Simulated Microbe Removal around Finger Rings Using Different Hand Sanitation Methods

Aim It is our op.nion that the CDC and the WHO have underestimated cross-contamination under examination gloves in dental clinics while wearing jewelry, such as finger rings. These agencies only ＂recommend＂ removing jewelry, and only washing hands for 15 seconds with soap.and warm water before donning gloves. This study examined several washing p.ocedures and finger rings using simulated microbes. Methodology A gloved robber hand manikin was made and fitted with a flesh disp.sable vinyl glove. Four fingers were fitted with rings or no ring, dusted with simulated microbes, and washed with a scrub brush for 5, 15, and 25 seconds under 20℃ and 40℃ water alone, or with liquid hand soap. Light levels （in lux） of fluorescent p.wder before and after washing were measured and delta scores calculated for changes in light levels, equivalent to effectiveness of hand washing p.ocedures. A full-factorial, 3-factor analysis of variance （ANOVA） was used to test for differences among levels of the three study factors-time, temp.rature, and soap.use. Tukey＇s p.st hoc honestly significant difference （HSD） test was ap.lied to significant factors to examine p.ir-wise differences between factor levels. Results It was found that the longer the hands with rings were washed with a scrub brush under flowing water, the more simulated microbes were removed. By 25 seconds, all methods were essentially the same. Simulated microbes were more difficult to remove from the p.lm comp.red to the back of the hand. The liquid hand soap.used in this study was more effective with warm water than cold. When given a choice of washing with cold water up.to 15 seconds, it would be p.eferable not to use soap.to remove simulated microbes. Qualitatively, the outer surface of finger rings were more effectively cleaned than the crevice below the ring, and the ring with a stone setting ap.eared to accumulate and retain simulated microbes more than other rings. Alemtuzumab: A p.ace in Therap. for Treatment of Multip.e Sclerosis

Alemtuzumab is a humanized mononclonal antibody known to cause rap.d dep.etion of B-and T-cell lymp.ocytes. Subsequent rep.etion of these lymp.ocytes leads to changes in adap.ive immunity. Alemtuzumab is ap.roved by the United States Food and Drug Administration (FDA) for the treatment of B-cell lymp.ocytic leukemia but has been investigated off-label in recent years for treatment of autoimmune diseases, including multip.e sclerosis (MS). In MS treatment, alemtuzumab is administered as p.lsed therap., given once daily initially for 5 consecutive days and then for 3 consecutive days at 12-month intervals. Alemtuzumab has recently been comp.red to interferon beta 1-a in one p.ase II and two p.ase III trials in p.tients with relap.ing-remitting MS. Results from the studies show alemtuzumab comp.red to interferon beta 1-a is associated with a greater reduction in the risk of sustained accumulation of disability and is more effective in reducing disease relap.e rates. The treatment of MS continues to be a healthcare challenge due to the modest clinical benefit and adverse effect p.ofiles of available disease modifying treatment op.ions. Available data suggest alemtuzumab may offer better efficacy outcomes comp.red to traditional disease modifying therap.es in p.tients with MS. However, the agent has not been comp.red to other new disease modifying medications that have been recently introduced.

Evaluation of the up.ated definition of early allograft dysfunction in donation after brain death and donation after cardiac death liver allografts

BACKGROUND:An up.ated definition of early allograft dysfunction(EAD) was recently validated in a multicenter study of 300 deceased donor liver transp.ant *** analysis did not differentiate between donation after brain death(DBD) and donation after cardiac death(DCD) allograft ***:We reviewed our p.osp.ctively entered database for all DBD(n=377) and DCD(n=38) liver transp.antations between January 1,2006 and October 30,*** incidence of EAD as well as its ability to p.edict graft failure and survival was comp.red between DBD and DCD ***:EAD was a valid p.edictor of both graft and p.tient survival at six months in DBD allograft recip.ents,but in DCD allograft recip.ents there was no significant difference in the rate of graft failure in those with EAD(11.5%) comp.red with those without EAD(16.7%)(p.0.664) or in the rate of death in recip.ents with EAD(3.8%) comp.red with those without EAD(8.3%)(p.0.565).The graft failure rate in the first 6 months in those with international normalized ratio ≥1.6 on day 7 who received a DCD allograft was 37.5% comp.red with 6.7% for those with international normalized ratio p.0.022).CONCLUSIONS:The recently validated definition of EAD is a valid p.edictor of p.tient and graft survival in recip.ents of DBD *** initial assessment,it does not ap.ear to be a useful p.edictor of p.tient and graft survival in recip.ents of DCD allografts,however a study with a larger samp.e size of DCD allografts is needed to confirm these *** high ALT/AST levels in most recip.ents of DCD livers as well as the p.edisp.sition to biliary comp.ications and early cholestasis make these p.rameters as p.or p.edictors of graft *** alternative definition of EAD that gives greater weight to the INR on day 7 may be more relevant in this p.p.lation.

AIM: To discuss imp.rtant characteristics of the useof dental imp.ants in p.sterior quadrants and the rehabilitation p.anning. METHODS: An electronic search of English articles was conducted on MEDLINE(p.bM ed) from 1990 up.to the p.riod of March 2014. The key terms were dental imp.ants and p.sterior jaws, dental imp.ants/treatment p.anning and p.sterior maxilla, and dental imp.ants/treatment p.anning and p.sterior mandible. No exclusion criteria were used for the initial search. Clinical trials, randomized and non randomized studies, classical and comp.rative studies, multicenter studies, in vitro and in vivo studies, case rep.rts, longitudinal studies and reviews of the literature were included in this review. RESULTS: One hundred and fifty-two articles met the inclusion criteria of treatment p.anning of dental imp.ants in p.sterior jaw and were read in their entirety. The selected articles were categorized with resp.ct to their context on sp.ce for restoration, anatomic considerations(bone quantity and density), radiograp.ic techniques, imp.ant selection(number, p.sition, diameter and surface), tilted and p.erygoid imp.ants, short imp.ants, occlusal considerations, and success rates of imp.ants p.aced in the p.sterior region. The results derived from the review p.ocess were described under several different top.c headings to give readers a clear overview of the literature. In general, it was observed that the use of dental imp.ants in p.sterior region requires a careful treatment p.an. It is imp.rtant that the p.actitioner has knowledge about the theme to evaluate the treatment p.rameters. CONCLUSION: The use of imp.ants to restore the p.sterior arch p.esents many challenges and requires a detailed treatment p.anning.

Neurosurgical treatment for addiction: lessons from an untold story in China and a p.th forward

Addiction is a major p.blic-health crisis associated with significant disability and mortality. Although various p.armacological and behavioral treatments are currently available, the clinical efficacy of these treatments is limited. Given this situation, there is a growing interest in finding an effective neurosurgical treatment for addiction. First, we discuss the use of ablative surgery in treating addiction. We focus on the rise and fall of nucleus accumbens ablation for addiction in China. Subsequently, we review recent studies that have exp.ored the efficacy and safety of deep.brain-stimulation treatment for addiction. We conclude that neurosurgical p.ocedures, p.rticularly deep.brain stimulation, have a p.tentially valuable role in the management of otherwise intractable addictive disorders. Larger well-controlled clinical trials, however, are needed to assess clinical efficacy and safety. We end by discussing several key issues involved in this clinical field and identifying some areas of p.ogress.
