Background:The initial randomized,double-blinded,actively controlled,phase III ANEAS study(NCT03849768)demonstrated that aumolertinib showed superior efficacy relative to gefitinib as first-line therapy in epidermal growth factor receptor(EGFR)-mutated advanced non-small cell lung cancer(NSCLC).Metastatic disease in the central nervous system(CNS)remains a challenge in the management of *** study aimed to compare the efficacy of aumolertinib versus gefitinib among patients with baseline CNS metastases in the ANEAS ***:Eligible patients were enrolled and randomly assigned in a 1:1 ratio to orally receive either aumolertinib or gefitinib in a double-blinded *** with asymptomatic,stable CNS metastases were ***-up imaging of the same modality as the initial CNS imaging was performed every 6 weeks for 15 months,then every *** responsewas assessed by a neuroradiological blinded,independent central review(neuroradiological-BICR).The primary endpoint for this subgroup analysis was CNS progression-free survival(PFS).Results:Of the 429 patients enrolled and randomized in the ANEAS study,106 patients were found to have CNS metastases(CNS Full Analysis Set,cFAS)at baseline by neuroradiological-BICR,and 60 of them had CNS target lesions(CNS Evaluable for Response,cEFR).Treatment with aumolertinib significantly prolonged median CNS PFS compared with gefitinib in both cFAS(29.0 vs.8.3 months;hazard ratio[HR]=0.31;95%confidence interval[CI],0.17-0.56;P<0.001)and cEFR(29.0 vs.8.3 months;HR=0.26;95%CI,0.11-0.57;P<0.001).The confirmed CNS overall response rate in cEFRwas 85.7%and 75.0%in patients treated with aumolertinib and gefitinib,*** risk analysis showed that the estimated probability of CNS progression without prior non-CNS progression or death was consistently lower with aumolertinib than with gefitinib in patients with and without CNSmetastases at *** new safety findings were ***:The

EGFR inhibitors have revolutionized the treatment of advanced non-small-cell lung cancer(NSCLC).Mefatinib is a novel,bioavailable,second-generation,irreversible pan-EGFR *** phase Ib/II open-label,single-arm,multi-center study investigated the efficacy,safety,biomarker,and resistance mechanisms of mefatinib in the first-line treatment of patients with advanced EGFR-mutant *** study included 106 patients with EGFR-mutant stage IIIB-IV NSCLC who received first-line mefatinib at a daily dose of either 60 mg(n=51)or 80 mg(n=55).The primary endpoint was progression-free survival(PFS).Secondary endpoints were overall response rate(ORR),disease control rate(DCR),overall survival(OS),and *** cohort achieved an ORR of 84.9%and DCR of 97.2%.The median PFS was 15.4 months and the median OS was 31.6 *** metastasis was detected in 29%of patients(n=31)at diagnosis and demonstrated an ORR of 87.1%,PFS of 12.8 months,and OS of 25.2 *** events primarily involved skin and gastrointestinal toxicities,which were well-tolerated and *** of mutation profiles were performed using targeted sequencing of plasma samples at baseline,first follow-up 6 weeks from starting mefatinib therapy(F1),and at *** with concurrent TP53 mutations had comparable PFS as wild-type TP53(14.0 vs 15.4 months;p=0.315).Furthermore,circulating tumor DNA clearance was associated with longer PFS(p=0.040)and OS(p=0.002).EGFR T790M was the predominant molecular mechanism of mefatinib resistance(42.1%,16/38).First-line mefatinib provides durable PFS and an acceptable toxicity profile in patients with advanced EGFR-mutant NSCLC.

Responding to the fast-spreading SARS-CoV-2 Omicron variant, to improve screening efficiency, rapid antigen tests(RATs) were first added as a supplementary detection method in China in mid-March, 2022. What and how big a role RATs should play need to be supported by clinical data. Here, RAT performance and relevant factors in comparison with nucleic acid amplification tests(NAATs) were assessed in Omicron-infected inpatients. From the NAAT results, nasopharyngeal swabs(NPs) performed better than oropharyngeal swabs(OPs). RATs tested on NAAT positive NPs performed better than those with OP-positive samples. The RAT positivity rate was strongly associated with high levels of N and OFR1ab genes, especially in NPs where patients also had significantly longer hospital stays and shorter days from symptom onset to RAT testing. Self-performed RATs had a detection accuracy that was comparable to professionally performed RATs when the subjects were well guided. The antigen negative rate of the studied patients was 100% at discharge. These findings suggest that, in addition to a supplementary detection role, RATs can be an important strategy for evaluating the disease progression of Omicron-infected inpatients. This study provides important clinical data to support better rules regarding RATs under China’s COVID-19 prevention and control policy.

Dear Editor,Uncommon mutations in exons 18-21 of the epidermal growth factor receptor(EGFR)gene account for 10%–15%of all EGFR mutations when considered as a whole group[1,2].However,each variant confers heterogeneous clinical outcomes to different generations of EGFR tyrosine kinase inhibitors(TKIs)with G719X,L861Q,and/or S768I showing adequate sensitivity to EGFR inhibition[1–3].Osimertinib,based on its superior survival outcomes,has become the preferred first-line treatment for patients diagnosed with advanced non-small cell lung cancer(NSCLC)harboring common EGFR mutations[4];however,its efficacy in patients harboring G719X,S768I,and/or L861Q mutations was comparable or even inferior to Afatinib[5].Afatinib,a second-generation EGFR-TKI,has received approval for extended clinical indication in treating previously untreated patients with metastatic NSCLC harboring G719X,L861Q,and/or S768I based on the findings from the pooled analysis of three clinical trials(LUX-Lung 2/3/6)[2].The real-world clinical efficacy of Afatinib for treating this patient subset has been consistently demonstrated by two large retrospective studies[6,7].In China,chemotherapy remains a standard first-line treatment for this patient subset,with Afatinib available only as an off-label treatment option.

元治理主张政府回到治理中心,采用制度设计的基本策略,以元治理框架下治理主体权责划分为依据重构社会各子系统的自组织过程。国家网信办是短视频社区元治理的代理者,国家网络空间安全是符合社会各子系统共同利益的元治理框架。在短视频社区专项治理中,国家网信办依法行政,以元治理框架下各治理主体的权责划分为界面,以制度文本为信息中介,实现对政府网络管理机构的元组织治理、对企业平台的元市场治理、对虚拟公民社会自组织的元差异性结构治理,推动国家网络空间安全框架下相关配套法规制度的完善。国家网信办的元治理制度路径呈现为“国家法律—国家网信办部门规章—国家网信办规范性文件—平台用户服务协议”的嵌套式路径。

目的提高对肺淋巴管平滑肌瘤病(Pu lmonary lymphangiole iomyom atosis,PLAM)的认识。方法对2例PLAM患者的临床资料进行分析,并结合文献进行复习。结果PLAM是一种持续发展的弥漫性肺间质疾病,临床表现为反复发作的气胸,乳糜胸,活动后呼吸困难,肺功能呈阻塞性或混合性通气功能障碍,弥散功能障碍,动脉血气示低氧血症。高分辨CT(HRCT)示两肺广泛弥漫性分布的薄壁小囊状病变。病理特点为肺淋巴管,小血管,小气道的管壁及其周围平滑肌细胞弥漫增生。结论育龄期妇女如反复出现自发性气胸或乳糜胸,胸部HRCT示两肺弥漫性囊状改变,即应考虑为PLAM可能。PLAM预后差,目前无有效治疗方法。

目的对复合性外伤患者的急救方法和护理措施进行探究和总结。方法选取我院于2014年4月-2015年6月收治的96例复合性外伤患者,将其作为研究对象。运用随机数字表法,分为观察组和对照组,每组患者48例,对照组采用常规急救和护理办法,观察组患者则采用程序化急救护理,对比两组患者的抢救治疗效果。结果观察组急诊时病死率、住院后病死率分别为12.5%、4.1%,低于对照组的20.8%、10.4%（P〈0.05）;观察组经抢救后住院治疗比率为83.3%,高于对照组的68.8%（P〈0.05）。结论对于复合性外伤患者采取程序化急救护理办法,能大大提高抢救效率,为患者的抢救治疗争取宝贵时间,并实现抢救成功率的提高,患者死亡率下降。

目的：分析探讨优质服务在急诊患者护理工作中的应用效果。方法选取2013年8月-2014年8月来我院就诊的80例患者作为研究对象，随机分为实验组和对照组各40例。其中对照组患者实施一般护理服，实验组实施优质护理服务，比较两组患者的心理状态、对护理工作的满意度、护理质量及护理差错的发生率。结果试验组患者的焦虑和抑郁状态明显减轻，对护理工作更加满意，护理质量明显提高，护理差错发生率明显降低，与对照组比较有统计学意义（P〈0.05）。结论急诊患者护理中引入优质护理服务，能明显提高护理工作的质量和效率，降低其负性情绪，提高患者对护理工作的满意度。

目的：分析急性心肌梗死院前急救与急诊急救对患者的影响。方法选择我院2013年4月-2015年3月收治的急性心肌梗死患者88例，按照入院方式不同分为观察组和对照组，每组44例。对照组由患者家属自行送至医院进行救治，观察组由急诊医护人员出车接诊，并采取院前急救措施。比较两组发病至抢救时间、溶栓时间、住院时间、并发症发生率以及死亡率。结果与对照组比较，观察组发病至抢救时间、溶栓时间、住院时间、并发症发生率以及死亡率均明显降低，差异有统计学意义（P 〈0.05）。结论采取针对性的院前急救以及急诊急救护理措施可以明显的缩短急性心肌梗死患者的救治时间和住院时，降低并发症发生率和病死率。
