Validation of user-friendly models predicting extracapsular extension in prostate cancer patients

Objective:There are many models to predict extracapsular extension(ECE)in patients with prostate *** aimed to externally validate several models in a Japanese ***:We included patients treated with robotic-assisted radical prostatectomy for prostate *** risk of ECE was calculated for each patient in several models(prostate side-specific and non-side-specific).Model performance was assessed by calculating the receiver operating curve and the area under the curve(AUC),calibration plots,and decision curve ***:We identified ECE in 117(32.9%)of the 356 prostate lobes *** with ECE had a statistically significant higher prostate-specific antigen level,percentage of positive digital rectal examination,percentage of hypoechoic nodes,percentage of magnetic resonance imaging nodes or ECE suggestion,percentage of biopsy positive cores,International Society of Urological Pathology grade group,and percentage of core *** the sidespecific models,the Soeterik,Patel,Sayyid,Martini,and Steuber models presented AUC of 0.81,0.78,0.77,0.75,and 0.73,*** the non-side-specific models,the memorial Sloan Kettering Cancer Center web calculator,the Roach formula,the Partin tables of 2016,2013,and 2007 presented AUC of 0.74,0.72,0.64,0.61,and 0.60,***,the 95%confidence interval for most of these models *** side-specific models presented adequate *** the decision curve analyses,most models showed net benefit,but it overlapped among ***:Models predicting ECE were externally validated in Japanese *** side-specific models predicted better than the non-side-specific *** Soeterik and Patel models were the most accurate performing models.

Enzyme-linked immunosorbent assay of 3 Screen Islet Cell Autoantibody in patients with autoimmune thyroid disease

BACKGROUND In recent years,the emergence of multiplex technology that can simultaneously measure multiple anti-islet autoantibodies has become particularly valuable for the staging and early diagnosis of immune-mediated type 1 diabetes(T1D).While it has been established that 20%-30%of T1D patients suffer from autoimmune thyroid disease(AITD),there is limited available data regarding the presence of anti-islet autoantibodies in AITD *** commercially available anti-islet autoantibodies,glutamic acid decarboxylase 65 autoantibodies(GADAs)are often the first marker measured in general clinical *** To investigate the frequency of anti-islet autoantibodies in AITD *** Our study involved four hundred ninety-five AITD patients,categorized into three distinct groups:AITD with T1D(n=18),AITD with phenotypic type 2 diabetes(T2D)(n=81),and AITD without diabetes(n=396),and the enzyme-linked immunosorbent assay(ELISA)was employed to determine the frequencies of 3 Screen Islet Cell Autoantibody(3 Screen ICA),GADA,insulinoma-associated antigen-2 autoantibodies(IA-2As),and zinc transporter 8 autoantibodies(ZnT8As)within these *** The frequency of 3 Screen ICA in AITD patients with T1D,T2D,and those without diabetes were 88.9%,6.2%,and 5.1%,respectively,with no significant difference seen between the latter two ***,the frequency of 3 Screen ICA was 11.1%higher in AITD patients with T1D,1.3%higher in AITD patients with T2D,and 1.1%higher in AITD patients without diabetes compared to GADA,***,12.5%,20.0%,and 20.0%of the 3 Screen ICA-positive patients were negative for ***,1.3%of the AITD patients who tested negative for 3 Screen ICA in both the AITD with T2D and non-diabetic AITD groups were found to be positive for individual *** the 3 Screen ICA-positive patients,there was a significantly higher proportion of individuals with multiple autoantibodies in AITD patients with T1D com

Positive Effect of Transcranial Direct Current Stimulation on Visual Verbal Working Memory in Patients with Attention-Deficit/Hyperactivity Disorder

Background: Working memory is an executive function that plays an important role in many aspects of daily life, and its impairment in patients with attention-deficit/hyperactivity disorder (ADHD) affects quality of life. The dorsolateral prefrontal cortex (DLPFC) has been a good target site for transcranial direct current stimulation (tDCS) due to its intense involvement in working memory. In our 2018 study, tDCS improved visual-verbal working memory in healthy subjects. Objective: This study examines the effects of tDCS on ADHD patients, particularly on verbal working memory. Methods: We conducted an experiment involving verbal working memory of two modalities, visual and auditory, and a sustained attention task that could affect working memory in 9 ADHD patients. Active or sham tDCS was applied to the left DLPFC in a single-blind crossover design. Results: tDCS significantly improved the accuracy of visual-verbal working memory. In contrast, tDCS did not affect auditory-verbal working memory and sustained attention. Conclusion: tDCS to the left DLPFC improved visual-verbal working memory in ADHD patients, with important implications for potential ADHD treatments.

Negative refraction of deep water waves through water channel network

A design method of a negative refractive index for free surface water waves in deep water is presented. To obtain a condition of a negative index, an analogy between water waves in a water channel and electric waves in an electric circuit is employed. An analytical solution of waves around a periodic array of rectangular columns is obtained by a circuit analysis. Attention is not focused on columns but a space among columns, and then the space is regarded as the water channel. Therefore, the circuit analysis can be applied to water waves. One channel is treated as a unit structure and a number of channels are periodically cascaded as a network. The negative refraction and backward waves by a network of the water channels are shown. Since the negative refraction might occur even around simple periodic structures, it is meaningful to understand wave phenomenon around offshore structures and develop innovative technology.

The role of water in Earth's mantle

Geophysical observations suggest that the transition zone is wet locally. Continental and oceanic sediment components together with the basaltic and peridotitic components might be transported and accumulated in the transition zone. Low-velocity anomalies at the upper mantle–transition zone boundary might be caused by the existence of dense hydrous magmas. Water can be carried farther into the lower mantle by the slabs. The anomalous Q and shear wave regions locating at the uppermost part of the lower mantle could be caused by the existence of fluid or wet magmas in this region because of the water-solubility contrast between the minerals in the transition zone and those in the lower mantle. δ-H solid solution AlO2H–MgSiO4H2carries water into the lower mantle. Hydrogen-bond symmetrization exists in high-pressure hydrous phases and thus they are stable at the high pressures of the lower mantle. Thus, theδ-H solid solution in subducting slabs carries water farther into the bottom of the lower mantle. Pyrite FeO2Hxis formed due to a reaction between the core and hydrated slabs. This phase could be a candidate for the anomalous regions at the core–mantle boundary.

The added resistance of a ship in waves can be related to ship-generated unsteady *** the present study,the unsteady wave-pattern analysis is applied to calculate the added resistance in waves for two modified Wigley models using a Cartesian-grid *** the present numerical method,a first-order fractional-step method is applied to the velocity-pressure coupling in the fluid domain,and one of volume-of-fluid(VOF)methods is adopted to capture the fluid interface.A ship is embedded in a Cartesian grid,and the volume fraction of the ship inside the grid is calculated by identifying whether each grid is occupied by liquid,gas,and solid *** sensitivity to the location of measuring position of unsteady waves as well as the number of solution grids is *** added resistance computed by direct pressure integration and wave pattern analysis is compared with experimental *** addition,nonlinear characteristics of the added resistance in waves are investigated by detailed analyses of unsteady flow field and resulting wave pattern.

How does the 120-year cycle mast seeding of dwarf bamboo affect the rodent population?

Background:In 2017,large-scale flowering,seeding,and dying events of dwarf bamboo(Sasa borealis)occurred in a wide range in central Japan for the first time in 120 *** phenomenon of ***,like the mast seeding of trees,could be expected to provide a large amount of food for seed-eating rodents and gradually affect their ecology and population *** captured rodents in survey plots in a secondary broad-leaved forest with the phenomenon from 2018 to 2019 to investigate the species,the number of individuals,growing stage,sex,and body *** addition,we also compared the capture data for 2 years(2018–2019)with that for the mast seeding year(2017)and 6 years before it(2011–2016).Results:The mast seeding of *** greatly increased the population size of rodents,especially Apodemus speciosus and ***,Eothenomys smithii did not show such an *** of the captured rodent individuals were already adults at the time of new capture,and the proportion of male juveniles was extremely *** results suggest that the mast seeding of *** created unusually rich food availability for the population concentration of rodents due to their ***,body mass was not significantly different before and after the mast *** addition,the increased populations of the two Apodemus species did not decline 2 years after the *** masting,contrary to the previously reported decrease of rodent populations after tree ***:Our results indicate that the mast seeding of *** affected the population dynamics of Apodemus species over a long time and their individual composition of different growth ***,it did not affect their body mass.

Implications of excess ^(210)Pb and ^(137)Cs in sediment cores from Mikawa Bay, Japan

Four sediment cores were collected from Mikawa Bay, Japan, and excess ^210pb and ^137Cs were measured by gamma spectrometry. Sedimentation rates for the four cores were determined by ^210Pb method. The sedimentation rate range is 0.10-0.70 g/（cm^***）. The bio-mixing depth for each core is less than 7.0 cm, and was determined by the excess 210^pb profiles as well. Therefore, the bioturbation is slight. The 210^pb-derived dates coincided with the results from ^137Cs geochronology. Acceleration in sedimentation rate due to environmental alteration has been found in cores A2.5 and 05AS8, representing two depocenters due to their topography. Evidence of the Tokal Flood in 2000 was found in core 05AS8 according to the profiles of both radioisotopes and trace metals.

Postoperative functional evaluation of gastrectomy for gastric cancer

To improve the quality of surgical procedures for gastric cancer,it is essential to consider many components comprehensively,including gastric motility,small intestinal absorption,hormones that affect gastric motility and appetite,presence or absence of vagus nerve preservation,esophageal regurgitation on endoscopic findings,in addition to whether or not there is a physiological route for food passage through the ***,proper functional evaluation cannot be performed without considering the form and amount of energy in the nutritional supplement to be loaded,and the posture at the time of *** results of functional evaluation vary according to the method selected from many available options,but we believe that use of the most physiologically accurate,appropriate and selectable option will enable us to arrive at the best resection/reconstruction *** have reported that it is important to consider the preservation of three elements when performing gastrectomy:1)reduction of the extent of gastrectomy,2)preservation of the pylorus,and 3)preservation of the vagus nerve;among which preservation of the remnant stomach is the most ***,the selection of a reconstruction method that maintains secretion of hormones beneficial to gastric motility preserves the energy balance inherent in the human body,and also provides better quality of life.
