AIm: To evaluate the efficacy of intraoperative endoscopic retrograde cholangio-pancreatography(ERCP) combined with laparoscopic cholecystectomy(LC) forpatients with gall bladder stones(GS) and common bile duct stones(CBDS).mETHODS: Patients treated for GS with CBDS were included. LC and intraoperative transcystic cholangiogram(TCC) were performed in most of the cases. Intraoperative ERCP was done for cases with proven ***: Eighty patients who had GS with CBDS were included. LC was successful in all cases. Intraoperative TCC revealed passed CBD stones in 4 cases so intraoperative ERCP was performed only in 76 patients. Intraoperative ERCP showed dilated CBD with stones in 64 cases(84.2%) where removal of stones were successful; passed stones in 6 cases(7.9%); short lower end stricture with small stones present in two cases(2.6%) which were treated by removal of stones with stent insertion; long stricture lower 1/3 CBD in one case(1.3%) which was treated by open hepaticojejunostomy; and one case(1.3%) was proved to be ampullary carcinoma and whipple's operation was scheduled. CONCLUSION: The hepatobiliary surgeon should be trained on ERCP as the third hand to expand his field of therapeutic options.

AImTo detect risk factors for post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) and investigate the predictors of its severity. mETHODSThis is a prospective cohort study of all patients who underwent ERCP. Pre-ERCP data, intraoperative data, and post-ERCP data were collected. RESULTSThe study population consisted of 996 patients. Their mean age at presentation was 58.42 (± 14.72) years, and there were 454 male and 442 female patients. Overall, PEP occurred in 102 (10.2%) patients of the study population; eighty (78.4%) cases were of mild to moderate degree, while severe pancreatitis occurred in 22 (21.6%) patients. No hospital mortality was reported for any of PEP patients during the study duration. Age less than 35 years (P = 0.001, OR = 0.035), narrower common bile duct (CBD) diameter (P = 0.0001) and increased number of pancreatic cannulations (P = 0.0001) were independent risk factors for the occurrence of PEP. CONCLUSIONPEP is the most frequent and devastating complication after ERCP. Age less than 35 years, narrower median CBD diameter and increased number of pancreatic cannulations are independent risk factors for the occurrence of PEP. Patients with these risk factors are candidates for prophylactic and preventive measures against PEP.

Acute upper gastrointestinal bleeding (UGIB) remains an important emergency situation. In the last two decades, major developments took place influencing incidence, etiology and outcome of patients with acute UGIB. Peptic ulcer bleeding is the most significant complication of ulcer disease being responsible for 50% of all cases mortality. Aim of the study: To compare between endoscopic clip application versus argon plasma coagulation in management of bleeding peptic ulcer (BPU). Patients and methods: Sixty patients suffering from acute UGIB were randomly divided into two groups: group I included 30 patients treated with endoscopic clip application and Group II included 30 patients subjected to endoscopic APC. All patients were classified according to Forrest classification and the clinical Rockall score. Results: There were significant differences between the two groups as regard Forrest classification (P 0.05). Re bleeding was significant with higher Rockall score in group I (P 0.05). Conclusion: Endoscopic application of hemoclips have a less re bleeding rate than Argon plasma coagulation for treatment of bleeding peptic ulcer, although this was statistically insignificant, meanwhile APC is still less cost and easy. Clinical and endoscopic assessment (through Rokcall score and Forrest classification) could help in making best choice for endoscopic management.

Background: Green coffee as a functional food has an antioxidant effect, which can reduce the cancer incidence, promote weight-loss and improve glucose and lipid metabolism, as well as anti-oxidant and anti-inflammatory activity. Objective: Assessing the effect of green coffee administration on experimental cisplatin induced renal apoptosis in adult male albino rats of local strain. Design: Randomized Block Design was used. materials and methods: Forty adult male albino rats of local strain were randomly divided into four groups of 10 animals each. G1: control negative group was received isotonic saline (0.5 ml, i.p.) for 30 consecutive days, G2: sham operated group mice were received green coffee extract in water (40 mg/kg/day) for 30 consecutive days, animals of G3 and G4 were administered cisplatin (8 mg/kg/day, i.p.) at 10th day, which is well known to produce significant nephrotoxicity in rats. Animals in G3 were received green coffee extract in water (40 mg/kg/day) and G4 was received isotonic normal saline (0.5 ml, i.p.) for 30 consecutive days. All rats were sacrificed after 30 days and blood was withdrawn for biochemical examinations of kidney function tests (blood urea nitrogen, creatinine and uric acid). Kidneys were removed for determination of renal oxidative stress markers (H2O2) biochemically and caspase-3 by Immunohistochemical examination. Results: Cisplatin administration was associated with significant higher levels of BUN, creatinine, uric acid and H2O2 as compared with normal control group. Green coffee administration in cisplatin-induced renal apoptosis groups produced significant lower levels of BUN, creatinine, uric acid and H2O2 (24.4 ± 4.14, 1.730 ± 0.2830, 5.50 ± 0.850 and 0.51 ± 0.12 respectively) as compared with cisplatin-induced renal apoptosis group not administrated green coffee (27.4 ± 6, 2.04 ± .31, 7.00 ± 1.25 and 1.1 ± 0.16 respectively). Objective： The aim of our study was to assess and compare the potential dosimetric advantages and drawbacks of photon beams and electron beams as a boost for the tumor bed in superficial and deep seated early-stage breast cancer. methods： We planned CTs of 10 women with early breast cancer underwent breast conservative surgery were selected. Tumor bed was defined as superficial and deep with a cut of point 4 cm, those with less than 4 cm were defined as superficial tumors representing 4 patients and those with depth of 4 cm or more were classified as deep tumors representing 6 patients. The clinical target volume （C＇I＇V） was defined as the area of architectural .distortion surrounded by surgical clips. The plan- ning target volume （PTV） was the C＇I＇V plus margin 1 cm. A dose of 10 *** 2 Gy fractions was given concurrently at the last week of treatment. Organs at risk （OARs） were heart, lungs, contra-lateral breast and a 5 mm thick skin segment of the breast surface. Dose volume histograms were defined to quantify the quality of concurrent treatment plans assessing target coverage and sparing OARs. The following treatment techniques were assessed： photon beam with 3D-conformal technique and a single electron beam. Results： For superficial tumors better coverage for CTV and P＇I＇V with good homogeneity with better CI was found for the 3D conformal radiotherapy （3DCRT） but with no significant planning objectives over electron beam. For deep tumors, the 3DCRT met the planning objectives for C＇I＇V, PTV with better coverage and fewer hot spots with better homogeneity and CI. For superficial tumors, OARs were spared by both techniques with better sparing for the electron beam where as for deep tumors also OARs were well spared by both techniques. Conclusion： Boosting the tumor bed in early- stage breast cancer with optimized photon may be preferred to electron beam for both superficial and deep tumors. Objective: The objective of this work was to establish adequate tolerance limits based on a certain defined institutional indices and generate published data presenting our results to the radiotherapy community. methods: One hundred paediatric patients were treated using 6-mV X-ray beams produced by Siemens ONCOR Expression linear accelerator. The clinical step-and-shoot intensity-modulated radiation therapy (ImRT) treatment plans were designed using KonRad release 2.2.23. For two treatment sites (abdomen, head and neck), the fluence maps generated by the treatment planning system were all delivered for the quality assurance (QA) which included absolute dose verification for all treatment fields, relative dose verification for each treatment field. Results: The 724 fluence maps were analyzed at three different criteria using the gamma index tool. The 3% dose difference of local prescribed dose /3 mm was considered adequate. The passing rate for all fields of all plans always exceeded 70%. The dose differences between the measured and calculated doses ranged from -2.2% to +4% [mean and standard deviation (s): 1.4 ± 1.5] for the abdominal case, and from -3.3% to +5.6% (1.3 ± 1.6) for head and neck case with total confidence limit 0.046 (4.6%). The 14/100 (14%) of the absolute point dose measurements were out of ±3% from the dose predicted by the treatment planning system. Only two cases were below -3%, while 12 cases over +3%. Conclusion: At 3% dose difference of local prescribed dose /3 mm criteria, a 75% passing a gamma criterion and 3% for absolute point dose can be achieved for abdomen and head and neck treatments site. We considered the tolerance limits based on these indices for ImRT QA adequate.

A novel, simple approach to the synthesis of macrocyclic Schiff base ligand resulted from the condensation of bisaldehyde and ethylenediamine was prepared (7, 8, 15, 16, 17, 18-hexahydrodibenzo (a, g) (14) annulene) (L) and its complexes were synthesized and characterized using different physicochemical studies as elemental analysis, FT-IR, 1H NmR, conductivity, magnetic properties, thermal analysis, and their biological activities. The spectroscopic data of the complexes suggest their 1:1 complexe structures which are investigated by elemental analysis, FT-IR, 1H NmR, conductivity, magnetic properties, thermal analysis, and their biological activities. The spectroscopic studies suggested the octahedral structure for the all complexes. The spectroscopic data of the complexes suggest their structure in which (N2O2) group act as a tetradentate ligand and two chlorides as monodentate ligands. Also electronic spectra and magnetic susceptibility measurements indicate octahedral structure of these complexes. The synthesized Schiff base and its metal complexes also were screened for their antibacterial and antifungal activity. Here we report the effect of a neutral chelating ligand on the complexation with iron to determine it in different types of natural water using recovery test. The activity data show that the metal complexes to be more potent/ antibacterial than the parent Schiff base ligand against one or more bacterial species.

Pregnancy is a complex physiological process involving several interconnected systems. many researchers were concerned that the formation of a fetus with different genetic components may contradict the normal state of immunity, which attempts to reject and fight foreign bodies. This piqued the interest of biologists and immunologists, who set out to discover the immune system’s composition and mode of response in the uterus. According to several studies, natural killer (NK) cells are present in a significant percentage that differs from what is seen in peripheral blood. As a result, several scientific studies have been conducted on uterine NK cells, investigating their types, characteristics, receptors, secretions, and interactions with the surrounding environment. Research has also indicated the capacity of uterine NK cells to strike a balance between eradicating uterine infections and effectively contributing to different phases of pregnancy. Various studies have shown that NK cell activity is intimately related to the success or failure of pregnancy. In this review, we describe the uterine NK cell subtypes;decidual (dNK) cells and endometrial NK cells (eNK) cells and their important role during different phases of pregnancy.

AIm: To assess the practice of Egyptian physicians in screening patients for hepatocellular carcinoma(HCC). mETHODS: The study included 154 physicians from all over Egypt caring for patients at risk for HCC. The study was based on a questionnaire with 20 items. Each questionnaire consisted of two parts:(1) personal information regarding the physician(name, age, specialty and type of health care setting); and(2) professional experience in the care of patients at risk for HCC development(screening, knowledge about the cause and natural course of liver diseases and HCC risk). RESULTS: Sixty-eight percent of doctors with an mD degree, 48% of doctors with a master degree or a diploma and 40% of doctors with a Bachelor of medicine, Bachelor of Surgery certificate considered the hepatitis C virus(HCV) genotype as risk factor for HCC development(P medicine, internal medicine or gastroenterology and 67% of physicians in other specialties advise patients to undergo screening for HCV and hepatitis B virus infection as well as liver cirrhosis(P ministry of Health(mOH) doctors consider HCV infection as the leading cause of HCC in Egypt(P mD degree, 55% of doctors with a master degree or a diploma, 56% of doctors with an mBBCH certificate, 74% of doctors in University Hospitals and 46% of mOH hospital doctors consider abdominal ultrasonography as the most important investigation in HCC screening(P medicine, internal medicine or gastroenterology and 37% of physicians in other specialties recommend as HCC screening interval of 3 mo(P mD degree, 50% of doctors with a master degree or diploma and 60% of doctors with an mBBCH certificate follow the same ***: In Egypt, physicians spe
