Clinical Outcome of Definitive Radiotherapy and/or Surgery in T1-2N0m0 Glottic Squamous Cell Carcinoma: A Single Institution Retrospective Report

Background: The current treatment options of early glottic carcinoma are radiotherapy;trans laser microsurgery, and open surgery. However, the best treatment is still controversial due to lack of randomized controlled trials. We aimed to evaluate the treatment results and the prognostic factors of local control of early glottic squamous cell carcinoma patients (GSCC) T1-2N0m0 treated at our institution. material and methods: We retrospectively studied the charts of 52 patients with early GSCC T1-T2N0m0 from 2010-2015 at the Clinical Oncology Department, Ain-Shams University. 24 patients had T1 and 28 had T2 early glottic carcinoma. The overall survival OS, local control rate, and laryngeal preservation rate were evaluated. Kaplan-meier method, Cox proportional hazards model were used to analyze the data. Results: median duration of follow-up was 13 months. Thirty-eight patients received radiation treatment alone (73.1%), 7.7% of the patients underwent surgery alone, and 19.2% of the patients had surgery combined with radiotherapy. Local recurrence after radiation failure developed in 6/52 patients, all had T2 disease and were salvaged by total laryngectomy. The ultimate local control rate was 88.5%, and the ultimate laryngeal preservation rate was 77.2% (40/52 patients). The median OS of the 52 patients was 13 months (range 2 - 46 months). Univariate analysis of factors associated with poor local control showed that age > 60 years was the only significant factor (P = 0.048). Conclusion: Radiotherapy achieves high local control and laryngeal preservation rates for patients with early glottic carcinoma, and is associated with a low rate of severe complications compared to surgery. Salvage surgery is feasible after radiotherapy failure.

Chemical Composition, minerals and Antioxidants of the Heart of Date Palm from Three Saudi Cultivars

The aim of this study was to determine the chemical composition, minerals and antioxidants of heart of date palm from three Saudi cultivars. moisture (ranged from 80.44% - 82.82%) was the predominant component in the heart date palm. Sukkari cultivar had the highest protein (5.27%) and ash (7.9%) compared to the other two cultivars. Fat was low in the three cultivars. Sucrose was the predominant sugar in the heart date palm and ranged from 7.65% - 20.25%. Potassium was the predominant mineral in the heart date palm and was significantly (p mpared to the Naboat Saif cultivar. Sulfur and chloride were next to potassium in abundance in the three heart date palm cultivars. Total phenols and flavonoids were higher in Solleg compared to the other two cultivars. IC50 for Solleg, Sukkari and Naboat Saif were 0.12, 0.13 and 0.29 mg/ml, respectively. The results of the present study may highlight the potential importance of heart of date palm as a product rich in essential minerals and antioxidants. Future researches are needed to explore other characteristics of heart of date palm which could be considered for human nutrition.

Cholangiocellular cancer(CCC)is an oncological disease of the bile ducts characterized by a high mortality *** date,the use of standard methods for the diagnosis and treatment of CCC has not been able to reduce mortality from this *** work presents the results of fluorescence diagnostics(FD),which consists in using a modified optical fiber and photodynamic therapy(PDT)using a therapeutic laser instead of a low-intensity *** technique was tested on 43 patients in a clinical *** results obtained indicate a direct correlation between spectroscopic and video FD ***,a direct correlation was found between the photobleaching of a chlorin e6-based photosensitizer,with the commercial names of PhotoIon Radachlorin and Photoran and stricture *** findings demonstrate the possibility of using a therapeutic laser with a wavelength of 660 nm for both diagnosis and treatment of bile ducts cancer,which results in a significant reduction of the operation time without decreasing its effectiveness.

Complete rectal prolapse in young Egyptian males: Is schistosomiasis really condemned?

AIm To investigate the assumption that schistosomiasis is the main cause of rectal prolapse in young Egyptian males. mETHODS Twenty-one male patients between ages of 18 and 50 years with complete rectal prolapse were included in the study out of a total 29 patients with rectal prolapse admitted for surgery at Colorectal Surgery Unit, Ain Shams University hospitals between the period of January 2011 and April 2014. Patients were asked to fill out a specifically designed questionnaire about duration of the prolapse, different bowel symptoms and any past or present history of schistosomiasis. Patients also underwent flexible sigmoidoscopy and four quadrant midrectal biopsies documenting any gross or microscopic rectal pathology. Data from questionnaire and pathology results were analyzed and patients were categorized according to their socioeconomic *** Twelve patients(57%) never contracted schistosomiasis and were never susceptible to the disease, nine patients(43%) had history of the disease but were properly treated. None of the patients had gross rectal polypsand none of the patients had active schistosomiasis on histopathological examination. Fifteen patients(71%) had early onset prolapse that started in childhood, majority before the age of 5 years. Thirteen patients(62%) were habitual strainers, and four of them(19%) had straining dating since early childhood. Four patients(19%) stated that prolapse followed a period of straining that ranged between 8 mo and 2 years. Nine patients(43%) in the present study came from the low social class, 10 patients(48%) came from the working class and 2 patients(9%) came from the low middle social class. CONCLUSION Schistosomiasis should not be considered the main cause of rectal prolapse among young Egyptian males. Childhood prolapse that continues through adult life is likely involved. Childhood prolapse probably results from malnutrition, recurrent parasitic infections and diarrhea that induce straining and prolaps

Effect of Aqueous Extracts of Salvadora persica “miswak” on the Acid Eroded Enamel Surface at Nano-mechanical Scale

Understanding the effects of enamel loss and surface softening caused by acidic soft drinks consumption would help in the clinical treatment of tooth wear and aid in the development of novel dental restorative materials. Hence, this investigation was car-ried out to examine the effects of “miswak” on the acid eroded enamel surface at nano-mechanical scale. The first use of a stylus-based inductive gauge is introduced as means of measurement of acid attack on enamel surfaces and the effectiveness of aqueous extracts of Salvadora persica as dissolution inhibitors for acid eroded enamel surfaces. For pre-molar protective, after being exposed to aqueous extracts of Salva-dora persica solution, the performance of the enamel surface was improved by Ra ≈ 0.82 μm. Whereas, the surface was damaged after immersing in citric acid solution by Ra ≈ 0.63 μm. For pre-molar restoring, the enamel surface was degraded by Ra ≈ 0.34 μm when exposed to citric acid solution. While, the surface roughness was improved by Ra ≈ 0.95 μm when aqueous extracts of Salvadora persica solution was introduced. So, this study concludes that the aqueous extracts of Salvadora persica treatment work effectively on the eroded enamel surface.

Chronic pancreatitis(CP)is an inflammatory disease of the pancreas characterized by progressive fibrotic destruction of the pancreatic secretory *** the heterogeneity in pathogenesis and involved risk factors,processes such as necrosis/apoptosis,inflammation or duct obstruction are *** fibrosing process ultimately leads to progressive loss of the lobular morphology and structure of the pancreas,deformation of the large ducts and severe changes in the arrangement and composition of the *** conditions lead to irreversible morphological and structural changes resulting in impairment of both exocrine and endocrine *** prevalence of the disease is largely dependent on culture and *** etiological risk-factors associated with CP are multiple and involve both genetic and environmental *** this review the m-ANNHEIm classification system will be used,comprising a detailed description of risk factors such as:alcohol-consumption,nicotineconsumption,nutritional factors,hereditary factors,efferent duct factors,immunological factors and miscellaneous and rare metabolic *** knowledge of the different etiological factors may encourage the use of further advanced diagnostic tools,which potentially will help clinicians to diagnose CP at an earlier ***,in view of the multi factorial disease and the complex clinical picture,it is not surprising that treatment of patients with CP is challenging and often unsuccessful.

Highly Efficient Synthesis of Novel Fluorine Bearing Quinoline-4-carboxylic Acid and the Related Compounds as Amylolytic Agents

Highly efficient syntheses of novel fluorine bearing quinoline-4-carboxylic acids and the related compounds had been achieved from cyclocondensation of 2-amino-5-fluorophenyl glyoxylic acid 1 with benzoyle asetanilides 2 in boiling DmF, to give the target 3. Decarboxylation of 3 produced 6-fluoro-2-phenyl-3-(substituted amino)-keto-quinolines 4, while that reaction underwent refluxing, afforded 7-fluoro-1-(aryl)-3-phenyl-pyrrolo[3,4-c] quinoline-2,9-diones 5. Structure of the products has been established from their elemental and spectral analysis. All targets exhibited a high to moderate activity against some Aspergillus fungi as amylolytic agents.

Synthesis and thermal studies of mixed ligand complexes of Cu(II), Co(II), Ni(II) and Cd(II) with mercaptotriazoles and dehydroacetic acid

A series of new mixed ligand complexes of cobalt(II), nickel(II), copper(II) and cadmium(II) have been synthesized with 3benzyl1H4[(2 methoxybenzylidine) amino]1, 2, 4triazole5 thione (mBT), 3bezyl1H4[(4chlorobenzylidine) amino]1, 2, 4triazole5thione (CBT), 3benzyl 1H4[(4nitrobenzylidine)amino]1, 2, 4triazole 5thione (NBT) and dehydroacetic acid sodium salt (Nadha). The mixed ligand complexes have been characterized by elemental analysis, spec troscopic spectral measurements (IR, UVVis.), molar conductance, magnetic measurements and thermal studies. The stoichiometry of these com plexes is m:L1:L2 = 1:1:1, 1:2:1 or 1:1:2 where L1 = NBT, CBT and mBT and L2 = Nadha. Tetrahedral structure was proposed for all Cd(II) mixed ligand complexes while the square planar geometry was proposed for Cu(II) mixed ligand complex with NBT. Octahedral structure was proposed for Ni(II), Co(II) mixed ligand complexes and Cu(II) mixed ligand complexes with CBT and mBT ligands. The thermal decomposition study of the prepared complexes was monitored by TG, DTG and DTA analysis in dynamic nitrogen atmosphere. TG, DTG and DTA studies confirmed the chemical formulations of theses complexes. The kinetic parameters were determined from the the thermal decomposition data using the graphical methods of CoatsRedfern and Horwitzmetzger. Thermodynamic parameters were calculated using standard relations.

Weekday vs weekend endoscopy: Is there a true difference in patient outcomes? A meta-analysis

Feasibility and reliability of the revised Edmonton Symptom Assessment System(ESAS-r)in Egyptian patients with advanced cancer:A single institutional experience

Objective This study aims to test the acceptance, feasibility, and usefulness of the Arabic version of the revised Edmonton Symptom Assessment System(ESAS-r) among Egyptian patients with advanced cancer and to compare the rates of symptoms documented by patients and physicians. methods Between August 2014 and February 2015, a total of 140 patients at Ain Shams University Hospitals in Cairo, Egypt received the Arabic version of the ESAS-r. For each patient, the ESAS-r was completed twice, first by the treating physician(as part of the basic assessment) and a second time by the patient, with a maximum of 2 hours between the two assessments. An additional survey was included to assess patients' acceptance of the survey and their preferences. Results Out of 140 enrolled patients in the study, 11 patients refused to complete the questionnaire, and 10 patients were excluded due to incomplete records in their medical records. Complete data was retrieved for 119 patients who were included for further analyses. The 78(65%) patients declared that the test was clear and easy to complete. They were able to answer the test without help. Collectively, tiredness and sense of well-being were the most commonly encountered symptoms in ratings obtained by both patients and physicians. Tiredness was the only symptom showing a significant difference between the two rating methods, patient-rated scores being higher(P = 0.032). Cronbach's alpha showed that both tests completed by the physician and the patients were internally consistent: the physician-rated test had a coefficient of 0.877, and the patient-rated test had a coefficient of 0.863. All ESAS scores had good internal consistency, with a Cronbach's alpha coefficient of 0.88. The internal consistency remained high after removal of individual symptom scores, with Cronbach's alpha coefficients ranging from 0.823 to 0.902, indicating that no individual question had undue influence on the total ESAS *** The ESAS-r was eas
