Depression is a serious medical condition and is a leading cause of disability *** depression diagnostics and assessment has significant limitations due to heterogeneity of clinical presentations,lack of objective assessments,and assessments that rely on patients'perceptions,memory,and *** phenotyping(DP),especially assessments conducted using mobile health technologies,has the potential to greatly improve accuracy of depression diagnostics by generating objectively measurable *** includes two primary sources of digital data generated using ecological momentary assessments(EMA),assessments conducted in real-time,in subjects'natural *** includes active EMA,data that require active input by the subject,and passive EMA or passive sensing,data passively and automatically collected from subjects'personal digital *** raw data is then analyzed using machine learning algorithms to identify behavioral patterns that correlate with patients'clinical *** investigations have also shown that linguistic and behavioral clues from social media data and data extracted from the electronic medical records can be used to predict depression *** other sources of data and recent advances in telepsychiatry can further enhance DP of the depressed *** of DP endeavors depends on critical contributions from both psychiatric and engineering *** current review integrates important perspectives from both disciplines and discusses parameters for successful interdisciplinary collaborations.A clinically-relevant model for incorporating DP in clinical setting is *** model,based on investigations conducted by our group,delineates development of a depression prediction system and its integration in clinical setting to enhance depression diagnostics and inform the clinical decision making ***,challenges,and opportunities pertaining to clinical integration of DP of depressio

Use of a device that applies external kneading-like force on the abdomen for treatment of constipation

AIM: To evaluate the efficacy of a recently developed device that applies kneading-like motion on the abdomen in improving constipation in elderly long-term care patients. METHODS: Thirty constipated elderly patients were randomly selected from two nursing homes. They were instructed to use the device once daily for 20 min. Rate of bowel movements, volume and consistency of stool and the use of laxantia were all recorded during a 3-wk baseline period and for 12-wk treatment period. Colonic transit time (CTT) was measured in 13 patients by radiopaque markers during the baseline and at the end of treatment. RESULTS: Bowel movement rate (BM/week) increased from 1.4±0.4 BM/wk during baseline to 3.9±0.8 BM/wk during treatment (P<5.0×10-7). Stool amount that was 'low' in 30 patients during baseline increased in 21 patients at the end of the study period (x2 = 19.048-P= 1.3×10-5). Stool consistency,that was 'hard' in 25 patients and 'soft' in 5 patients during baseline, ameliorated in 23 patients at the end of the study (only 2 patients referred 'hard' stool) (x2 = 21.043-P= 4.0×10-6). The mean baseline CTT measured was 92.3±32.3 h at baseline and decreased to 49.4±31.3 h during the study period (P = 0.000208). No side effects were observed during the study period. CONCLUSION: External mechanical vibration of the abdomen reduced CTT and helped to relieve severe constipation in elderly constipated patients.

A cross-sectional study of post-COVID syndrome at a tertiary care center in Turkey

Objective:To investigate long-term symptoms after acute COVID-19,the link between symptoms and respiratory function,radiological changes in the post-COVID period,and risk factors for post-COVID ***:In this cross-sectional study,123 participants who were admitted within the first 3 months were categorized as group 1,and those who applied after 3 months were categorized as group *** to thoracic imaging and pulmonary function tests,patients were split into 3 groups as mild,moderate and ***:At least one symptom was present in 91.9%and 61.8%in acute and post-COVID period,*** function tests were normal in 60(70.6%)in the first three months,and 30(78.9%)in 91-days to 1-year period after acute COVID-19 *** 3 months,22.4%of chest X-rays and 7.9%of computerized tomography revealed *** who developed acute complications(OR 9.91,95%CI 1.93-50.87),had 2 or more symptoms at admission(OR 7.73,95%CI 2.56-23.33),had 1%to 14%CT involvement(OR 3.05,95%CI 1.06-8.79),or had 50%or more CT involvement(OR 14.68,95%CI 1.24-172.55)had a higher risk of developing post-COVID ***:COVID-19 symptoms can last for long *** of symptoms,acute complications,and the extent of radiological involvement may all contribute to elevated risk of post-COVID *** a result,patients with COVID-19 should be checked for long-term clinical difficulties on regular basis.

Background:Acute hemorrhagic edema of infancy (AHEI) is a rare leukocytoclastic vasculitis of the small vessels occurring at a young age and considered as a benign self-limited *** to its low prevalence,there are limited data on the presentation and complications of this ***:All computerized files of children who were hospitalized at a tertiary pediatric center due to AHEI over a 10 year period were ***,laboratory and histopathological data were ***:Twenty-six patients were included in our study,accounting for 0.7 cases per 1000 admissions of children aged 2 years or *** age was 12.9 *** than two thirds of the children had preceding symptoms compatible with a viral *** admission,all patients presented with typical findings of a rash and *** was most profound over the lower extremities (73%).Concomitant viral or bacterial infections were found in six *** biopsy was performed in six patients revealing leukocytoclastic *** children (50%) had systemic involvement including joint involvement (n=9),gastrointestinal hemorrhage (n=4),microscopic hematuria (n=1) and compartment syndrome of the limb (n=1).The latter was diagnosed in a patient with familial Mediterranean ***:Our largest data series highlighted what is known regarding clinical and histological findings in children with ***,contrary to what was previously reported,we found a higher rate of systemic *** AHEI is a rare entity,pediatricians should be familiar with its presentation,management and our reported complications.

Survival Effect of Tube Feeding in Patients with Advanced Dementia and Comorbidities

Patients suffering from advanced dementia who have difficulties in eating and swallowing are often tube fed (TF). They suffer, on most occasions, from additional diseases. The effect of tube feeding in advanced dementia patients is controversial and is largely discussed in the medical literature. The purpose of the present study, is to report the effect of tube feeding on the survival time in patients with advanced dementia who suffered from additional diseases. One hundred and forty nine patients with Functional Assessment Stage of Alzheimer Disease (FAST) stage 5>, hospitalized in skilled geriatric nursing department between July 2008 and December 2011 were included in the study. Oral and tub feeding was decided following the evaluation by speech therapists at admission. 99 (66.5%) of the patients were tube fed and 50 (33.5%) received oral feeding at admission. The mean age was 80 ± 11. Sixty four percent of the patients were admitted from general hospitals. Comorbidities were significantly higher in the tube fed group, median survival time was 164 days. The median survival time of the OF group was 78 days (χ2 = 0.94;p = 0.33). Prevalence of tube feeding increased significantly from 53.85% to 90.9% in patients with 3 risk factors such as pressure ulcers, low Hgb and low BMI. The survival time of the tube fed patients was longer compared to patients who were orally fed. Tube feeding in patients with dementia continues to be controversial, and additional researchable data are helpful in the decision to insert a tube.

Stress Relaxation of a Paper Sheet under Cyclic Load: An Experimental and Theoretical Model

Mechanical experiments have been performed to study the dynamic stress relaxation of a paper sheet material mainly used in food packaging industry. The material was cyclically tensile-loaded with a strain range between 2.4% and 4%. The time period for each cycle was 400 seconds. It was found that stress will decrease when the number of cycles increases in the case of upper load and vice versa in the case of lower load. At the same time, the stress to strain curves followed the same pattern as the one from the previous cycle. The stress relaxation behavior of each cycle has been analyzed and the dynamic relaxation modulus was derived. An improved model is proposed to describe the dynamic relaxation behavior of the paper sheet. This model shows a very good fit to the experimental results and trends of prediction are observed. Furthermore, the physical description of this model and the variation by the cycles is discussed.

The role of 3-dimensional ultrasound for the diagnosis of congenital uterine anomalies

Objectives: To demonstrate the value of 3-dimensional (3-D) ultrasound (US) in the diagnosis of congenital uterine anomalies. Methods: Fifty one infertile patients referred to our US unit during 12 years period, with suspected diagnosis of congenital uterine anomalies by previous HSG or 2D US examinations, were evaluated by transvaginal 3-D US. The 3-D US diagnoses were compared to the initial HSG diagnosis, and to hysteroscopic evaluation when performed. Results: 3-D scan confirmed the initial HSG diagnosis in 27 out of 51 (52.9%) women. The concordancy rates between the initial diagnosis by HSG and 3-D US results were 30.4% for bicornuate uterus;75% for arcuate uterus;83% for septate uterus;and 80% for unicornuate uterus. Of the 13 cases with normal HSG and suspicious 2-D US, 30.8% were found to be normal by 3D sonography. In cases where hysteroscopy was done, the results were 100% in concordance with the 3-D US evaluations. Conclusions: 3-D US is an accurate test for the assessment of uterine congenital anomalies. Its ability to concomitantly visualized, the external uterine contour with the uterine cavity on the same coronal plan, makes this noninvasive, easy to perform test the procedure of choice for the diagnosis of uterine anomalies.
