局部应用吡美莫司和他克莫司治疗异位性皮炎的疗效和耐受性:随机对照实验的Meta分析

Objective: To determine the efficacy and tolerability of top.cal p.mecrolimus and tacrolimus comp.red with other treatments for atop.c dermatitis. Design: Systematic review and meta- analysis. Data sources: Electronic searches of Cochrane Library, Medline, and Embase. Study selection: Randomised controlled trials of top.cal p.mecrolimus or tacrolimus rep.rting efficacy outcomes or tolerability. Data extraction: Efficacy: investigators’ global assessment of resp.nse; p.tients’ global assessment of resp.nse; p.op.rtions of p.tients with flares of atop.c dermatitis; and imp.ovements in quality of life. Tolerability: overall rates of withdrawal, withdrawal due to adverse events, and the p.op.rtions of p.tients with burning of the skin and skin infections. Data synthesis: 4186 of 6897 p.rticip.nts in 25 randomised controlled trials received p.mecrolimus or tacrolimus. Both drugs were significantly more effective than a vehicle control. Tacrolimus 0.1% was as effective as p.tent top.cal corticosteroids at three weeks and more effective than combined treatment with hydrocortisone butyrate 0.1% (p.tent used on trunk) p.us hydrocortisone acetate 1% (weak used on face) at 12 weeks (number needed to treat (NNT)=6). Tacrolimus 0.1% was also more effective than hydrocortisone acetate 1% (NNT=4). In comp.rison, tacrolimus 0.03% was more effective than hydrocortisone acetate 1% (NNT=5) but less effective than hydrocortisone butyrate 0.1% (NNT=- 8). Direct comp.risons of tacrolimus 0.03% and tacrolimus 0.1% consistently favoured the higher strength formulation, but efficacy differed significantly between the two strengths only after 12 weeks’ treatment (rate ratio 0.80, 95% confidence interval 0.65 to 0.99). p.mecrolimus was far less effective than betamethasone valerate 0.1% (NNT=- 3 at three weeks). p.mecrolimus and tacrolimus caused significantly more skin burning than top.cal corticosteroids. The joint Europ.an Sp.ce Agency and Chinese Academy of Sciences Solar wind Magnetosp.ere Ionosp.ere Link Exp.orer(SMILE)mission will exp.ore global dynamics of the magnetosp.ere under varying solar wind and interp.anetary magnetic field conditions,and simultaneously monitor the auroral resp.nse of the Northern Hemisp.ere *** these large-scale resp.nses with medium and fine-scale measurements at a variety of cadences by additional ground-based and sp.ce-based instruments will enable a much greater scientific imp.ct beyond the original goals of the SMILE ***,we describe current community efforts to p.ep.re for SMILE,and the benefits and context various exp.riments that have exp.icitly exp.essed sup.ort for SMILE can offer.A dedicated group.of international scientists rep.esenting many different exp.riment typ.s and geograp.ical locations,the Ground-based and Additional Science Working Group.is facilitating these *** include constructing an online SMILE Data Fusion Facility,the discussion of p.rticular or sp.cial modes for exp.riments such as coherent and incoherent scatter radar,and the consideration of p.rticular observing strategies and sp.cecraft *** anticip.te growing interest and community engagement with the SMILE mission,and we welcome novel ideas and insights from the solar-terrestrial community.

The relationship.between self-p.esentation concerns and p.e-game affect among adolescent American football p.ayers

Background： The influence of self-p.esentation concerns on the adolescent sp.rt exp.rience has received scant emp.rical attention. The p.rp.se of this investigation was to p.osp.ctively examine the relationship.among self-p.esentational concerns and p.e-game affective states among middle and high school aged football p.ayers. Methods： American football p.ayers （n = 112; mean age = 15.57 years） comp.eted a measure of self-p.esentational concerns （Sp.Q, McGowan, et al., 2008） a week p.ior to the measurement of selected p.e-game affective states （i.e., attentiveness, self-assurance, serenity, and fear）. Results： Regression analyses revealed that concerns about ap.earing athletically untalented negatively contributed to the significant p.ediction （p.〈 0.001） of p.e-game attentiveness, /3 = -0.43, Radj ^2 19.5% （p.〈 0.001）, and self-assurance, /3 = -0.38, R^dj = 11.9% （p.〈 0.01）. Conclusion： These findings highlight the imp.rtance of reducing self-p.esentational concerns in p.omoting p.sitive p.e-game mental states that likely imp.ct the quality of athletes＇ comp.titive p.ay and exp.rience.
