Introduction: Breast cancer is the most common cancer in women worldwide, accounting for an estimated 22% of all female cancers. It is the leading cause of cancer mortality in women, almost all of which is due to metastases, with 73% of metastases occurring in the bone. In oncology, metastable technetium 99-labelled methylene bisphosphonate bone scintigraphy (BS) remains the standard examination for detecting and assessing the extent of bone metastases. The aim of this study was to assess the role of BS in the evaluation and follow-up of breast cancer in Senegal. Methodology: This was a retrospective study of breast cancer patients who underwent bone scintigraphy with 99mTc-HMDP in the nuclear medicine department of Idrissa Pouye General Hospital (IPGHO), from July 2009 to June 2022. Results: We enrolled 165 patients, mean age 46.79 years (27 - 87 years). BS was performed in 94.37% of cases for post-therapeutic monitoring and in 5.63% for pre-therapeutic assessment. Results were contributory in 131 patients (92.25%), of whom 72 cases (50.70%) were normal and 59 cases (41.55%) positive or presenting bone metastases;and non-contributory or doubtful in 11 cases (7.75%). Secondary bone locations were multiple in 57 cases (96.61%) and single or solitary in 2 cases (3.39%). The scintigraphic appearance of bone metastases was hyper-fixative in 58 cases (98.31%) and mixed in 1 case (1.69%). Bone lesions were quantified using the Soloway’s grading classification. Conclusion: BS with 99mTc-labelled bisphosphonates remains the examination of choice for skeletal exploration, in the detection and extension of bone metastases in breast cancer. Performance has been enhanced by the development of SPECT coupled with CT (SPECT-CT).

Results of the First 10 Cases of Coronary Bypass Surgery in Senegal

This is a review of the first 10 coronary artery bypass surgeries performed by the local team. The mean age was 62 years old [45 - 74]. The patients were predominantly male, with a M/F ratio of 4:1. Cardiovascular risk factors were mainly myocardial infarction (MI) (60%), hypertension (50%), obesity (40%) and diabetes (30%), with at least two risk factors per patient. Angina was the main symptom (80%). The average time from presentation to surgery was 8 months. The mean Euroscore 2 was 2.92 ± 1.65 [1.33 - 6.60]. Coronary angiography revealed an average of 2 lesions per patient, with 3-vessel involvement in 70% of cases: the Interventricular artery (IVA) (100%), the right coronary artery (90%) and the circumflex artery (70%). On echocardiography, the mean Left ventricular ejection fraction (LVEF) was 59% [33% - 76%]. All patients underwent median sternotomy with bypass grafting. The average duration of the cardiopulmonary bypass was 150 min [46 - 275 min];that of aortic clamping, 120 min [43 - 232 min]. The grafts used were internal thoracic artery (ITA) in 100% of cases (80% on the left and 20% on the right), and the great saphenous vein (GSV) in 60% of cases (50% on the left and 10 on the right). Double bypass was performed in 60% of cases, single bypass in 30% and triple bypass in 10%. The bypasses were performed on the IVA (100%), the middle lateral of the circumflex (30%) and the bisector (20%). The average time to extubation was 11 hours and the length of stay in the intensive care unit was 7 days [03 - 17 days]. One patient had a reoperation on Day 0 post-op. The average hospital stay was 13 days [06 - 27 days]. Complications occurred in nine of the patients (90%), with a predominance of infectious and neurological complications. Overall operative mortality was 3%, all in intensive care.

Giant cerebriform nevus cell nevus of the scalp is an extremely rare form of congenital melanocytic nevus. Giant cerebriform nevus of the scalp has a major psychosocial impact because of its unsightly appearance with fetid maceration. We report the case of a 35-year-old woman who had a painless, malodorous swelling of the cerebriform scalp measuring 20 × 17 cm in diameter with a wide base of insertion at the occipital level adhering to the deep planes. The excision associated with a skin plasty was carried out. Histology concluded that there was a giant cerebriform naevo-cellular nevus of the scalp.

Introduction: Numerous studies show the involvement of the cingulate gyrus in affective disorders, particularly in depression. With a preventive and curative aim, the authors raise questions leading to therapeutic applications such as focal brain stimulation. The cingulate gyrus is the primary target of these brain stimulation therapies for the treatment of depression. The objective of this work is to establish anatomoclinical correlations and to deduce the therapeutic implications. Methodology: Our work is a review of the literature. The inventory of the cingulate gyrus and depression was based on the development of a critical synthesis of bibliographic knowledge. Results: We found a bipartite Brodmann subdivision which evolved into a subdivision into four regions of the cingulate gyrus. Descriptions of the cingulate gyrus boundaries are imprecise and divergent. The anterior end of the anterior cingulate cortex is a confirmed target of stimulation in the treatment of major and resistant depression, thus requiring the authors, a consensus in its delineation. Brodmann’s area 25 has been described as the main target of brain stimulation therapies. Dysfunction by local lesion or by alteration of the connectivity of Brodmann’s area has repercussions on these different structures to which it is interconnected. These disturbances when they are in the direction of collapse paint a picture similar to major depression. Conclusion: The anterior cingulate cortex is involved in depression. The functional system organization of affectivity will allow new brain stimulation techniques to act on the entire functional system or on one of its components.

Perpetrators of Sexual Violence against Minors in Casamance (Senegal)

Introduction: The causes of sexual delinquency against children are multifactorial and the motivations that drive the perpetrators of sexual assault are diverse. Nowadays, there is still no consensus on a single portrait of child sex offenders and studies that focus on sex offenders are still rare in Senegal and Africa. Objectives: To determine the profile of the perpetrators and to identify the circumstances of sexual violence against minors in Casamance. Materials and Methods: A retrospective descriptive and analytical study from April 1, 2014 to October 31, 2019 was conducted. The study population was made up of perpetrators of sexual violence against minors whose legal proceedings were dealt with at the Ziguinchor High Court. The data were entered into an Excel spreadsheet and then analyzed using Epi InfoTM 7 software. Results: A total of 106 cases of sexual violence against minors were collected for 113 aggressors and 106 victims. Rape accounted for 66.03% of sexual violence. Male aggressors were 98.23%. Female victims accounted for 95.28%. The average age of the attackers was 32.52 years (±13.68). Offenders over the age of 35 committed the most rape (p = 0.043). The average age of the victims was 12.62 years (±3.42). Victims aged ten and over (p = 0.015) and who had a low level of education (p = 0.016) were more exposed to rape. The assault took place during the day (65.71%) and at the home of the aggressor (62.26%). Victims knew their attackers (84.47%). Victims were assaulted more than once by the same assailant (40.57%). Those aged 10 years and over (p = 0.0037) and who knew their attackers (p = 0.014) were more at risk of being repeatedly sexually abused. Conclusion: In Senegal, sexual violence against minors is perpetrated by young adult men, single, with little or no education and exercising low-income professions. Context: HIV infection is an infectious, viral, chronic disease described as a public health problem. It has an impact on bio-psycho-social well-being. Thus, the objective of our study is to decrypt the representations of HIV infection in our patients. Methodology: 31 consecutively recruited HIV-infected patients participated in our study. The completed questionnaire included socio-demographic characteristics, association membership, psycho-social aspects of HIV infection, mode of transmission and adherence to antiretroviral treatment. Secondly, we proceeded to an assessment of the psychological state of the patient. Results: Almost 77% of HIV patients do not date peers in an association. At the time of the study, more than half of patients or 19/31 with HIV had a positive image of the disease. Before the diagnosis of the disease, more than 2/3 of patients with HIV, i.e. 25/31, have a negative image of the disease. The 31 PLHIV have been aware of their positive HIV status for an average of 11 years with extremes of 2 years and 21 years. Almost 21/31 of PLHIV have shared their positive serological status. The main barrier in patient communication was the risk of disclosure of HIV status. Most of the patients (45.2%) thought of death when they announced their positive HIV status, and 19.4% thought of their children. In 80.6% of cases, relationships with family, friends and neighbors were normal and remained unchanged for 74% of patients. The patients thought they had been infected through sexual intercourse in 84% of cases. Our study showed that 6 of the 31 PLHIV experienced stigma. Among these 6 patients, 3 had psychological repercussions and a voluntary interruption of treatment. Conclusion: The representations are dynamic. In Senegal, the eradication of discrimination among people living with HIV must imperatively involve a reconstruction of individual, collective and social representation

This present study comes in addition to overcome the problems of separation of fine particles of TiO2 in heterogeneous photocatalysis after treatment. It aims to show the potential for using titaniferous sand as a new semiconductor under solar irradiation. The photocatalytic efficiency of this titaniferous sand was tested on a pesticide (Azadirachtin). A tubular photocatalytic reactor with recirculation of the polluting solution was designed for the elimination of the pesticide in an aqueous solution. Before its use as a photocatalyst, the titaniferous sand has undergone a specific treatment that consists of calcination at 600℃ followed by extraction of the calcined natural organic materials, which can interfere with the measurement of analytical parameters such as COD. The titaniferous sand was also characterized by X-ray fluorescence spectroscopy (XRF). XRF analyses have shown that TiO2 is predominant in the titaniferous sand with a percentage that has been estimated at 46.34%. The influence of various experimental parameters such as the flow rate of the polluting solution, the concentration of titaniferous sand, the presence of oxygen and the intensity of the overall rate of sunshine, was studied to optimize the photocatalytic degradation of the pesticide. The results showed that the highest removal rate (70%) was observed under the following conditions: a pH of 6, a titaniferous sand concentration of 150 g/L, a flow rate of 0.3 mL/min, and a sunshine rate of 354 W/m2 and in the presence of atmospheric oxygen. Under these experimental conditions, the rate of photodegradation of the pesticide follows the pseudo first order kinetic model of Langmuir Hinshelwood with a coefficient of determination R2 of 0.9869 and an apparent rate constant of 0.0029 min-1. Comparative Study of the Effect of Shading Rate on the electrical Parameters of CIGS and CdTe/CdS Solar Modules

In this paper, a comparative study of the maximum power on the shading rate on the maximum power of thin film PV modules. Thus two thin film PV modules of type Copper indium gallium selenide, CIGS, of 90W power and a CdTe (Cadmium telluride)/CdS (Cadmium sulfide) module, of maximum power 75 W. These modules, reference SN-CIGS90 and CX3 75 were tested under the conditions of the installation site to ensure their proper functioning and to determine the initial values of electrical parameters before shading. The results obtained are as follows: for the CIGS: Pm (80.717 W);Vco (23.06 V), Icc (3.5 A) and for the CdTe:Pm (54.914 W);Vco (35.52 V), Icc (1.546 A). After this characterization test, the modules are exposed to real operating conditions at the Center for Study and Research on the renewable energy (CERER), Cheikh Anta Diop University in Dakar. Four types of shading are performed on each module with the same mask: partial shading at 25%, 50%, 75% and complete shading at 100%. The comparison of the variation rates obtained on the experimental values of the 4 types of shading carried out on each module, shows that, the phenomenon of shading constitutes an environmental factor which influences negatively the maximum power of the thin film PV modules. But this reduction depends on the surface of the shaded module, the nature of the mask but also the technology used. Indeed, for a shading of 25% of the surface of the two modules, we note a reduction of 21.32% of power for the CIGS, against 40.53% for the CdTe/CdS, that is to say a difference which approaches 20%.

Contribution to the Study of Diabetic Kidney Disease in a Sub-Saharan Environment: An Example of the Aristide Le Dantec University Hospital in Dakar

Introduction: Diabetic kidney disease (DKD) is a leading cause of chronic kidney disease and dialysis admission. Few studies are available in Sub-Saharan Africa. The objective of this work was to study the epidemiological, clinical, diagnostic and therapeutic characteristics of DKD in our context. Patients and Methods: We conducted an observational, exhaustive and retrospective study focusing on diabetic patients seen in consultation or hospitalized in the Nephrology Department of at the Aristide Le Dantec University Hospital in Dakar during a period of 5 years from January 1, 2017 to December 31, 2021. Results: Of 4735 patients seen during the study period, 491 had DKD, i.e. a hospital prevalence of 10.36%. The average age was 59.1 ± 11.4 years with a sex ratio of 0.95. Type 2 diabetes predominated with 93.4%. The average duration of diabetes was 11.5 ± 7.6 years. Diabetes was associated with high blood pressure in 78.81% of cases, dyslipidemia in 23.2% of cases, active smoking in 6.7% of cases and obesity in 1.6% of cases. Renal failure was the main reason for referral 72.3%. One hundred and forty-eight patients (30.1%) had uncontrolled diabetes. Macroalbuminuria was found in 64.8% and microalbuminuria in 18.7% of cases. One hundred and eighty-five patients (37.7%) were in Stage V of kidney disease and 137 patients were in Stage III (18.1% in Stage IIIb and 9.8% in Stage IIIa). Diabetic nephropathy was the main etiology at 61.30%. Nephropathy was mixed (diabetic and hypertensive) in 18.12 cases. Renin-angiotensin-aldosterone system (RAAS) blockers were prescribed in 83.5% of patients. Conclusion: The different etiologies encountered during the study show the diversity of diabetic kidney disease. Diabetic nephropathy is not the only kidney damage that can occur in diabetics in our context.
