BACKGROUND During pancreaticoduodenectomy in patients with celiac axis(CA)stenosis due to compression by the median arcuate ligament(MAL),the MAL has to be divided to maintain hepatic blood flow in many ***,MAL division often fails,and success can only be determined *** overcome this problem,we performed endovascular CA stenting preoperatively,and thereafter safely performed *** present this case as a new preoperative treatment strategy that was *** SUMMARY A 77-year-old man with a diagnosis of pancreatic head cancer presented to our department for *** assessment revealed CA stenosis caused by *** performed endovascular stenting in the CA preoperatively because we knew that going into the operation without a strategy could lead to ischemic ***-antiplatelet therapy(DAPT)–which is needed when a stent is inserted–was then administered in parallel with neoadjuvant chemotherapy(NAC).This allowed us to administer DAPT for a sufficient period before the main pancreaticoduodenectomy procedure while obtaining therapeutic effects from *** stomach-preserving pancreaticoduodenectomy was *** operation did not require any unusual techniques and was performed ***,the patient progressed well,without any ischemic ***,curative resection was confirmed,and the patient had no recurrence or complications due to ischemia up to six months *** Preoperative endovascular stenting,with NAC and DAPT,is effective and safe prior to pancreaticoduodenectomy in potentially resectable pancreatic cancer.

Conjecture with water and rheological control for subducting slab in the mantle transition zone

Seismic observations have shown structural variation near the base of the mantle transition zone （MTZ） where subducted cold slabs, as visualized with high seismic speed anomalies （HSSAs）, flatten to form stagnant slabs or sink further into the lower mantle. The different slab behaviors were also accompanied by variation of the ＂660 kin＂ discontinuity depths and low viscosity layers （LVLs） beneath the MTZ that are suggested by geoid inversion studies. We address that deep water transport by subducted slabs and dehydration from hydrous slabs could affect the physical properties of mantle minerals and govern slab dynamics. A systematic series of three-dimensional numerical simulation has been conducted to examine the effects of viscosity reduction or contrast between slab materials on slab behaviors near the base of the MTZ. We found that the viscosity reduction of subducted crustal material leads to a sepa- ration of crustal material from the slab main body and its transient stagnation in the MTZ. The once trapped crustal materials in the MTZ eventually sink into the lower mantle within 20 30 My from the start of the plate subduction. The results suggest crustal material recycle in the whole mantle that is consistent with evidence from mantle geochemistry as opposed to a two-layer mantle convection model. Because of the smaller capacity of water content in lower mantle minerals than in MTZ minerals, dehydration should occur at the phase transformation depth, ~660 kin. The variation of the disconti- nuity depths and highly localized low seismic speed anomaly （LSSA） zones observed from seismic P waveforms in a relatively high frequency band （~ 1 Hz） support the hypothesis of dehydration from hydrous slabs at the phase boundary. The LSSAs which correspond to dehydration induced fluids are likely to be very local, given very small hydrogen （H＋） diffusivity associated with subducted slabs. The image of such local LSSA zones embedded in HSSAs may not be necessarily captured

Checkpoint with forkhead-associated and ring finger promoter hypermethylation correlates with microsatellite instability in gastric cancer

Surgical resection of a solitary para-aortic lymph node metastasis from hepatocellular carcinoma

Lymph node (LN) metastases from hepatocellular carcinoma (HCC) are considered uncommon. We describe the surgical resection of a solitary para-aortic LN metastasis from HCC. A 65-year-old Japanese man with B-type liver cirrhosis was admitted for the evaluation of a liver tumor. He had already undergone radiofrequency ablation, transcatheter arterial chemoembolization, and percutaneous ethanol injection therapy for HCC. Despite treatment, viable regions remained in segments 4 and 8. We performed a right paramedian sectionectomy with partial resection of the left paramedian section of the liver. Six months later, serum concentrations of alpha-fetoprotein (189 ng/mL) and PIVKA-2 (507 mAU/mL) increased. Enhanced com- puted tomography of the abdomen revealed a tumor (20 mm in diameter) on the right side of the abdominal aorta. Fluorine-18 fluorodeoxyglucose positron emission tomography revealed an increased standard uptake value. There was no evidence of recurrence in other regions. Esophagogastroduodenoscopy and colonoscopy revealed no malignant tumor in the gastrointestinal tract. Para-aortic LN metastasis from HCC was thus diagnosed. We performed lymphadenectomy. Histopathological examination revealed that the tumor was largely necrotic, with poorly differentiated HCC on its surface, which confirmed the suspected diagnosis. After 6 mo tumor marker levels were normal, with no evidence of recurrence. Our experience suggests that a solitary para-aortic LN metastasis from HCC can be treated surgically.

One-step palliative treatment method for obstructive jaundice caused by unresectable malignancies by percutaneous transhepatic insertion of an expandable metallic stent

AIM： To describe a simple one-step method involving percutaneous transhepatic insertion of an expandable metal stent （EMS） used in the treatment of obstructive jaundice caused by unresectable malignancies. METHODS： Fourteen patients diagnosed with obstructive jaundice due to unresectable malignancies were included in the study. The malignancies in these patients were a result of very advanced carcinoma or old age. Percutaneous transhepatic cholangiography was performed under ultrasonographic guidance. After a catheter with an inner metallic guide was advanced into the duodenum, an EMS was placed in the common bile duct, between a point 1 cm beyond the papilla of Vater and the entrance to the hepatic hilum. In cases where it was difficult to span the distance using just a single EMS, an additional stent was positioned. A drainage catheter was left in place to act as a hemostat. The catheter was removed after resolution of cholestasis and stent patency was confirmed 2 or 3 d post-procedure. RESULTS： One-step insertion of the EMS was achieved in all patients with a procedure mean time of 24.4 min. Out of the patients who required 2 EMS, 4 needed a procedure time exceeding 30 min. The mean time for removal of the catheter post-procedure was 2.3 d. All patients died of malignancy with a mean follow-up time of 7.8 mo. No stent-related complication or stent obstruction was encountered. CONCLUSIONS： One-step percutaneous transhepaticinsertion of EMS is a simple procedure for resolving biliary obstruction and can effectively improve the patient＇s quality of life.

We report the discovery of a substellar companion around the intermediate- mass giant HD 175679. Precise radial velocity data of the star from the Xinglong Station and the Okayama Astrophysical Observatory revealed a Keplerian velocity variation with an orbital period of 1366.8±5.7 d, a semiamplitude of 380.2±3.2 m s-1 and an eccentricity of 0.378± 0.008. Adopting a stellar mass of 2.7± 0.3 M, we obtain that the minimum mass of the HD 175679 b is 37.3± 2.8 M and the semimajor axis is 3.36±0.12 AU. This discovery is the second brown dwarf companion candidate from a joint planet-search program between China and Japan.

Protein phosphatase 4 is involved in the late development of <i>Dictyostelium discoideum</i>

A cDNA clone SSJ337 (accession no. AF161253) of 1230 bp, encoding a catalytic subunit of protein phosphatase 4, was selected as one of the clones expressed specifically in prestalk cells from a cDNA library of D. discoideum slugs. Cells transformed with a knockout construct of SSJ337 showed an aberrant and tiny fruiting-body formation with a short stalk. A knockout mutant, SSJ337KO was allowed to develop much slower than a wild-type AX2 after the post-aggregation stage. This suggested that the SSJ337 cDNA clone has played an important role especially in the later development of Dictyostelium discoideum. Results from Northern blotting, analysis showed that transcripts for SSJ337 were accumulated at 16 h to 24 h after starvation began.

Improving grain quality is a primary objective in contemporary rice *** modern rice breeding has developed two different types of rice,eating and sake-brewing rice,with different grain characteristics,indicating the selection of variant gene alleles during the breeding *** the critical importance of promptly and efficiently identifying genes selected in past breeding for future molecular breeding,we conducted genome scans for divergence,genome-wide association studies,and map-based ***,we successfully identified two genes,OsMnS and OsWOX9D,both contributing to rice grain *** encodes a mannan synthase that increases the white core frequency in the endosperm,a desirable trait for sake brewing but decreases the grain appearance ***9D encodes a grass-specific homeobox-containing transcription factor,which enhances grain width for better sake ***,haplotype analysis revealed that their defective alleles were selected in East Asia,but not Europe,during modern *** addition,our analyses indicate that a reduction in grain mannan content during African rice domestication may also be caused a defective OsMnS allele due to breeding *** study not only reveals the delicate balance between grain appearance quality and nutrition in rice but also provides a new strategy for isolating causal genes underlying complex traits,based on the concept of“breeding-assisted genomics”in plants.

Preoperative liver functional volumetry performed by 3D-99mTc-GSA scintigraphy/vascular fusion imaging using SYNAPSE VINCENT: a preliminary study

Aim:The present study was designed to evaluate the feasibility of preoperative liver functional volumetry performed by 3D-technetium-99m-diethylenetriaminepentaacetic acid-galactosyl-human serum albumin(99mTc-GSA)scintigraphy/vascular fusion imaging using SYNAPSE VINCENT and to examine the discrepancy between conventional and functional ***:The study group comprised 15 patients who underwent preoperative 3-dimensional(3D)-99mTc-GSA scintigraphy/vascular fusion imaging using SYNAPSE VINCENT software before hepatectomy between July 2014 and August *** diagnosis was hepatocellular carcinoma(n=4),metastatic liver tumor(n=10),or intrahepatic cholangiocarcinoma(n=1).Right hepatectomy was performed in 2 patients,left hepatectomy in 3 patients,right posterior sectionectomy in 3 patients,segmentectomy in 2 patients,and partial hepatectomy in 4 patients.99mTc-GSA scintigraphy and computed tomography(CT)were performed to construct 3D-99mTc-GSA scintigraphy/vascular fused *** conventional volume ratio of the planned resection region without tumor(%CT),and the functional volume ratio of the planned resection region without tumor(%GSA)were *** discrepancy ratio was calculated as follows:discrepancy ratio=100-%GSA/%CT×100(%).Results:The%GSA(17.9±16.7%)was significantly lower than the%CT(21.5±17.6%)(P<0.036).In all except 2 patients,the%GSA was lower than the%*** discrepancy ratio ranged from-4%to 75%(median,20.7%).Conclusion:3D-99mTc-GSA scintigraphy/vascular fused images constructed using SYNAPSE VINCENT were useful for noninvasively performing functional liver volumetry in patients scheduled to undergo various patterns of *** planned resection regions without tumor,the functional volume ratio was about 20%lower than the conventional volume ratio.

Effects of Magnetic Field on Photodegradation of Methylene Blue over ZnO and TiO2 Powders using UV-LED as a Light Source

The magnetic field effects （MFEs） are studied on photocatalytic degradation of methylene blue （MB） solution using ZnO and TiO2 particles. The UV-VIS-NIR spectrometer is used to monitor the MB concentrations, and the dependence of the reaction rate on the initial dye concentration and settling duration is studied under UV light irradiation. It is found that the MFEs exist on the heterogeneous reaction systems for both ZnO and TiO2 powders and that the extraordinary phenomenon is reproducible. For ZnO powder, the results are in good agreement with the second-order reaction kinetics following the Langmuir-Hinshelwood （L-H） model, while the reaction for TiO2 follows first-order kinetics. It enhances the photodegradation for ZnO, while it reduces or enhances the reaction for TiO2 depending on the initial dye concentrations. The MFEs become small or negligible when the same photodecomposition experiment is carried out after settling the MB solution for more than three hours for both ZnO and TiO2. It is suggested that the key factors of MFEs on photocatalytic degradation is the condition of the MB solution as well as the characteristics of photocatalysts. The alteration of the MFEs ascribed to the solution condition caused by variation of the settling time.
