102204例婚检女性疾病检出率及分类

目的　进一步强调女性婚前保健的必要性。　方法　婚前医学检查 (简称婚检 )按照卫生部《婚前保健工作规范》及《青岛市婚前保健服务基本标准》执行 ,对青岛市 2 0 0 0年至 2 0 0 1年10 2 2 0 4例婚检女性检出疾病情况进行回顾性分析。　结果　疾病检出率 8.1% ( 82 2 4 /10 2 2 0 4 ) ;生殖系统疾病占 36 .2 % ( 2 973/82 2 4 ) ,内科系统疾病占 19.8% ( 16 2 4 /82 2 4 ) ,严重遗传病及先天性疾病占 3.9% ( 32 2 /82 2 4 ) ,指定传染病占 3.7% ( 30 1/82 2 4 ) ,有关精神病占 0 .6 % ( 4 5 /82 2 4 ) ,其它系统疾病占 36 .0 % ( 2 95 9/82 2 4 ) ;有人流史者占婚检人群的 2 8.5 % ( 2 912 8/10 2 2 0 4 ) ;对患影响婚育疾病的 375人提出了医学处理意见 ,其中暂缓结婚 346人 ( 92 .3% ) ,不宜生育 2 9人( 7.3% )。　结论　做好婚前保健 ,是实现生殖健康的重要环节。

Oncofertility is an extremely signiﬁcant topic that is increasingly being discussed owing to increased evidence indicating that fertility preservation does not affect the treatment outcomes of patients with cancer but signiﬁcantly contributes to preserving life *** effect of chemotherapy can range from minimal effects to complete ovarian *** data are available on the effects of monoclonal antibodies and targeted therapies on the ovaries and *** ovarian suppression by administering a gonadotropin-releasing hormone agonist(GnRHa)during chemotherapy decreases the gonadotoxic effect of chemotherapy,thereby diminishing the chance of developing premature ovarian insufﬁciency(POI).At present,the concomitant administration of GnRH analogs during chemotherapy is the only accepted pharmacological method for preserving ovarian ***,most randomized studies on the effectiveness of luteinizing hormone-releasing hormone agonists during chemotherapy in preventing POI have been conducted in women with breast cancer,with a considerably small number of studies on patients with hematological ***,most randomized controlled trials on breast cancer have revealed a decrease in treatment-induced POI risk,regardless of the hormone receptor *** addition,studies on hematological malignancies have yielded negative results;nevertheless,theﬁndings must be interpreted with caution owing to numerous *** guidelines from the American society of Clinical Oncology and EsMO Clinical Practice Guidelines recommend sperm,oocyte,and embryo cryopreservation as a standard practice and only offering GnRHa to patients when proven fertility preservation methods are not *** this manuscript,we present a comprehensive literature overview on the application of ovarian suppression with GnRHa during chemotherapy in patients with cancer by addressing preclinical and clinical data,as well as future perspectives in thisﬁeld

Objective To investigate the effect of different down-regulation protocol on the in vitro fertilization-embryo transfer （IVF-ET） outcomes in infertile patients with endometriosis （EMs）.Methods A retrospective case control study was performed. Totally 294 infertile patients with EMs were enrolled. And 109 patients （116 cycles） received prolongedprotocol as the control, 185 patients （193 cycles） received long protocol as case group, all followed by standard controlled ovarian hyperstimulation （COH）. Response to gonadotropins, the fertilization rate, the cleavage rate, the implantation rate, the clinical pregnancy rate and the miscarriage rate were measured and analyzed between the two groups. Results A trend toward better ovarian response was observed in long protocol group. Higher fertilization rate, lower total dose of rFsH, shorter duration of stimulation and more endometrial thickness on the day of hCG injection were observed in long protocol group compared with those of prolonged protocol group, and the difference was significant （P〈0.05）. In addition, the clinical pregnancy rate, the cleavage rate and the implantation rate also had an increase trend in long protocol group compared with those of prolonged protocol group, but without significant differences. Conclusion Long protocol regimen before IVF-ET in patients with EMs resulted in a trend toward better ovarian response and higher clinical pregnancy rates than prolonged protocol regimen did.

Objective To analyze the possible cause leading to death during the procecture oy IUD removal under hysteroscopy and how it could be prevented. Methods All （11 110） cases of IUD removal under hysteroscopy performed at 87 hospitals in shanghai from 2001 to 2007 were reviewed. Four dead cases were reported. Results Air embolism was suspected as the cause of death. Associated risk factors included insufficient preoperative preparation, short duration between surgeries, non-standard procedure, careless observation and untimely resuscitation. Conclusion This emphasizes the necessity of early interventions taken such as prevention, early detection and management of the fatal complications.

Coronavirus disease 2019(COVID-19)is a public health disaster that has not been encountered for a hundred *** January 12,2020,the World Health Organization(WHO)confirmed and named the coronavirus which caused unexplained pneumonia as 2019 novel coronavirus(2019-nCoV).since then,the disease caused by this virus had been named as coronavirus disease 2019(COVID-19).On February 11,2020,the International Committee on Taxonomy of Viruses announced that the English name of the novel coronavirus was severe acute respiratory syndrome coronavirus 2(sARs-CoV-2)[1].At present,sARs-CoV-2 infection is still rampant *** of september 10,2021,there were about 222 million confirmed cases of COVID-19 and more than 4.5 million deaths worldwide[2].According to the American centers for Disease Control and Prevention,by July 29,2021,4.19 million pediatric COVID-19 cases had been reported in the United states。

Objective： The aim of the study was to investigate the effect of sG1-1776 combination with DDP in sub-toxic concentration on induction of apoptosis of human ovarian cancer HO-8910 ceiis in vitro and to unravei the associated mechanisms. Methods： Human ovarian cancer HO-8910 cells were cultured in vitro. The inhibitory effect of sG1-1776 combination with DDP in sub-toxic concentration on induction on viability of human ovarian cancer HO-8910 cells was evaiuated by the MTT assay. Cell apoptosis rate was analyzed by flow cytometry. The proteins expression level related to apoptosis were analyzed by Western blot. Results： sG1-1776 combination with DDP in sub-toxic concentration significantiy inhibited the proliferation of human ovarian cancer HO-8910 cells, and proliferation inhibition rate was increased drastically compared with normai saline （Ns） group or DDP group in sub-toxic concentration or sG1-1776 group in sub-toxic concentration （P 〈 0.01）. Apoptosis rate markedly increased after the treatment of sG1-1776 combination with DDP in sub-toxic concentration for 48 h. Western blot showed that the expression of bcl-2 protein was down-regulated and protein level of Bax and Cyto-c were depressed by sG1-1776 combination with DDP in sub-toxic concentration. Cenclusion： sG1-1776 combination with DDP in sub-toxic concentration could inhibit the cell proliferation and lead to cell apoptosis inhuman ovarian cancer HO-8910 cells, and its mechanism may be related to through mitochondrial apoptotic pathway.

患者男,38岁,因双侧鼻塞伴嗅觉丧失2年余于2016年3月12日入院.患者2年来无明显诱因出现双侧鼻塞,呈持续性,伴嗅觉丧失、脓涕,无头昏、头痛等不适,经外院药物治疗及2次内镜鼻窦手术（具体不详）后,鼻塞仍进行性加重,为求进一步诊治收入天津市第一中心医院.既往有哮喘病史7年.专科查体：外鼻无畸形,鼻窦各区无压痛,鼻内镜下见：双侧鼻腔黏膜高度肿胀,大量荔枝肉样新生物堵塞鼻腔,伴大量黏脓性分泌物,鼻中隔大致居中,双侧下鼻甲肥大,鼻咽部结构不可见.鼻窦CT示：双侧上颌窦、筛窦、额窦、蝶窦及双鼻腔见软组织影,窦壁骨化明显（图1）,Lund-Mackay评分为24分.

目的分析早产儿和足月儿神经发育结局,探讨高危儿门诊对早产儿早期神经发育干预的作用,为临床早期干预提供一定依据。方法选择2008—2011年在我院高危儿门诊规律随访的早产儿为观察组,随机抽取同期在本院儿童保健门诊保健的健康足月儿为对照组,在早产儿校正年龄/足月儿生后年龄3个月、6个月时分别进行Gesell测试和DDsT筛查,评估并比较两组婴儿的神经发育。结果校正年龄3个月时,早产儿大运动、语言、适应性与个人社交落后于足月儿[(62.2±22.7)分比(82.6±20.0)分,(91.8±49.3)分比(117.0±22.5)分,(63.1±29.5)分比(88.7±19.4)分,(66.5±35.7)分比(92.2±17.5)分,P均0.05);校正年龄6个月时,早产儿大运动、精细运动、语言、适应性、个人社交等发育商与足月儿差异均无统计学意义(P>0.05)。早产儿校正年龄6个月时大运动、适应性和个人社交发育商与校正年龄3个月时相比均提高,差异有统计学意义(P<0.05)。结论早产儿早期发育商水平低于足月儿,尤其是大运动、语言、适应性和个人社交落后于足月儿。通过早期干预,早产儿发育商后期能追赶上足月儿。应重视早产儿出院后干预,促进早产儿潜在能力得到最大程度的发挥。

妊娠期缺钙严重影响母婴的安全健康 ,其原因是由于妊娠期母体钙代谢发生变化 ,使母体血清游离钙离子浓度降低 ,血清铅浓度增高。有关研究表明 ,母体缺钙导致血铅竞争性过高 ,使胎儿身长、体重均小于胎龄儿 ,胎儿宫内发育迟缓的发生率增... 详细信息

妊娠期缺钙严重影响母婴的安全健康 ,其原因是由于妊娠期母体钙代谢发生变化 ,使母体血清游离钙离子浓度降低 ,血清铅浓度增高。有关研究表明 ,母体缺钙导致血铅竞争性过高 ,使胎儿身长、体重均小于胎龄儿 ,胎儿宫内发育迟缓的发生率增高 ,甚至发生早产、死胎等。同时发现血清钙离子可能对内源性一氧化氮合成释放起调节作用 ,而内皮素是最强的缩血管物质之一 ,母体补钙可调节一氧化氮与内皮素的平衡 ,从而降低妊高征的发生率。因此孕期补充钙剂是非常重要的。
