Drug-abusing Offenders with Co-morbid Mental Disorders:Gender Differences in Problem Severity,Treatment Participation,and Recidivism

Objective This study examined the gender differences in drug‐related problems and predictors of recidivism among a sample of 1444 offenders with co‐morbid drug abuse and mental disorders participating in California＇s Proposition 36 *** Background characteristics and problem severity in multiple key life areas were assessed at intake by using Addiction Severity Index,and drug treatment participation,mental health diagnoses and arrests were based on official *** Women demonstrated greater problem severity than men in family relationships,health,psychological health,and sexual and physical abuse *** on the other hand had greater criminal history,high rates of attention disorder,and psychotic *** men than women were rearrested during the year after treatment *** regression analyses showed that for the combined sample,male,young age,cocaine use（relative to methamphetamine）,drug abuse severity,methadone treatment,arrest history and fewer prior treatment history were associated with higher recidivism at 12‐month follow‐up;lower education,cocaine use,and arrest history were related to women＇s recidivism,while young age,outpatient treatment,and arrest history were predictors of men＇s *** Although the specific type of mental disorder did not seem to be predictive of recidivism,the high rates of mental health disorder and arrest of this population is *** strategies taking into consideration gender‐specific problems and needs can improve outcomes for both.

RECENT PROGRESS IN INCOMPRESSIBLE REYNOLDSAVERAGED NAVIER-STOKES SOLVERS

A new numerical approach based on a multiblock, multigrid, local refinement method has been developed. The multiblock structure makes grid generation for complex geometries easier,multigrid techniques significantly accelerate the rate of convergence and the local refinement method provides high spatial resolution of boundary lay6r and separated vortical flows with much reduced computer memory and CPU time. Sample three-dimensional flow computations for complex geometries are presented to illustrate the advantages in term of memory savings and reductions of CPU time in obtaining high spatial resolution solutions. High spatial resolution in the inner viscous layer is achieved by using the values of y+ for the first grid centers rom the wall in the order of 1 to 7. The required solution resolution in the dominantly viscous flow region (boundary layer, vortex core, or three-dimensional separated zone) dictates the acceptable spacing of grid cells in that region. It is very desirable to be able to reduce grid spacing locally in the critical region without changing the overall grid. The local refinement technique doubles grid numbers in all three directions for every level of refinement. Multiple levels of local grid refinement with moderate grid stretching can be used to provide fine grid spacing in rapidly changing flow regions such as near the wall, vortex core and separated shear layer. Thus, the method provides the required fine spatial resolution in the dominantly viscous region which is relatively small compared to the entire computational domain. This also avoids the used of excessively large aspect ratios of the grid cells near the rapidly changing now regions thus reduces truncation error and improves robustness. Furthermore, the results of numerical experimentation indicate that the present multigrid local refinement technique provides very effective numerical communications across the fine and coarse grid interfaces because the finer mesh is embedde

Effects of age on ocular anterior segment dimensions measured by optical coherence tomography

Background Older subjects tend to have smaller ocular anterior segment. The present study aimed to measure anterior segment dimensions with optical coherence tomography （OCT） and quantitatively assess the effect of age and other factors. Methods Anterior segment OCT images were obtained in normal subjects residing in the greater Los Angeles area. Four line scans were acquired at the 90%, 45%, 0% and 135% meridians of each eye. Computer calipers acquired anterior segment dimensions of corneal diameter, anterior chamber width, corneal vault and anterior chamber depth on OCT images. Measurements from 4 meridians were averaged. Axial length and corneal power were measured by partial coherence interferometry. Univariate and multivariate analyses were performed to assess correlations. Results Sixty-six eyes of 33 normal subjects （aged 22-65 years, 19 Asians, 14 Caucasians） were enrolled. For every 1 year of age, corneal diameter was 0.03：3 mm narrower （P 〈0.01）, anterior chamber width was 0.031 mm narrower （P 〈0.01）, corneal vault was 0.016 mm lower （P 〈0.01）, and anterior chamber depth was 0.025 mm lower （P 〈0.01）. Asian eyes had smaller corneal diameter （P=-0.035） and anterior chamber width （P=0.015） compared with those of Caucasian eyes. Body height showed positive correlation with corneal diameter （0.039 mm per centimeter of height, P 〈0.01） and corneal vault （0.024 mm per centimetre of height, P 〈0.01）. Gender did not have an independent effect on anterior segment dimensions. Conclusions Anterior segment dimensions were smaller in older subjects. Age-related changes may affect the tolerability of long-term implants such as phakic intraocular lens.

Bacteremia Caused by <i>Acinetobacter baumannii</i>: Epidemiologic Features, Antimicrobial Susceptibility, and Outcomes

Acinetobacter baumannii bacteremia is becoming more prevalent and is associated with increasing morbidity and mortality. Escalating antibacterial resistance further contributes to therapeutic dilemmas, enhanced infection control support and poorer outcomes in patients infected with these bacteria. A retrospective analysis of patients whose blood cultures produced A. baumannii from January 2007 through January 2013 was performed. Data regarding the epidemiologic features, antimicrobial susceptibility and outcomes of patients with A. baumannii bacteremia were collected and analyzed. Sixty A. baumannii isolates each from a different patient were identified. The Charlson Comorbidity Index (≥3) was the greatest among patients with multi-drug resistance (MDR) compared to intermediate drug resistance (IDR) and pan-sensitive (PS) A. baumannii. The mean APACHE II scores for MDR, IDR and PS A. baumannii bacteremia were 21, 15 and 11, respectively (P < 0.05, MDR v. PS). Seventy-three percent of the isolates were resistant to quinolones, 44% to piperacillin/tazobactam, 45% to amikacin, 22% to imipenem, 0% to ticarcillin/clavulanate, and 0% to polymyxin. Among 28 patients with MDR A. baumannii bacteremia, 20 received inadequate empiric treatment, and 16 of these patients died (80%). Of the remaining eight patients with MDR bacteremia who received adequate empiric antibiotics, only two died (25%). The severity of underlying illness, degree of antibiotic resistance and receiving inadequate initial antibiotic therapy are associated with mortality among patients with bacteremia due to A. baumannii.

Characteristics, Treatment, and Outcomes Associated with <i>Clostridium difficile</i>Associated Diarrhea in a Veterans Affairs Medical Center

We conducted a study to assess the characteristics, treatment and outcomes associated with Clostridium difficile associated diarrhea in a Veterans Affairs Medical Center. Fifty-eight consecutive individual cases of C. difficile infection in 2013 were observed within the Veterans Affairs New Jersey Health Care System (VA NJHCS). We molecularly typed all 58 individual strains and identified the associated characteristics, treatment and outcomes. Forty-four out of 58 specimens (76%) which were probed had characteristics of the epidemic strain BI/NAP1/027 making this virulent strain to be the predominate strain at the VA NJHCS. All C. difficile BI/NAP1/027 strains were resistant to fluoroquinolones and sensitive to fidaxomicin, metronidazole and vancomycin. Fidaxomicin had the most potent in vitro activity (MIC90 = 0.5 μg/ml) against the BI/NAP1/027 strain. Twenty-six of 44 patients (59%) with the virulent strain were from a long-term care facility (LTCF). Patients possessing the virulent strain from the LTCF had a mean APACHE II score of 14.1 and a predicted death rate of 21.9%. Two-third of patients were treated with metronidazole alone (mean APACHE II scores 9.6), and one-third required oral vancomycin and metronidazole (mean APACHE II scores 14.1). There were no C. difficile infection related deaths. C. difficile BI/NAP1/027, an epidemic strain, is the endemic strain at the VA NJHCS, but no increased mortality was seen with infection with this strain.

Post-layout simulation driven analog circuit sizing

Post-layout simulation provides accurate guidance for analog circuit design, but post-layout performance is hard to be directly optimized at early design stages. Prior work on analog circuit sizing often utilizes pre-layout simulation results as the optimization objective. In this work, we propose a post-layoutsimulation-driven(post-simulation-driven for short) analog circuit sizing framework that directly optimizes the post-layout simulation performance. The framework integrates automated layout generation into the optimization loop of transistor sizing and leverages a coupled Bayesian optimization algorithm to search for the best post-simulation performance. Experimental results demonstrate that our framework can achieve over 20% better post-layout performance in competitive time than manual design and the method that only considers pre-layout optimization.

Spatial distribution characteristics of climate-induced landslides in the Eastern Himalayas

Risk factors for lower limb swelling after primary total knee arthroplasty

Total knee arthroplasty （TKA） is a successful surgical technique for patients with advanced knee osteoarthritis; however, some peri-operative complications can not be predicted or avoided completely. This study aimed to investigate the factors affecting limb swelling after primary total knee arthroplasty, to guide and improve patient rehabilitation. Methods Using a hospital database, we retroactively analyzed the mean changes in limb circumferences of 286 consecutive patients who underwent primary unilateral total knee arthroplasty between October 2007 and August 2009. The lower limb circumference change was calculated and analyzed statistically. The influence of age, gender, body mass index, the presence of deep vein thrombosis, methods of anti-coagulation, operation time, hidden blood loss, and type of prosthesis on post-operative lower limb swelling was studied. Results Swelling was most pronounced from the third to the fifth post-operative day and usually occurred in both lower limbs. Swelling was significantly more pronounced in the operated limb than in the non-operated limb. The swelling above the knee was also significantly greater than that below the knee. The change in limb circumference at 10 cm above the knee was significantly different between the patients with body mass index 〈25 kg/m2 and those with body mass index 〉25 kg/m2. However, the change in limb circumference at 10 cm below the knee was not significantly different between the two groups. There was no statistically significant difference in limb swelling between different age groups （P 〉0.05）. Similarly, gender, methods of anti-coagulation, the presence of deep vein thrombosis, the type of prosthesis, and operation time did not significantly affect post-operative limb swelling. Multivariate linear regression showed that the factors affecting post-operative limb swelling were body mass index and hidden blood loss.
