Frozen Section in Postmenopausal Women Presented with Suspicious Ovarian Masses, Does It Have a Role?

Background: Frozen section (FS) has a valuable role in the diagnosis of ovarian tumors. It is considered a pivotal point in guiding the surgical therapy, particularly in premenopausal women. In postmenopausal women, it may be required as well to avoid unnecessary surgical staging in benign ovarian tumors. Aim: This study aims to evaluate the accuracy of intraoperative frozen section in ovarian neoplasms in postmenopausal women. Materials and Methods: A retrospective analysis was done for intraoperative FS for suspected ovarian neoplasms. The study was conducted in Oncology Center, Mansoura University from March 2008 to December 2014. The frozen and paraffin section reports were compared, and overall accuracy, sensitivity, specificity, positive and negative predictive values were determined. Results: The study included 105 patients and the overall accuracy of FS in determining malignancy was 81.7%. The sensitivity for malignant tumors was 75.32% with specificity of 100%. For benign tumors, the sensitivity and specificity were 100% and 93%, respectively. Borderline tumors had the lowest sensitivity of 100% with specificity of 95.19%. Conclusion: The present study concurs that frozen section is an accurate test for diagnosis of benign and malignant tumors in postmenopausal women thus determining the extent of surgery done for them. On the other hand, accuracy rates for borderline tumors are low.

BACKGROUND Our understanding of the severe acute respiratory syndrome coronavirus 2 has evolved since the first reported cases in December 2019,and a greater emphasis has been placed on the hyper-inflammatory response in severely ill *** purpose of this study was to determine risk factors for mortality and the impact of anti-inflammatory therapies on *** To determine the impact of various therapies on outcomes in severe coronavirus disease 2019 patients with a focus on anti-inflammatory and immune-modulating *** A retrospective analysis was conducted on 261 patients admitted or transferred to the intensive care unit in two community hospitals between March 12,2020 and June 17,*** 167 patients received glucocorticoid(GC)*** patients received GC alone,94 received GC and tocilizumab,28 received tocilizumab monotherapy,and 66 received no anti-inflammatory *** Patient survival was associated with GC use,either alone or with tocilizumab,and decreased vasopressor *** administration of GC was found to decrease the survival benefit of GC *** difference in survival was found with varying anticoagulant doses,convalescent plasma,tocilizumab monotherapy;prone ventilation,hydroxychloroquine,azithromycin,or intravenous ascorbic acid *** This analysis demonstrated the survival benefit associated with anti-inflammatory therapy of GC,with or without tocilizumab,with the combination providing the most *** studies are needed to assess the optimal timing of anti-inflammatory therapy initiation.

Hypertriglyceridemic pancreatitis(HTGP) accounts for up to 10% of acute pancreatitis presentations in nonpregnant individuals and is the third most common cause of acute pancreatitis after alcohol and gallstones. There are a number of retrospective studies and case reports that have suggested a role for apheresis and insulin infusion in the acute inpatient setting. We report a case of HTGP in a male with hyperlipoproteinemia type Ⅲ who was treated successfully with insulin and apheresis on the initial inpatient presentation followed by bi-monthly outpatient maintenance apheresis sessions for the prevention of recurrent HTGP. We also reviewed the literature for the different inpatient and outpatient management modalities of HTGP. Given that there are no guidelines or randomized clinical trials that evaluate the outpatient management of HTGP, this case report may provide insight into a possible role for outpatient apheresis maintenance therapy.

近年来,赛莱菲主义的全球传播引起了学界的广泛关注。而从土耳其社会发展看,受到伊斯兰教逊尼派哈乃斐教法学派和马图里迪教义学派影响,后又经共和制度、西方哲学思想以及世俗主义的洗礼,逐步形成独具特色的苏菲社会组织形式。由此,过去30多年来,赛莱菲主义者虽通过出版书籍、新媒体推介等多种宣传方式及受到其他外部因素影响,在土耳其部分地区得以传播,但基于历史遗痕、教义本身的严苛性以及"圣战赛莱菲"的负面效应,赛莱菲主义在土耳其的崛起还很遥远,在土耳其苏菲社会也很难有所发展。

目的:研究封闭式负压引流(VSD)不同负压值对猪皮肤软组织感染创面中绿脓杆菌生长的影响。方法:猪皮肤软组织创伤感染创面模型,创面样本共40例,分别给予1-40编号,随机分为5组,每组8例样本。对照组(常规换药)和-25,-20,-15,-10 kPa治疗组。对照组常规换药,各治疗组分别用相应的负压的封闭负压引流技术治疗。治疗时间均为3 d。于治疗前和治疗后取创面中心的活组织标本进行细菌计数,并观察创面一般情况。结果:与对照组比较,治疗组(VSD治疗组)细菌计数明显减少(P<0.01),分泌物少,未见明显黄白色组织,肉芽新鲜,伤口明显缩小,无伤口周围组织水肿,愈合时间短,伤口愈合后疤痕不明显。结论:采用不同负压力的封闭负压引流技术均能减少创面分泌物,抑制绿脓杆菌增殖生长,刺激肉芽组织生成,缩短创面愈合时间。

Cardiac resynchronization therapy in acute pulmonary edema: A case report

We are reporting a case of 71-year old lady with a dual chamber demand pacemaker,who developed acute pulmonary edema due to an acute left ventricular(LV)dysfunction and worsening in mitral valve regurgitation after atrioventricular nodal ablation for uncontrolled atrial *** was attributed to right ventricular apical pacing leading to LV *** dramatically improved within 12-24 h after upgrading her single chamber pacemaker to biventricular *** case demonstrates that biventricular pacing can be an effective modality of treatment of acute congestive heart *** particular,it can be used when it is secondary to LV dysfunction and severe mitral regurgitation attributed to significant dyssynchrony created by right ventricular pacing in patients with atrioventricular nodal ablation for chronic atrial fibrillation.

Bioactivity Investigations with Calcia Magnesia Based Composites

The bioactivity and physico-mechanical properties of calcia magnesia based composites developed in this study were investigated. Different composite mixtures containing calcia-magnesia have been processed with the addition of alumina, silica or zircon. These system powders were formed and fired at two different temperatures. The produced composites were characterized by means of X-ray diffraction, SEM (scanning electron microscope) equipped with EDS (energy dispersive X-ray spectrometry), density and apparent porosity measurements, mechanical testing and in-vitro evaluation in a SBF (simulated body fluid) solution. The compositions termed "I", "II" and "III" gave clear tendency towards the formation-ability of HA (hydroxyapatite). Composite "1" gave cubic and spindle HA crystallite, while composites "II" and "III" fired at 1,300 and 1,400 °C formed typically "cauliflower" morphology and their evaluated physico-mechanical properties are similar to the properties of human cortical bone. Thus, composites "II" and "III" might be a promising bone implant materials. Beside the bioactivitiy of composite "I", it also contains highly CA (cementing phase) and MA (bioinert) phases, therefore, it might be nominated as a promising bioceramic material especially for different purposes such as scaffold, bone replacement, bone repair and coating.
