Irbiben is a small gold deposit of the Precambrian inlier Tagragra d’Akka located in the Moroccan western Anti-Atlas, approximately 160 km southeast from Tiznit. Petrographic study shows that host rocks of Irbiben deposit are green schist facies metamorphic rocks, like metamorphic sandstone and schist, meta-pelite and quartzite. Gold is located in quartz veins or host rock with frequent injections of quartz. The petrographic characterization of alteration through the description of the alteration profiles and the comparative study of protolith and altered rocks showed a development of dominant quartz and calcite in mineralization wall-rocks. In moving away from it, there is rather sericite and chlorite. These phenomena are materialized respectively by the development of SiO2 and CaO in the wall-rocks but rather that ferromagnesian elements (FeO, MgO) and K2O away from the mineralization. These trends were demonstrated by the geochemical study through Harker diagrams, description of alteration profiles, comparative study of protolith and altered rocks, and use of a new alteration index, MIA, elaborated during this work.

Context/Objectives: Hepatocellular carcinoma occurs mainly and increasingly in developing countries, where the prognosis is particularly poor. The Barcelona Clinic Liver Cancer classification is used to guide the treatment of hepatocellular carcinoma. The aim of this retrospective study was to describe the Barcelona Clinic Liver Cancer classification and the treatment of hepatocellular carcinoma in a University Hospital in Côte d’Ivoire. Methods: Patients with hepatocellular carcinoma hospitalized in the hepato-gastroenterology unit of the University Hospital of Yopougon from 01 January 2012 to 30 June 2017 were included. The diagnosis of hepatocellular carcinoma was based on the presence of hepatic nodules on the abdominal ultrasound scan, typical images with the helical scanner associated or not with an increase of the α-fetoprotein higher than 200 ng/ml or with histology. Demographic, clinical, biological and radiological data were determined at the time of diagnosis. Patients were classified according to the Barcelona Clinic Liver Cancer classification. Their treatment was specified. Results: There were 258 patients whose median age was 48.1 years. Viral hepatitis B virus was the primary cause of hepatocellular carcinoma in 64.7% of cases. The severity of the underlying cirrhosis was Child-Pugh A in 12.1%, B in 63.6% and C in 24.3% of cases. The median size of the tumor was 63 mm. The α-fetoprotein level was higher than 200 mg/ml in 56.03% of cases. The Eastern Cooperative Oncology Group (ECOG)/World Health Organization (WHO) system was ≥2 in 82.9%. The Barcelona Clinic Liver Cancer classification was A in 1.3%, B in 0%, C in 55.2% and D in 43.5% of patients. There was no transplantation or hepatic resection. Very few patients (1.9%) received radio-frequency curative therapy. The treatment was predominantly symptomatic in 97.8% of patients. During hospitalization 43.7% of patients died. Conclusion: Hepatocellular carcinoma occurs on a liver with severe cirrhosis

Management of Priapism in Sickle Cell Patients: Experience of the Urology Department of the University Hospital Centre of Libreville

Introduction: Priapism is a rare pathology, known since antiquity. Sickle cell disease is the main aetiology in Africa. The aim of our work was to report our experience in its treatment. Material and Methods: This is a prospective, descriptive study carried out at the urology department of the Libreville University Hospital from January 2018 to December 2020. All sickle cell patients admitted to urology for priapism were included. The parameters studied were socio-demographic, clinical and therapeutic parameters as well as the evolution after treatment. Result: We collected 19 priapisms in sickle cell patients. The average age was 20.9 years with extremes of 4 and 53 years. Fifteen patients were homozygous SS. All patients had stasis priapism. The average consultation time was 22.4 hours. All patients had perioperative medical management combining hyperhydration, analgesia and antibiotic prophylaxis. A vasoactive drug was administered to 13 patients. Sixteen patients had a puncture of the corpus cavernosum. A distal cavernosal-spongiosum shunt under penile block was performed in 6 patients. The outcome was favorable from the outset in 12 cases, marked by complete detumescence of the corpus cavernosum. Partial detumescence was noted in 7 patients with the need for a new puncture of the cavernous body. A complication such as edema of the penis was in only one of our patients. A recurrence was noted in 2 patients. After an average follow-up of 6 months, no sequelae erectile dysfunction was observed. Conclusion: Priapism is a frequent complication among sickle cell patients in Libreville. Medical management associated with a cavernous puncture with administration of vasoactive drugs allows a favourable evolution without after-effects.

Multiple Autoimmune Syndrome:A Study of 25 Senegalese Cases

Introduction: Multiple autoimmune syndrome is rarely described in the African literature. We report 25 cases. Patients and Method: A Retrospective, descriptive study was carried out in the Rheumatology Department of Aristide Le Dantec University Hospital in Dakar from 2013 to September 2019. Patients with at least 3 distinct autoimmune diseases were included. Results: Twenty-five cases of multiple autoimmune syndrome were collected in 23 women and 2 men, with a mean age of 42.4 years (extremes: 24 and 67 years). The mode of discovery of the disease was a connective tissue disease in 19 cases (76%) and an organ-specific autoimmune disease in 6 cases. According to Humbert and Dupond’s classification, 18 patients (72%) had both type 2 and type 3 criteria. An isolated type 2 was noted in 5 cases (20%) and an isolated type 3 in 1 case (4%). One case (4%) was unclassifiable. The evolution under treatment was marked by remission in 84% of cases. Conclusion: Multiple autoimmune syndromes account for 5.66% of the autoimmune diseases in our study. They predominate in young women with a mean age of 42.4 years. Not all entities were classifiable by the Humbert and Dupond’s classification. The evolution under treatment was marked by remission in 84% of cases.

Purpose: During the COVID-19 pandemic, closing schools was discussed to limit the virus transmission, despite a lack of evidence on the role of young children in the virus spread. We undertook a study among Belgian primary schools to assess the SARS-CoV-2 antibody seroprevalence and its relationship with local incidence, school size, and socioeconomic status (SES). Methods: Schools were purposively selected using three criteria: an area with either a low or a high COVID-19 incidence, either a small or a large size, and either a low or a high SES. 932 (/2488, 38%) children and 242 (/444, 55%) staff signed informed consent. COVID-19 antibodies were tested using rapid finger prick tests. Results: Children participation was positively correlated with staff (r = +0.33;95% CI [−0.34;0.78]), and strongly with SES (r = +0.81;95% CI [0.40;0.95]). Seroprevalence was 21% in children (191/922) and 25% in staff (61/240). Seroprevalence did not correlate with local cumulative incidence (children: r = +0.06;95% CI [−0.59;0.67];staff: r = +0.26;95% CI [−0.40;0.74]). In staff, seroprevalence was higher in schools with higher SES (r = +0.37;95% CI [−0.29;0.79]), but not in children (r = −0.10;95% CI [−0.66;0.53]). Conclusion:The lower seroprevalence in children suggests they are lower transmitters than adults, but poor socioeconomic levels were less representative.

Current available treatment options for erectile dysfunction （ED） are effective but not without failure and/or side effects. Although the development of phosphodiesterase type 5 （PDE5） inhibitors （i.e. sildenafil, tadalafil and vardenafil） has revolutionized the treatment of ED, these oral medications require on-demand access and are not as effective in treating ED related to diabetic, post-prostatectomy and severe veno-occlusive disease states. Improvement in the treatment of ED is dependent on understanding the regulation of human corporal smooth muscle tone and on the identification of relevant molecular targets. Future ED therapies might consider the application of molecular technologies such as gene therapy. As a potential therapeutic tool, gene therapy might provide an effective and specific means for altering intracavernous pressure ＂on demand＂ without affecting resting penile function. However, the safety of gene therapy remains a major hurdle to overcome before being accepted as a mainstream treatment for ED. Gene therapy aims to cure the underlying conditions in ED, including fibrosis. Furthermore, gene therapy might help prolong the efficacy of the PDE5 inhibitors by improving penile nitric oxide bioactivity. It is feasible to apply gene therapy to the penis because of its location and accessibility, low penile circulatory flow in the flaccid state and the presence of endothelial lined （lacunar） spaces. This review provides a brief insight of the current role of gene therapy in the management of ED.

This study aims to contribute to improve knowledge on geological formations of Comoé basin. The petrographic study of the geological formations of Koun Fao has highlighted two major lithological families: magmatic rocks consisting of monzonites, monzogranites, diorites, biotite granodiorites, syenites, porphyritic micromonzonites and porphyritic dacite and metamorphic rocks from sedimentary origin (quartz schists, meta-greywackes, schists, andalusite chloritoschist and paragneiss). These formations are affected by amphibolite to greenschist facies metamorphism and hydrothermal alteration (pervasive and vein) marked by the presence of quartz, epidotes, chlorites and sericite. Minerals such as andalusite, muscovite and chlorite characterize a local low pressure contact metamorphism in the andalusite chloritoschist. Remote sensing data (Landsat 8 image) coupled with field data allowed the production of a geological map of the area. The study of the structures and microstructures highlighted two deformation mechanisms. These are flattening and simple shear (ductile and brittle). The study area is affected by four deformation phases: D1 marked by a N-S to NNE-SSW elongation, D2 marked by a NW-SE to NNW-SSE compression phase, D3 which is a NE-SW to NNE-SSW transpression phase and D4, responsible for late structures, marked by a NW-SE to NNW-SSE transpression phase.
