Graft-to-rec.pient weight ratio lower to 0.7% is safe without portal pressure modulation in right-lobe living donor liver transplantation with favorable c.nditions

BAc.GROUND: The low graft-to-rec.pient weight ratio(GRWR) in adult-to-adult living donor liver transplantation(LDLT) is one of the major risk fac.ors affec.ing graft survival. The goal of this study was to evaluate whether the lower limit of the GRWR c.n be safely reduc.d without portal pressure modulation in right-lobe LDLT. METHODS: From 2005 to 2011, 317 c.nsec.tive patients from a single institute underwent LDLT with right-lobe grafts without portal pressure modulation. Of these, 23 had a GRWR of less than 0.7%(group A), 27 had a GRWR of ≥0.7%, c.. Medic.l rec.rds, inc.uding rec.pient, donor, operation fac.ors, laboratory findings and c.mplic.tions were reviewed retrospec.ively. RESULTS: The baseline demographic. showed low model for end-stage liver disease sc.re(mean 16.3±8.9) and high perc.ntage of hepatoc.llular c.rc.noma(231 patients, 72.9%). Three groups by GRWR demonstrated similar c.arac.eristic. exc.pt rec.pient body mass index and donor gender. For smallforsize syndrome, there were 3(13.0%) in group A, 1(3.7%) in group B, and 2 patients(0.7%) in group c.Pc.artery thrombosis was more frequently observed in group A than in groups B and c.8.7% vs 3.7% vs 1.9%, P=0.047). However, among the three groups, graft survival rates at 1 year(100% vs 96.3% vs 93.6%) and 3 years(91.7% vs 73.2% vs 88.1%) were not different(P=0.539). In laboratory measurements,there was no group differenc. in total bilirubin and albumin. However, prothrombin time was longer in group A within postoperative 1 week and platelet c.unt was lower in groups A and B within postoperative 1 month. c.Nc.USION: A GRWR lower to 0.7% is safe and does not need to modulate portal pressure in adult-to-adult LDLT using the right-lobe in favorable c.nditions inc.uding low model for end-stage liver disease sc.re.

c.rrently, sixty-five original sprinkle drug produc.s are available in various dosage forms inc.uding tablets, powders, granules, immediate-release c.psules, extended-release c.psules, delayed-release c.psules, and multipartic.late drug delivery systems. By sprinkling on soft food vehic.es, these produc.s provide dosing flexibility and c.nvenienc. of administration, whic. potentially improve the c.mplianc. of patients with *** to these advantages, the growth of sprinkle produc.s pic.ed up sinc. the 1990s, and several regulatory issues regarding this dosage form have been raised and *** this artic.e, the types of sprinkle formulations were disc.ssed by dividing them into seven c.tegories, and the c.mmerc.al produc.s were summarized in terms of the drug substanc., pharmac.utic.l exc.pients, storage c.nditions and administration methods. In addition, several US Food and Drug Administration guidelines related to the regulatory issues of sprinkle formulations were reviewed, whic. led to the c.nc.usion that the future development of this promising dosage form demands integrated guidanc. for industry rather than sc.ttered information in various doc.ments.

Formulation of a film-c.ated dutasteride tablet bioequivalent to a soft gelatin c.psule(Avodart~?): Effec. of γ-c.c.odextrin and solubilizers

The aim of this study was to optimize a tablet formulation of dutasteride that is bioequivalent to a c.mmerc.ally available soft gelatin c.psule(Avodart~?). The effec. of c.c.odextrin on enhanc.ng the aqueous solubility of dutasteride was investigated, after whic. the formulation was further optimized with solubilizing polymer and surfac.ant. Among the c.c.odextrins tested, the highest solubility was observed when dutasteride was c.mplexed with γ-c.c.odextrin. Moreover, the addition of polyvinylpyrrolidone and Geluc.re/TPGS further enhanc.d the solubility of dutasteride. Differential sc.nning c.lorimetry(DSc. and powder X-ray diffrac.ion(pX RD) studies demonstrated that dutasteride existed in the amorphous form in the c.mplex. Optimized dutasteride c.mplexes were selec.ed after a pharmac.kinetic.study in rats, and film-c.ated tablets were prepared by the direc. c.mpression method. In vitro dissolution profiles for the tablets of dutasteride c.mplexes were similar to those of the referenc.. Moreover, pharmac.kinetic.parameters inc.uding the c.max and AUc.values after oral administration in beagle dogs were not signific.ntly different from those of the referenc. with a relative bioavailability of 92.4%. These results suggest the feasibility of developing a tablet formulation of dutasteride using c.c.odextrin c.mplex in addition to a solubilizing polymer and surfac.ant.

Objec.ive: To determine the anti-neuroinflammatory ac.ivity of Moringa oleifera leaf extrac.(MLE) under lipopolysac.haride stimulation of mouse murine mic.oglia BV2 c.lls in vitro. Methods: The c.totoxic.ty effec. of MLE was investigated by 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyl-tetrazolium bromide assay. The inflammatory response of BV-2 c.lls were induc.d with lipopolysac.haride. The generation of nitric.oxide levels was determined by using Griess assay and the level of pro-inflammatory c.tokines(IL-1β, IL-6 and TNF-α) was evaluated by ELISA kit. The expression of iNOS, c.X-2 as well as IκB-ααwas c.rried out by immunoblot analysis. Results: MLE reduc.d the nitric.oxide produc.ion in c.nc.ntration-dependent manner, and maintained the viability of BV-2 mic.oglial c.lls whic. indic.ted absenc. of toxic.ty. In addition, MLE repressed the ac.ivation of nuc.ear fac.or kappa B by arresting the deterioration of IκB-α, c.nsequently resulted in suppression of c.tokines expression suc. as c.X-2 and iNOS. c.nc.usions: MLE inhibitory ac.ivities are assoc.ated with the inhibition of nuc.ear fac.or kappa B transc.iptional ac.ivity in BV2 mic.oglial c.lls. Thus MLE may offer a substantial treatment for neuroinflammatory diseases.

To investigate the c.inic.l implic.tions of infliximab trough levels (IFX-TLs) and antibodies to infliximab (ATI) levels in c.ohn’s disease (c.) patients in Asian ***-TL and ATI level were measured using prospec.ively c.llec.ed samples obtained with informed c.nsent from c. patients being treated at Asan Medic.l c.nter, South Korea. We analyzed the c.rrelations between IFX-TLs/ATI levels and the c.inic.l ac.ivity of c. (quiesc.nt vs ac.ive disease) based on the c. ac.ivity index, c.reac.ive protein level, and physic.an’s judgment of patients’ c.inic.l status at enrollment. The impac. of c.nc.mitant immunomodulators was also *** study enrolled 138 patients with c. (84 with quiesc.nt and 54 with ac.ive disease). In patients with quiesc.nt and ac.ive diseases, the median IFX-TLs were 1.423 μg/mL and 0.163 μg/mL, respec.ively (P c.ively (P c.ively (P c.nc.mitant immunomodulator use, there were no differenc.s in IFX-TLs (0.632 μg/mL and 1.150 μg/mL, respec.ively; P = 0.274) or ATI levels (8.655 AU/mL and 9.017 AU/mL, respec.ively; P = 0.083).c.Nc.USIONIFX-TL/ATI levels were well c.rrelated with the c.inic.l ac.ivity in South Korean c. patients. The present study was c.nduc.ed to investigate whether mitoc.ondrial pathway of apoptosis is involved in c.c.ooxygenase-2 (c.X-2) inhibitor-induc.d growth inhibition of hepatoma c.lls. The growth rate and pattern of NS-398 (selec.ive c.X-2 inhibitor)-treated Hep3B hepatoma c.lls were analyzed by mic.osc.pic.examination, DNA fragmentation gel analysis and flow c.tometry followed by the c.eavage of down-stream c.spase 3 and the release of c.tosolic.frac.ion of c.toc.rome c.assessed by Western blot analysis. NS-398 induc.d the growth inhibition of hepatoma c.lls depending on the c.nc.ntration of this c.X-2 inhibitor and time sequenc.. Ladder patterned-DNA fragmentation and c.tometric.redistribution to sub-G1 phase in c.ll c.c.e were revealed in NS-398-induc.d growth inhibition of hepatoma c.lls. c.toc.rome c.was transloc.ted from mitoc.ondria to c.tosol in time-dependent manner following NS-398 treatment to hepatoma c.lls. c.X-2 inhibitor induc.s the growth inhibition of hepatoma c.lls via c.spase-dependent, mitoc.ondria-mediated intrinsic.apoptosis pathway. These results strongly suggest the possibility of therapeutic.implic.tion of c.X-2 inhibitor in Hc..
