BACKGROUND： Bilateral stent-in-stent （SIS） self-expandable metal stent placement is technically chghallenging for palliation of unresectable malignant hilar obstruction. In the SIS technique, the uniform large cell type biliary stent facilitates contralateral stent deployment through the mesh of the first metallic stent. This study aimed to assess the technical success and clinical effectiveness of this technique with a uniform large cell type biliary stent. METHODS： Thirty-one patients who underwentlbilateral SIS placement using a large cell type stent were reviewed retrospectively. All patients showed malignant hilarl obstruction （Bismuth types Ⅱ, Ⅲ, Ⅳ） with different etiologies. RESULTS： Sixteen （51.6%） patients were male. The mean age of the patients was 67.0±14.0 years. Most patients were diagnosed as having hilar cholangiocarcinoma （58.1%） and gallbladder cancer （29.0%）. Technical success rate was 83.9%. Success was achieved more frequently in patients without masses obstructing the biliary confluence （MOC） than those with MOC （95.2% vs 60.0%, P=0.03）. Functional success rate was 77.4%. Complications occurred in 29.0% of the patients. These tended to occur more frequently in patients with MOC （50.0% vs 19.0%, P=0.11）. Median time to recurrent biliary obstruction was 188 days and median survival was 175 days. CONCLUSIONS： The large cell type stent can be used efficiently for bilateral SIS placement in malignant hilar obstruction. However, the risk of technical failure increases in patients with MOC, and caution is needed to prevent complications for these patients.

Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP has not been well ***: This study was undertaken to compare the pain-relieving effects of EA and conventional treatment in patients with ***, setting, participants and interventions: This study was conducted using a randomized, controlled,three-arm, parallel-group and multi-center design. Patients diagnosed with AP were randomly and equally assigned to EA1, EA2 or control groups. All participants received conventional standard-of-care therapy for AP. Local EA alone was administered in EA1, and local plus distal EA was given in *** EA included two abdominal acupoints, while distal EA included twelve peripheral acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or *** outcome measures: The primary outcome measure was the change in the visual analogue scale(VAS;0–100) pain score between baseline and day ***: Eighty-nine participants were randomized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score change(median [interquartile range]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the change in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) than in the control group(median 2.0 days vs 3.0 days), with a hazard ratio of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events ***: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.

In order to prevent the oxide formation on the surface of nano-size iron particles and thereby to improve the oxidation resistance in ordinary condition, iron nanoparticles synthesized by a chemical vapor condensation method were directly soaked in hexadecanethiol solution to coat them with a polymer layer. Oxygen content in the polymer-coated iron nanoparticles was significantly lower than that in usual air-passivated particles possessing iron-core/oxide-shell structure. Accordingly, oxidation resistance of the polymer-coated particles at an elevated temperature below 130℃ in air was 10-40 times higher than that of the normally passivated particles.

Poly(ethylene) oxide(PEO)-based electrolytes have been widely studied for solid-state lithium batteries while their ionic conductivity and lithium-ion transference number still need to be further ***, using the combined experimental and theoretical approach, we demonstrate a novel, solidstate PEO-deep eutectic solvent(DES) electrolyte for the first time. We found that the in situ formation of DES can reduce the crystallinity of PEO matrix and more Li+ions can move freely owing to the weakened coordination between ether oxygens and Li-ions. Besides, we show that more Li+ions can be dissociated from Li salts in PEO-DES electrolyte using the molecular dynamics simulations. Such liquid-free PEO-DES electrolytes showed good ionic conductivity(2.1 × 10^(-4) S cm^(-1)) which is 160% higher than that of conventional PEO-Li TFSI(8.1 × 10^(-5) S cm^(-1)) electrolyte at 60 ℃. Additionally, the PEO-DES electrolyte showed 136% increase of Li-ion transference number(0.33) compared with ionic liquid-doped PEO-Li TFSI(0.14) at 60 ℃. Moreover, the PEO-DES exhibited good compatibility with Li metal and stable Li plating/stripping behavior with little morphology change of Li metal. This research also provides new insights into the enhancement mechanisms of novel polymer electrolytes, improving our fundamental understanding of critical challenges that have impeded the adoption of solid-state lithium metal batteries.

One-dimensional alkali metal titanates containing potassium,sodium,and lithium are of great concern owing to their high ion mobility and high specific surface *** those titanates are combined with conductive materials such as graphene,carbon nanotube,and carbon nanofiber,they are able to be employed as efficient electrode materials for *** hexa-titanate(K2Ti6O13,KTO),in particular,has shown superior electrochemical properties compared to other alkali metal titanates because of their large lattice parameters induced by the large radius of potassium ***,we present porous rGO crumples(PGC)decorated with KTO nanoparticles(NPs)for application to *** KTO NP/PGC composites were synthesized by aerosol spray pyrolysis and post-heat *** NPs less than 10 nm in diameter were loaded onto PGCs ranging from 3 to 5μ*** porous structure of the composites was obtained by the activation of rGO by adding an excessive amount of KOH to the *** KTO NP/PGC composite electrodes fabricated at the GO/KOH/TiO2 ratio of 1:3:0.25 showed the highest performance(275 F g−1)in capacitance with different KOH concentrations and cycling stability(83%)after 2000 cycles at a current density of 1 A g−1.

An inverse spinel-type metal oxide, magnesium-manganese-titanium oxide （Mg2Mn0.5Ti0.5O4）, were prepared using the coprecipitation/thermal crystallization method. The extraction/insertion reaction with this material was investigated by X-ray, saturation capacity of exchange, pH titration, and distribution coefficient （Kd） measurement. The acid treatments of Mg2Mn0.5Ti0.5O4 caused Mg^2＋ extractions of more than 81%, whereas the dissolutions of Mn^4＋ and Ti^4＋ were less than 10%. The experimental results proved that the acid-treated sample has a capacity of exchange 56 mg·g^-1 for Li^＋ in the solution. The chemical analysis showed that the Li^＋ extraction/insertion progressed mainly by ion-exchange mechanism and surface adsorption.

Differentiation of acute and chronic hepatitis B in IgM antiHBc positive patients

AIM: To identify the factors that differentiate acute hepatitis B(AHB) from chronic hepatitis B with acute exacerbation(CHB-AE).METHODS: From 2004 to 2013, a total of 82 patients(male n = 52, 63.4%; female n = 30, 36.6%) with clinical features of acute hepatitis with immunoglobulin M antibodies to the hepatitis B core antigen(Ig M antiHBc) were retrospectively enrolled and divided into two groups; AHB(n = 53) and CHB-AE(n = 29). The AHB group was defined as patients without a history of hepatitis B virus(HBV) infection before the episode and with loss of hepatitis B surface antigen within 6 mo after onset of acute hepatitis. Biochemical and virological profiles and the sample/cutoff(S/CO) ratio of Ig M anti-HBc were compared to determine the differential diagnostic ***: The multivariate analysis demonstrated that, the S/CO ratio of Ig M anti-HBc and HBV DNA levels were meaningful factors. The S/CO ratio of Ig M anti-HBc was significantly higher in the AHB group, while the HBV DNA level was significantly higher in the CHB-AE group. The optimal cutoff values of Ig M anti-HBc and HBV DNA levels for differentiating the two conditions were 8 S/CO ratio and 5.5 log10 IU/m L, respectively. The sensitivity and specificity were 96.2% and 89.7% for the S/CO ratio of Ig M anti-HBc and 81.1% and 72.4% for HBV DNA levels, respectively. The area under receiver operating characteristic curves of both the S/CO ratio of Ig M anti-HBc and HBV DNA levels were not significantly different(0.933 vs 0.844, P = 0.105). When combining Ig M anti-HBc and HBV DNA, the diagnostic power significantly improved compared to HBV DNA alone(P = 0.0056). The combination of these factors yielded a sensitivity and specificity of 98.1% and 86.2%, ***: The combination of the S/CO ratio of Ig M anti-HBc and HBV DNA levels was a useful tool for differentiating AHB from CHB-AE in patients with positive Ig M anti-HBc.
