Low Cost Equipment and Short Duration Program Are Not Barriers to Good Outcomes of Cardiac Rehabilitation in Senegalese Patients with Coronary Artery Disease

Cardiac rehabilitation is still underused in Africa, despite its cost effectiveness widely demonstrated around the world. Aim: To evaluate the results of a cardiac rehabilitation program in the first Senegalese outpatient center in low resource context. Patients and Method: We carried out a prospective and interventional study. It compared on one hand a group of coronary patients at baseline and after 6 months of our rehabilitation program and on the other hand a control group of patients not enrolled in rehabilitation. Results: We included 70 patients (30 in the rehabilitation group and 40 in the control group). After rehabilitation, the functional capacity improved: 6.99 ± 2.9 Mets at M0 vs 8.8 ± 2.23 Mets at M6, p = 0.0001. Patients of intervention group significantly increased motivation to lifestyle changes and knowledge about their disease. We found better control at 6 months of hypertension (84.50% vs 33.70%, p = 0.003), diabetes (70.00% vs 26.70%, p = 0.0042), LDL cholesterol (33.00% vs 5.00%, p = 0.002) in the group “Rehabilitation”. Prevalence of psychosocial issues like anxiety and depression decreased in intervention group: 50.00% at M0 to 23.33% at M3 (p = 0.021), then 30.00% at M6 (p = 0.18). Return to work and resuming sexual activity were not significantly different. Purpose: Our purpose was to describe the epidemiological and therapeutic aspects of the vaginal hydroceles. Methodology: It was a forward-looking and descriptive study of Mars 2014 in July, 2017, which realized in the service of general surgery of the Reference Health Center of Kati (Mali). All the patients operated in the service for hydrocele were included. Other causes of big stock exchange and hydroceles operated outside our service were not including. Results: We operated 92 hydroceles or 5.9% of the surgical activities of the service. The average age was of 34.5 years with extremes of 2 years and 82 years. The most frequent signs of hydrocele were the big painless stock exchange (100%);a transillumination positive (90%) and the effusion of the vaginal in the ultrasound (100%). The cure of hydrocele was realized according to the techniques of Bergmann (94.6%) and of Lord (5.4%). The mortality was nil and the morbidity was 3.3% (2 operating infections of the site and 1 bruise). Conclusion: The hydrocele is a frequent pathology in Africa. The therapeutic strategies are multiple and varied. However, we prefer the Bergmann technique, which is easy, fast and has few complications.

Background: COVID-19, an infectious viral disease, has caused a global health crisis. Most cases remain asymptomatic. The majority of patients have mild symptoms while about 15% develop a severe form. The clinical spectrum of SARS-CoV-2 infection appears broad, encompassing asymptomatic infection, upper respiratory tract symptoms, and severe viral pneumonia with respiratory failure that can lead to death. Laboratory tests play an important role in the management of COVID-19 patients. In addition to being essential for the diagnosis, several biological analyses make it possible to identify the inflammatory processes and the potential complications of this disease. This study attempted to identify biochemical assays that could help in the prognosis of the disease to ensure early management. Methods: This was a descriptive study. It focused on patients hospitalized for COVID-19 from March 19, 2020, to January 26, 2021, at the Infectious Disease Management Centre in Lomé (Togo). Medians were compared using the (Mann-Whitney and Wilcoxon) test and frequencies were compared using the Chi-square test or Fisher’s exact test. Results: We included 782 patients. The median age was 41 years IQR from 32 to 55. We observed several biochemical abnormalities in varying proportions for all biochemical parameters studied. Compared to non-serious patients, critically ill patients at admission had more frequently elevated urea, creatinine, transaminases, TG, GGT, CRP and blood glucose. Also, they had more frequent decreases in total cholesterol, HDL-c, blood chloride, and blood calcium. As for patients who died during hospitalization, compared with healed patients, they had more frequent elevations of urea, creatinine, AST, ALT, GGT. CRP and blood glucose. They also had a more frequent decrease in total cholesterol, HDL-c, blood chloride, blood calcium, and blood glucose (p = 0.025). Cardiac myxoma is one of the primary intracardiac tumours. We report the case of a left atrial myxoma in a 26-year-old female patient admitted for investigation of dyspnoea. Echocardiography led to the diagnosis of a giant hypermobile myxoma prolapsing into the left ventricle. The patient died before being evacuated for surgical treatment.

Introduction: Small bowel obstruction (SBO) is defined as a complete and persistent cessation of the transit of materials and gases. It occurs in a segment of the digestive tract located between the pylorus and the colorectal junction. Herein, we report a case of small bowel obstruction (SBO) following excessive consumption of wild grapes with no previous abdomen medical or surgical history. Presentation of case: A 65-year-old male farmer presented with cessation of materials and gas associated with vomiting of food and then bilious. The abdomen was bloated and painful on palpation with a moving, painful mass in the left iliac fossa. The hypothesis of acute occlusion of the colon was retained and confirmed on the X-Ray which showed a dilation of small bowel with air-fluid levels. The diagnostic of acute SBO was retained. We performed urgently a median laparotomy. We found the dilation of the loops from the jejunum to the sigmoid colon with a compact mass at the sigmoid colon of 15 cm in length and 6 cm in diameter. We proceeded to crush the mass that was made up with wild grape seed fecal, and push it back to the rectum. The phytobezoar made up with wild grape seeds was extracted through the anus with the fingers after anal dilation. After extraction of the bezoar, the patient’s complaints completely subsided. Discussion: SBO is a frequent emergency surgical pathology. SBO induced by phytobezoar often occurs in patients with abdomen surgery history. However, it is particularly rare in virgin abdomen. Conclusion: This case report aims to raise awareness of phytobezoar as a cause of SBO in elderly even though without abdomen surgery history.

Introduction: Influenza is an acute respiratory infectious disease, highly contagious due to influenza viruses. The objective of this work was to identify, understand the epidemiology of circulating strains and estimate disease transmission. Patients and Methods: The study was carried out in the pediatric department of the Sikasso Hospital. This was a prospective, longitudinal descriptive study over a five-year period (January 1, 2015 to December 31, 2019). She was interested in severe acute respiratory infections (SARI) for hospitalized patients in the pediatric department. Results: During the study period the prevalence of severe acute respiratory infections among hospitalized children was 21.85‰. The majority of cases were observed in 2019 with 58 cases, the sex ratio was 1.8. The age group from 0 to 1 was the most represented with 100 cases (48.30%) followed by 2 to 4 years 73 cases (35.24%) and 5 to 15 years 34 cases (16.46%). More than half of the patients lived in rural areas 129 (62.31%). Fever and cough were present in the majority of patients. No children had received influenza vaccination. In study 36 (17.39%) cases were positive for influenza A and B.

Introduction: A cardiovascular risk factor (FDRCV) is defined as a physiological, pathological or environmental attribute or characteristic that results in an increased likelihood of developing cardiovascular disease in the individual in whom it is detected. The objective of this study was to determine the prevalence of cardiovascular risk factors in hemodialysis patients on hemodialysis at the University Hospital of Point G. Patients and Methods: This was a cross-sectional descriptive study with prospective data collection from March 3, 2009 to March 5, 2010 (13 months). Included were all patients with end-stage renal disease (ESRD) receiving chronic hemodialysis in the Nephrology and Hemodialysis Department of the University Hospital of Point G during the study period. Results: Eighty-eight patients were enrolled. The M/F sex ratio was 1.26. The mean age was 41.32 years with extremes of 17 and 81 years. The classic cardiovascular risk factors in order of frequency were: hypertension (90.9%), sedentary lifestyle (71.6%), male sex (54.5%), age ≥ 55 years (21.6%), obesity (13.6%), diabetes (8%), tobacco (8%), alcohol (3.4%). Cardiovascular risk factors related to CKD were: anemia (98.9%), phosphocalcic disorders (85.2%), arteriovenous fistula (AVF) (80%), lipid abnormalities (36.5%), hydrosodium inflation (29.5%). Predominant cardiovascular complications were hypertensive heart disease (62.5%), acute pulmonary oedema (APO) (50%), rhythm disorders (23.9%), coronary insufficiency (18.2%). The mortality rate was 17%. Conclusion: Cardiovascular risk factors are frequent in chronic hemodialysis. They contribute to the excess mortality of these patients. Adequate management of these risk factors can slow down serious cardiovascular complications and reduce the risk of mortality in this fragile population.

Correlation between Computed Tomography and Histopathology Findings of Sinonasal Tumors

Background: A diverse range of pathologies affect the sinonasal cavities, because of the nonspecific nature of the symptoms, most patients with sinonasal masses are diagnosed late when the disease is at an advanced stage. The diagnostic challenge of identifying the nature of sinonasal masses before treatment would guide therapeutic strategies. Our objective was to evaluate the accuracy of computed tomography in the preoperative diagnosis of sinus masses at the Central Hospital of Yaoundé. Method: A retrospective chart review was conducted;CT scan reports and pathology reports of patients operated for sinonasal masses between January 2012 and November 2022 were compared. Results: Forty-five patients met the inclusion criteria for the study. The average age was 39.98 ± 18.34 years, with extremes of 15 and 87 years. The sex ratio H/F is 0.76 with a female predominance. The median time to consultation was 14 [12 - 18] months, with extremes of 11 and 36 months. The most frequently encountered histological type was benign tumors with sinonasal polyposis (PNS) in the lead, i.e. 34.1% of cases. The types adenocarcinoma, adenoid cystic carcinoma and non-Hodgkin’s lymphoma were represented equally in 6.8% of cases. Management was dominated by Caldwell-Luc surgery, i.e. 59.1% of cases. The frequency of disease recurrence after surgical treatment was 18.2%. As for the mortality rate, it was 11.4%. The concordance rate of the character of malignancy on imaging compared to histology was 90.9%. This level of concordance was statistically significant according to the Gamma test (p Conclusion: Preoperative CT scans correlate with histology and have a prognostic role in surgically treated sinonasal masses.

Introduction: Secondary hyperparathyroidism is a common complication in chronic hemodialysis patients. It is characterized by hypersecretion of parathormone by the parathyroid glands to maintain phosphocalcium homeostasis in response to hypocalcemia, lowering of 1.25 dihydroxy vitamin D3 and hyperphosphatemia. Objective: To analyze the results of parathyroidectomy (PTX) in cases of secondary hyperparathyroidism (HPTS), report the post-operative course and early and late complications. Patients and Methods: We conducted a retrospective study of ten (10) patients with chronic renal failure operated on at the CHU of Point G over a 32-month period, from January 1, 2016 to August 31, 2019. We analyzed the clinical, biological, radiological and histological parameters of the thyroid glands. Results: We have collected 33 cases of secondary hyperparathyroidism. Ten (10) patients met the inclusion criteria. They were 45 years old on average, seven (7) women and three (3) men, in dialysis for an average of 6.8 years before parathyroidectomy. Half of the initial kidney disease was of vascular origin (HTA). No cases of diabetic nephropathy were identified. Symptoms were in order of frequency: bone pain (60%), paresthesia (50%), functional impotence of the lower limbs (50%). Radiological signs included demineralisation (5 out of 6 cases) and brown tumour associated with a fracture (1 out of 6 cases). The most frequent indication for parathyroidectomy (100%) was persistence despite treatment of a serum PTH concentration above 1000 pg/ml. Subtotal PTX (7/8) was performed after cervical ultrasound in all patients. Histological analysis of the parathyroid glands showed adenoma (60%) and hyperplasia in 40% of cases. The Twin Bambili Maars (Bamenda Mountains, Cameroon Volcanic Line): Simulative Model of Formation

The Bambili maars are twin contemporaneous maars embedded in trachytic rocks. The two maars are separated by a low lying inter-Maar dome whose inner walls dip towards the Southern Maar. The Bambili Northern Maar (BNM) contains water and its floor is found at an altitude lower than the Bambili Southern Maar (BSM) (50 m) which is swampy and drains into the BNM. Both maars have very steep vertical walls. Three different units are identified in the area. Unit 1 is 1 m thick sequence, characterize by the presence of matrix supported fine-grained fallouts (ash and lapilli size) with stratified diffused bedding planes. This unit is further subdivided into three different layers: L1, L2 and L3 from bottom to top with increasing sizes of particles respectively. This layer shows reverser grading and its grain sizes rarely exceed 2 cm and this unit shows no depositional structures. Unit 2 is an unstratified unit predominantly made up of tephra deposits. Within these deposits are found blocks of sizes ranging between 2 and 10 cm of different petrographic types. They include scoria, basalts, trachyte, and granite cognate. The last unit, Unit 3 is distinctly stratified characterized by well sorted, heterolithologic, graded bedding layers which are matrix supported. Base on the composition and grain sizes, this unit was subdivided into 7 horizons: h1 - h7. Within this unit, two sequences of deposits are distinguished: U1 and U2. Both sequences are characterized by alternation of lapilli and tuff beds. The clast comprises of juvenile materials, mudstone which are highly vesicular and altered rock fragments. Other products identified within the area are ferolithic volcanic bombs (5 - 20 cm), xenocrystals (2 - 4 cm) within finer deposits. They are elongated and show visible twinned planes.
