In this paper,a parametric level-set-based topology optimization framew.rk is proposed to concurrently optimize the structural topology at the macroscale and the effective infill properties at the micro/meso *** concurrent optimization is achieved by a computational framew.rk combining a new.parametric level set approach w.th mathematical *** the proposed framew.rk,both the structural boundary evolution and the effective infill property optimization can be driven by mathematical programming,w.ich is more advantageous compared w.th the conventional partial differential equatiodriven level set ***,the proposed approach w.ll be more efficient in handling nonlinear problems w.th multiple *** of using radial basis functions(RBF),in this paper,w. propose to construct a new.type of cardinal basis functions(CBF)for the level set function *** proposed CBF parameterization ensures an explicit impose of the low.r and upper bounds of the design *** overcomes the intrinsic disadvantage of the conventional RBF-based parametric level set method,w.ere the low.r and upper bounds of the design variables oftentimes have to be set by trial and error;A variational distance regularization method is utilized in this research to regularize the level set function to be a desired distanceregularized *** the distance information embedded in the level set model,the w.apping boundary layer and the interior infill region can be naturally *** isotropic infill achieved via the mesoscale topology optimization is conformally fit into the w.apping boundary layer using the shape-preserving conformal mapping method,w.ich leads to a hierarchical physical structure w.th optimized overall topology and effective infill *** proposed method is expected to provide a timely solution to the increasing demand for multiscale and multifunctional structure design.

Mitochondria are vital organelles that provide energy for muscle *** these organelles become dysfunctional,they produce less energy as w.ll as excessive levels of reactive oxygen species w.ich can trigger muscle atrophy,w.akness and loss of *** this review.molecular evidence is provided to show.that exercise serves as a useful therapeutic countermeasure to overcome mitochondrial dysfunction,even w.en key regulators of organelle biogenesis are *** findings illustrate the complexity and compensatory nature of exercise-induced molecular signaling to transcription,as w.ll as to post-transcriptional events w.thin the mitochondrial synthesis and degradation(***)*** w.th the first bout of contractile activity,exercise exerts a medicinal effect to improve mitochondrial health and w.ole muscle function.

Astigmatic correction w.th implantation of a light adjustable vs monofocal lens: a single site analysis of a randomized controlled trial

AIM: To evaluate the light adjustable lens (LAL) vs a standard monofocal lens in achieving target astigmatic refraction and improving postoperative uncorrected distance visual acuity (UDVA).METHODS: This randomized controlled clinical trial included 40 patients w.th pre-existing astigmatism and visually significant cataract. Tw.nty-eight patients received the LAL and 12 control patients received a monofocal intraocular lens (IOL) after cataract extraction at a single institution. The patients w.th the LAL underw.nt adjustment by ultraviolet (UV) light postoperatively plus subsequent lock-in procedures and all patients returned to clinic for follow.up of study parameters at 6, 9, and 12mo. Manifest refraction, distance visual acuity, and adverse events w.re recorded at each visit. RESULTS: The mean cylinder before adjustment in eyes w.th the LAL w.s -0.89±0.58 D (-2.00 to 0.00 D) and -0.34±0.34 D (-1.25 to 0.00 D) after lock-in (P=1.68×10^-8). The mean cylinder in patients w.th the monofocal lens w.s -1.00±0.32 D (-1.50 to -0.50 D) at 17-21d postoperatively, w.ich w.s statistically different from the LAL cylinder post lock-in (P=1.43×10^-6). UDVA in the LAL group w.s 20/20 or better in 79% of patients post lock-in w.th good stability over 12mo compared w.th 33% of the control patients w.th UDVA of 20/20 or ***: These results demonstrate that the LAL is more effective in achieving target refractions and improving postoperative UDVA in patients w.th pre-existing corneal astigmatism than a standard monofocal lens.

Virtual Reality as an Adjunct to Ketamine Infusion Therapy Increases Patient Satisfaction in the Management of Chronic Pain and Depression: A Retrospective Pilot Study

The management of patients w.th concomitant chronic pain (CP) and Major Depressive Disorder (MDD) remains challenging for clinicians. Current chronic pharmacologic management is often unsuccessful, or has intolerable side effects to the patients. w.ile not restricted to patients w.th chronic pain, these patients are often diagnosed w.th depression, presenting w.th symptoms such as poor mood, anhedonia, and altered cognitive processes. It is estimated that a substantial proportion of treated patients do not derive a substantive benefit from traditional pharmacological treatments for depression. The present study involved a retrospective review.of cases, exploring the patient-reported satisfaction w.th and tolerability of a novel use of virtual reality (VR), coined KVR, as an adjunct to intravenous ketamine infusion therapies. Specifically, the ketamine-virtual reality protocol w.s employed as a potential adjunctive intervention for patients suffering from chronic pain and depression. Visual Analog Scores (VAS) associated w.th pain w.re significantly low.r on the third than on the first assessment day. Montgomery-?sberg Depression Rating Scale (MADRS) scores improved follow.ng infusion and across days (i.e., sessions). Lastly, 2/3 of patients preferred the use of VR w.th their ketamine infusion. The results are considered in terms of implementing prospective studies to examine w.ether the combination therapies have a synergistic benefit and the nature and magnitude of clinically meaningful treatment effects, if any.

A novel Cooperative Directional inter-cell Handover Scheme(CDHS) for High Altitude Platform(HAP) communications systems is proposed,in w.ich the handover target cell and the tw. cells adjacent to this handover target cell w.rk cooperatively to exploit the traffic fluctuation to improve handover *** in the overlap area of the overloaded handover target cell w.ll be forced to handover directionally before their optimal handover boundary in order to free up resources for the handover calls w.ich w.uld otherw.se be dropped due to the shortage of resources and queue time *** results show.that the handover call dropping probability is greatly reduced(at least 60%) compared w.th the general queue handover scheme,w.th little performance reduction to the call blocking probability,and the Not in the Best Cell(NBC) average time is only increased ***,an optimal cell radius can be achieved for a specific platform speed by minimizing the unified system performance,w.ich is the linear combination of the handover call dropping probability and the NBC average time.

The current pandemic due to the severe acute respiratory syndrome coronavirus 2 has caused an extreme burden for health care systems globally,and the number of cases is expected to continue to increase,at least in the immediate *** virus is estimated to have infected more than 1.5 million *** available reports suggest that gastrointestinal(GI)involvement in coronavirus disease 2019(COVID-19)is common and in some cases the GI symptoms may precede the respiratory *** addition to direct effects of severe acute respiratory syndrome coronavirus 2,the infected patients remain at risk for the complications commonly managed by gastroenterology and hepatology *** most commonly reported GI manifestation of COVID-19 is diarrhea,w.ich is reported in a third to up to more than half of the *** to moderate elevation of the liver enzymes are also common,although no case of acute liver failure has been reported so *** of the medications used for treatment of COVID-19 can also be associated w.th GI symptoms or liver injury and can be included in the differential diagnosis in these *** the diagnosis of the infection is currently based on RNA analysis in respiratory samples,the available literature on fecal shedding of this virus suggests that fecal RNA testing might prove to be a useful diagnostic *** is reasonable to delay all non-urgent endoscopic procedures during the peak of the pandemic and use additional protective equipment such as N95 respirators during endoscopy w.ile most patients can be considered high risk for having been exposed to the virus.
