Combining a Low-Cost Bio-Simulator and Single Use or Disposable Bronchoscope: A Platform for Remote Training

Introduction: The COVID-19 pandemic has resulted in a reconsideration of our traditional teaching modalities and develop newer, dynamic methods. Both high and low fidelity bronchoscopy simulators are costly. The objective was to assess whether a combination of a low-cost bio-simulator made of recyclable materials (ALFIETM Airway Low Fidelity including EBUS (endobronchial ultrasound)) and single use flexible bronchoscopy (SUFB) has the capability of differentiating novices from experts and the ability to train novices in bronchoscopy. Materials and Methods: Trainees were evaluated individually before and after training using a modified validated Bronchoscopy Skills and Tasks Assessment Tool (B-STAT) and SUFB. Results: 18 trainees were included (14 residents and 4 fellows). Pre-training assessment of scope handling differentiated novices from experienced bronchoscopists (p = 0.0025, 95% confidence intervals (CI) 3.12 - 12.17). Training of novices was associated with an improvement in scope handling and sampling (p = 0.0001, 95% CI 4.73 - 10.27). Conclusion: ALFIETM and SUFB have the potential to create a low-cost platform to teach bronchoscopy remotely.

Economic development of coastal community in Kotabaru Regency has become strategic in the future. Kotabaru Regency is the geo-economic strategic position due to the position of Kotabaru region boundaries between Kalimantan Island and Sulawesi Island. Geo-economic strategic of Kotabaru should increase the coastal community welfare in Kotabaru Regency. The objective of the research is to evaluate economic characteristic of coastal community at mining area in Kotabaru Regency. The number of samples in this study is 50 respondents in each village who are in the mining area. The number of companies in the mining area is 17 companies. Data were collected by using questionnaires. The results show that economic characteristics have low category. Mining and plantation companies do not increase the economic conditions of the community. Research suggestions are: (1) to examine the potential and importance of economic empowerment model of coastal communities in the region both mining companies and plantations; (2) the important role of companies and local governments to work together in formulating a strategy for management of CSR (corporate social responsibility), and social mapping to be done by the company in the preparation of the CSR program.

Visual Analysis of the Daily QA Results of Photon and Electron Beams of a Trilogy Linac over a Five-Year Period

The motivation for this research stems from the potential of biofuels to assist Ireland in meeting obligations outlined in the ‘Renewable Energy Source Directive’（2009/28/EC）.Under the ‘Renewable Energy Source Directive’ Ireland must increase renewable energy use by over five fold;it is required that a minimum of 10% of all transport fuel consumed in 2020 must be of renewable origin and that feedstocks used to achieve these targets must save a minimum of 60% of greenhouse gas emissions by 2018.This makes research into the energy balance of indigenous crops for biomethane and bioethanol production of significant relevance.Biofuels can help Ireland reduce its over reliance on imported fossil fuels,thus reducing economic reliance on countries from war torn regions of the world,encourage indigenous energy creation and provide indigenous jobs.The purpose of this work is to investigate and establish the energy consumed and created in the production of biomethane and bioethanol in Ireland;allowing for calculation of the true net energy.The two energy crops investigated are wheat and grass silage（assuming a three cut silage system）.Both grass silage and wheat have long been grown,cultivated and harvested effectively in Ireland,therefore the knowledge and expertise for optimal crop production are already present.Grass accounts for 91% of the crop land area in Ireland with typical yields of 60t-FM/ha while wheat is consider as one of Irelands highest yielding cereals at 7.5 t-FM/ha.This analysis shall be broken into three distinct stages,the energy consumption stage（crop production and biofuel processing）,the energy creation stage（biofuel processing – biomethane and bioethanol） and the energy balance stage.Each of these stages takes into consideration both the direct and indirect energy sinks in the effective production of each biofuel.The results show that biomethane from grass silage illustrates superior performance in relation to gross energy and net energy than biomethane and bioethanol from wheat.Biomethane from grass

Data visualization technique was applied to analyze the daily QA results of photon and electron beams. Special attention was paid to any trend the beams might display. A Varian Trilogy Linac equipped with dual photon energies and five electron energies was commissioned in early 2010. Daily Linac QA tests including the output constancy, beam flatness and symmetry (radial and transverse directions) were performed with an ionization chamber array device (QA Beam Checker Plus, Standard Imaging). The data of five years were collected and analyzed. For each energy, the measured data were exported and processed for visual trending using an in-house Matlab program. These daily data were cross-correlated with the monthly QA and annual QA results, as well as the preventive maintenance records. Majority of the output were within 1% of variation, with a consistent positive/upward drift for all seven energies (^+0.25% per month). The baseline of daily device is reset annually right after the TG-51 calibration. This results in a sudden drop of the output. On the other hand, the large amount of data using the same baseline exhibits a sinusoidal behavior (period = 12 months;amplitude = 0.8%, 0.5% for photons, electrons, respectively) on symmetry and flatness when normalization of baselines is accounted for. The well known phenomenon of new Linac output drift was clearly displayed. This output drift was a result of the air leakage of the over-pressurized sealed monitor chambers for the specific vendor. Data visualization is a new trend in the era of big data in radiation oncology research. It allows the data to be displayed visually and therefore more intuitive. Based on the visual display from the past, the physicist might predict the trend of the Linac and take actions proactively. It also makes comparisons, alerts failures, and potentially identifies causalities.

AIM To noninvasively investigate tumor cellularity measured using diffusion-weighted magnetic resonance imaging(DW-MRI) and glucose metabolism measured by 18 Flabeled fluorodeoxyglucose positron emission tomography/computed tomography(18F-FDG-PET/CT) during radiation therapy(RT) for human papillomavirus negative(HPV-) head and neck squamous cell carcinoma(HNSCC).METHODS In this prospective study, 6 HPV- HNSCC patients underwent a total of 34 multimodality imaging examinations(DW-MRI at 1.5 T Philips MRI scanner [(n = 24) pre-, during-(2-3 wk), and post-treatment(Tx), and 18F-FDG PET/CT pre- and post-Tx(n = 10)]. All patients received RT. Monoexponential modeling of the DW-MRI data yielded the imaging metric apparent diffusion coefficient(ADC) and the mean of standardized uptake value(SUV) was measured from 18F-FDG PET uptake. All patients had a clinical follow-up as the standard of care and survival status was documented at 1 year.RESULTS There was a strong negative correlation between the mean of pretreatment ADC(ρ =-0.67, P = 0.01) and the pretreatment 18F-FDG PET SUV. The percentage(%) change in delta(?) ADC for primary tumors and neck nodal metastases between pre- and Wk2-3 Tx were as follows: 75.4% and 61.6%, respectively, for the patient with no evidence of disease, 27.5% and 32.7%, respectively, for those patients who were alive with disease, and 26.9% and 7.31%, respectively, for those who were dead with disease.CONCLUSION These results are preliminary in nature and are indicative, and not definitive, trends rendered by the imaging metrics due to the small sample size of HPV- HNSCC patients in a Meixoeiro Hospital of Vigo Experience.

AIM: To investigate the merits of texture analysis on parametric maps derived from pharmacokinetic modeling with dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) as imaging biomarkers for the prediction of treatment response in patients with head and neck squamous cell carcinoma(HNSCC). METHODS: In this retrospective study,19 HNSCC patients underwent pre- and intra-treatment DCEMRI scans at a 1.5T MRI scanner. All patients had chemo-radiation treatment. Pharmacokinetic modeling was performed on the acquired DCE-MRI images,generating maps of volume transfer rate(Ktrans) and volume fraction of the extravascular extracellular space(ve). Image texture analysis was then employed on maps of Ktrans and ve,generating two texture measures: Energy(E) and homogeneity.RESULTS: No significant changes were found for the mean and standard deviation for Ktrans and ve between pre- and intra-treatment(P > 0.09). Texture analysis revealed that the imaging biomarker E of ve was significantly higher in intra-treatment scans,relative to pretreatment scans(P < 0.04). CONCLUSION: Chemo-radiation treatment in HNSCC significantly reduces the heterogeneity of tumors.
