Erectile dysfunction after transurethral prostatectomy for lower urinary tract symptoms:results from a center with over 500 patients

Aim： To identify possible risk factors for erectile dysfunction （ED） after transurethral resection of prostate （TURp） for benign prostatic hyperplasia （BpH）. Methods： Between March 1999 and March 2004, 629 patients underwent TURp in our department for the treatment of symptomatic BpH. All patients underwent transrectal ultrasound examination. In addition, the flow rate, urine residue, International prostate Symptom Score （IpSS） and quality of life （QOL） were recorded for those who presented without a catheter. Finally, the erectile function of the patient was evaluated according to the International Index of Erectile Function Instrument （IIEF-5） questionnaire. It was determined that ED existed where there was a total score of less than 21. The flow rate, IpSS and QOL assessment were performed at 3 and 6 months post-treatment. The IIEF-5 assessment was repeated at a 6-month follow-up. A logistic regression analysis was used to identify potential risk factors for ED. Results： At baseline, 522 （83 %） patients answered the IIEF-5 questionnaire. The mean patient age was （63.7 ± 9.7） years. The ED rate was 65%. After 6 months, 459 （88%） out of the 522 patients returned the IIEF questionnaire. The rest of the group was excluded from the statistical analysis. Six months after TURp, the rate of patients reporting ED increased to 77 %. Statistical analysis revealed that the only important factors associated with newly reported ED after TURp were diabetes mellitus （p = 0.003, r = 3.67） and observed intraoperative capsular perforation （p = 0.02, r = 1.12）. Conclusion： The incidence of postoperative, newly reported ED after TURp was 12%. Risk factors for its occurrence were diabetes mellitus and intraoperative capsular perforation. （Asian j Androl 2006 jan; 8： 69-74）

Extensive Medical Absenteeism among Secondary School Students: An Observational Study on Their Health Condition from a Biopsychosocial perspective

An adequate approach to reducing school absenteeism should focus on medical absenteeism as this is the most prevalent form of school absenteeism. The objective of this study is to explore the health condition of pre-vocational secondary students with extensive medical absenteeism from a biopsychosocial perspective. Data were obtained from medical assessments and Strengths and Difficulties Questionnaires (SDQs) of students with medical absence above threshold criteria (i.e. reported sick four times in 12 school weeks or more than six consecutive school days) who were referred to a youth health care physician. The results showed that the students had a mean absence rate of 14% in 12 school weeks. Of all students, 43.5% had a diagnosed disease and 81.5% had problems such as physical complaints not yet diagnosed, psychosocial problems, lifestyle problems and sleeping difficulties. Four groups could be distinguished: 13.4% with a diagnosed disease and no problem, 30.1% with a diagnosed disease and a problem, 51.5% with a problem and no diagnosed disease and 5.1% without a diagnosed disease or problem. Significantly higher scores of the Total difficulties-scale on the SDQ were found (mean 10.5;SD 5.8) in the study group, compared to a reference group (mean 9.1;SD 4.9). In conclusion, this study shows that when using the aforementioned criteria for extensive medical absenteeism to intervene with the absence, students with a mean absence rate of 14% in 12 school weeks are identified. If there was a diagnosed disease, it was accompanied by problems about twice as often. More than half of the students’ absence was caused by problems rather than a disease. The great diversity of these problems calls for a personalized approach. A broad perspective, including medical expertise, is needed to distinguish between emerging mental and physical diseases, psychosocial and lifestyle problems.

目的探讨小柴胡汤45%乙醇提取物对体外培养人肝癌细胞株HepG2增殖的影响及其可能的机理。方法采用ATp-Lite法测定不同浓度小柴胡汤对体外培养的HepG2增殖的作用,同时利用氧自由基清除能(ORAC)法测定样品的抗氧化活性;以LpS刺激小鼠巨噬细胞RAW264.7产生一氧化氮(NO),Griess法测定NO的终产物亚硝酸盐的含量,观察样品对NO生成的影响。结果小柴胡汤在62.5～500mg/L浓度范围内可以显著抑制HepG2增殖,抑制作用呈剂量相关性,计算半数抑制浓度(IC50)为(90.7±23.7)mg/L。小柴胡汤提取物具有一定的抗氧化活性,其抗氧化能力为阳性对照维生素C的1.3倍。小柴胡汤在3.125～50mg/L浓度范围内均可以显著抑制NO的生成(pppG2增殖的作用,其抑制作用可能与其抗氧化活性及抑制NO生成有关。

Aim: To examine the correlation between neonatal cranial ultrasound and school age magnetic resonance imaging (MRI) and neurodevelopmental outcome. Methods: In a prospective 2 year cohort study, 221 children (gestational age ≤ 32 weeks and/or birth weight ≤ 1500 g) participated at a median age of 8.1 years (inclusion percentage 78% ). Conventional MRI, IQ (subtests of the WISC), and motor performance (Movement Assessment Battery for Children) at school age were primary outcome measurements. Results: Overall, there was poor correspondence between ultrasound group classifications and MRI group classifications, except for the severe group (over 70% agreement). There was only a 1% chance of the children with a normal cranial ultrasound having a major lesion on MRI. Mean IQ (standard deviation) was significantly lower in children with major ultrasound or MRI lesions, but was also lower in children with minor lesions on MRI compared to children with a normal MRI (91 ± 16, 100 ± 13, 104 ± 13 for major lesions, minor lesions, and normal MRI, respectively). Median total impairment score (TIS) was significantly higher in children with major lesions on ultrasound or MRI as well as in children with minor lesions on MRI (TIS 4.0 and 6.25 for normal and minor lesions on MRI, respectively; p ded a severe lesion on MRI in 99% of cases. MRI correlated more strongly with mean IQ and median TIS than ultrasound. Subtle white matter lesions are better detected with MRI which could explain the stronger correlation of MRI with IQ and motor performance.

用于垂直磁记录的 Co-Cr 膜是采用射频(RF)溅射的方法沉积的。几个最重要的溅射参数,即RF 溅射电压 VRF、氩气压力 par 和基片保持器温度 Tsh,决定着磁化垂直取向的最佳值。其晶体结构在各种参数变化的范围内总是 hcp(六方密排—译注)。如果不使用最佳溅射参数,则会观察到 C 轴沿膜面取向的附加 h■p 化合物(an additional hcp compound)。这种取向会使平面剩磁 S■=(Mr/Ms)增加。我们对溅射参数引起的基片温度变化进行了测量,发现只有在 Ts 大约150℃时才能获得纯粹的 C 轴垂直取向。若因改变 Tsh 或溅射条件而导致改变 Ts 的取值,结果就会出现 C 轴沿膜面取向的附加 hcp 化合物。因此认为 Ts 对于 Co-Cr 膜的溅射是一个相当重要的参数。
