Prevention of Nephropathy by Some Natural Sources of Antioxidants

Nephrotoxicity is one of the most common kidney problems and occurs when the body is exposed to a drug or toxin. Natural sources of antioxidants may serve as a vital source of potentially useful new compounds for the development of an effective therapy to combat a variety of kidney problems. Natural antioxidants have a variety of biochemical actions such as inhibition of reactive oxygen species production, scavenging of free radicals. The present review aims to summarize the recent articles which studied some of the nephrotoxic agents, and alleviation of nephrotoxicity using of some natural products possessing antioxidant properties. Our review shows the oxidative damage and renal disorders induced in human and experimental animals by nephrotoxic agents such as gentamicin, alcohol, nicotine, adenine, glycerol, ethylene glycol, sodium nitrite, mercuric chloride, AlCl3, lead acetate, carbon tetrachloride (CCl4), furosemide, carbendazim, diazinon, heat stress, and γ-radiation. Also, nephrotic disorders caused in diabetic rats, patients, cirrhotic ascetic patients, and ischemia-reperfusion. Administration of natural sources of antioxidants such as curcumin, garlic, fenugreek, parsley, peppermint, pomegranate, propolis, olive leaves, rosemary, and sesame attenuated both physiological and histopathological alterations induced in the kidney by the nephrotoxic agent and certain diseases. The nephroprotective effect of the former natural sources of antioxidants may be due to the enhancement of antioxidant activity and inhibition of tissue lipid peroxidation. It can be concluded that administration of curcumin, garlic, fenugreek, parsley, peppermint, pomegranate, propolis, olive leaves, rosemary, and sesame showed a remarkable kidney protection against nephrotoxic agents, and diseases induced renal dysfunctions in human and experimental animals. Trabeculectomy with Ologen implant versus perfluoropropane gas bubble for open angle glaucoma in pseudophakic eyes

AIM:To evaluate the safety and efficacy of augmented trabeculotomy with Ologen versus perfluoropropane in management of pseudophakic ***:This is a comparative randomized study included 57 pseudophakic eyes of 57 patients with medically uncontrolled open angle glaucoma(OAG).Twentynine patients were allocated in group I(trabeculectomy with Ologen;trab-ologen group),while 28 patients were assigned in group II(trabeculectomy with perfluoropropane gas bubble;trab-C3 F8 gas bubble group).RESULTS:The intraocular pressure(IOP)was significantly reduced in both study groups at all postoperative follow up intervals(1 wk,3,6,12,18,24,30 and 36 mo,P<0.001).The differences between the mean IOP values of both groups remained statistically insignificance during the early 12 months of follow ***,the trab-ologen group achieved a statistically significant reduction over the trab-C3 F8 gas bubble group during the last 24 months of follow ***:Augmentation of trabeculectomy with either Ologen implant or perfluoropropane gas bubble are associated with strict long term IOP control and evident safety in medically-uncontrolled pseudophakic eyes with OAG.

Missed acute pulmonary embolism and sudden death: A case report

Rationale: Pulmonary embolism is a severe cardiovascular disease. Acute pulmonary embolism is an extremely common and potentially the serious pattern of venous thromboembolic disease. Unfortunately, missed diagnosis of pulmonary embolism is lethal and common because of its non-specific symptoms and signs. Patient concerns: A 42-year-old male patient presented with acute chest pain that was treated as gastroesophageal reflux disease. Diagnosis: Suspected acute pulmonary embolism. Interventions: O2inhalation, urgent electrocardiography, and cardiopulmonary resuscitation. Outcomes: Deterioration and sudden cardiac death. Lessons: Physicians should pay much attention to the symptoms and signs of pulmonary embolism to reduce the rate of missed diagnosis.

On the Quantum Statistical Distributions Describing Finite Fermions and Bosons Systems

A century old methodology for deriving statistical distribution using approximate Stirling’s formulation of the factorial becomes questionable. By avoiding the use of exaggerated approximations, a new picture of the energy distribution of fermions and bosons are presented. Energy distribution among fermions (or bosons) in systems with finite degeneracy are found to be degeneracy dependent. The presented point of view explains, successfully, presence of degeneracy pressure in ultra-cooled Fermi gas and predicts the minimum accessible temperature for finite degeneracy fermions system.

Management of Spontaneous Superficial Intracerebral Hematomas

Background: Treatment of primary spontaneous superficial intracerebral hemorrhage (SSICH) is still controversial. The aim of this study is to investigate the effectiveness of craniotomy and early hematoma evacuation vs conservative (non operative) management in patients with SSICH. Methods: A prospective study of craniotomy and early hematoma removal vs. medical management was performed in 20 patients with cortical and subcortical (superficial) primary SSICH admitted to Assiut university hospital in one year period (March 2016-March 2017). Surgical or medical treatment was initiated immediately when patients admitted to hospital. We divided the patients into two groups according to type of treatment. Results: Age of the patients ranged from 2 months to 73 years (mean: 45.84 ± 23.28 years) among the 20 patients studied;13 (65%) were males and 7 (35%) were females. Glasgow coma scale (GCS) at admission was less than 8 in 25%, between 8 - 12 in 45% and 15 in 30% of the patients. Arterial hypertension was the most common associated medical disease (65%). Eleven of patients (55%) had ventricular extension. GCS at admission was the strongest factor affecting outcomes and determined the type of treatment. All patients with a GCS ≤ 8 (5 cases (25%) were died regardless the type of treatment and there was no significant statistical difference in this group in relation to type of treatment p-value (0.606). We had 6 cases (30%) with a GCS 15;all of them were treated conservatively and all of them survived with significant p-value (0.003). Nine patients (45%) had GCS 8 - 12;two of them were treated conservatively and died while the remaining seven patients were treated surgically;five of them survived with significant p-value (0.025). No significant statistical difference was found in the outcome between the surgical and the conservative group regarding having associated intracranial hemorrhage. The chemical studies suggest the use of woody young stems as a newly investigated bioactive *** extraction of unsaturated fatty acids from the seeds of the plant would serve as a good health and nutritive product.

It has been well recognized that interactions between the gut microbiota and host-metabolism have a proven effect on *** gut lumen is known for harboring different bacterial *** by-products and structural components,which are derived through the gut microbiota,generate a signaling response to maintain *** microbiota is not only involved in metabolic disorders,but also participates in the regulation of reproductive hormonal *** phyla,which are localized in the gut,allow for the metabolization of steroid hormones through the stimulation of different *** hormones such as progesterone,estrogen and testosterone play a pivotal role in the successful completion of reproductive *** in this mechanism may lead to reproductive *** bacteria can affect the metabolism,and degrade steroid hor-mones and their relevant *** behavior of the bacteria can safely be implemented to eliminate steroidal compounds from a polluted *** this review,we summarize the meta-bolism of steroid hormones on the regulation of gut microbiota and vice-versa,and also examined the significant influence this process has on various events of reproductive ***,the evidence suggests that steroid hormones and gut microbiota exert a central role in the modification of host bacterial action and impact the reproductive efficiency of animals and humans.

Yolk Sac Tumor of the Ovary in 18 Egyptian Cases: Does It Really Differ?

Background: Ovarian Yolk sac tumor (OYST) is a rare entity of malignant ovarian germ cell tumors (MOGCT). abdominal pain, a rapidly growing distending mass or irregular vaginal bleeding is the main presentation. Serum AFP is elevated in nearly all cases. The standard management is fertility preserving surgery with adjuvant chemotherapy. Aim of Work: To report and analyze retrospectively recorded cases that were either treated at National Cancer Institute/Egypt or referred there for advice about therapy. Materials and Methods: This is a retrospective single-institutional analysis of 18 cases of OYST treated at National Cancer Institute-Cairo University from January 2011 till December 2015. The clinical and pathological characteristics, treatment, and outcomes of these patients were analyzed. Results: Data from eighteen patients were obtained. The median age was 18 years (range: 15 - 22). abdominal pain was the most common presentation (89%). The mean tumor size was 21cm (range: 8 - 30 cm). Eleven of our cases (61%) were stage I, seven cases and (39%) were stage IV at presentation. Fifteen cases (83%) underwent fertility preserving procedure & the standard surgical staging. Panhysterectomy & formal staging procedure was done only in two cases (11%). One case (6%) underwent bilateral salpingo-oophorectomy. 2 cases (11.1%) only underwent lymph node biopsy. 11 patient (61.1%) showed pure type YST while mixed type was present in the remaining 7 cases (38.8%): Dysgerminoma (one case, 5.6%), Dysgerminoma + immature teratoma (one case, 5.6%), Immature teratoma (2 cases, 11.1%) and Teratoma (3 cases, 16.7%). AFP was extremely elevated in all cases at presentation (median 4191 ng/mL;ranging: 725 ng/mL - 402,908 ng/mL). It showed decreased level after surgery (median 145 ng/ mL;ranging: 2 ng/mL - 38,000 ng/mL) & normalized after chemotherapy except for progressive disease.
