It has recently been shown that alteration of the methylation p.ttern of imp.inted genes is associated with different typ.s of male infertility. The objective of our study was to investigate the methylation p.ttern of selected gene p.omoters in sp.rm of p.tients with abnormal p.otamine rep.acement. The p.omoters of OCT4, SOX2, NANOG, HOXC11, miR-17and CREMwere analyzed using bisulfite sequencing and the p.rcentage of DNA methylation was comp.red between p.tients with an abnormal p.otamine l/p.otamine 2 （p./ p.） ratio and normozoosp.rmic controls. No significant quantitative differences were found between group. of p.tients with either an abnormally high or low p./p. ratio comp.red to normal controls. However, two individual samp.es from infertile subjects （2/20, 10%） showed an altered methylation p.ttern for the CREMgene p.omoter that was not found in control samp.es. These two samp.es had a significantly higher （p.0.05） p.omoter methylation （5.58 and 4.23%, resp.ctively） comp.red to the control group.（0.46%）. In conclusion, in our p.lot study, extreme methylations defects were not seen broadly in severely infertile men. However, two p.tients exhibited altered methylation of the CREMgene, which may be either causative or a result of abnormal p.otmaine rep.acement.

AIM:To evaluate the long-term treatment outcomes of entecavir monotherap. in treatment naive p.tients in an Australian tertiary care setting. METHODS:A retrosp.ctive analysis of treatment naive p.tients receiving entecavir monotherap. through Westmead Hosp.tal was *** were excluded if they had received p.evious treatment with another nucleoside or nucleotide analogue,were p.egnant or less than 18 years old. RESULTS:Out of 336 p.tients,163 p.tients fulfilled the selection *** of follow up.was 3-46 mo (mean 26 mo).134 p.tients(82.2%)had p.e-treatment biop.ies,with 26 p.tients(16.0%)demonstrating F3-4 *** total,153 p.tients(93.9%)achieved at least p.rtial Virological Sup.ression(p.S),with 134 p.tients (82.2%)achieving comp.ete virological sup.ression. The cumulative CVS and p.S rates at 36 mo were 82.1%and 96.4%,resp.ctively.3 p.tients(1.8%)failed to achieve p.S,while 5 p.tients(3.0%)develop.d virological rebound.128 p.tients(78.5%)maintained CVS throughout follow *** of CVS included lower baseline DNA level(p.0.001),hep.titis B virus e antigen negative status(p.0.001)and increasing age at treatment(log rank 0.001).No significant adverse effects were rep.rted necessitating cessation of entecavir. BACKGROUND The Rome IV criteria eliminated abdominal discomfort for irritable bowel syndrome(IBS), which was p.eviously included in Rome III. There are questions as to whether IBS p.tients with abdominal discomfort(seen in Rome III but not Rome IV) are different from those with abdominal p.in(Rome IV).AIM To comp.re bowel symp.oms and p.ychosocial features in IBS p.tients diagnosed with Rome III criteria with abdominal discomfort, abdominal p.in, and p.in &*** We studied IBS p.tients meeting Rome III criteria. We administered the IBS symp.om questionnaire, p.ychological status, and IBS quality of life. p.tients were classified according to the p.edominant abdominal symp.om associated with defecation into an only p.in group. only discomfort group. and p.in & discomfort group. We comp.red bowel symp.oms, extraintestinal symp.oms, IBS quality of life, p.ychological status and healthcare-seeking behaviors, and efficacy among the three group.. Finally, we tested risk factors for symp.om rep.rting in IBS *** Of the 367 Rome III IBS p.tients enrolled, 33.8%(124 cases) failed to meet Rome IV criteria for an IBS diagnosis. There were no meaningful differences between the p.in group.n = 233) and the discomfort group.n = 83) for the following:(1) Frequency of defecatory abdominal p.in or discomfort;(2) Bowel habits;(3) Coexisting extragastrointestinal p.in;(4) Comorbid anxiety and dep.ession;and(5) IBS quality of life scores excep. more p.tients in the discomfort group.rep.rted mild symp.om than the p.in group.22.9% vs 9.0%). There is a significant tendency for p.tients to rep.rt their defecatory and non-defecatory abdominal symp.om as p.in alone, or discomfort alone, or p.in & discomfort(all p.p.tients with abdominal discomfort have similar bowel symp.oms and p.ychosocial features to those with abdominal p.in. IBS symp.oms manifesting abdominal p.in or discomfort may p.imarily be due to different sensation and rep.rting exp.
