Effects of glycaemic management on diabetic kidney disease

Hyperglycaemia contributes to the onset and progression of diabetic kidney disease(DKD). Observational studies have not consistently demonstrated a glucose threshold, in terms of HbA1c levels, for the onset of DKD. Tight glucose control has clearly been shown to reduce the incidence of micro-or macroalbuminuria. However, evidence is now also emerging to suggest that intensive glucose control can slow glomerular filtration rate loss and possibly progression to end stage kidney disease. Achieving tight glucose control needs to be balanced against the increasing appreciation that glucose targets for the prevention of diabetes related complications need be individualised for each patient. Recently, empagliflozin which is an oral glucose lowering agent of the sodium glucose cotransporter-2 inhibitor class has been shown to have renal protective effects. However, the magnitude of empagliflozin's reno-protective properties are over and above that expected from its glucose lowering effects and most likely largely result from mechanisms involving alterations in intra-renal haemodynamics. Liraglutide and semaglutide, both injectable glucose lowering agents which are analogues of human glucagon like peptide-1 have also been shown to reduce progression to macroalbuminuria through mechanisms that remain to be fully elucidated. Here we review the evidence from observational and interventional studies that link good glucose control with improved renal outcomes. We also briefly review the potential reno-protective effects ofnewer glucose lowering agents.

Effect of gamification applications on success of dentistry students

Exploration of the shared pathophysiological mechanisms of gestational diabetes and large for gestational age offspring

Gestational diabetes mellitus (GDM) and large for gestational age (LGA) offspring are two common pregnancy complications. Connections also exist between the two conditions, including mutual maternal risk factors for the conditions and an increased prevalence of LGA offspring amongst pregnancies affected by GDM. Thus, it is important to elucidate potential shared underlying mechanisms of both LGA and GDM. One potential mechanistic link relates to macronutrient metabolism. indeed, derangement of carbohydrate and lipid metabolism is present in GDM, and maternal biomarkers of glucose and lipid control are associated with LGA neonates in such pregnancies. The aim of this paper is therefore to reflect on the existing nutritional guidelines for GDM in light of our understanding of the pathophysiological mechanisms of GDM and LGA offspring. Lifestyle modification is first line treatment for GDM, and while there is some promise that nutritional interventions may favourably impact outcomes, there is a lack of definitive evidence that changing the macronutrient composition of the diet reduces the incidence of either GDM or LGA offspring. The quality of the available evidence is a major issue, and rigorous trials are needed to inform evidence-based treatment guidelines.

Optimization of constitutive parameters of foundation soils k-means clustering analysis

The goal of this study was to optimize the constitutive parameters of foundation soils using a k-means algorithm with clustering analysis. A database was collected from unconfined compression tests, Proctor tests and grain distribution tests of soils taken from three different types of foundation pits： raft foundations, partial raft foundations and strip foundations. k-means algorithm with clustering analysis was applied to determine the most appropriate foundation type given the un- confined compression strengths and other parameters of the different soils.

Production of nanocrystalline silver particles by hydrogen reduction of silver nitrate aerosol droplets

Nanocrystalline silver particles were produced by hydrogen reduction of silver nitrate aerosol droplets formed by high frequency ultrasonic *** dependences of the particle size,morphology and crystallite size on the precursor concentration and the reaction temperature were *** spray pyrolysis process was combined with hydrogen reduction to research the effects on the silver particle *** silver particles including slight oxide structure were prepared at temperature as low as 200 ℃ from silver nitrate under hydrogen atmosphere.X-ray diffraction（XRD） studies showed that pure silver particles were obtained above 200 ℃ reaction *** crystallite sizes of the samples ranged from 29 to 47 *** results indicate that the crystallite sizes hardly ever depended on the reaction *** slightly enlarged by increasing precursor *** observations showed that particles were obtained in spherical morphology with particle sizes between 210 and 525 *** temperature and precursor concentration strongly influenced the particle size.

Goblet cell carcinoid of the appendix and mixed adenoneuroendocrine carcinoma: Report of three cases

Neuroendocrine neoplasms are the most common epithelial tumors among appendix tumors. Appendix tumors that are completely or partially composed of neuroendocrine cells are divided into two categories: Classic carcinoid tumors and goblet cell carcinoid tumors(GCCT). They are known to progress more aggressively than classic(neuro) endocrine tumors. in this study, three cases with acute appendicitis symptoms are presented, including their clinical and histopathological findings. Microscopic examination detected GCCT in two cases and mixed adenoneuroendocrine carcinoma in one case, in addition to acute appendicitis.

Prolonged QT dispersion in inflammatory bowel disease

AiM:To investigate the frequency and factors of prolonged QT dispersion that may lead to severe ventricular arrhythmias in patients with inflammatory bowel disease(iBD).METHODS:This study included 63 ulcerative colitis(UC) and 41 Crohn's disease(CD) ***-seven healthy patients were included as the control *** rate was calculated using electrocardiography,corrected QT dispersion(QTcd) and the Bazett's *** model assessment(HOMA) was used to determine insulin resistance(iR).HOMA values idered normal and values > 2.5 indicated a high probability of ***:Prolonged QTcd was found in 12.2% of UC patients,and in 14.5% of CD patients compared with the control group(P ignificant difference was found between the insulin values(CD:10.95 ± 6.10 vs 6.44 ± 3.28,P in UC and CD patients with and without prolonged *** behavior types were determined in CD patients with prolonged *** systolic arterial pressure(125 ± 13.81 vs 114.09 ± 8.73,P in UC patients were significantly associated with prolonged ***:Our data show that iBD patients have prolonged QTcd in relation to *** routine followup of iBD patients should include determination of HOMA,insulin values and electrocardiogram examination.

Radiotherapy for Gastrointestinal Stromal Tumors

Objective： Gastrointestinal stromal tumors （GiSTs） are rare mesenchymal tumors of the gastrointestinal tract, which fiequently cause intraabdominal metastases. The current standard of care is surgery for localized cases, and adjuvant imatinib is recommended for tumors with a high risk of recurrence. To date. radiotherapy has not been commonly accepted as a part of l-nultimodality treatment approach other than palliation. However, recently published case reports and some small series suggest that radiotherapy is a valuable option for controlling locally progressive, drug-resistant disease. The aim of this review is to provide a viewpoint from a radiation oncologist concerning the management of GiSTs, especially rectal GiST, and clarify the role and technical aspects of radiotherapy in the treatment approach. Data Sources： A comprehensive search in PubMed using the keywords ＂＇radiotherapy for rectal GiST＂ and ＂＇rectal GiST＂ was undertaken. The literature search included the related articles after 1995. Study Selection： The main articles including rectal GiST case reports and GiST series containing rectal cases were the primary references. Results： Surgery is tile mainstay of treatment, However, to date, radiotherapy is included in the multidisciplinary treatment strategy of rectal GiSTs in some circumstances with palliative, adjuvant, or definitive intent using different treatment doses and fields. Conchlsions： Recently reported iong-terrn local control rates indicate that GiST is a radiosensitive disease. This makes radiotherapy a valuable alternative in GiST management with curative intent, especially in patients who （ i ） cannot tolerate or are resistant to chemotherapy agents. （2） have an unresectable disease, （3） have a gross or microscopic residual disease after surgery, and （4） have a recurrent disease.
