No difference in mortality among ALPPS,two-staged hepatectomy,and portal vein embolization/ligation:A systematic review by updated traditional and network meta-analyses

Background:There is an ongoing debate on the feasibility,safety,and oncological efficacy of the associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)*** aim of this study was to compare ALPPS,two-staged hepatectomy(TSH),and portal vein embolization(PVE)/ligation(PVL)using updated traditional meta-analysis and network meta-analysis(NMA).Data sources:Electronic databases were used in a systematic literature *** traditional metaanalysis and NMA were performed and *** and major morbidity were selected as primary ***:Nineteen studies including 1200 patients were selected from the pool of 436 *** these patients,315(31%)and 702(69%)underwent ALPPS and portal vein occlusion(PVO),*** mortality based on updated traditional meta-analysis,subgroup analysis of the randomized controlled trials(RCTs),and both Bayesian and frequentist NMA did not demonstrate significant differences between the ALPPS cohort and the PVE,PVL,and TSH ***,analysis of RCTs did not demonstrate significant differences of major morbidity between the ALPPS and PVO *** ALPPS cohort demonstrated significantly more favorable outcomes in hypertrophy parameters,time to operation,definitive hepatectomy,and R0 margins rates compared with the PVO *** contrast,1-year disease-free survival was significantly higher in the PVO cohort compared to the ALPPS ***:This study is the first to use updated traditional meta-analysis and both Bayesian and frequentist NMA and demonstrated no significant differences in 90-day mortality between the ALPPS and other hepatic hypertrophy ***,two high quality RCTs including 147 patients demonstrated no significant differences in major morbidity between the ALPPS and PVO cohorts.

Hepatocellular carcinoma(HCC)is the most common primary liver tumor and has been considered a very immunogenic *** treatment with radiofrequency ablation(RFA)has been established as the standard ablative therapy for early HCC,and is currently recognized as the main ablative tool for HCC tumors<5 cm in size;however,progression and local recurrence remain the main disadvantages of this *** solve this clinical problem,recent efforts were concentrated on multimodal treatment,combining different strategies,including the combination of RFA and *** article reviewed the combination treatment of RFA with immunotherapy and found that this treatment strategy leads to an increased response of anti-tumor T cells,significantly reduces the risk of recurrence and improves survival rates compared to RFA *** review highlighted scientific evidence that supports the current recommendations for pre-clinical studies,and discuss the need for further research on this topic.

Affective responses to supervised 10-week programs of resistance exercise in older adults

Background:Resistance exercise has numerous,well-documented benefits on the health and functional performance of older ***,little information exists on the affective responses to resistance exercise in this *** affective responses can predict continued exercise behavior,examining if and how they differ between resistance exercise intensities and frequencies in older adults may provide important data to improve resistance exercise ***:We monitored the affective responses of older adults when performing high-velocity,low-load(HVLL)or low-velocity,high-load(LVHL)resistance exercise once or twice weekly.A total of 40 moderately to highly active,but resistance-exercise naïve,older adults(60-79 years)completed 1 of 4 randomly assigned progressive resistance exercise conditions:HVLL once weekly(HVLL1:n=10),HVLL twice weekly(HVLL2:n=10),LVHL once weekly(LVHL1:n=10),or LVHL twice weekly(LVHL2:n=10).The Physical Activity Affect Scale,Felt Arousal Scale,Feeling Scale,rating of perceived exertion,Visual Analog Scale,and the Physical Activity Enjoyment Scale were used to assess enjoyment and affective ***:The results from Physical Activity Enjoyment Scale(out of 35)suggest that all exercise conditions were largely and similarly enjoyable to older adults(HVLL1:30.9±2.4;LVHL1:31.9±3.6;HVLL2:30.9±3.5;LVHL2:30.2±3.7)despite greater fatigue(p=0.033;ηp^2p=0.22)and perceived workload(p=0.042;ηp^2=0.20)in LVHL(results from Visual Analog Scale).Conclusion:Moderately to highly physically active older adults may tolerate higher intensities of resistance exercise performed once or twice weekly without experiencing a negative impact on ***,the role that supervision and social interaction played in shaping the participants’responses is unclear.

Kallistatin,a new and reliable biomarker for the diagnosis of liver cirrhosis

Kallistatin, which protects organs and cells against inflammation, fibrosis and oxidative stress, is mainly synthesized and secreted in liver. However, its relationship to human liver disease remains unclear. The purpose of this study was to explore the relationship between serum kallistatin and clinical evidence of both cirrhosis and hepatocellular carcinoma (HCC), and to determine if serum kallistatin levels could be used as a diagnostic indicator of hepatic health status, especially human liver cirrhosis (LC). Our cohort consisted of 115 patients with clinically proven liver fibrosis (IT), LC, or HCC by liver biopsies, and 31 healthy controls (CON). Serum kallistatin levels were quantified by ELISA. Results of the present study demonstrated that irrespective of the underlying etiology, serum kallistatin levels were significantly lower in the LF/LC group when compared with the CON group. A decrease in serum kallistatin levels appeared to reflect the extent of cirrhosis, with the lowest levels associated with higher grades of cirrhosis. Patients with 111: had a noticeable correlation between serum kallistatin levels and other serum biochemical indicators. 'the area under the curve (AUC) for LC, viral liver cirrhosis (VLC) and alcoholic liver cirrhosis (ALC) was 0.845, 0.757 and 0.931, respectively. in conclusion, our findings demonstrated that kallistatin, a plasma protein produced by the liver, can be a useful and reliable diagnostic indicator of hepatic health status, especially for LC. Treatment of RuCl2(PPh3)3 with 6-dimethylaminopentafulvene in THF in the presence of water produced(η5-C5H4CHO) RuCl(PPh3)2, which was reduced by NaBH4 to give the Ru–H···HO dihydrogen bonded complex(η5-C5H4CH2OH) RuH(PPh3)2. The dihydrogen bonded complex(η5-C5H4CH2OH)RuH(PPh3)2 could also be synthesized by the reduction of complex(η5-C5H4CHO)RuH(PPh3)2, which was obtained by the reaction of RuHCl(PPh3)3 with 6-dimethylaminopentafulvene in the presence of water. The analogous dihydrogen bonded osmium complex(η5-C5H4CH2OH)OsH(PPh3)2 was similarly prepared. Single crystal structures and DFT calculations support the presence of intra-molecular H···H interaction, with separations of around 1.9 to 2.0 .

Cotton stomatal closure under varying temperature and vapor pressure deficit,correlation with the hydraulic conductance trait

Background:Cotton(Gossypium hirsutum L.)is often grown in locations characterized by high atmospheric evaporative *** has been hypothesized that plants which resist hydraulic flow under this condition will limit water use and conserve soil ***,in a series of controlled environment experiments ten cotton cultivars were exposed to two different temperature and vapor pressure deficit(VPD)conditions(i.e.,38℃,>3 kPa and 32℃,1∼1.5 kPa)as well as a progressive soil ***,individual differences in shoot hydraulic conductance(K_(shoot))was measured using a hydraulic conductance flow meter(HCFM).Physiological parameters were reported included leaf area,dry leaf weight,stomatal conductance(g_(s)),and water use efficiency coefficient(WUE_(k)).Results:Differences were observed in K_(shoot) among cultivars under the 38℃,>3 kPa but not the 32℃,1∼1.5 kPa *** the 38℃,>3 kPa environment,correlations were found between K_(shoot),stomatal conductance(gs),VPD breakpoint,WUEk,total leaf area,dry leaf weight,fraction transpirable soil water(FTSW)threshold,and slope of TR decline after FTSW ***:Results show that the ability of some cotton cultivars to restrict water loss under high evaporative demand through early stomatal closure is associated with the cultivars’K_(shoot).The K_(shoot) is influential in the limitation of TR trait under high temperature and VPD.

We report the discovery of three new pulsars in the globular cluster(GC)NGC 6517,namely NGC 6517 E,F and G,made with the Five-hundred-meter Aperture Spherical radio Telescope(FAST).The spin periods of NGC 6517 E,F and G are 7.60 ms,24.89 ms and 51.59 ms,*** dispersion measures are 183.29,183.713 and 185.3 pc cm^(-3),respectively,all slightly larger than those of the previously known pulsars in this *** spin period derivatives are at the level of 1×10^(-18) s s^(-1),which suggests these are recycled *** addition to the discovery of these three new pulsars,we updated the timing solutions of the known isolated pulsars,NGC 6517 A,C and *** solutions are consistent with those from Lynch et *** with smaller timing *** the timing solution,NGC6517 A,B(position from Lynch et al.),C,E and F are very close to each other on the sky and only a few arcseconds from the optical core of NGC *** currently published and unpublished discoveries,nine pulsars have been discovered in NGC 651,ranking it 6 thfor GCs with the most known *** discoveries take advantage of the high sensitivity of FAST and a new algorithm used to check and filter possible candidate signals.

Demographic variation and habitat specialization of tree species in a diverse tropical forest of Cameroon

Background： Many tree species in tropical forests have distributions tracking local ridge-slope-valley topography. Previous work in a 50-ha plot in Korup National Park, Cameroon, demonstrated that 272 species, or 63% of those tested, were significantly associated with topography. Methods： We used two censuses of 329,000 trees ≥1 cm dbh to examine demographic variation at this site that would account for those observed habitat preferences. We tested two predictions. First, within a given topographic habitat, species specializing on that habitat （＇residents＇） should outperform species that are specialists of other habitats （＇foreigners＇）. Second, across different topographic habitats, species should perform best in the habitat on which they specialize （＇home＇） compared to other habitats （＇away＇）. Species＇ performance was estimated using growth and mortality rates. Results： In hierarchical models with species identity as a random effect, we found no evidence of a demographic advantage to resident species. Indeed, growth rates were most often higher for foreign species. Similarly, comparisons of species on their home vs. away habitats revealed no sign of a performance advantage on the home habitat. Conclusions＂ We reject the hypothesis that species distributions along a ridge-valley catena at Korup are caused by species differences in trees _〉1 cm dbh. Since there must be a demographic cause for habitat specialization, we offer three alternatives. First, the demographic advantage specialists have at home occurs at the reproductive or seedling stage, in sizes smaller than we census in the forest plot. Second, species may have higher performance on their preferred habitat when density is low, but when population builds up, there are negative density-dependent feedbacks that reduce performance. Third, demographic filtering may be produced by extreme environmental conditions that we did not observe during the census interval.

Chronic obstructive pulmonary disease in adults with human immunodeficiency virus infection: a systematic review

Objective: To determine the prevalence of chro- nic obstructive pulmonary disease (COPD) in adults with Human Immunodeficiency virus infection (HIV). Design: Systematic review of Medline, Embase, CINAHL, PsycINFO and references from identified papers. Study selection: Studies determining the prevalence of COPD in adults with HIV infection. Independent duplicate data extraction. Study quality was assessed in terms of whether consecutive patients were en- rolled, recruitment and follow-up periods were defined, 60% males, 60% on antiretroviral therapy. COPD was diagnosed by post-bronchodilator FEV1/FVC < 0.7 in three studies, by International Classification of Diseases (ICD-9) codes in three studies, by FEV1/FVC < 5% of lower adjusted normal in one and by pre- bronchodilator FEV1/FVC < 0.7 in another study. The prevalence was 10% - 35%, except for one study that recorded prevalence of 4% by postbronchodilator FEV1/FVC < 0.7, but <38% of patients with prebronchodilator FEV1/FVC < 0.7 had post-bronchodilator spirometry in that study. Conclusion: COPD is becoming increasingly common in HIV infected as they smoke and live longer due to efficient antiretrovirals. However, definite conclusions cannot be drawn and more longitudinal studies are needed. In the meantime health care providers should be vigilant to screen for undiagnosed COPD and hesitant to attribute respiratory symptoms solely to HIV infection.
