Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIm To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Amphetamine Versus Non Amphetamine-Related First Episode Psychosis in Saudi Arabian Patients

Background: Amphetamines are illicit psychostimulant drugs that can induce psychotic symptoms. Very few studies have been conducted in Kingdom of Saudi Arabia (SA) on amphetamine abuse and related psychosis. Recently, the pattern of amphetamine abuse in SA showed a significant trend of increased frequency. Objectives: To investigate the extent of amphetamine abuse in a sample of Saudi patients hospitalized for first episode of acute psychosis. Also, to compare in that sample between amphetamine psychosis and other psychoses regarding demographic data, premorbid personality and symptoms profile. method: 106 patients with acute psychosis were hospitalized and screening of urine for amphetamine was conducted for all. Patients’ psychiatric evaluation included interviewing, and ICD-10 criteria for personality disorders. 30 healthy subjects were also included for comparison with patients. Results: 34/106 of psychotic patients (32%) were positive for amphetamine in urine (≥ 1000 ng/ml). The frequency of personality disorders was significantly higher (P mpared with healthy subjects (6/30;20%). Also, the incidence of personality disorders was significantly higher (P mphetamine positive psychosis (25/34;73.5%) compared with amphetamine negative psychosis (29/72;40%). Cluster B personality disorders particularly the antisocial and borderline were significantly higher in amphetamine positive psychosis (13/34;38%) compared with amphetamine negative psychoses (6/72;8%). The symptom profile showed significant difference between amphetamine positive and amphetamine negative psychosis as regards ideas of reference (50% vs. 14%), suspiciousness (44% vs. 11%), suicidal ideation (38% vs. 23%), paranoid delusions (29% vs. 17%) and increased pulse or blood pressure (29% vs. 7%) respectively. Conclusion: Screening of amphetamine in urine among patients with first episode of acute psychosis can help and support the clinical distinction of

为了提高桑叶发酵品质,制作良好的青贮饲料以满足动物营养需要,本试验研究了发酵底物(葡萄糖,糖蜜)和乳酸菌制剂对桑叶青贮发酵品质的影响。1.不同浓度葡萄糖(morus Alba)对桑叶青贮发酵品质,化学成分和体外消化率的影响。本试验研究目的是研究添加不同水平葡糖糖对桑叶青贮发酵品质,化学成分和体外消化率的影响。将采收自生长阶段的新鲜桑叶切碎处理,采用实验室小型青贮窖进行青贮。试验设不同浓度的葡萄糖添加组,分别为(1)对照无添加;(2)1%葡萄糖组,G1;(3)2%葡萄糖组,G2;(4)3%葡萄糖组,G3,在青贮第1,7,14,30,60和90天开窖分析发酵品质。结果表明,桑叶干物质(dry matter;Dm)为 424.92 g/kg 鲜重(fresh matter;Fm),可溶性糖(WSC)为 80.31 g/kg干重,缓冲能和粗蛋白含量分别为223.02 mE/kg Dm和21.86%Dm。表面附着乳酸菌数量为1.0×103CFU/gFm。青贮14天后,G2和G3组pH显著(Pm,AN/TN,ADF和NDF均无显著影响。G3组中有较高含量的PA表明梭菌发酵或异型发酵的发生。不同处理组在整个青贮过程中产气量均无显著差异。添加葡萄糖对桑叶体外干物质消化率(IVDmD)和体外中性洗涤纤维消化率(IVNDF)均无显著影响。2.葡萄糖和乳酸菌对桑叶青贮发酵品质,化学成分和体外消化率的影响本试验采用小型实验室青贮窖。试验设:对照无添加(C);商业乳酸菌(Lactobacillus Plantarum)(LAB1);乳酸片球菌(Pediococuss acidilactici)(LAB2);鼠李糖乳杆菌(Lactobacillus rhamnosus)(LAB3)。所有乳酸菌组添加水平为106(CFU)g-1。青贮窖置于室温下贮藏(25-30℃),分别于1,7,14,30,60和90天开窖分析发酵品质。原材料Dm WSC和CP含量分别为424.92 g/kg FW,80.31g/kg Dm和21.86%Dm,缓冲能为61.17 mE/kg Dm。表面附着乳酸菌数量为1.0×105cfu/gFm,酵母菌数量少于1.0×103CFU/gFm。青贮7天后,所有处理组pH均显著(Pm。不同乳酸菌添加组在整个培养过程中产气量存在显著差异(PmD)和体外中性洗涤纤维消化率(IVNDF)显著差异。3.添加糖蜜,葡萄糖和乳酸菌对桑叶青贮特性、发酵品质和化学成分的影响。为开发桑叶作为非常规饲料资源,本试验探讨了添加乳酸菌、葡萄糖或糖蜜对桑叶青贮发酵品质的影响,试验设对照组(C)、乳酸菌组(P)、葡萄糖组(G)、糖蜜组(m)、乳酸菌+葡萄糖组(P+G)、乳酸菌+糖蜜组(P+m),青贮后第7,14,30和60天开窖取样分析桑叶青贮饲料发酵品质。结果表明,添加乳酸菌加速了桑叶青贮过程中乳酸发酵,青贮第7天P,P+G和P+m组乳酸含量已达到C组的6倍以上,pH值降至4.30以下,其中P+G和P+m在青贮结束时降至4.0左右。补充发酵底物并未有效改善桑叶青贮发酵品质,C、G和m组pH值在青贮前30 d始终保持在5.85以上,青贮60d时C组为5.96,G和m组分别下降至5.35和5.24,显著(Pm组,青贮7 d后开始显著(Pm组。青贮第7天p组氨态氮/总氮显著(Pm组氨态氮/总氮显著(Pm组直至青贮结束。认为单独添加乳酸菌明显提高了桑叶青贮发酵品质,而组合添加并未进一步得到大的改善.

This study examined if the anti-tumorigenesis effect of Exendin-4 in HT29 and HCT116 colorectal cancer(CRC)involves modulation of SIRT1 and Akt/GSR3K/β-catenin/NF-κB ***29 and HCT116 cells were treated either with increasing levels of Exendin-4(0.0-200μm)or with Exendin-4(at its IC50)in the presence or absence of EX-527(10μm/a selective SIRT1 inhibitor)or Exendin-4(9-39)amide(E(9-39)A)(1μm/an Exendin-4 antagonist).In a dose-dependent manner,Exendin-4 inhibited cell survival,but enhanced levels of lactate dehydrogenase(LDH)and single-stranded DNA(ssDNA)in both HT29 and *** both cell lines and at it has an IC50(45μm for HT29 and 35μm for HCT1165),Exendin-4 also significantly reduced cell survival,migration,and invasion of both cell types,with no effect on the expression GLP-1 receptors(GLPRs)nor of the activity of *** these doses,Exendin-4 also increased the expression of SIRT1 but reduced the acetylation of NF-κB and the expression of Bax and cleaved caspase-3 and in both cell ***,protein levels of p-GS3Kβ(Ser9),total and acetylatedβ-catenin,and Anix2 were significantly decreased,but levels of p-GS3Kβ(Ser9)and p-β-catenin(Ser33/37/Thr41)were significantly increased in both HT29 and HCT116-exendin-4 treated *** the effects exerted by Exendin-4 were completely prevented by Ex527 or E(9-39)*** conclusion,Exendin-4 suppresses the tumorigenesis of HT29 and HCT116 CRC cell activation of GS3Kβ-induced inhibition ofβ-catenin and NF-κβin a SIRT1-dependent mechanism.

The Value of Serum midKine as a marker of Hepatocellular Carcinoma on Top of Hepatitis C Virus Related Cirrhosis

Background: Hepatocellular carcinoma (HCC) is characterized by the deficiency of specific symptoms in the early stages of the disease and a poor prognosis. Biomarkers that have the diagnostic ability to distinguish between inflammation, cirrhosis, and HCC are needed for early case detection. midKine (mK) is overexpressed in many malignancies, including hepatocellular carcinoma, gastric carcinoma, colon carcinoma, lung carcinoma, urinary bladder carcinoma and prostate carcinoma. Objective: The objective is to evaluate serum midKine as a marker of HCC in HCV-related liver cirrhosis. Patients and methods: This study included 50 patients with HCC (on top of hepatitis C virus-related cirrhosis), 20 cirrhotic patients due to chronic HCV infection, and 10 normal subjects as a control group. History, examination, and laboratory investigations were performed on all subjects in the three groups including mK and Alpha fetoprotein (AFP) serum levels. Results: Serum levels of AFP and mK were higher in the HCC group than in the other study groups (P Conclusion: midKine is a novel sensitive biomarker that could be used for early detection of HCC and has a high significance in differentiation between cirrhosis and HCC cases. midKine levels are correlated with HCC stages and sizes.

Egypt has one of the highest prevalence rates of hepatitis C virus(HCV) in the world,mostly with genotype 4 that is highly associated with severe fibrosis. As a consequence,hepatocellular carcinoma has become the leading cause of cancer in this country. mauritania is a highly endemic area for hepatitis B virus(HBV). HBV and HCV could both be iatrogenically transmitted through infected blood products,infected needles,and medical equipment improperly sterilized. Adequate and efficient healthcare and public health measures with good surveillance programs,access for screening,prevention strategies,and successful treatment are needed to halt the spread of these diseases. Herein,we have reviewed the epidemiology,modes of transmission,predisposing factors,and novel treatment modalities of these viruses. We have proposed practices and interventions to decrease the risk of transmission of HCV and HBV in the affected countries,including strict adherence to standard precautions in the healthcare setting,rigorous education and training of patients and healthcare providers,universal screening of blood donors,use of safetyengineered devices,proper sterilization of medical equipment,hepatitis B vaccination,as well as effective direct-acting antiviral agents for the treatment of HCV.

Role of Sulforaphane on Histone Deacetylase Activity in Solid Ehrlich Carcinoma

Histone acylation is one in every of the posttranslational modification that incorporates a role within the regulation of sequence expression. This study was aimed to ameliorate neoplasm cell model Solid Ehrlich malignant neoplastic disease with sulforaphane extracted from cabbage alone and together with immune suppressant drug (mTX) by study the activity of simple protein deacetylase accelerator (HDAC). In this study, sulforaphane was extracted from cabbage leaves and evaluated victimization GC-mS and ultraviolet illumination spectrophotometry. 60 white male rats were divided into six equal groups. Group I: management ordinarily. The remaining mice were subjected to Ehrlich neoplasm cells. Group II: Tumor-bearing group. Group III: immune suppressant drug “mTX” treated group. Group IV, V treated with SFN before and when Paul Ehrlich cells implantation group VI (methotrexate and sulforaphane-treated group). This result showed that sulforaphane was extracted from cabbage (Brassica oleracea) in concentration of 833 μg/g leave. HDAC was attenuated when treatment with sulforaphane after treatment with methotrexate or sulforaphane alone. The desoxyribonucleic acid injury was attenuated in neoplasm tissue of tumor-bearing mice when treatment with immune suppressant drug and hyperbolic considerably in tumor tissue of tumor-bearing mice treated with sulforaphane before and after carcinogenesis and together with methotrexate treatment after carcinogenesis.

Dosimetric Comparison of Intensity-modulated Radiotherapy versus Three-Dimensional Conformal Radiotherapy for Patients with Brain Tumors

Purpose: To compare target dose distribution and dose to normal tissue for brain tumors using intensity-modulated and three-dimensional conformal radiotherapy. methods and materials: Twelve patients selected in the present study compared the dosimetry of intensity-modulated radiation therapy (ImRT) and three-dimensional conformal radiation therapy (3DCRT) techniques in patients treated with different diagnosis brain tumor. All patients underwent computed tomography planning in conjunction with magnetic resonance imaging fusion. Prescription dose and normal-tissue constraints were identical for both plans. The tolerance level for maximum dose was 6.0 Gy for lenses and 54.0 Gy for brain stem, and optical nerves. Target coverage was evaluated with the D98%, D95% and D2%. Normal tissue dose was evaluated with the maximum dose to lenses, brain stem, and optical nerves. Results: mean and standard deviation values of PTV 98%, PTV 95% and PTV 2% in ImRT these values were 94.6 ± 2.3, 96.56 ± 1.67 and 106.5 ± 1.87 respectively;for 3DCRT were 92.7 ± 3.09, 95.58 ± 1.24 and 104.45 ± 2.73 respectively. ImRT plan provided reduced dose to brainstem (72.17 ± 29.967) and lenses (2.43 ± 2.39) relative to the 3DCRT plans for brainstem (73.05 ± 34.59) and lenses (7.65 ± 23.24). But optic nerves with ImRT significantly received higher doses (38.86 ± 32.15) than 3D-CRT (27.57 ± 31.45). Conclusion: Dose coverage of the planning target volume (PTV) and organ at risk (OAR) was better with ImRT. If PTV is distant to optical nerves and brainstem 3DCRT technique can be applied, and if the PTV is nearby OAR intensity-modulated treatment technique should be used.

The Effects of Energy Drink Consumption on Kidney and Liver Function: A Comparative Study

The current study aimed to investigate the potential health impacts associated with Energy Drink (ED) consumption and its effects on general health, liver function, and kidney function among students at Jazan University. EDs contain a variety of ingredients such as taurine, arginine, caffeine, acidity regulators, B vitamins, vitamin C, herbal extracts, and sugar. Previous studies have reported that EDs contain more caffeine content more a cup of coffee. Fifty-seven volunteer female students from the same socioeconomic background were eligible for the study based on their ED consumption habits. The students were divided into four groups: the control group (1) which did not consume EDs (19 students), group (2) which consumed 2 - 3 cans per week (19 students), group (3) which consumed 4 - 7 cans per week (14 students), and group (4) which consumed more than 7 cans per week (5 students). Five milliliters of venous blood were drawn from all subjects to determine ALT, AST, and GGT levels for liver function tests and creatinine, urea, uric acid, and BUN levels for kidney function tests. The results of the kidney function analysis showed that uric acid levels significantly increased (P 0.05) in all groups compared to the control group. The liver function analysis illustrated that, compared to the control group, only AST was significantly increased (P m ED consumption had a significant relationship to increased uric acid and AST concentrations, while urea and BUN decreased significantly. Further research is needed to understand the long-term health implications of ED consumption.

The integration of voice and data traffics in our proposed centralized switch node architecture is considered. Specifically,the unfairness of the Free Round Robin(FRR) scheduling strategy towards low priority traffic types is demonstrated by a comparison with a restricted bandwidth strategy called Reserved Round Robin(RRR).The simulation results show that under heavy load conditions,RRR provides a more balanced scheduling of traffic types than the FRR,with only a moderate increase in the delay times of higher priority traffic types.
