Weight regain after intragastric balloon for pre-surgical weight loss

BAcKGROUND:Ectopic pancreas is defined as pancreatic tissue found outside its usual anatomical position,with no ductal or vascular communication with the native pancreas. We describe a case of ectopic pancreas of the small bowel and mesentery causing recurrent episodes of pancreatitis, initially suspected on computed tomography(cT)and magnetic resonance cholangiopancreatography(MRcP),and confirmed on histological review of the resection. METHODS:A 67-year-old woman presented with clinical symptoms and biochemical evidence of *** had similar episodes over the past 30 years with unrevealing investigations,and was concluded to have idiopathic pancreatitis. She underwent cT and MRcP,with findings suggestive of ectopic pancreas,a diagnosis confirmed on histology of the resection. RESULTS:MRcP identified a mass in the proximal small bowel mesentery isointense to the native pancreas,with a small duct draining into a proximal jejunal *** resected specimen consisted of normal parenchyma with lobulated acinar tissue with scattered islets of Langerhans,an occasional ductular structure,and admixed areas of adipose *** patient remained asymptomatic with normal biochemistry six months post-operatively. cONcLUSION:In an individual with abdominal pain,elevated serum amylase/lipase,but imaging findings of a normal native pancreas,ectopic pancreatitis should be considered,and can be evaluated by cT and MRcP.

Brine shrimp(Artemia)has existed on Earth for 400 million years and has major ecological importance in hypersaline *** a crucial live food in aquaculture,brine shrimp cysts have become one of the most important aquatic products traded ***,our understanding of the biodiversity,prevalence and global connectedness of viruses in brine shrimp is still very limited.A total of 143 batches of brine shrimp(belonging to seven species)cysts were collected from six continents including 21 countries and more than 100 geographic locations worldwide during 1977–*** total,55 novel RNA viruses were identified,which could be assigned to 18 different viral families and related *** viruses were dsRNA viruses,16 were+ssRNA viruses,and 28 were−ssRNA *** analyses of the RNA-directed RNA polymerase(RdRp)showed that brine shrimp viruses were often grouped with viruses isolated from other invertebrates and ***,most brine shrimp viruses were related to those from different hosts that might feed on brine shrimp or share the same ecological niche.A notable case was the novel brine shrimp noda-like virus 3,which shared 79.25%(RdRp)and 63.88%(capsid proteins)amino acid identity with covert mortality nodavirus(cMNV)that may cause losses in *** addition,both virome composition and phylogenetic analyses revealed global connectedness in certain brine shrimp viruses,particularly among Asia and Northern *** highlights the incredible species diversity of viruses in these ancient species and provides essential data for the prevalence of RNA viruses in the global aquaculture *** broadly,these findings provide novel insights into the previously unrecognized RNA virosphere in hypersaline ecosystems worldwide and demonstrate that human activity might have driven the global connectedness of brine shrimp viruses.

The bioconversion of lignocellulose has attracted global attention,due to the significant potential of agricultural and forestry wastes as renewable zero-carbon resources and the urgent need for substituting fossil *** cellulosome system is a multi-enzyme complex produced by anaerobic bacteria,which comprises cellulases,hemicellulases,and associated enzymatic and non-enzymatic components that promote biomass *** enhance their efficiency in degrading recalcitrant lignocellulosic matrices,cellulosomes have been employed to construct biocatalysts for lignocellulose bioconversion,such as consolidated bioprocessing and consolidated ***,the second most abundant polysaccharides in plant cell walls,hold valuable application potential but can also induce inhibitory effects on cellulose hydrolysis,thus highlighting the indispensable roles of hemicellulases within the cellulosome *** review evaluated current research on cellulosomal hemicellulases,comparing their types,abundance,and regulation,primarily focusing on eight known cellulosome-producing species of different *** also reviewed their growth conditions,their hemicellulose-degrading capabilities,and the inhibitory effects of hemicellulose on cellulosome-based lignocellulose ***,we proposed strategies for targeted enhancement of hemicellulase in cellulosomes to improve lignocellulose bioconversion in future studies.

背景：微骨折技术常用于修复膝关节软骨损伤。虽然该技术目前已广泛应用，但有关临床疗效的前瞻性研究尚不多，本研究旨在分析骨修复技术影响临床结果的因素。方法：48例关节稳定性正常、有症状的单发股骨髁全层软骨损伤患者采用微骨折技术进行治疗。术后随访至少24个月，采用公认的功能评分系统、主观评分及软骨敏感MRI扫描序列进行前瞻性评价。结果：膝关节功能优良者32例（67％），可12例（25％），差4例（8％）。日常生活能力评分、国际膝关节论证委员会（Inter-national Knee Documentation committee，IKDc）评分及SF-36评分均明显增加（p〈0．05）。身高体重指数小可以获得日常生活能力和SF-36的高评分，身高体重指数〉30kg／m^2者得分最差。术前病程短于12个月者可以明显改善日常生活能力评分（p〈0．05）。24例接受MR检查的病例显示13例（54％）软骨修复、充填好，7例（29％）充填中等，4例（17％）充填差。充填程度与膝关节功能评分密切相关。所有充填好的病例功能均有所改善，而充填差的病例不仅功能改善有限，而且24个月后出现功能评分降低。结论：通过2年以上的随访证实，采用微骨折技术修复膝关节软骨损伤可以明显改善术后功能。软骨修复、充填好，身高体重指数小及术前病程短者可以获得最佳的短期疗效。身高体重指数大对短期疗效有负面影响，软骨修复、充填差的病例疗效维持时间短。可信水平：治疗性研究，Ⅳ级，进一步可信度参见作者介绍。

The combination of urbanization and global warming leads to urban overheating and compounds the frequency and intensity of extreme heat events due to climate ***,the risk of urban overheating can be mitigated by urban green-blue-grey infrastructure(GBGI),such as parks,wetlands,and engineered greening,which have the potential to effectively reduce summer air *** many reviews,the evidence bases on quantified GBGI cooling benefits remains partial and the practical recommendations for implementation are *** systematic literature review synthesizes the evidence base for heat mitigation and related co-benefits,identifies knowledge gaps,and proposes recommendations for their implementation to maximize their *** screening 27,486 papers,202 were reviewed,based on 51 GBGI types categorized under 10 main *** GBGI(green walls,parks,street trees)have been well researched for their urban cooling ***,several other GBGI have received negligible(zoological garden,golf course,estuary)or minimal(private garden,allotment)*** most efficient air cooling was observed in botanical gardens(5.0±3.5℃),wetlands(4.9±3.2℃),green walls(4.1±4.2℃),street trees(3.8±3.1℃),and vegetated balconies(3.8±2.7℃).Under changing climate conditions(2070–2100)with consideration of RcP8.5,there is a shift in climate subtypes,either within the same climate zone(e.g.,Dfa to Dfb and cfb to cfa)or across other climate zones(e.g.,Dfb[continental warm-summer humid]to BSk[dry,cold semi-arid]and cwa[temperate]to Am[tropical]).These shifts may result in lower efficiency for the current GBGI in the *** the importance of multiple services,it is crucial to balance their functionality,cooling performance,and other related co-benefits when planning for the future *** global GBGI heat mitigation inventory can assist policymakers and urban planners in prioritizing effective interventions to reduce the risk of urban overheating,fi

The Soft X-ray Imager(SXI)is part of the scientific payload of the Solar wind Magnetosphere Ionosphere Link Explorer(SMILE)*** is a joint science mission between the European Space Agency(ESA)and the chinese Academy of Sciences(cAS)and is due for launch in *** is a compact X-ray telescope with a wide field-of-view(FOV)capable of encompassing large portions of Earth’s magnetosphere from the vantage point of the SMILE *** is sensitive to the soft X-rays produced by the Solar Wind charge eXchange(SWcX)process produced when heavy ions of solar wind origin interact with neutral particles in Earth’s *** provides a mechanism for boundary detection within the magnetosphere,such as the position of Earth’s magnetopause,because the solar wind heavy ions have a very low density in regions of closed magnetic field *** sensitivity of the SXI is such that it can potentially track movements of the magnetopause on timescales of a few minutes and the orbit of SMILE will enable such movements to be tracked for segments lasting many *** is led by the University of Leicester in the United Kingdom(UK)with collaborating organisations on hardware,software and science support within the UK,Europe,china and the United States.

背景：微骨折是常用于修复膝关节软骨病损的技术。尽管该项技术应用很普遍，但有关远期临床结果的信息仍很有限。本研究的目的是找出该项软骨修复技术临床疗效的影响因素。方法：采用微骨折技术治疗48例膝关节稳定、股骨髁孤立、全层软骨缺损并有症状的患者。最短随访24个月，采用公认的疗效评分和主观临床评级及软骨敏感NRI扫描对患者的结果进行前瞻性评估。结果：末次随访时，膝关节功能优、良32例（67％），可12例（25％），差4例（8％）。日常活动评分、国际膝关节信息委员会（IKDc）评分、SF-36体格检查评分术后均有显著性增加（p〈0.05），体重指数（BM）较低，则日常活动评分SF-36体格检查评分较高，BMI〉30kg/m^2则效果最差。术前症状时间少于12个月，则日常活动评分显著增加（p〈0.05）。24膝中MRI证实缺损填充，修复良好13例（54%），中等填充7例（29%），填充不良4例（17%），填充度与膝关节功能评分相关。全部缺损填充良好的膝关节，功能均有改善，而填充不良的关节，功能改善有限，且24个月后功能评分下降。结论：微骨折技术修复膝关节软骨病损在最短2年的随访期间功能有显著改善，软骨缺损填充良好，BMI较低，术前症状期短者可取得最佳短期疗效；高BMI对短期疗效有负面影响，软件缺损填充不良则短期疗效较差。

Residential building performance analysis at near extreme weather conditions in Hong Kong through a thermal-comfort-based strategy

The precise building performance assessment of residential housings in subtropical regions is usually more difficult than that for the commercial premises due to the much more complicated behavior of the occupants with regard to the change in indoor *** conventional use of a fixed schedule for window opening,clothing insulation and cooling equipment operation cannot reflect the real situation when the occupants respond to the change in thermal comfort,thus affecting the appropriateness of the assessment *** rectify the situation,a new modeling strategy in which the modification of the various operation schedules was based on the calculated thermal comfort(Tc),was developed in this *** this new Tc-based strategy,the realistic building performances under different cooling provision scenarios applied to a high-rise residential building under the near extreme weather conditions were investigated and *** was found that sole provision of ventilation fans could not meet the zone thermal comfort by over 68%of the time,and air-conditioning was *** optimal use of ventilation fans for cooling could only help reduce the total cooling energy demand by less than 12%at best which could only be realistically evaluated by adopting the present *** studies were conducted which revealed that some design factors could offer opportunities for reducing the total cooling energy under the near extreme weather conditions.
