BACKGROUND: Some factors have been reported to besassociated with a greater likelihood of sustained viral re-sponse ( SVR) in the interferon (IFN) treatment of chronichepatitis C. The factors include HCV genotype, HCVRNA level in serum, state of liver disease, baseline bodyweight, age, sex, and race. The aim of this trial was to in-vestigate the influence of HCV genotype on the IFN treat-ment of patients with chronic hepatitis ***: The genotypes of HCV virus were determinedin the patients with chronic hepatitis C from several hospi-tals of china enrolled into the randomized, opened andcontrolled trial of Peg-IFN alpha-2a (pegasys) treatment,controlled with IFN-α-2a (roferon-A). The serum ALTlevels and HCV RNA concentrations of the patients weredetected before and at the end of treatment and during thefollow-up. The influence of HCV genotype on the IFNtreatment of patients with chronic hepatitis C was analyzedin intention-to-treat (ITT) ***: The HCV genotypes of 202 patients were deter-mined. Of these patients, 158(78.22%) were infected withgenotype 1 HCV and 44(21.78%) with genotype *** viral response at the end of treatment (ETVR) andsustained viral response (SVR) rates were 53.80% and25.32% respectively in patients with genotype 1 HCV, butthey were 61.36% and 43.18% in patients with genotypenon-1. The difference of SVR between patients with geno-type 1 HCV and those with genotype non-1 was significant(P =0.021). After being grouped by the used drugs, theETVR rates of patients infected with genotype 1 and non-1HCV were 76.83% and 80.95% in the patients treated withpegasys (P =0.686); but their SVR rates were 35.37% and66.67% (P =0. 01). The viral relapse rate of genotype 1HCV (55.56%) was significantly higher than that of geno-type non-1 HCV (23.53%) (P=0.02). The second envelope glycoprotein （E2） of hepatitis C virus has been shown to bind human target cells and has become a major target for the development of anti-HCV vaccines. Anti-E2 antibodies have been suggested to be of clinical significance in diagnosis, treatment and prognosis of hepatitis C. However, large-scale expression and purification of E2 proteins in mammalian cells is difficult. As an alternative, E2 fragment （aa 385-730） with a four-amino-acid mutation （aa 568-571 PCNI to RVTS） was expressed as hexa-histidine-tagged full length protein [E2N730（m）] in E. coli and purified to over 85% purity. Purified E2N730（m） was specifically recognized by homologous hepatitis C patient serum in Western blot, suggesting that it displayed E2specific antigenicity. Rabbit antiserum raised against E2N730（m） recognized E2 glyco-proteins expressed in mammalian cells in Western blot. BACKGROUND:Proximal migration of pancreatic stent (PMPS)is an infrequent event but its management can be technically challenging and there are no standard retrieval *** study aimed to determine the results of an endoscopic stent retrieval algorithm in terms of feasibility and efficacy of the endoscopic procedure. METHODS:During the period from January 2008 to December 2009,15 patients(8 women and 7 men with a mean age of 51.9 years)with PMPS were included in this *** retrieval was approached initially with balloon extraction followed by rat-tooth forceps and basket.A rescue approach such as using a stent retriever was attempted when other approaches failed. RESULTS:All the PMPSs(six 5Fr,nine 7Fr)were retrieved successfully within one ERCP *** extraction was successful in 9(60%)*** the 6 failed cases of balloon extraction,wire-guided rat-tooth forceps grasp was successful in 4,and stone extraction basket grasp was successful in 1 in whom forceps grasp *** stent was finally rescued with a stent retriever when balloon extraction,forceps and basket grasp all *** patients with successful balloon extraction,44.4% (4/9)developed post-ERCP hyperamylasemia but none of them developed post-procedure pancreatitis. CONCLUSIONS:With this algorithm,5Fr and 7Fr PMPS were successfully retrieved in all of the *** PMPSs can be safely retrieved with the commonly-used approaches in this *** less used approaches can be used as a rescue method.

Effect of transforming growth factor-beta on activity of connective tissue growth factor gene promoter in mouse NIH/3T3 fibroblasts

AIM: To investigate the regulatory mechanism of transforming growth factor-betaon activity of connective tissue growth factor promoter in mouse NIH/3T3 fibroblasts. METHODS: The regulation fragment of the 5' flanking region of the human CTGF gene was linked to pGL3-Basic vector, a firefly luciferase reporter construct without promoter. The recombinant plasmid pCTGF-luc was transiently transfected to NIH/3T3 fibroblasts. The activity of CTGF promoter after treatment of TGF-β1 and MAPK pathway inhibitors were assayed with luciferase reporter gene assay system. RESULTS: TGF-β1-induced increase of CTGF promoter activity was concentration-dependent, with a plateau at 5 μg/L by 2.67-fold vs control (P<0.05). The TGF-β1 stimulation of CTGF promoter activity was time-dependent, too. After exposure to TGF-β1(5 μg/L), the maximal level of luciferase activity was reached at 12 h and maintained to 24 h by 2.76- and 2.20-fold vs control, respectively (P<0.05). Blockade of mitogen-activated protein kinases (MAPK) pathway with PD98059 (10 μmol/L), the MAP kinase kinase 1 inhibitor, and SB203580 (10 μmol/L), the p38 MAP kinaseinhibitor, decreased basal and TGF-β1-induced activation of CTGF promoter. However, inhibition of c-Jun-N-terminal kinase/stress-activated protein kinase by SP600125 (20 μmol/L) was without effect. CONCLUSION: TGF-β1stimulated the transcriptional activity of CTGF gene promoter in NIH/3T3 fibroblasts in a dose- and time-dependent manner. MAPK pathway may play a role in the regulation of TGF-β1-induced CTGF expression.

AIM:To analyze the clinical manifestations and risk fac-tors of complications in laparoscopic low anterior resection(LAR)for rectal cancer ***:A series of 132 consecutive patients who received laparoscopic LAR for rectal cancer in our center were *** etiology,diagnosis,treatment and prevention of rectal cancer were studied among the patients with surgery-related complications using both univariate and multivariate regression ***:No conversion to open surgery was observed and 5 cases converted to hand-assisted laparoscopic *** overall morbidity rate was 20.5%.Complications occurred during the operation in 7 patients(5.3%),within 30 postoperative days in 24 patients(18.2%),and within 3 mo in 2 patients(1.5%).The most significant complications were anastomotic leak-age(9.1%)and anastomotic hemorrhage(5.3%).Sizeand location of tumor,pathological staging and preoperative nutrition were significant factors associated with LAR complications,while gender,age and pathological type showed no *** logistics regression showed that the size and location of tumor,and pathological staging were independent factors of laparoscopic *** the complications were treated during their onset of clinical manifestations by interventional or conservative ***:Anastomotic leakage is a major com-plication in laparoscopic *** complications may be associated with tumor size and site,and pathological *** therapies are of value in the management of laparoscopic LAR complications.

CT manifestations and features of solid cystic tumors of the pancreas

Objective: To study retrospectively the manifestations and characteristics of solid cystic tumor of the pan- creas(SCTP) on CT. Methods: Nine patients with solid cystic tumor of the pancreas underwent resection with pathological and immunohistochemical confirmation. All the patients were female, aged 27.7 years on average at onset of the disease. CT scan was performed in patients with or without intravenous injection of contrast medium; it was also performed in the arterial phase in 4 pa- tients. Results: Solid and cystic structures were observed in tumors of the pancreas. Solid structure demonstrated low or iso-density on unenhanced CT, poor enhance- ment in the arterial phase and marked enhancement in the portal venous phase on contrast material-en- hanced CT. Cystic structure showed low density on both pre- and pro-contrast views. The CT findings of tumors with predominantly cystic or equal propor- tion of solid and cystic structure: mural nodules for solid structure, 'floating cloud' signs, or solid and cystic crossing distributions. Cystic structure of tumors predominately solid presenting as a beading appearance under capsule. In round, oval and com- pletely encapsulated tumors in the pancreas, the greater vessel enhanced with well-defined margins on pro-contrast images. No dilatation was observed of the common bile and pancreatic ducts. Conclusion: The characteristics of solid cystic tumor of the pancreas revealed by CT could confirm the di- agnosis of the disease in combination with onset age and sex of patients.
