Intellectual Capacity of Children Exposed to Environmental Pollution in the Extreme South of Brazil

Objective： The study evaluated the intellectual capacity of children who live in areas that were E （exposed） or NE （not exposed） to environmental pollution. Method： 246 students between the ages of 8 and 11 years and 8 months were evaluated. Data were collected between May and June 2010 using a semi-structured questionnaire to examine factors that were related to compromised intellectual capacity. Intellectual capacity was evaluated with Raven＇s Colored Progressive Matrices Scale. The intellectual capacity of children who live in areas that were exposed or not exposed was compared using chi-squared tests and raw and adjusted analyses were conducted using Poisson regression. Results： The general occurrence of intellectual impairment was 28.9%, with 22.0% in the non-exposed area and 36.1% in the exposed area （p = 0.01）. The outcome was associated with the following： a lack of companionship （RP = 2.77; p 〈 0.00）, maternal education of less than five years （RP = 2.13; p 〈 0.00）, the mother not being Caucasian （RP = 1.57;p 〈 0.00）, residing in an exposed area （RP = 1.88;p 〈 0.00）, and nutritional risk/malnutrition （RP = 2.83; p 〈 0.00）. Conclusions： The results showed a high occurrence of intellectual impairment and a significant difference between the environmental areas. Additional research is needed to improve the understanding of several results. The town＇s priorities should include efforts related to environmental health.

Strategies for Fermentable Sugar Production by Using Pressurized Acid Hydrolysis for Rice Husks

This study investigated the use of leftover biomass(rice husks)as the raw material for the biotechnological production of platform chemicals and *** the biorefinery concept,different acid hydrolysates were studied and resulted into a wide range of treatment *** were applied throughout the procedures in multivariate experimental *** using the best hydrolytic conditions of 6.0%H3PO4,135oC(45 MPa)and reaction time of 62 min,21.0 g/L sugar hydrolysates were produced;by using the best hydrolytic condition of 4.5%HNO3,135oC/35 min,16.1 g/L sugar hydrolysates were produced;and with the hydrolysates use of 1.5%H2SO4 and 1.5%HCl,135oC/62 min,18.2 and 17.8 g/L sugar hydrolysates were produced,*** highest productivity,in terms of fermentable sugars,reached 68%of integral cellulose/hemicellulose fraction and surpassed those found in the literature,with regard to the processing of rice husks,by considering just one step *** hydrolysate,detoxified with active carbon,was used to prove this proposal viability,resulting in a fermentation substrate for ***(ATCC10020)and ***(LMG196)strains(natural producers of bioproducts),which certified the feasibility of the *** production of fermentable sugars from leftover biomass should encourage a search for new bioconversion routes,which can result in economic and environmental benefits and a spread of knowledge.

Preliminary developments of Miniplate-Type Fuel of U-2.5Zr-7.5Nb Alloy Dispersed and Cladded in Zircaloy

Nuclear fuel based on uranium metal alloys is utilized in research and test reactors. For the purpose of the reduction of fuel enrichment, high densities of uranium-235 in this kind of fuel are needed. This can be achieved when uranium alloys are used containing elements such as Zr, Mo and Nb. The construction of fuel element with high-uranium density requires materials with low cross sections for neutron absorption, stability under irradiation and absence of the chemical interactions between the fuel and cladding elements. In case of U-Zr-Nb alloys, Zry （zircaloy） cladding is a better option due to the fact that they have a higher chemical compatibility when compared with the use of aluminum alloys. This study aims to develop plate type nuclear fuel using the U-2.5Zr-7.5Nb alloy dispersed in Zry. Powders of this uranium based alloy and Zry were obtained by hydriding-dehydriding process. These powders were homogenized, compacted in pellet that was sandwiched in plates and frame of Zry. This assembly was hot rolled forming the dispersion fuel miniplate.

Endoscopic management and prevention of migrated esophageal stents

The use of self-expandable metallic stents has increased recently to palliate inoperable esophageal neoplasia and also in the management of benign strictures. Migration is one of the most common complications after stent placement and the endoscopist should be able to recognize and manage this situation. Several techniques for managing migrated stents have been described, as well as new techniques for preventing stent migration. Most stents have a "lasso" at the upper flange which facilitates stent repositioning or removal. An overtube, endoloop and large polypectomy snare may be useful for the retrieval of stents migrated into the stomach. External fixation of the stent with Shim's technique is efficient in preventing stent migration. Suturing the stent to the esophageal wall, new stent designs with larger flanges and double-layered stents are promising techniques to prevent stent migration but they warrant validation in a larger cohort of patients.

Influence of the Hydriding Cycles on U-IOMo Alloy Comminution Process

Hydriding-dehydriding process has been widely employed to comminute uranium alloys. developments have been made to improve this process because it is a cheaper way to obtain powder of uranium alloys compared to the other methods, such as atomization. This work presented an innovative comminution process of U-10Mo alloy in laboratory scale where a Sievert type equipment was utilized and only hydriding cycles were employed without repetition of the dehydriding procedure. Experiments of comminution of the U-10Mo alloy were carried out by employing alloy samples which were submitted to different heat treatment conditions and, subsequently, to different hydriding cycles. The heat treatments considered were hot rolled at 800 ℃, homogenization at 900 ℃ or 1,000 ℃ and aging at 520℃. It was observed that the number of hydriding cycles and heat treatment conditions have influenced the particle size distributions. Samples that were hot-rolled, homogenized at 1,000 ℃ and aged, and comminuted with five cycles of hydriding have produced powders with particle size distribution more uniform with the major quantity of particles in the range from 50μm to 200 μm while the other conditions have obtained a great quantity above 200 μm.

Corynebacterium pseudotuberculosis is an infectious agent that occurs in small ruminants causing caseous lymphadenitis, and more rarely in humans causing lymphadenitis and pneumonia. The breeding small ruminants have great economic importance in Brazil. Rural farm workers and veterinary students who acquired this disease suffered from weakening symptoms for weeks, and the identification of the etiological agent was time-consuming and complex. Due to the low prevalence of case records, there is probably no available commercial diagnostic kit for C. pseudotuberculosis infection in humans. This study aimed to describe human seroreactivity to secreted antigens from C. pseudotuberculosis. Reactivity of serum from farm workers (n = 14), individuals who work with the bacillus at laboratory (n = 8) or individuals without contact (n = 25) was tested with secreted proteins from PAT10 strain of C. pseudotuberculosis by Western blotting. Samples of all (100%) farm workers showed reactivity to 31 kDa, 71 kDa and 164 kDa proteins, while laboratory workers showed 87.5%, 62.5 % and 37.5%, and no-contact 20%, 0% and 16%, respectively. All sera recognized the 275 kDa protein. Our data suggest that C. pseudotuberculosis secreted proteins are antigenic in humans and the recognition profiles allowed the identification of individuals with and without prior contact with this bacillus. This is the first paper which describes human reactivity to C. pseudotuberculosis in serum samples of workers in Brazil.

Dabigatran in the Treatment of Extensive Cerebral Venous Thrombosis:A Case Report

Background: Cerebral venous thrombosis (CVT) is a rare type of cerebrovascular disease associated with a 15% rate of death or function dependence. The mainstay of treatment for CVT is systemic anticoagulation, despite venous hemorrhagic infarction. Vitamin K antagonists have long been the only available option for anticoagulation;however, the past few years have brought the development of many new target-specific drugs, collectively called non-vitamin K antagonist oral anticoagulants (NOACs). Although emerging evidence suggests NOACs have an acceptable safety and tolerability profile in CVT, there are limited data available and no randomized controlled trials have been performed to date. Case Presentation: This describes the case of a patient with CVT occurring during an infection who was successfully treated with a NOAC, dabigatran, after a difficult time on warfarin. Conclusions: A case of extensive and deep CVT was identified. Dabigatran 150 mg treatment twice daily in this patient resulted in no additional damage to the brain. This case study illustrates that the use of NOACs such as dabigatran can be safe and effective in patients with CVT.

AIM To evaluate the parathyroid ultrasonography and define parameters that can predict poor response to treatment in patients with secondary hyperparathyroidism due to renal *** This cohort study evaluated 85 patients with chronic kidney disease stage V with parathyroid hormone levels above 800 pg/mL. All patients underwent ultrasonography of the parathyroids and the following parameters were analyzed: demographic characteristics(etiology of chronic kidney disease, gender, age, dialysis vintage, vascular access, use of vitamin D), laboratory(calcium, phosphorus, parathyroid hormone, alkaline phosphatase, bone alkaline phosphatase), and the occurrence of bone changes, cardiovascular events and death. The χ~2 test were used to compare proportions or the Fisher exact test for small sample frequencies. Student t-test was used to detect differences between the two groups regarding continuous *** Fifty-three patients(66.4%) had parathyroid nodules with higher levels of parathyroid hormone, calcium and phosphorus. Sixteen patients underwent parathyroidectomy and had higher levels of phosphorus and calcium × phosphorus product(P = 0.03 and P = 0.006, respectively). They also had lower mortality(32% vs 68%, P = 0.01) and lower incidence of cardiovascular or cerebrovascular events(27% vs 73%, P = 0.02). Calcium × phosphorus product above 55 mg^2/dL^2 [RR 1.48(1.06, 2.08), P = 0.03], presence of vascular calcification [1.33(1.01, 1.76), P = 0.015] and previous occurrence of vascular events [RR 2.25(1.27, 3.98), P dentification of nodules at ultrasonography strengthens the indication for parathyroidectomy in patients with secondary hyperparathyroidism due to renal failure.
