An NSQIP evaluation of practice patterns and outcomes following surgery for anorectal abscess and fistula in patients with and without Crohn’s disease

Background:Patients with Crohn’s disease(CD)are believed to have more aggressive anorectal abscess and fistula *** assessed the types of procedures performed and perioperative complications associated with the surgical management of anorectal abscess and fistula disease in patients with and without ***:The American College of Surgeons National Surgical Quality Improvement Program database(ACS-NSQIP,2005-2010)was used to calculate 30-day outcomes using regression modeling,accounting for demographics,comorbidities and surgical ***-9 codes for anorectal abscess or fistula were used for initial *** were then stratified,based on the presence or absence of underlying *** procedures included incision and drainage of abscesses,fistulotomy and seton *** fistulas were considered simple,while all others were classified as complex(-vaginal,-urethral and-vesical).Results:A total of 7,218 patients(mean age 45 years;64%male)met inclusion criteria,with underlying CD in 345(4.8%).CD patients were more likely to have a seton placed(9.9 vs 8.2%,Pe on steroids(15.4 vs 4.3%,Peven percent of CD patients underwent local procedures,while 46%had a proctectomy and8%underwent *** was more common in those without underlying CD(16 vs 11%,Pe overall complication rate after local treatment was 4.9%,with no difference between patients with and without CD(7.7 vs 4.9%,P=0.144).This was not affected by fistula type-simple(7.9 vs 3.9%,P=0.194)vs complex(33 vs 7.1%,P=0.21)—or when stratified by wound(3.8 vs 2.4%;P=0.26)or systemic complications(3.8 vs 2.5%;P=0.53).Yet,complications following emergency procedures were higher in patients with CD(21.4 vs 5.9%,P=0.047).Factors significantly associated with increased complications were Crohn’s disease(OR=8.2),lack of functional independence(OR=2.0),pre-operative weight loss(OR=2.6)and pre-operative acute renal failure(OR=5.6).Steroids were al

Kerogen Kinetic Distributions and Simulations Provide Insights into Petroleum Transformation Fraction (TF) Profiles of Organic-Rich Shales

Two hundred and fifty single first-order Arrhenius reactions are simulated to generate S2 pyrograms at three heating rates 25,15,and 5°C·*** activation energy(e)and pre-exponential factor(A)of the reactions simulated follow a long-established trend of those variable values displayed by shales and *** characteristics of the transformation fraction(TF)profiles(product generation window temperatures)of the simulated single reactions are compared to the TF profiles of recorded shale pyrograms generated by multiple reactions with different e-A values lying near the defined e-A *** similarities and differences are observed between the TF profile values of the two *** similarities support the spread of e-A values involved in shale pyrogram best *** differences are most likely explained by the complexity of the multiple kerogen first-order and second-order reactions contributing to the recorded shale pyrograms versus the simplicity and crispness of the single first-order reactions *** results also justify the validity of using the previously described“variable e-A pyrogram-fitting method”of multi-heating-rate shale pyrograms enabling optimizers to choose multiple reactions from an unlimited range of e-A *** contrast,further doubt is cast on the validity of the constant-A pyrogram-fitting method used by the easy%Ro technique,in that a distribution of reactions with a single A value is unlikely to represent the complex variety of kerogen macerals observed in shale *** profiles generated by the variable e-A pyrogram-fitting method lie close to the established e-A trend and are likely to provide more realistic TF generation window temperatures than TF profiles generated by the constant-A pyrogram-fitting method.

Role of climatic variability in shaping intraspecific variation of thermal tolerance in Mediterranean water beetles

The climatic variability hypothesis(CVH)predicts that organisms in more thermally variable environments have wider thermal breadths and higher thermal plasticity than those from more stable ***,due to evolutionary trade-offs,taxa with greater absolute thermal limits may have little plasticity of such limits(trade-off hypothesis).The CVH assumes that climatic variability is the ultimate driver of thermal tolerance variation across latitudinal and altitudinal gradients,but average temperature also varies along such *** explored intraspecific variation of thermal tolerance in three typical Mediterranean saline water beetles(families Hydrophilidae and Dytiscidae).For each species,we compared two populations where the species coexist,with similar annual mean temperature but contrasting thermal variability(continental *** population).We estimated thermal limits of adults from each population,previously acclimated at 17,20,or 25℃.We found species-specific patterns but overall,our results agree with the CVH regarding thermal ranges,which were wider in the continental(more variable)*** the two hydrophilid species,this came at the cost of losing plasticity of the upper thermal limit in this population,supporting the trade-off hypothesis,but not in the dytiscid *** results support the role of local adaptation to thermal variability and trade-offs between basal tolerance and physiological plasticity in shaping thermal tolerance in aquatic ectotherms,but also suggest that intraspecific variation of thermal tolerance does not fit a general pattern among aquatic *** such intraspecific variation could lead to inaccurate predictions of the vulnerability of aquatic insects to global warming.

The great catastrophe: causes of the Permo-Triassic marine mass extinction

The marine losses during the Permo-Triassic mass extinction were the worst ever experienced. All groups were badly affected, especially amongst the benthos(e.g. brachiopods, corals, bryozoans, foraminifers,ostracods). Planktonic populations underwent a fundamental change with eukaryotic algae being replaced by nitrogen-fixing bacteria, green-sulphur bacteria, sulphate-reducing bacteria and prasinophytes. Detailed studies of boundary sections, especially those in South China, have resolved the crisis to a ～55 kyr interval straddling the Permo-Triassic boundary. Many of the losses occur at the beginning and end of this interval painting a picture of a two-phase extinction. Improved knowledge of the extinction has been supported by numerous geochemical studies that allow diverse proposed extinction mechanisms to be studied. A transition from oxygenated to anoxic-euxinic conditions is seen in most sections globally, although the intensity and timing shows regional variability. Decreased ocean ventilation coincides with rapidly rising temperatures and many extinction scenarios attribute the losses to both anoxia and high *** kill mechanisms include ocean acidification for which there is conflicting support from geochemical proxies and, even less likely, siltation(burial under a massive influx of terrigenous sediment) which lacks substantive sedimentological evidence. The ultimate driver of the catastrophic changes at the end of the Permian was likely Siberian Trap eruptions and their associated carbon dioxide emissions with consequences such as warming, ocean stagnation and acidification. Volcanic winter episodes stemming from Siberian volcanism have also been linked to the crisis, but the short-term nature of these episodes(ecades) and the overwhelming evidence for rapid warming during the crisis makes this an unlikely cause. Finally, whilst the extinction is well studied in equatorial latitudes, a different history is found in northern Boreal latitudes

extreme cold temperatures were observed in July and August 2023, coinciding with the WINFLY(winter fly-in)period of mid to late August into September 2023, meaning aircraft operations into Mc Murdo Station and Phoenix Airfield were adversely impacted. Specifically, with temperatures below-50℃, safe flight operation was not possible because of the risk of failing hydraulics and fuel turning to gel onboard the aircraft. The cold temperatures were measured across a broad area of the Antarctic, from east Antarctica toward the Ross Ice Shelf, and stretching across West Antarctica to the Antarctic Peninsula. A review of automatic weather station measurements and staffed station observations revealed a series of sites recording new record low temperatures. Four separate cold phases were identified, each a few days in duration and occurring from mid-July to the end of August 2023. A brief analysis of 500-hPa geopotential height anomalies shows how the mid-tropospheric atmospheric environment evolves in relation to these extreme cold temperatures. The monthly 500-hPa geopotential height anomalies show strong negative anomalies in August. examination of composite geopotential height anomalies during each of the four cold phases suggests various factors leading to cold temperatures, including both southerly off-content flow and calm atmospheric conditions. Understanding the atmospheric environment that leads to such extreme cold temperatures can improve prediction of such events and benefit Antarctic operations and the study of Antarctic meteorology and climatology.
