Comparative Study of Sublingual and Vaginal Misoprostol in Second Trimester Induced Abortion

Background: To identify an effective misoprostol-only regime for the termination of second trimester pregnancy. Objectives: To compare the efficacy, safety and acceptability of sublingual and vaginal misoprostol for second trimester pregnancy termination. Methods: In a prospective randomized comparative study, over 138 pregnant women at 13 - 20 weeks (91 - 140 days) of gestation requiring medical abortion were randomly assigned to the sublingual or vaginal route for misoprostol administration with dose schedule of 400 mcg every 3 hours up to 5 doses within 24 hours. The course of misoprostol was repeated if the woman did not abort within 24 hours. Primary outcome was the efficacy of the treatment to terminate pregnancy completely at 24 and 48 hours. Secondary outcomes measured were induction-abortion interval, side effects, failure rate, and women’s perception to these treatments. Results: At 24 h, the complete abortion rate was?87.88% in the vaginal administration group and 79.41% in sublingual group (difference 8.5%, 95%?CI: 3.8 to 13.2). No significant difference in the complete abortion rates was observed at 48 h(90.91% versus 88.24% difference: 2.7%, 95% CI: ?0.04 to 5.4) when vaginal and sublingual?groups were compared. Mean induction-abortion interval in sublingual and vaginal groups was 12.28 h (95% CI of mean 11.019 - 13.541 h) and 13.11 h (95% CI of mean 12.0301 - 14.1899 h) respectively;p = 0.485. The rates of side effects were similar in both groups except for fever, which was more common in vaginal group. Significantly more women in the sublingual group preferred the route as compared to vaginal administration (RR 1.618. 95% CI: 1.277 - 2.050;p < 0.0001). Conclusion: Both sublingual and vaginal routes of misoprostol are equally effective in medical termination of pregnancy in second trimester but sublingual route was preferred by the women.

The Impact of Heat Treatment and Surface Treatment on Thermal Conductivity of Heat Sinks Made of Aluminium Die Casting Alloys

This research contributes to understand the thermal management capabilities of Plate Fin Heat Sinks(PFHS)fabricated from *** uniqueness in this study is that the heat sinks were exposed to abrasive blasting,heat treatment,and graphene coating,and a full evaluation of the influence of the aforementioned treatments on the thermal management capacities of PFHS was *** PFHS is compared with 1)abrasive blasted and graphene coated heat sink,and 2)heat treated and graphene coated heat *** assess the thermal efficiency of the PFHS variants,a dedicated experimental set up was meticulously *** is noteworthy that a junction temperature of 60℃was assumed as the reference point for the *** results revealed that the charging cycle time which denotes the time required attaining the junction temperature,increased 1.3 times for the sample being abrasive-blasted at 0.5 MPa pressure and graphene-coated for 0.5 mm when the maximum heat input of 45 W is *** low heat input of 15 W is evaluated,the results revealed that there is no significant difference in charging cycle when compared to the untreated heat *** charging cycle time increased 2 times for the sample which is heat-treated at 450℃and graphene-coated for 0.5 mm at heat input of 15 *** finding unequivocally underscores the heightened capacity of the heat treated and graphene coated PFHS made of AlSi10Mg to withstand elevated junction temperatures.

Two-Stent Strategy for Bifurcation Lesions in Percutaneous Transluminal Coronary Angioplasty: Real-World Evidence

Background: Bifurcation lesions pose a high risk for adverse events after percutaneous coronary intervention (PCI). Evidence supporting the benefits of the two-stent strategy (2SS) for treating coronary bifurcation lesions in India is limited. This study aimed to evaluate the clinical outcomes of various 2SSs for percutaneous transluminal coronary angioplasty for bifurcation lesions in India. Materials and Methods: This retrospective, observational, multicentric, real-world study included 64 patients over 8 years. Data on demographics, medical history, PCI procedures, and outcomes were recorded. Descriptive statistics were computed using the SPSS software. Results: Patients (n = 64) had an average age of 65.3 ± 11.1 years, with 78.1% males. Acute coronary syndrome was reported in 18.8%, chronic stable angina in 40.6%, and unstable angina in 34.4% of participants. Two-vessel disease was observed in 98.4% of patients, and 99.4% had true bifurcation lesions. The commonly involved vessels were the left anterior descending artery (50%), left circumflex coronary artery (34.4%), and first diagonal artery (43.8%). Mean percent diameter stenosis was 87.2% ± 10.1%. The mean number of stents used was 2.00 ± 0.34. The 2SS techniques included the T and small protrusion (TAP) (39.1%), double kissing (DK) crush (18.8%), and the culotte techniques (14.1%). Procedural and angiographic success rate was 92.18%. Major adverse cardiovascular events at 1-year follow-up occurred in 7.8% of cases. Conclusion: The 2SS for bifurcation lesions showed favorable in-hospital and follow-up outcomes. Findings can serve as a resource for bifurcation angioplasty in India. Larger real-world studies with robust methodology are needed to validate these results.

Reliability assessment of three-dimensional bearing capacity of shallow foundation using fuzzy set theory

The aim of this study is to investigate the applicability of reliability theory on surface square/rectangular footing against bearing capacity failure using fuzzy set theory in conjunction with the finite element *** is modeled as a three-dimensional spatially varying medium,where its parameters(cohesion,friction angle,unit weight,etc.)are considered as fuzzy variables that maintain some membership *** is idealized as an elastic-perfectly plastic material obeying the Mohr-Coulomb failure criterion,where both associated and non-associated flow rules are considered in estimating the ultimate bearing capacity of the *** spatial variability of the soil is incorporated for both isotropic and anisotropic fields,which are determined by the values of scales of fluctuation in both the horizontal and vertical directions.A new parameter namely,limiting applied pressure at zero failure probability is proposed,and it indirectly predicts the failure probability of the *** effect of the coefficient of variation of the friction angle of the soil on the probability of failure is analyzed,and it is observed that the effect is ***,the effect of the scale of fluctuation on the probability of failure is investigated,and the necessity for considering spatial variability in the reliability analysis is well proven.

Remote sensing and GIS for land use/cover mapping and integrated land management： case from the middle Ganga plain

In India, land resources have reached a critical stage due to the rapidly growing population. This challenge requires an integrated approach toward harnes- sing land resources, while taking into account the vulnerable environmental conditions. Remote sensing and Geographical Information System （GIS） based technologies may be applied to an area in order to generate a sustainable development plan that is optimally suited to the terrain and to the productive potential of the local resources. The present study area is a part of the middle Ganga plain, known as Son-Karamnasa interfluve, in India. Alternative land use systems and the integration of livestock enterprises with the agricultural system have been suggested for land resources management. The objective of this paper is to prepare a land resource development plan in order to increase the productivity of land for sustainable development. The present study will contribute necessary input for policy makers to improve the socio-eeonomic and environmental conditions of the region.

An all-optical adder/subtractor （A/S） unit with the （TOAD） is proposed. The all-optical A/S unit with help of terahertz optical asymmetric demultiplexer a set of all-optical full-adders and optical exclusive- ORs （XORs）, can be used to perform a fast central processor unit using optical hardware components. We try to exploit the advantages of TOAD-based optical switch to design an integrated all-optical circuit which can perform binary addition and subtraction. With computer simulation results confirming the described methods, conclusions are given.

Impacts of soil fertility management on productivity and economics of rice and fodder intercropping systems under rainfed conditions in Odisha,India

Under small and marginal farm conditions,allocation of land exclusively for forages is not ***,integration of forages in existing crop geometry can ensure production of grain and fodder,simultaneously under rainfed *** experiment was conducted to study the effect of different nutrient management practices on rice and fodder intercropping systems under rainfed conditions during *** intercropping system comprised(i)sole rice(R),(ii)rice and cowpea(5:2)(CP)and(iii)rice and ricebean(5:2)(RB)whereas the different nutrient management practices comprised(i)application of farm yard manure(FYM)at 5t ha^(-1)(farmers'practice)(N_(1)),(ii)application of inorganic fertilizer(recommended dose of fertilizer(RDF)of rice,60:30:30 kg ha^(-1) of N:P_(2)O_(5):K_(2)O)(N_(2))and(iii)application of both FYM at 5t ha^(-1) and 50%of RDF inorganic fertilizer(N3).The results of the experiment revealed that the growth attribute such as leaf area was influenced significantly when fodder crops were taken as intercrops because rice plant was getting more nitrogen from soil due to nitrogen fixation of leguminous fodder *** the nutrient management practices,significant differences in leaf area were found beween N2 and and between N_(3)and N_(1)***,regarding total number of effective tillers,significant differences were found neither between nutrient management practices nor between cropping *** rice equivalent yield(REY)based on price(REY_(P))was found to be significantly lower in CP(2615 kg ha^(-1);-6.4%)and RB intercropping systems(2571 kg ha^(-1);-8.0%)than in R monocropping system(2794 kg ha^(-1)).However,the REY based on energy(REY_(E))of CP(2999 kg ha^(-1);+7.3%)and RB(2960 kg ha^(-1);+5.9%)were found to be significantly higher than that of R(2794 kg ha^(-1))irrespective of nutrient management *** different nutrient management practices,the N3 treatment recorded the highest REY_(P)and REY_(E)which was at par with

A brief survey of computerized expert systems for crop protection being used in India

In the recent years, a plethora of computerized expert systems has been developed for various sectors of agriculture in India. The availability of low-cost computers, agricultural knowledge and information technology professionals are the principal reasons for the development of so many agricultural expert systems. Among all agricultural expert systems, the expert systems for crop protection need special mention. These expert systems are meant to be used by farmers and other persons without much experience of using computers. Hence, special care must be taken while developing them. The current paper develops a taxonomy for the expert systems for crop pro- tection and briefly discusses four such expert systems for crop protection being used in India.

Stability study of fortified low-fat spreads with preservatives

Physicochemical variations in products are known to occur depending on storage *** study examined the effects of storage time on the physicochemical,sensory,rheological,and microbiological properties of fortified low-fat *** spreads containing potassium sorbate(PS;0.05%and 0.1%)or MicroGard 100(MG;0.5%and 1%)as preservatives and a control spread(CS)were packed in presterilized airtight polyethylene jars and their shelf lives were investigated by storage at(5±1)℃for 91 *** on the physicochemical properties,the addition of preservatives inhibited the formation of free fatty acids and thiobarbituric acid and influenced the pH and *** emulsion stability of the preserved samples was greater than that of the CS after 14 *** storage progressed,the sensory and rheological properties of the CS deteriorated continuously,whereas significantly smaller changes were observed for the samples containing 0.1%PS and 1%*** surface growth of yeasts and molds was the main cause of spoilage,particularly in the *** all the samples,the standard plate counts and the yeast and mold counts increased during *** contrast,the coliform counts were zero,indicating that the samples were manufactured and stored under hygienic *** samples containing lower levels of preservatives(0.05%PS and 0.5%MG)were acceptable for 70 d and 77 d as compared to 49 d for the ***,the addition of 0.1%PS and 1%MG extended the shelf life to 91 d.
