Objective： The aim of this work was to quantify the extent of set-up errors to conduct a quality assurance （QA） aspect of treatment delivery, verification of the treatment field＇s position on different days using electronic portal. methods： This study was carried out on 12 patients, treated for pelvis tumor; and total of 240 images obtained by electronic portal image device （EPID） were analyzed. The EPIs acquire using EPID attached to the Siemens linear accelerator. The anatomy match- ing software （Theraview） was used and displacement in two dimensions were noted for each treatment field to study patient setup errors. Results： The percentages of mean deviations less than 5 mm in X direction were 65% ＆ 92%, from 5-10 mm were 31% ＆ 19% and more than 10 mm were 11% ＆ 9% forNP and lateral direction respectively. The percentages of mean deviations less than 5 mm in Y direction were 65% ＆ 63%, from 5-10 mm were 33% ＆ 28% and more than 10 mm were 22% ＆ 29%. The mean deviations in 2D-vector errors were 〈 5 mm in 47% and 46%, 5-10 mm in 36% and 37% and 〉 10 mm in 37% and 37% of images in the NP and lateral direction respectively. Conclusion： The results revealed that the ranges of set up errors are immobilization method to improve reproducibility. The observed variations were not within the limits..

Background: Individualization of high dose regimen of methotrexate (mTX) in pediatric patients with acute lymphoblastic leukemia based on pharmacokinetic (PK) parameters can help in optimization of the dose and better control of the disease. Building up of a pharmacokinetic model can help dose optimization. Objectives: A NONmEm based population (POP) PK model has been subsequently developed to evaluate the effect of demographics as covariates to address variability in pharmacokinetics of mTX. method: Forty one patients (24 males & 17 females) with ranges of age, body weight and height of 3 - 15 years, 13 - 54 kg and 100 - 177 cm respectively and diagnosed as acute lymphoblastic leukemia (ALL) were involved in the study. mTX was administered as i.v. infusion at a dose of 2 gm/m2 over a period of two hour and its plasma concentrations were determined primarily at 24 hr post-dose to be utilized in the building-up of PK *** initial/prior estimates of volumes of central (V1) and peripheral compartments (V2) and clearance (CL) and inter-compartmental clearance (Q2) for mTX were extracted from literature. The inter-subject variability was estimated for V1 & CL. The influence of different covariates on model performance and parameter estimates was assessed by evaluating the difference in objective function value. The final POP PK model was validated by bootstrap re-sampling procedures (1000 runs) and the 95% confidence intervals for the estimates were calculated. Results: The POP estimates for CL, V1 and V2 were 2.18 L/h, 5.74 L and 11.2 L respectively. The inter-individual variability for the CL and V1 was 23% and 30% respectively. The covariates analysis showed effect of body surface area and sex on the CL and weight on V1. Conclusions: The POP-PK model developed adequately defines the population PK of mTX in pediatric patients with lymphoblastic leukemia. Objective： The purpose of this study was to present the dosimetric study and evaluation the dose delivered to the skin tumor by using diode detector with total skin electron therapy （TSET）. methods： The total skin electron irradiation （TSEI） technique was used to treat ten patients with histological confirmed mycosis fungoides according to the Stanford staging system at the Radiotherapy Department, National Cancer Institute, Cairo University, Egypt. High dose rate electron beams with low electron energy 5 meV from a Siemens linear accelerator were used for treatment. Diodes were calibrated at TSET distance 300 cm and field size （35 × 35） cm^2. Results： The result of diodes measurements showed the dose to flat surface of the body was within ：1：10 % from the prescribed dose. Special areas of the body such as the perineum ＆ eyelid showed large deviation up to 30% variation from the prescription dose. Conclusion： The diode results of this study will be used as a quality assurance check for all new patients treated with TSET and to compare it to the prescribed dose delivered to the patients. It is recommends to evaluate the diodes measurements for all patients throughout the full treatment cycle and to identify individually the boost dose areas.

Objective: The dosimetric characteristics for linear accelerators with the same model, and nominal energy are known to be very similar, as long as the machines are unaltered from the manufacturer's original specifications. In this preliminary study, a quantitative investigation of the similarity in the basic photon and electron dosimetry data from the Siemens Oncor linear accelerators at our hospital(Children's Cancer Hospital, Cairo, Egypt) was reported. methods: The output factor(OF), wedge factors(WF), percentage depth dose(PDD), and beam profile for the 6 and 10 mV photon beams were measured. Results: The measured output factors varied by less than about 1% for each field size. The difference between the maximum and minimum PDD values at each depth was less than about 1%. The difference between the beam flattnes and symetry was no more than 1% at all off-axis distances. For electron the results showed that the PDD, OF, and the beam profiles were matched within 1% differences. Conclusion: These results strongly suggest that it is feasible to establish one reference photon and electron dosimetry data set for the two machines and nominal energies.

Background: Radiotherapy (RT) techniques after Conservative Breast Surgery (CBS) vary. Three Dimension (3D) planning allows for better plan optimization compared to 2 Dimension (2D) plans and also allowing for creating Dose Volume Histograms (DVHs) for both Planning Target Volume (PTV) and Organs at Risk (OAR). Patients and methods: Twenty consecutive patients with CBS planned for whole breast and supraclavicular (SCV) RT at the National Cancer Institute (NCI), Egypt between January and June 2016 were included in this study. All patients were planned clinically in 2D fashion with no more than 2 cm of ipsilateral lung allowed in the tangential fields “Limited 2D” (Limit-2D) then Target and OAR volumes were drawn according to the Radiation Therapy Oncology Group (RTOG) guidelines and 3D plans and a central slice PTV-based 2D plan, “modified 2D” (mod-2D), were performed in the same Computerized Tomography (CT) slices for each patient. mono-Iso-Centeric technique (mIT) was used in 3D plans. DVH parameters were used to compare the three plans. Results: In 3D plans, compared to Limit-2D, coverage improved for the intact breast (V95% = 95% versus (Vs) 69%, p = 0.036) and SCVPTV (V90% = 90% Vs 65%, p = 0.01). The breast and SCV V 107%, V112% and Dmax were better with 3D plan however not statistical significant (NS). Junctional hot spots were 120% and 107% in the Limit-2D and 3D plans respectively (p = 0.04). The dose to the heart, mean (333 Vs 491 cGy), V10 (5% Vs 10%) and V20 (3% Vs 7%), Ipsilateral lung V20 (19% Vs 26%), and contra lateral breast D-max (205 Vs 462 cGy) were higher in 3D plans however NS, and the dose to the cord was the same. Comparison between 3D and mod-2D showed better OAR sparing with 3D with mean heart dose (491 cGy Vs 782 cGy, p = 0.025) and Ipsilateral lung V20 (26% Vs 32%, p = 0.07% with statistically comparable target coverage. The present work deals with the detailed geology,mineralogy,geochemistry,and spectrometric prospection of the felsic volcanic rocks at the Eastern Desert,Egypt of the Arabian-Nubian *** volcanic rocks are an essential source for rare earth elements(REEs)and uranium occurrences in this *** are compositionally uniform with tholeiitic to calc-alkaline affinities,peraluminous and belong to the series of rhyolite with high-K *** exhibit more enrichment in high field strength elements(HFSE,***,Ta,Nd,Th,and U)and large-ion lithophile elements(LILE,*** and Rb)compared to the country rocks of the studied area,with REE ranging from 188.20 to 442.70 ppm and strong depletion in Ti,Sr,P with deep negative Eu *** felsic volcanic rocks were mostly generated from the partial melting of quartz-amphibolite facies accreted during the *** oddities of Zr-U-Th for the felsic volcanic rocks determine the involvement of crustal *** volcanic rocks are found in A-type suites of magma and represent highly fractionated rocks derived from rhyolitic magma,with insignificant interaction with continental crust in the low-pressure environment and during fractional *** volcanic rocks have higher values of radioactivity in which eU range from 0.5 to 121 ppm and eTh from 1.0 to 415.10 *** high values of eU and eTh can be ascribed to the mineralization of uranium and the presence of accessory minerals of radiogenic nature such as uranophane,uranothorite,zircon,and *** is considered as the mineral with most enriched uranium contents in the studied felsic volcanic rocks in which(UO2=87.30 wt%).Also,they are enriched with REE-bearing accessory minerals comprising allanite,titanite,and *** geological investigations of the felsic volcanic rocks in the studied areas are inappropriate to clear the feasible economic potentialities of rare earth elements and U occurrences;itemized and invaluable

AIm:To determine predictors of long term survival after resection of hilar cholangiocarcinoma(HC) by comparing patients surviving > 5 years with those who survived mansoura University,Egypt between January 2002 and April *** data of the patients were collected from the medical *** were divided into two groups according to their survival:patients surviving less than 5 years and those who survived > 5 ***:There were 34(14%) long term survivors(5 year survivors) among the 243 *** survivors were younger at diagnosis than those surviving less than 5 years(mean age,50.47 ± 4.45 vs 54.59 ± 4.98,p = 0.001).Gender,clinical presentation,preoperative drainage,preoperative serum bilirubin,albumin and serum glutamic-pyruvic transaminase were similar between the two *** level of CA 19-9 was significantly higher in patients surviving monstrated nine variables to be significantly associated with survival > 5 year,includingyoung age(p = 0.001),serum CA19-9(p = 0.0001),non-cirrhotic liver(p = 0.02),major hepatic resection(p = 0.001),caudate lobe resection(p = 0.006),well differentiated tumour(p = 0.03),lymph node status(0.008),R0 resection margin(p = 0.0001) and early postoperative liver cell failure(p = 0.02).CONCLUSION:Liver status,resection of caudate lobe,lymph node status,R0 resection and CA19-9 were demonstrated to be independent risk factors for long term survival.
