all cases. Conclusion: Surgery for hidden penis using full thickness skin graft achieves very satisfactory aesthetic and functional improvement.

Role of endoscopic-ultrasound-guided biliary drainage with electrocautery-enhanced lumen-apposing metal stent for palliation of malignant biliary obstruction

Machine and deep learning driven models for the design of heat exchangers with micro-finned tubes

The design of micro-finned tube heat exchangers is a complex task due to intricate geometry, heat transfer goals, material selection, and manufacturing challenges. Nowadays, mathematical models provide valuable insights, aid in optimization, and allow us to explore various design parameters efficiently. However, existing empirical models often fall short in facilitating an optimal design because of their limited accuracy, sensitivity to assumption, and context dependency. In this scenario, the use of Machine and Deep Learning (ML and DL) methods can enhance accuracy, manage nonlinearity, adjust to varying conditions, decrease dependence on assumptions, automatically extract pertinent features, and provide scalability. Indeed, ML and DL techniques can derive valuable insights from datasets, contributing to a comprehensive understanding. By means of multiple ML and DL methods, this paper addresses the challenge of estimating key parameters in micro-finned tube heat exchangers such as the heat transfer coefficient (HTC) and frictional pressure drop (FPD). The methods have been trained and tested using an experimental dataset consisting of over a thousand data points associated with flow condensation, involving various tube geometries. In this context, the Artificial Neural Network (ANN) demonstrates superior performance in accurately estimating parameters with MAEs in the range below 4.5% for both HTC and FPD. Finally, recognizing the importance of comprehending the internal mechanisms of the black-box ANN model, the paper explores its interpretability aspects.
