To describe the feasibility and results of total laparoscopic radical hysterec tomy with intraoperative sentinel lymph node identification in patients with ear ly cervical cancer. between March 2001 and October 2003, 12 patients with FIGO s tage IA2 (n = 1)or Ib1 (n = 11) cancer of the cervix underwent surgical treatmen t through the laparoscopic route. All patients underwent a laparoscopic sentinel node identification with preoperative lymphoscintigraphy (technetium-99 m coll oid albumin injection around the tumor) and intraoperative lymphatic mapping wit h isosulfan blue dye and a laparoscopic gamma probe followed by systematic bilat eral pelvic lymphadenectomy and laparoscopic type II (n = 5) or type III (n = 7) hysterectomy. A mean of 2.5 sentinel nodes per patient (range 1-4) was detecte d, with a mean of 2.33 nodes per patient by gamma probe and a mean of 2 per pati ent after blue injection (combined detection rate 100%). The most frequent loca lization of the nodes was the interiliac region. Histopathologic examination of sentinel nodes including cytokeratin immunohistochemical analysis did not show m etastasis. Microscopic nodal metastases were not found. The mean number of resec ted pelvic nodes was 18.6 per patient (range 10-28). The operation was performe d entirely by laparoscopy in all patients and no case of laparotomy conversion w as recorded. The mean duration of operation was 271 min (range 235-300), with a mean blood loss of 445 mL (range 240-800), and a mean length of stay of 5.25 d ays (range 3-10). No major intraoperative complications occurred. After a media n follow-up of 20 months (range 5-34), all patients are free of disease. This study shows the feasibility of the combination of laparoscopic intraoperative se ntinel node mapping and laparoscopic radical surgery in the context of minimally invasive surgery for the management of patients with early cervical cancer.

Study of corneal biomechanical properties in patients with childhood glaucoma

AIM:To study of corneal biomechanical properties and intraocular pressure(IOP)measured with Corvis Scheimpflug Technology(ST)in patients with childhood glaucoma(CG).METHODS:Cross-sectional study in which 89 eyes were included 56 of them with *** one eye per patient was *** following variables were obtained from the clinical history and the ophthalmological examination:age,sex,IOP,number of surgeries,and the cup/disc ratio(CDR).The following parameters were recorded using Corvis ST:corrected by biomechanics IOP(b IOP),not corrected IOP(nct IOP),central corneal thickness(CCT),maximum concavity[radius,peak distance(PD)and deformation amplitude],applanation 1 and 2(length and velocity).The mean age was 23±14.55 and 33±19.5 years old for the control group and CG group,*** 36 were males and 53 were *** the CG group,7 patients were controlled only with medical *** had at least one previous goniotomy,19 had at least one trabeculectomy,and 11 had an Ahmed ***:A significant and positive intraclass correlation coefficient was found between Goldman IOP and the IOP measured by Corvis in both *** differences were found between the IOP measured with Corvis and Goldman using a student *** biomechanical parameters,there were differences in the applanation length 2(A-L2),in the applanation velocity 2(A-V2)and in the *** sex,only the applanation length 1(A-L1)was found to be different in control group.A positive and significant Pearson correlation was found between CDR and the ***:Corneal biomechanical properties have shown differences between CG and healthy subjects and also between men and women.

Emphysematous Pyelonephritis in a black African Woman Managed with Antibiotics Alone

Introduction: Emphysematous pyelonephritis (EPN) is a severe, life-threatening infection of the renal parenchyma. This condition is characterized by the production of intrarenal and perinephric gas. In the world, the EPN is currently limited to case series reported. No cases have been described in black Africa. Aims: Related a first case of EPN diagnosed and treated in the University Hospital Center Sylvanus Olympio of Lomé in Togo. Observation: A 40 years old woman, with antecedent of diabetes presented pyelonephritis emphysematous class 2. She was treated successfully with antibiotic alone without using percutaneous drainage or nephrectomy. Conclusion: As reported in every case series, it was a young diabetic patient with a clinical features of acute pyelonephritis which CT scan had helped lay the EPN class 2 diagnosed. The germ was Klebsiella pneumoniae. She was treated with adapted antibiotic alone.

Dependence of dynamic contour and Goldmann applanation tonometries on peripheral corneal thickness

AIM：To determine the effects of peripheral corneal thickness（PCT）on dynamic contour tonometry（DCT）and Goldmann applanation tonometry（GAT）.METHODS：A cross-sectional *** created a software which calculates the corneal contour（CC）as a function of the radius from the corneal apex to each pixel of the *** software generates a central circumference with a radius of 1 mm and the remainder of the cornea is segmented in 5 rings concentric with corneal apex being its diameter not constant around the corneal circumference as a consequence of the irregular CC but keeping constant the diameter of each ring in each direction of the *** was determined as the mean thickness of the most eccentric *** weighted scatterplot smoothing（LOWESS）regression was used to determine the pattern of the relationship between PCT and both DCT and GAT ***,two multivariable linear regression models were *** each of them,the dependant variable was intraocular pressure（IOP）as determined using GAT and DCT *** both of the models the predictive variable was PCT though LOWESS regression pattern was used to model the relationship between the dependant variables and the predictor *** and sex were also introduced control variables along with their first-degree interactions with *** outcome measures include amount of IOP variation explained through regression models（R2）and regression coefficients（b）.RESULTS：Subjects included 109 eyes of 109 healthy *** regression suggested that a 2nd-degree polynomial would be suitable to model the relationshipbetween both DCT and GAT with *** PCT was introduced in both models as a linear and quadratic *** age nor sex nor interactions were statistically significant in both *** GAT model,R2was 17.14%（F=9.02;P=0.0002）,PCT linear term b was-1.163（95%CI：-1.163,-0.617）.PCT quadratic term b was 0.00081（95%CI：0.00043,0.00118）.For DCT model R2was 14.28%（F=9.29;P=0.0002

Erectile Dysfunction in Hemodialysis in Togo

Objectives: To determine the prevalence and risk factors of erectile dysfunction (ED) in men on hemodialysis in Togo. Methods: Prospective cross-sectional study, descriptive and analytical ranging from December 2015 to February 2016 in the hemodialysis unit of CHU SO, only dialysis center in the country. The evaluation of the ED was made through IIEF-5 questionnaire. Results: 39 men (67.2%) of 58 hemodialysis is a sex ratio M/F of 2.05. The average age was 43.5 years with extremes of 25 and 59 years. The majority of men had a profession in 19 cases (48.7%). The mean duration of dialysis was 30 months with a range of 4 months and 90 months. Initial glomerular nephropathy was in 19 cases (48.7%) and vascular in 14 cases (35.9%). 38 patients were hypertensive (97.4%) and 6 diabetic men (15.4%). The prevalence of ED was 74.3% (29 cases). ED score was less than 10 points in 14 cases (35.9%);01 case (2.6%) between 11 and 16 points;06 cases (15.4%) between 17 to 21 points;08 cases (20.5%) between 22 and 25 points;and 10 cases (25.6%) between 26 and 30 points. Age, profession and duration of dialysis were risk factors associated with ED (P 0.001). Conclusion: The prevalence of ED is very high in Togo.

Anatomical characterization of central, apical and minimal corneal thickness

·AIM: To anatomically locate the points of minimum corneal thickness and central corneal thickness(pupil center) in relation to the corneal apex.·METHODS: Observational, cross-sectional study, 299 healthy volunteers. Thickness at the corneal apex(AT),minimum corneal thickness(MT) and corneal thickness at the pupil center(PT) were determined using the pentacam. Distances from the corneal apex to MT(MD)and PT(PD) were calculated and their quadrant position(taking the corneal apex as the reference) determined:point of minimum thickness(MC) and point of central thickness(PC) depending on the quadrant position. Two multivariate linear regression models were constructed to examine the influence of age, gender, power of the flattest and steepest corneal axes, position of the flattest axis, corneal volume(determined using the Pentacam)and PT on MD and PD. The effects of these variables on MC and PC were also determined in two multinomial regression models.·RESULTS: MT was located at a mean distance of 0.909 mm from the apex(79.4% in the inferior-temporal quadrant). PT was located at a mean distance of 0.156 mm from the apex. The linear regression model for MD indicated it was significantly influenced by corneal volume(b =-0.024; 95% CI:-0.043 to-0.004). No significant relations were identified in the linear regression model for PD or the multinomial logisticregressions for MC and PC.·CONCLUSION: MT was typically located at the inferiortemporal quadrant of the cornea and its distance to the corneal apex tended to decrease with the increment of corneal volume.

Aim:To report an unusual case of cutaneous presentation on the eyelid of systemic(or nodal),CD30+,anaplastic large-cell lymphoma(ALCL).Methods:A 39-year-old man presented with a rapidly growing exophytic mass on the left upper eyelid,with a protuberant,ulcerated aspect and with *** patient showed lymph node involvement 3 months after the appearance of the lesion on the eyelid(the lesion itself appeared 1 week before examination).Results:The histopathologic and immunohistoc hemical diagnosis was ALCL,T-cell phenotype,strongly positive for CD43 and CD30,and negative for CD3,anaplastic lymphoma kinase(ALK),and b-cell *** was by radiotherapy and,later,chemotherapy(cyclophosphamide,adriamycin,vincristine,and prednisolone,CHOP)for skin recurrences and lymphadenopathies over 5 *** has been no recurrence for more than 6 ***:Primary,systemic,CD30+,ALK-negative,ALCL presentations generally have a poor prognosis and tend to occur in older individuals,although the clinical outcome is highly variable and difficult to predict in individual *** three cases of ALCL have been described in the ocular adnexae and none was ALK-negative.

The β^+ decay of ^(31)Ar was investigated in an experiment at the GSI-FRS *** ions of interest have been produced in the fragmentation of a ^(36)Ar beam at 880 Me V/nucleon and implanted in a time projection chamber with optical readout. In addition to β-delayed one and two proton emission, for the first time the emission of β-delayed 3 protons has been observed. The branching ratio for this decay mode is found to be(0.07 ± 0.02)%.

b-dazL:A novel gene in bovine spermatogenesis

The dazL gene plays an important role in *** and mutation of this gene has been frequently associated with the sterility in both vertebrates and ***,a cDNA containing the complete open reading frame of dazL gene,which encodes putative RRM RNA-binding protein,was isolated from bovine tissues by *** isolated gene was referred to as ***-based location analysis showed that b-dazL is located near the end of chromosome *** b-dazL gene and its vertebrate homologue shared a high nucleotide similarity (90-94%) in the coding region and in the genomic organization,each consisting of 10 exons and 9 *** protein products contained a highly conserved RNA-binding motif,a unique daz repeat, and PUM2 and PAbP interacting ***-PCR assay revealed that b-dazL was expressed in the testis and ***,no signals were detected in the testes of sterile male *** microstructure of the testes of sterile males showed very few spermat- ocytes but mostly somatic *** observations are consistent with the typical phenotype of testes with defective dazL expression. Subsequent gene detection studies indicated that transcription arrest might lead to a b-dazL transcript defect in testes of cattle-yaks. Our results suggest that b-dazL may be involved in spermatogenesis,and that transcription arrest of the gene is associated with male sterility in cattle-yaks.
