Protein-losing enteropathy caused by a jejunal ulcer after an internal hernia in Petersen's space: A case report

BACKGROUND The incidence of internal hernias has recently increased in concordance with the popularization of laparoscopic *** particular concern are internal hernias occurring in Petersen's space,a space that is surgically created after treatment for gastric cancer and *** hernias cause devastating sequelae,such as massive intestinal necrosis,fatal Roux limb necrosis,and superior mesenteric vein *** addition,protein-losing enteropathy(PLE)is a rare syndrome involving gastrointestinal protein loss,although its relationship with internal Petersen’s hernias remains *** SUMMARY A 75-year-old man with a history of laparotomy for early gastric cancer developed Petersen's hernia 1 year and 5 mo after *** was successfully treated by reducing the incarcerated small intestine and closure of Petersen’s defect without resection of the small *** 3 mo after his surgery for Petersen’s hernia,he developed bilateral leg edema and *** was diagnosed with PLE with an alpha-1 antitrypsin clearance of 733 mL/24 ***-balloon enteroscopy revealed extensive jejunal ulceration as the etiology,and it facilitated minimum bowel *** analysis showed extensive jejunal ulceration and collagen hyperplasia with nonspecific inflammation of all layers without lymphangiectasia,lymphoma,or vascular *** postoperative course was unremarkable,and his bilateral leg edema and hypoalbuminemia improved after 1 *** was no relapse over the 5-year follow-up *** PLE and extensive jejunal ulceration may occur after Petersen's *** enteroscopy helps identify and resect these lesions.

Dear Editor,Beige adipocytes,which increase energy expenditure by dissipating energy as heat,have gained attention as a therapeutic target for combating obesity.1 Adipocytes express many types of G-protein-coupled receptors(GPCRs),each of which has a unique preference for the Gs,Gi,Gq,and G12 *** the function of Gs-coupledβ-adrenergic receptors in beige adipocyte induction is well established,2 little is known about the function of G12-coupled GPCRs beyond its suppressive roles in white adipocyte maturation.3 In this study,we generated transgenic mice conditionally expressing a G12-coupled designer GPCR using a Cre-loxP system and investigated the potential effects of G12 signaling on adipocyte biology.

Synthesis of powdered silver nanoparticles using hydrogen in aqueous medium

Environmentally benign one-pot protocol is used in the synthesis of silver nanoparticles （AgNPs） in powder form, involving the reduction of silver nitrate in the presence of fully hydrolyzed polyvinyl alcohol （PVA） as stabilizer by hydrogen （H2） as reducing agent in aqueous medium. Fully hydrolyzed biodegradable PVA has extremely low cytotoxicity, making it a favorable stabilizer from green perspective. In the present methodology, essentially naked silver nanoparticles are obtained in good yield. The prepared sil- ver nanoparticles were characterized by TEM, XRD, EDAX, UV-Vis spectroscopy and DLS histogram, and studied for its activity as a recyclable catalyst for synthesis of different enaminones.

Perception gaps for recognition behavior between staff nurses and their managers

Nurse managers play a critical role in improving the work environment. Important leadership characteristics for nurse managers include visibility, accessibility, communication, recognition, and support. The nurse manager’s recognition behaviors strongly influence the job satisfaction of staff nurses. In our previous study, we investigated how staff nurses perceived the nurse manager’s recognition behaviors and revealed that there was a divergence in practical approaches to these behaviors between the nurse manager and the staff. We assume that one factor causing this divergence could be perception gaps between the nurse manager and the staff. The aim of this study, therefore, was to uncover what types of perception gaps exist between the nurse manager and staff nurses and whether the background of staff nurses, such as years of experience or academic background, could affect the staff nurses’ perceptions. This quantitative, cross-sectional study involved 10 hospitals in Japan. A total of 1425 nurses completed the questionnaire. The results showed that staff nurses considered “Respect job schedule preferences” to be the most important of the recognition behaviors. In contrast, nurse managers gave “Nurse manager meets with the staff nurses to discuss patient care and unit management” the highest score for importance. Four factors (marriage status, age, years of clinical experience, and training background) affected the professional awareness of recognition behaviors. Our results suggest that nurse managers need to consider these factors when they conduct recognition behaviors.

Nurse manager’s recognition behavior with staff nurses in Japan-based on semi-structured interviews

Objective: The purpose of this qualitative study was to obtain a better understanding of nurse manager’s recognition behavior. Methods: This study, consisting of semi-structured interviews, was conducted in five hospitals with 100 beds or more in the Kanto, Kansai, and Kyushu regions of Japan. Fifteen nurse managers, who each had more than one year of professional work experience as a nurse manager, participated in this study. Results: We extracted four categories and fourteen subcategories as the factors related to the recognition behaviors in nurse managers. The first category is the basis of the recognition behaviors, which were divided into the following four subcategories: recognition behaviors that they received, perception of recognition behaviors, construction of confidential relationships with staff nurses, and the organizational climate. The second category is the issues that make recognition behaviors difficult, which were classified into the following three subcategories: multiple duties, number of staff nurses, and characteristics of the recent staff nurses. The third category is the factors regarding the staff nurses that must be considered, which consist of the following two subcategories: the characteristics and motivation of staff nurses and recognition behaviors that the staff nurses expect. The forth category is the methods of the recognition behaviors, which consist of the following five categories: watching over and consideration of individuals, evaluation of routine work, development as a professional, opinion sharing and delegating work, and promotion of work-life balance. Conclusions: The recognition behavior by nurse managers is influenced by their own experience, and nurse managers practice recognition behaviors in response to the characteristics of their staff nurses in a busy environment. Our results suggest that nurse managers need expertise in management for them to identity appropriate recognition behavior.

Wound Closure on the Neonatal Rat Skin II. The Potential Ability of Epidermis to Close Small-Sized Wounds Independently of the Underlying Dermis

Reepithelialization of skin which comprises epidermis and dermis has not been fully elucidated due to the complexity of the participants as well as the interactions therein. In this study, the intrinsic roles and behaviors of epidermis itself during wound closure on neonatal rat skin were explored by developing and utilizing a novel in vivo wound model, termed “shallow incisional wound” in which the injury of dermis was minimized. The shallow wounds were closed by 12 h postwounding (PW) by the migration of the wound-marginal epidermal sheets in which activated myosin light chain (p-MLC) was predominantly detected at the lateral plasma membrane of individual cells. By local administration of Rho-associated protein kinase (ROCK) inhibitor Y27632, p-MLC disappeared at the wound margin and wounds were not closed by 12 h PW. Inhibition of Rac 1 by NSC23766 also resulted in hold of wound closure by 12 h PW, though NSC23766 somewhat slowly acted on p-MLC expression. These results suggest that, without joining of dermis, epidermal cells have a potential ability of closing wounds by active epithelial sheet movement integrated by Rho family small GTPases-dependent extension and contraction of the individual cell bodies.

Balloon-occluded transcatheter arterial chemoembolization for hepatocellular carcinoma

Transcatheter arterial chemoembolization(TACE) is widely accepted as a treatment for patients with hepatocellular carcinoma(HCC) in the intermediate stage according to the Barcelona Clinic Liver Cancer(BCLC) guidelines. Recently, balloon-occluded TACE(B-TACE) was developed in Japan. Despite the lack of a clear definition, B-TACE is generally defined as the infusion of emulsion of chemotherapeutic agents with lipiodol followed by gelatin particles under the occlusion of feeding arteries by a microballoon catheter, which leads to the dense lipiodol emulsion(LE) accumulation in HCC nodules. This phenomenon cannot be explained only by the prevention of proximal migration and leakage of embolization materials; it further involves causing local changes in the hemodynamics of the surrounding occlusion artery and targeted HCC nodules. Balloon-occluded arterial stump pressure plays an important role in the dense LE accumulation in targeted HCC nodules. Although randomized controlled trials comparing the therapeutic effect and the prognosis of B-TACE to those of the other TACE procedures, such as conventional-TACE and drug-eluting beads TACE, are still lacking, B-TACE is thought to be a promising treatment. The purpose of this review is to summarize the mechanism, therapeutic effect, indication, prognosis and complications of BTACE.

Wound Closure on the Neonatal Rat Skin I. The Modulation of the Thickness of Epidermis at the Closing Incisional Wounds

Full-thickness incisional wounds were made on the dorsal skin of 1-day-old rats to elucidate the mechanism of the fluctuation of the epidermal thickness after the wound closure. The thickness of the epidermis covering the wound reached a peak around 96 h post-wounding (PW), and became thinner thereafter. The analyses of the cell proliferation and apoptosis at the epidermal wound regions revealed that the rate of TUNEL-positive cells that displays the cells undergoing apoptosis increased as the epidermis became thinner around 120 h PW. Next, immunohistochemical analyses using antibodies against keratinocyte differentiation marker proteins indicated that the delay or interruption of the spinous to granular transition from 96 to 120 h PW might result in the epidermal thickening in the wound region. Third, the region undyed with anti-caspase-14 antibody extended downward in the thickened epidermis by 96 h PW, and in turn, it became intensely and widely stained with this antibody in the thinning epidermis by 120 h PW. Taken together, it is likely that the delay and acceleration of the terminal differentiation, including cornification of the epidermal keratinocytes may coordinately cause the fluctuation of the thickness of the epidermis at the wound site in rat neonates.

Background Vascular remodeling is an important pathologic process in vascular injury for various vascular disorders such as atherosclerosis, postangioplasty restenosis and transplant arteriopathy. Recently, pathologic change and the role of bone marrow derived cells were wildly studied in atherosclerosis and restenosis. But the manner of lesion formation in neointima and cell recruitment in vascular remodeling lesion in the present of hypercholesterolemia is not yet fully understood. Methods Double-transgenic mice knockout of LDL receptor gene （LDL-/-） and expressing ubiquitously green fluorescent protein （GFP） were obtained by cross-breeding LDL -/- mice with the GFP-expressing transgenic mice. LDL-/- mice （22-24 weeks of age） fed high fat diet containing 1.25% （w/w） cholesterol were subjected to 9Gy irradiation and received bone marrow （BM） cells from the double-transgenic mice. Four weeks later, a nonconstrictive cuff was placed around the right femoral artery. After another 2 weeks, both right and left femoral arteries were harvested and subjected to histochemical analysis. Apoptosis was analyzed in situ using TUNEL assay. Results Two weeks after cuff placement, atherosclerotic lesions developed in the intima consisting of a massive accumulation of foam cells. The tissue stained with anti-α smooth muscle actin （SMA） antibody, showed a number of SMA-positive cells in the intimal lesion area. They were also positive for GFP, indicating that BM-derived cells can differentiate to SMCs in the intima in cuff-induced vascular remodeling lesions. Numerous small vessels in the adventitia as well as the endothelial lining of the intima were positive both for CD31 and GFP. The intima and media showed a large number of TUNEL-positive signals after 2 weeks cuff injury, indicating the presence of apoptosis in vascular remodeling. Conclusions Atherosclerotic lesions in mice can be developed in the intima after 2 weeks of cuff-induced vascular injury under the hypercholes

Aspect of QOL Assessment and Proposed New Scale for Evaluation

According to the reports from Ministry of Health in each country, the average life span is elongated over the past decade. This trend is expected to be keeping constant further in the future and at the same time continued efforts should be made. Next, it is necessary for us to consent about improving the quality of life. In an effort of improving the quality of life, other than the western medicine, we have attempted to introduce many traditional medical practices, including the oriental medicine, from various parts of the world into the medical field as a complementary & alternative medicine (CAM). Judging both positive and negative aspects by the evaluation standard of the Western medicine, we try to obtain numerical aspects in termed of QOL. Some methods of alternative medicine involved in this review are acupuncture, moxibustion, TCM and other traditional medicine. Above all, WHO suggests selecting the suzerain nation of acupuncture moxibustion, which has been developed in the oriental countries as a specialized cure, by identifying merits & demerits and ascertaining which nation can be a the best model for special field. At the moment for evaluating CAM, for example, what kind of methods is/are suitable for evaluating each CAM. We have been trying to propose the peripheral leukocyte, that is one of the best marker for evaluating CAM. Our trial is a hot spring hydrotherapy, acupuncture and moxibution, light exercise and Ondle/floor heating etc. In these trials, we find the common results from leukocyte effects which exhibit a strong correlation for the regulation in number and function, as a new scale for evaluating QOL. The contents are the result from the data showing the much in number of volunteer tend to down regulate, on the other hand, lower number of volunteer is up-regulated at next day by each menu of CAM as a constitution dependent manner. However, even in western medical methodology, double blind and cross over system are the better way to evaluation
