Correlation between DKK-1 Level and Bone Density Status in Children on Maintenance Haemodialysis

Background: Renal osteodystrophy (ROD) is a bone disorder resulting from chronic kidney disease (CKD) and related metabolic diseases. Dickkopf-related protein-1 (DKK-1) is critical in regulating bone biology. This study aimed to evaluate the serum DKK-1 level as a bone marker in children with CKD who undergo regular hemodialysis (HD). Subjects and Methods: This case-control study involved 40 children with CKD on HD and 40 healthy children as controls. The study measured serum DKK-1 levels and performed a dual-energy X-ray absorptiometry scan (DeXA) in line with routine laboratory investigations. Results: There was a significant increase in the serum level of DKK-1 in the patient group compared to the control group. The DKK-1 levels were 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, respectively, with a p-value of less than 0.001. In the hemodialysis group, 25 patients (62.5%) had low bone mineral density (BMD) with a Z-score of under -2.0. eighteen of these patients had low BMD in both the neck of the femur and lumbar spines. Additionally, there was a significant increase in serum DKK-1 level in patients with low BMD (2567.35 (2303.8 - 3108.1) pg/ml) compared to patients with normal BMD (2454 (1859 - 2820) pg/ml) (p = 0.041). There was also a significant positive correlation between DKK1 level and phosphorus, alkaline phosphatase, and Parathormone serum levels. In conclusion, the study indicates a clear correlation between DKK-1 and BMD in children undergoing maintenance hemodialysis. DKK1 is a promising biomarker for CKD-MBD.

Background: Acute pancreatitis (AP) is an inflammatory process affecting the pancreas. Hypertriglyceridemic AP (HTAP) is its third leading cause after gallstones followed by alcohol use. It develops when stars align viz. triggers (acquired surges) in patients with genetic hypertriglyceridemia. Case: severe and acute pancreatitis had developed in a 57-year-old man with type IIb familial hyperlipidemia after heavy fatty meals. The acute phase was controlled by holding oral intake with weekly evolocumab therapy and long-term prevention by weight reduction and Fenofibrate alone. His pancreatic endocrine and exocrine functions remained normal after 2 years of follow-up. Conclusion: Those four measures were safe, practical and effective for short- and long-term management of HTAP.

Background: The simultaneous presence of a pituitary adenoma and a meningioma is a rare and underreported condition. The randomly encountered cases need to be reported to further allow our understanding of this collision tumors that are still not understood nor fully described. Patients and Method: We report a case of coexisting nonfunctioning pituitary adenoma and a left temporal lobe meningioma revealed by a 1-year history of progressive vision loss and occasional headaches in a 56-year-old woman. Her clinical condition worsened in the last 5 months with ptosis, cavernous sinus syndrome, and ophthalmoplegia of the right eye without papilledema. There was an improvement in the visual symptoms after subtotal resection of both lesions through a right frontotemporal craniotomy. Histology confirmed a collision tumor. The patient was referred for adjuvant treatment with gamma knife radiosurgery. He was doing well and back to his usual duties 6 months later. Conclusion: A gross total or subtotal resection with adjuvant therapy is the gold standard for the surgical management of collision tumors for a favorable patient outcome.

Recurrent Transient Ischemic Attacks Revealing Cerebral Amyloid Angiopathy: A Comprehensive Case

This case report investigates the manifestation of cerebral amyloid angiopathy (CAA) through recurrent Transient Ischemic Attacks (TIAs) in an 82-year-old patient. Despite initial diagnostic complexities, cerebral angiography-MRI revealed features indicative of CAA. Symptomatic treatment resulted in improvement, but the patient later developed a fatal hematoma. The discussion navigates the intricate therapeutic landscape of repetitive TIAs in the elderly with cardiovascular risk factors, emphasizing the pivotal role of cerebral MRI and meticulous bleeding risk management. The conclusion stresses the importance of incorporating SWI sequences, specifically when suspecting a cardioembolic TIA, as a diagnostic measure to explore and exclude CAA in the differential diagnosis. This case report provides valuable insights into these challenges, highlighting the need to consider CAA in relevant cases.

While egypt’s canola production per unit area has recently grown,productivity remains low,necessitating increased *** are water-absorbent polymer compounds that can optimize irrigation schedules by increasing the soil’s ability to retain ***,twoﬁeld experiments were conducted to examine hydrogel application to sandy soil on canola growth,biochemical aspects,yield,yield traits,and nutritional quality of yielded seeds grown under water deﬁcit stress *** experiments were conducted by arranging a split-plot layout in a randomized complete block design(RCBD)with three times replications of each *** water stress at 75%or 50%of crop evapotranspiration(eTc)lowered chlorophyll a,chlorophyll b,caro-tenoids,and total pigments content,indole-3-acetic acid,plant development,seed yield,and oil and total carbo-hydrates of seed yield,hydrogel treatment enhanced all of the traits mentioned ***,hydrogel enhanced to gather compatible solutes(proline,amino acids,total soluble sugars),phenolics content in leaves,seed protein,and crop water productivity,which increased while the plants were under water *** results revealed that the full irrigation(100%eTc)along with hydrogel compared to water-stressed(50%eTc)led to enhanced seed yield(kg ha^(-1)),Oil(%),and Total carbohydrates(%)of rapeseed by 57.1%,11.1%and 15.7%,***,under water-stressed plots with hydrogel exhibited enhancement by 10.0%,3.2%and 5.1%in seed yield(kg ha^(-1)),oil(%),and total carbohydrates(%)of rapeseed by 57.1%,11.1%and 15.7%,respec-tively compared to *** a result,the use of hydrogel polymer will be a viable and practical solution for increasing agricultural output under water deﬁcit stress situations.

Modelling the hydrological balance in semi-arid zones is essential for effective water resource management,encompassing both surface water and *** study aims to model the monthly hydrological water cycle in the Wadi Mina upstream watershed(northwest Algeria)by applying the Soil and Water Assessment Tool(SWAT)hydrological *** modelling integrates spatial data such as the Digital elevation Model(DeM),land use,soil types and various meteorological parameters including precipitation,maximum and minimum temperatures,relative humidity,solar radiation and wind *** SWAT model was calibrated and validated using data from January 2012 to December 2014,with a calibra-tion period from January 2012 to August 2013 and a validation period from September 2013 to December *** and parameter calibration were conducted using the SWAT-SA program,and model performance evaluation relied on comparing the observed discharge at the outlet of the basin with model-simulated discharge,assessed through statistical coefficients including Nash-Sutcliffe efficiency(NSe),coefficient of determination(R2)and Percent Bias(PBAIS).Calibration results indicated favourable objec-tive function values(NSe=0.79,R2=0.93,PBAIS=-8.53%),although a slight decrease was observed during validation(NSe=0.69,R2=0.86,and PBAIS=-11.41%).The application of the SWAT model to the Wadi Mina upstream watershed highlighted its utility in simulating the spatial distribution of different components of the hydrological balance in this *** SWAT model revealed that approximately 71%of the precipitation in the basin evaporates,while only 29%contributes to surface runoff or infiltration into the soil.

Objective:To assess the nephroprotective potential of agmatine in a rat model of streptozotocin-induced diabetic ***:A single dose of streptozotocin(40 mg/kg)coupled with a fructose diet induced diabetes in Wistar ***(40 and 80 mg/kg)was administered to rats for 12 *** body weight and fasting blood glucose were measured *** level,urine output,total protein,albumin,blood urea nitrogen,creatinine,and cystatin-C were also determined at the end of the ***,superoxide dismutase,glutathione,interleukin-1β,interleukin-6,and tumor necrosis factor-alpha were evaluated in kidney *** study was also performed using hematoxylin and eosin ***:Agmatine at both doses significantly increased final body weight,and lowered fasting blood glucose,urine output,insulin,total protein,albumin,blood urea nitrogen,creatinine,and cystatin-C levels compared with the diabetic group(Pers and antioxidant effect were significantly improved in agmatine-treated ***,the histopathological changes in renal structure were.a.eliorated by agmatine ***:Agmatine alleviates diabetic nephropathy by improving renal functions and reducing inflammation and oxidative *** molecular mechanisms of its nephroprotective actions need to be investigated in future study.
