Although there are a variety of definitions that describe plastic surgery,confusion persists in the minds of both the lay public and doctors.A common lay misconception is that actual pieces of plastic are used to repair injured tissues or placed over defects to make them appear *** the other hand,resident doctors from specialities such as ear-nose-throat and general surgery fail to appreciate the vast scope that plastic surgery specialty *** recognition of this,trainee plastic surgeons are often asked to briefly define our specialty in their final exams,so that subsequent misconceptions are minimized.

Fetus in fetu is an extremely rare condition embryologically arising in monozygotic diamniotic twins with an incidence of 1:500,000 births. To the best of our knowledge, number of reported cases should not be more than 100 with majority of cases presenting in post natal life with only 5 cases detected prenatally. We are presenting a case diagnosed at 24 weeks of gestation on ultrasound with presence of well formed skull, facial skeleton and bony orbit.

随着系统结构变得更加复杂,可用性分析成为工业系统设计领域中的一个重要问题。此外,系统的可用性受诸多因素影响,如设计、制造、安装等,所以针对系统的故障行为建模、分析和预测是非常困难的。本文目的是开发一种新的方法,用于计算任何工业系统中的各种性能指标,即可靠性、可用性、MTBF(平均故障间隔时间),ENOF(预期的故障次数),故障率和维修时间。在该方法中,采用遗传算法获得所有组成部件的故障率和维修时间,然后使用模糊λ-tau技术计算各种性能指标。本研究涉及的清洗系统是造纸工业的主要组成部分。运用Petri网络对工作部件之间的相互作用进行建模。使用三角模糊数表示故障率和修复频率,因为他们允许将可靠信息中的专家观点、语言变量、操作条件、不确定性和不精确性纳入系统模型中。基于计算的可靠性参数形成一个结构框架,可帮助维护工程师分析和预测系统行为。

目的探讨舒芬太尼复合罗哌卡因用于连续硬膜外无痛分娩的效果和安全程度。方法将2018年12月至2019年4月期间昆明医科大学附属延安医院100例在产房正常待产的初产妇随机分为2组(n=50例),实验组常规于L2~3间隙行连续硬膜外穿刺,试验量后头侧置硬膜外管4cm接PCEA泵,协助产妇平卧位后给予舒芬太尼复合罗哌卡因的首剂量和维持剂量,对照组产妇实施常规阴道分娩。观察2组产妇硬膜外自控镇痛效果、产程时间、出血量和新生儿Apgar评分情况,记录是否需要助产和紧急更改剖宫产,是否出现产后出血和胎儿窘迫等不良事件。结果 2组产妇的年龄、身高、体重、孕周值比较差异无统计学意义(P>0.05),实验组产妇分娩镇痛效果的优良率明显高于对照组(P0.05),实验组产妇第一产程和第二产程时间明显短于对照组(P<0.05),实验组产妇出现需要阴道助产、紧急更改剖宫产和产后出血的比例明显低于对照组(P<0.05)。结论舒芬太尼复合罗哌卡因用于连续硬膜外无痛分娩,镇痛效果显著,能够显著缩短产程时间,提高产妇自然分娩率,降低分娩过程中不良事件的发生率,可以安全有效地应用于临床。

目的鲍曼不动杆菌是医院内感染常见的细菌。文中旨在分析其分子流行特征,检测外排泵和调节蛋白基因的携带率,并研究替加环素对外排泵和调节蛋白基因表达的影响。方法选取2017年5月至2019年3月江苏大学附属医院住院患者183株鲍曼不动杆菌。分为耐药组(一类及以上抗菌药物耐药的菌株,139株),敏感组(药敏试验中的药物均为非耐药的菌株,44株)。应用重复序列PCR作菌株的同源性分析,并以脉冲场凝胶电泳(PFGE)为同源性分析的金标准对部分菌株进行验证和比较。PCR检测耐药相关基因。依据同源性分析、外排泵携带率检测及药敏试验结果,选取6株外排泵基因全携带但耐药表型不同的临床菌株作为实验株,包括敏感株(SAB)、多药耐药株(MDRAB)、广泛耐药株(XDRAB),进行替加环素体外诱导,诱导后的菌株为诱导组,以不含替加环素LB培养的相同菌株为对照组。检测AdeB、TetB等外排泵基因及AdeS、BaeS等调节蛋白的基因表达变化。结果同源性分析显示,183株临床分离株包含有45个克隆群。耐药组AdeB、TetB基因(97.84%、98.56%)检出率均高于敏感组(63.64%、20.45%),CraA、AbeM等基因亦高于敏感组(P<0.05)。替加环素诱导前MDRAB、XDRAB中AdeB(4.17±0.94、6.86±3.23)、TetB(13.06±0.38、13.96±0.63)表达较SAB(0.00±3.70、0.00±0.70)升高(P<0.05),MDRAB菌株中MacB、BaeS表达较SAB菌株亦升高(P<0.05)。MDRAB、XDRAB菌株中AbaQ表达较SAB菌株降低(P<0.05)。诱导组AdeB、TetB在各菌株的表达较对照组明显升高(P<0.01);除SAB1菌株外,AbaQ、AbeM在其余菌株中的表达较对照组明显升高(P<0.05);AdeS在SAB1、SAB2、MDRAB1、XDRAB2菌株中的表达较对照组升高(P<0.05);负调节蛋白SoxR在SAB1、XDRAB1菌株中表达较对照组降低(P<0.05)。结论AdeB、TetB、AbeS、AdeS、BaeS在临床鲍曼不动杆菌多药耐药过程中发挥着重要的作用,其中AdeB和TetB在菌株对替加环素耐药过程中的作用尤为明显,可能是替加环素耐药的优势外排泵基因。
