BACKGROUND:The traditional hand-sewn Roux-en-Y choledochojejunostomy is technically complicated,and the incidence of postoperative complications has remained high.A set of novel magnetic compressive anastomats was introduced to facilitate choledochojejunostomy and improve the prognosis of ***:After ligating the common bile duct for 7 days,16 dogs were randomly divided into two groups (n=8 per group).Anastomats were used in the study group,and the traditional hand-sewn method was used in the control group for standard Roux-en-Y *** compared the operation time,incidence of complications,gross appearance,and pathological disparity in stoma between the two groups in 1-month and 3-month follow-up ***:The time spent on constructing the anastomosis for the study group was significantly *** no anastomotic stenosis occurred in the two groups,the narrowing rate of biliary-enteric anastomosis was much higher in the control *** was one case of bile leakage in the control group,whereas no bile leakage occurred in the study group.A smoother surface,an improved layer apposition,and a lower local inflammatory response were identified in the anastomosis of the study ***:The structures of the novel magnetic compressive anastomats are simple,and they are time-saving,safe and efficient for performing Roux-en-Y choledocho- jejunostomy procedures in a canine model of obstructive jaundice.

Relationship between peritoneal macrophages and inflammatory reaction in a rat model of severe acute pancreatitis

Application of laparoscopic cholecystectomy in patients with cirrhotic portal hypertension

BACKGROUND: Laparoscopic cholecystectomy (LC) has been widely adopted in treating benign gallbladder disea- ses. Cirrhosis and cirrhotic portal hypertension (CPH) are contraindicated for LC in its early period. In recent years, several studies have reported liberal use of LC in patients with cirrhosis. But its benefits and successful use in patients with CPH are less documented. This study was designed to evaluate the feasibility, safety and technical characteristics of LC in CPH patients. METHODS: In 38 patients with symptomatic gallbladder disease and CPH, 19 belonged to Child A class, 15 Child B class and 4 Child C class. Perioperative data of these pa- tients were collected and analyzed. RESULTS: LC was successfully performed in 36 patients, and 2 patients (5.3%) were converted to open cholecystec- tomy (OC) for difficulty in management of bleeding under laparoscopy and dense adhesion of Calot' s triangle. The sur- gical time was 62.6±15.2 minutes. The estimated amount of intraoperative hemorrhage was 75.5±15.5 ml. No blood transfusion was necessary. The time to resume diet was 18.3 ±6.5 hours. Seven postoperative complications in 5 patients (13.2%) included port-site infection (1 patient), respiratory infection (2), upper digestive tract bleeding (1), slight hepatic encephalopathy (1) and increased ascites (2). All patients were cured and discharged from the hospital within 5.6±2.4 days after LC. CONCLUSIONS: Despite LC is difficult for CPH patients, it is feasible and relatively safe. To make LC successful in patients with CPH, it is necessary for surgeons to acquaint with the technical characteristics of LC and emphasize me- ticulous perioperative management.

Objective: To investigate the procedure choice of pal-liative operation for carcinoma of the head of the pan-creas (CHP).Methods: The clinical data from 187 patients withCHP treated in the last 20 years were analyzed ***: The operation mortality rate was 8.6%, themortality of hepatic duct-jejunostomy (HDJS) was nothigher than that of cholecystojejunostomy (CJS) (P>0.05). The postoperative relapse of jaundice and cholan-gitis was significantly lower than that of CJS (P0.05), whereas the survival was significantlyhigher than that of the simple procedure without GJS(P<0.01). The occurrence of duodenal obstructionafter HDJS or CJS was 29.3%.Conclusion: As a palliative operation, Roux-en-Y chole-dochojejunostomy especially in combination with pre-ventive gastrojejunostomy is strongly recommended.

A modified rat model for hepatocellular carcinoma

BACKGROUND: Rat hepatocellular carcinoma ( HCC ) model which has a high analogy to clinical liver cancer is of great value in understanding the pathogenesis and evolution of liver cancer, in searching effective anti-cancer treat- ments ( drug, hepatectomy and liver transplantation ), and designing cancer prevention strategies. In this study we es- tablished a modified rat model of hepatocellular carcinoma to enhance rats' physique and surgical endurance. METHODS: Wistar rats were fed with diethylnitrosamine (DENA) by three methods for evaluation of general condi- tions for 130 days: Doppler ultrasonographic measurement, laparotomy and histopathological examination. RESULTS: No rat died in control group ( group A) and modified DENA-induction-HCC group ( group C), but 6 deaths in classical DENA-induction-HCC group (group B) (survival rate 80%). All survived rats in groups B and C de- veloped diffusive hepatocellular carcinoma and liver cirrho- sis. General appearance of rats in the group C was better than that in the group B. CONCLUSION: With good general conditions for surgery, the modified rat model for hepatocellular carcinoma has a high carcinogenic rate and a high survival rate.

Hepatic segmentectorny for treatment of hepatic tuberculous pseudotumor

BACKGROUND: This study was designed to explore the preoperative diagnosis and surgical modality of patients with hepatic tuberculous pseudotumor. METHODS: Of 682 patients who had undergone liver resection from January 1988 to December 2004, 8 were confirmed pathologically as having hepatic tuberculous pseudo-tumor after operation. Their clinical features, laboratory findings, results of preoperative imaging and surgical modality of the 8 patients were analyzed. RESULTS: In these patients,5 were misinterpreted as having other types of liver tumor and 3 were confirmed as having liver tuberculous pseudotumor preoperatively. All the 8 patients underwent hepatic segmentectomy and local hepatic resection. Seven had no tumor recurrence after follow-up for 4 years. CONCLUSIONS: Hepatic tuberculous pseudotumor was highly suspected for the patients with hepatic occupying-space lesions who had a history of tuberculosis. Fine needle aspiration liver biopsy guided by B-mode ultrasound and CT scan could confirm the diagnosis. They are of vital importance in the pathological diagnosis of the tumor. Therapeutic modalities included all kinds of hepatic segmentectomy and postoperative administration of antituberculous agents for the enhancement of the therapeutic effects.

Low-concentration sodium hydroxide solution injection in normal liver parenchyma of rabbits

BACKGROUND: Percutaneous ethanol injection has been widely used as a non-surgical therapy for liver cancer, but it has some shortcomings such as local diffusion and une- qual permeation. This study was designed to observe the volume, controllability and completeness of necrosis after injection of low concentration sodium hydroxide in the normal liver parenchyma so as to assess its possibility in treatment of liver cancer instead of ethanol. METHODS: Twenty-seven New Zealand rabbits were di- vided randomly into 9 groups (Aa, Ab, Ac, Ba, Bb, Bc, Ca, Cb, and Cc) by a 3 × 3 (three-by-three) factorial de- sign, each consisting of 3 rabbits. Group A was given sodi- um hydroxide solution at a concentration of 5%, while B at 2.5% and C at 1% in liver parenchyma. Each group re- ceived three doses of the solution: a (0.2 ml), b (0.5 ml) and c (1.0 ml). Then another 3 rabbits as side-effect group were dropped with sodium hydroxide solution in their liver lobe space. Liver and renal function changes in all the rab- bits were compared after injection with pre-injection. RESULTS: All the lesions were localized. At the concentra- tion of 2.5% and 5%, the lesion volume increased with the dose increased from 0.2 ml to 1.0 ml (P < 0. 05). No sig- nificant differences were found in the lesion volume of the groups receiving the same dose but different concentration. Changes in liver and renal function were not significant 7 days after injection, compared with those before injection. CONCLUSIONS: 2.5% and 5% sodium hydroxide solution could control local complete necrosis in normal liver. With regard to safety, 2.5% alkali solution is considered promis- ing as a new agent for intratumoral injection therapy in- stead of ethanol.

BACKGROUND: The incidence of extrahepatic bile duct carcinoma (EBDC) has been increasing, especially in aged people, but the glycobiology of the tumor is not elucida- ted. In this study we investigated the expressions of three glycosyltransferases in 35 patients with EBDC and 35 pa- tients with benign biliary duct disease (BBDD) as well as their clinicopathological significance. METHOD: The patients were divided into several sub- groups by tumor differentiation, TNM stage, and invasion by the standards recommended by UICC. Tumor samples were immediately frozen in liquid nitrogen after resection, followed by mRNA determination of enzymes in the tissue using a mRNA selective reverse trancriptase-polymerase chain reaction kit. The mRNA levels of different groups were semi-quantitatively compared. RESULTS: The mRNA levels of N-acetylglucosaminyltrans- ferase V (GnT-V) and a subtype of α2,3 sialyltransferases for N-glycans, ST3Gal- were elevated 7.75 and 5.39 times in EBDC as compared with BBDD, respectively, and they were correlated to several clinicopathological factors including tumor advancement, differentiation, metastasis, and invasiveness. The mRNA expression of another sialyl- transferase, ST6Gal- , was also 0.63-fold higher in EBDC than in BBDD, but not involved in the clinicopathological characteristics. CONCLUSION: The elevated expression of these three gly- cosyltransferases can be considered as an important molecu- lar event in the occurrence and progression of EBDC.
