Screening for hepatocellular carcinoma by Egyptian physicians

AIM: To assess the practice of Egyptian physicians in screening patients for hepatocellular carcinoma(HCC). METHODS: The study included 154 physicians from all over Egypt caring for patients at risk for HCC. The study was based on a questionnaire with 20 items. Each questionnaire consisted of two parts:(1) personal information regarding the physician(name, age, specialty and type of health care setting); and(2) professional experience in the care of patients at risk for HCC development(screening, knowledge about the cause and natural course of liver diseases and HCC risk). RESULTS: Sixty-eight percent of doctors with an MD degree, 48% of doctors with a master degree or a diploma and 40% of doctors with a Bachelor of Medicine, Bachelor of Surgery certificate considered the hepatitis C virus(HCV) genotype as risk factor for HCC development(P f physicians specialized in tropical medicine, internal medicine or gastroenterology and 67% of physicians in other specialties advise patients to undergo screening for HCV and hepatitis B virus infection as well as liver cirrhosis(P f doctors in University Hospitals and 69% of Ministry of Health(MOH) doctors consider HCV infection as the leading cause of HCC in Egypt(P f doctors with an MD degree, 55% of doctors with a master degree or a diploma, 56% of doctors with an MBBCH certificate, 74% of doctors in University Hospitals and 46% of MOH hospital doctors consider abdominal ultrasonography as the most important investigation in HCC screening(P five percent of physicians in tropical medicine, internal medicine or gastroenterology and 37% of physicians in other specialties recommend as HCC screening interval of 3 mo(P f doctors with an MD degree, 50% of doctors with a master degree or diploma and 60% of doctors with an MBBCH certificate follow the same ***: In Egypt, physicians spe

Results of the first 10 Cases of Coronary Bypass Surgery in Senegal

This is a review of the first 10 coronary artery bypass surgeries performed by the local team. The mean age was 62 years old [45 - 74]. The patients were predominantly male, with a M/f ratio of 4:1. Cardiovascular risk factors were mainly myocardial infarction (MI) (60%), hypertension (50%), obesity (40%) and diabetes (30%), with at least two risk factors per patient. Angina was the main symptom (80%). The average time from presentation to surgery was 8 months. The mean Euroscore 2 was 2.92 ± 1.65 [1.33 - 6.60]. Coronary angiography revealed an average of 2 lesions per patient, with 3-vessel involvement in 70% of cases: the Interventricular artery (IVA) (100%), the right coronary artery (90%) and the circumflex artery (70%). On echocardiography, the mean Left ventricular ejection fraction (LVEf) was 59% [33% - 76%]. All patients underwent median sternotomy with bypass grafting. The average duration of the cardiopulmonary bypass was 150 min [46 - 275 min];that of aortic clamping, 120 min [43 - 232 min]. The grafts used were internal thoracic artery (ITA) in 100% of cases (80% on the left and 20% on the right), and the great saphenous vein (GSV) in 60% of cases (50% on the left and 10 on the right). Double bypass was performed in 60% of cases, single bypass in 30% and triple bypass in 10%. The bypasses were performed on the IVA (100%), the middle lateral of the circumflex (30%) and the bisector (20%). The average time to extubation was 11 hours and the length of stay in the intensive care unit was 7 days [03 - 17 days]. One patient had a reoperation on Day 0 post-op. The average hospital stay was 13 days [06 - 27 days]. Complications occurred in nine of the patients (90%), with a predominance of infectious and neurological complications. Overall operative mortality was 3%, all in intensive care.

Sinonasal Polyposis: About 60 Cases at fann University Hospital Center, Senegal

Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/f) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. Introduction: Malaria remains a public health priority in Senegal, particularly in Tambacounda, where it is one of the main causes of child mortality. The objective of this work was to evaluate the care of children under 10 years of age hospitalized at the Tambacounda Health Center and the factors associated with recovery. Methods: An analytical, retrospective, and descriptive cross-sectional study with exhaustive recruitment of children 0 to 120 months hospitalized at the Tambacounda reference health center for severe malaria (according to WHO criteria) between 1 January 2018 and 31 December 2021 was performed. Data collection was done through a questionnaire. Records, hospitalization records, and treatment records were the sources of collection. Data entry and analysis were performed on Epi Info 7.2 and R. Results: A total of 481 children hospitalized with severe malaria were recruited. The highest number of cases was recorded in 2018 (33.05%). In the four years of the study, peaks were always observed between October and November and the highest peak in November 2020 with 95 cases. The mean age was 65.64 months with a standard deviation of 29.28 months and a predominance of male (53.43%). The majority of people were admitted from the outpatient clinic (57.79%) and the rest (42.21%) on the recommendation of a peripheral health post. All hospitalized patients had a positive RDT and/or a positive thick drop. However, the sharp decline at admission or during hospitalization was positive in 93.80% of patients in our series, negative in 5.20% and not achieved in 1.00%. Seizures and severe anemia topped the list of signs of severity with 45.94% and 8.11%, respectively. In terms of evolution, for all hospitalized patients, there were 81.29% recovery, 10.19% referral to the Tambacounda regional hospital center for hospitalization, 4.99% death, 0.83% discharge and 2.70% unknown evolution. INTRODUCTION Preservation of the freshwater resources is a big challenge for a healthy civilized *** water consumption increased more than twice the population increase rate,drinking water pollution and freshwater shortage increased dramatically during the last decades(Kar et al.,2012;Mueller and Nowack,2010).Only 0.01%of freshwater resources are suitable for human needs(Rashid et al.,2019),even in the areas of abundant freshwater(Yeston et al.,2006).By 2050,over 5 billion of the world population will be under severe fresh water supply conditions due to climate change and urbanization exacerbate(Sealy,2019;Oves et al.,2015;Kar et al.,2012).Currently,one billion people drink unhygienic water,especially in developing countries which leads to severe waterborne diseases which might cause death(Aminiyan et al.,2018).Twelve million people die every year due to this problem(Zhao et al.,2019;Oves et al.,2015).

framework for COVID-19 Segmentation and Classification Based on Deep Learning of Computed Tomography Lung Images

Corona Virus Disease 2019(COVID-19) has affected millions of people worldwide and caused more than6.3 million deaths(World Health Organization, June 2022). Increased attempts have been made to develop deep learning methods to diagnose COVID-19 based on computed tomography(CT) lung images. It is a challenge to reproduce and obtain the CT lung data, because it is not publicly available. This paper introduces a new generalized framework to segment and classify CT images and determine whether a patient is tested positive or negative for COVID-19 based on lung CT images. In this work, many different strategies are explored for the classification ***50 and VGG16 models are applied to classify CT lung images into COVID-19 positive or negative. Also,VGG16 and ReNet50 combined with U-Net, which is one of the most used architectures in deep learning for image segmentation, are employed to segment CT lung images before the classifying process to increase system performance. Moreover, the image size dependent normalization technique(ISDNT) and Wiener filter are utilized as the preprocessing techniques to enhance images and noise suppression. Additionally, transfer learning and data augmentation techniques are performed to solve the problem of COVID-19 CT lung images deficiency, therefore the over-fitting of deep models can be avoided. The proposed frameworks, which comprised of end-to-end, VGG16,ResNet50, and U-Net with VGG16 or ResNet50, are applied on the dataset that is sourced from COVID-19 lung CT images in Kaggle. The classification results show that using the preprocessed CT lung images as the input for U-Net hybrid with ResNet50 achieves the best performance. The proposed classification model achieves the 98.98%accuracy(ACC), 98.87% area under the ROC curve(AUC), 98.89% sensitivity(Se), 97.99 % precision(Pr), 97.88%f-score, and 1.8974-seconds computational time.

Utilization of Egyptian Ilmenite Ore for Steel Surface Preparation in Petroleum field

The construction of gas and oil plants include many items such as steel structures, equipments, piping, tanks and vessels at different conditions such as offshore, onshore, above and underground shall be protected against corrosion by coating. The protective value of a coating system is proportional to the surface preparation;the objectives of the surface preparation are the surface cleanliness and the surface profile of the substrate. The surface preparation is usually achieved by blast cleaning using different types of abrasive. Due to its high hardness and low cost, silica sand has been used extensively for impact abrasion. It is, however, very brittle with most grain disintegrating on impact, causing excessive levels of siliceous dust which is a major health hazard and causes serious environmental pollution. Consequently, it has been banned as impact abrasive material in most countries around the world. Considerable research has been done in the last twenty years to find an impact abrasive material as hard as silica and that respects. So it was important to search for sand substitution among the local abundant minerals in Egypt. Waste product obtained from ilmenite ore extracted from the mine was found to be the most suitable alternative abrasive. This work shows the abrasive types, the occurrence of the ilmenite ore. The chemical properties, physical properties and microscopic examination of the waste were investigated. The obtained result indicates that, the waste product obtained from ilmenite ore can be utilized for abrasion as sand substitution.
