Isothermal Transport (Core-Flow Type) of Heavy and Ultraviscous Oil in Curved Pipes: A Transient Study by CFD

In the scenery of the oil industry, the remaining resources associated with light oils have an increasingly smaller share in the natural energy resources available to man, and in return the importance of resources associated with heavy oils has increased significantly. One of the drawbacks of this type of oil is associated with its low mobility due to the high viscosity in reservoir conditions, making the transport in pipelines very difficult, especially through pumping methods that require high powers. Thus, the development of new techniques and optimization of some existing technologies, aiming at the commercial use of heavy oil accumulations plays an important role. A viable technique that has been used is the core annular flow, in which small amounts of water are injected close to the pipe wall, lubricating the oil core, reducing friction and decreasing the pressure drop during the flow. In this sense, this work aims to perform, numerically, an energetic and hydrodynamic analysis of a heavy oil-water two-phase flow, using the core-flow technique, in curved pipes, in the Ansys CFX software. Results of the velocity, pressure, and volume fraction distribution of the involved phases are presented and analyzed. It was observed that the proposed mathematical model was able to accurately represent the analyzed phenomena and that a reduction factor in the pressure drop of 28.4 was obtained as compared to the heavy oil single-phase flow.

Inguinal Lymphadenectomy Assessment Associated with Penile Carcinoma Undergone New Strategies for Nodal Staging

Penile carcinoma is an uncommon urological tumour and provides an opportunity for curation on early stage of the disease. Nodal metastases are one of the most important prognostic factors for survival although detection of inguinal adenopathies could be related with an inflammatory or infectious etiology. A suspicion of bilateral metastasic nodal involvement should be taken with caution. Radical inguinal lymphadenectomy has been associated to a great deal of complications. Several anatomical studies have reported the true lymphatic drainage pathways in order to reduce the area of groin dissection. Nonetheless, a prophylactic modified inguinal lymphadenectomy should not be a systematic surgical procedure in all patients due to morbidity and questioned usefulness when there are not nodal metastasis. Classical imaging studies have a limited contribution to the diagnosis of lymph node metastasis. Nowadays, lymph node involvement may be diagnosed both minimally invasive and noninvasive techniques, such as dynamic sentinel lymph node biopsy in intermediate and high risk patients with nonpalpable lymph nodes, and fine needle aspiration biopsy in cases with palpable nodes. Their high effectiveness has facilitated the radical pelvic or inguinal lymphadenectomy that is only performed when there is histological confirmation of nodal involvement. A new video endoscopic technique has been developed at present to reduce postoperative complications although prospective studies are needed to assess outcomes. The appearance of adenopathies after surgical treatment of the primary tumour could be supported at the same *** inguinal lymphadenectomy should be carried out in selected patients to support a benefit on early stages with an extended survival.

Oily Water Treatment by Ceramic Membrane: Modeling and Simulation

The separation process of oily water using membranes has attracted the attention of researchers and engineers. The greater problem in the use of membrane separation process is the reduction in permeate flux due to clogged pores by oil deposition inside the membrane or by the effect of the concentration polarization. For this purpose, a theoretical study of a water/oil separation module was performed. This device consists of a tubular ceramic membrane provided with a rectangular inlet section. Numerical simulations were performed using Ansys CFX software to solve the mass and momentum conservation equations in the fluid and porous domains. Here was adopted the RNG k-ε turbulence model. The effect of the membrane porosity and the inlet velocity of the fluid mixture on the two-phase flow behavior inside the separation module were evaluated. Results of the volumetric fraction, velocity and pressure fields of the oil and water phases are presented and analyzed. The results indicate a higher oil concentration within the membrane for the cases of higher porosity, and that the inlet fluid mixture velocity does not substantially affect the velocity profile within the separation module. It is found that the maximum separation efficiency of the module was obtained with feed velocity of 40 m/s and membrane porosity of 0.44.

Effects of selenomethionine on acute toxicities from concurrent chemoradiation for inoperable stage Ⅲ nonsmall cell lung cancer

AIM: To prospectively determine the safety and tolerability of oral L-selenomethionine(SLM) with concurrent chemoradiation(CCRT) for Stage Ⅲ non-small cell lung cancer(NSCLC) and estimate if the incidence and/or severity of adverse events could be reduced by its ***: Sixteen patients with stage Ⅲ NSCLC were accrued to this single arm, phase Ⅱ study. CCRT consisted of radiation given at 2 Gy per fraction for 30-33 fractions, 5 d per week with concurrent weekly Ⅳ paclitaxel 50 mg/m2 followed by carboplatin dosed at an area under the time-concentration curve of 2. SLM was dosed in a loading phase at 4800 μg twice daily for one week prior to CCRT followed by once daily dosing during treatment. RESULTS: No selenium-related toxicity was observed. Analysis revealed grade 3 or higher esophagitis in 3 of 16 patients(19%), pneumonitis in 0, leukopenia in 2(12.5%), and anemia in 1(6%); the latter two were significantly reduced when compared to the protocolstated expected rate of 35%(P = 0.045 for leukopenia, and P < 0.01 for anemia). Median overall survival was 14.9 mo and median failure-free survival was 9 mo(95%CI: 3.3-21.5).CONCLUSION: There may be some protective benefit of selenium in the setting of CCRT for inoperable NSCLC. The data suggests decreased rates of myelosuppression when compared to similarly-treated historical and contemporary controls. Further evaluation of selenium in this setting may be warranted.

由工农业废料配制的两种堆肥作为试验用的基质:C1——酿造啤酒等废料(酵母和麦芽)加修剪下的柠檬树枝;C2——橄榄研磨厂废水的固体级分加橄榄树叶。以每种堆肥与泥炭藓泥炭或商品基质(CS)按不同的比例混合,可配制成16种基质。尽管基质的物理和理化特性例如总孔隙度和电导率(EC)都是合适的,但由堆肥提供的养分(氮和钾)释放缓慢,对金盏花发育的影响尤为明显。另一方面,由于对盐分含量敏感,EC和氯化物浓度是影响蒲包草生长的主要因素。为供金盏花发育的适宜的基质可以通过含量高达75%C1与泥炭或高达50%C1与CS混合,以及高达50%C2与泥炭或CS混合使用。对蒲包花来说,基质中堆肥比例应较低,可用50%C1与泥炭或CS混合,也可以用25%C2与泥炭或CS混合使用。因此,只要基质至少含有25%的泥炭或CS,源于工农业废料的堆肥可作为培育观赏植物的泥炭和CS的替代品。

横纹肌肉瘤(RMS)是一类异质性的软组织肿瘤,目前包括4种不同组织学亚型,但这些亚型尚不足描述日常病理工作中遇到的所有RMS类型。作者报道一类最新发现特异性RMS组,以期进一步完善WHO RMS分类。在对RMS进行常规RNA测序过程中,作者发现了3例分化良好的RMS含有新的融合基因,并应用比较基因组杂交技术对该组肿瘤行进一步分析。临床上,3例肿瘤均为深部脊柱旁肿瘤,发于新生儿和幼儿,均接受手术切除和辅助化疗,在最后一次随访(12~108个月)时均无瘤存活。组织学上这3例肿瘤生长模式相似:周边的肌母细胞核中度非典型性,在纤维间质中形成小束状结构,可见浸润性边界,中央的梭形细胞和横纹肌母细胞核非典型性明显,以大片的两种细胞混杂生长模式为特征,核仁明显,但核分裂少见。RNA测序显示该组肿瘤含有新的融合基因即SRF-FOXO1和SRF-NCOA1。与另外33例骨骼肌肿瘤包括胚胎型RMSs、腺泡状RMSs、VGLL2融合RMSs、MyoD1突变RMSs、骨的EWSR1/FUS-TFCP2上皮样和梭形细胞RMSs及PTCH1缺失横纹肌瘤表达谱进行比较,并根据聚类分析,作者发现这3例SRF融合肿瘤形成一组不同于其他类型骨骼肌肿瘤的特定表达谱,经aCGM分析显示11号染色体重复获得。

Therapeutic Management in Proximal Tracheal Rupture during Total Thyroidectomy

Tracheal disruption is a life-threatening rare complication of total thyroidectomy that it should be prevented. The own-patient risk factors, procedure of tracheal intubation and a meticulous surgical technique are three main keys to keep in mind. Both medical and surgical management of this injury depends on its size, location and patient’s symptoms although neither of them prevents late complications. An early diagnosis could improve with prognosis although it is mandatory to perform either respiratory functional or imaging study in order to assume its healing. When the tracheal laceration is diagnosed during the surgery, we should take care with a continuous monitoring of the patient’s vital signs. A good oxygenation will be succeeded in high volume and low pressure with endotracheal tube distal to the lesion. We advice placing a tube drain near the disruption to prevent emphysema and an early extubation to avoid an ischaemic damage of the mucosa. A postoperative suspicion of tracheal rupture could demand reintubation with the patient in full relaxation and an examination by CT-scan or fiberscope before deciding a simple supportive therapy. We present our therapeutic experience in a female patient who suffered from tracheal injury during total thyroidectomy and describe a review of literature.

目的　比较柯萨奇B3m株病毒 (CVB3m)和柯萨奇B3o株病毒 (CVB3o)在实验中引起小鼠心肌炎的发病情况 ,为心肌炎造模者提供客观的实验参考。方法　用CVB3m和CVB3o感染Balb/c实验小鼠 ,测定两株病毒在鼠心肌细胞中的滴度 ;用常规病理法观察两株病毒致小鼠心肌炎的情况。结果CVB3m和CVB3o感染组小鼠心肌病毒滴度的峰值出现的时间基本相同 ,CVB3m的滴度较高 ;CVB3m感染组小鼠死亡率明显高于CVB3o,两株病毒致小鼠心肌炎的发病率分别为 76 %和 17%。结论　CVB3m和CVB3o致小鼠心肌炎的差异明显 ,在制造小鼠心肌炎模型时建议选择CVB3m。

Background:With the rising global prevalence of fatty liver disease related to metabolic dysfunction,the association of this common liver condition with chronic kidney disease(CKD)has become increasingly *** 2020,the more inclusive term metabolic dysfunction-associated fatty liver disease(MAFLD)was proposed to replace the term non-alcoholic fatty liver disease(NAFLD).The observed association between MAFLD and CKD and our understanding that CKD can be a consequence of underlying metabolic dysfunction support the notion that individuals with MAFLD are at higher risk of having and developing CKD compared with those without ***,to date,there is no appropriate guidance on CKD in individuals with ***,there has been little attention paid to the link between MAFLD and CKD in the Nephrology *** and Results:Using a Delphi-based approach,a multidisciplinary panel of 50 international experts from 26 countries reached a consensus on some of the open research questions regarding the link between MAFLD and ***:This Delphi-based consensus statement provided guidance on the epidemiology,mechanisms,management and treatment of MAFLD and CKD,as well as the relationship between the severity of MAFLD and risk of CKD,which establish a framework for the early prevention and management of these two common and interconnected diseases.
