objective: To evaluate the morbidity due to diabetes mellitus type 2 within the State of San Luis Potosí, México, through a strong methodology, through which the multivariate relations were identified of the main social and environmental determiners in the disease, thus managing to quantify their respective levels of responsibility. Material and Methods: This evaluation began as a hypothesis of a multicasual theoretical model on diabetes mellitus and its main determining factors, which was analyzed through the application of multivariate exploratory statistical methodologies and confirmed as it is the case of the principal components analysis and the structural equation models. Results: Three components were extracted that explain the 96% of the total variance of the indicators;the main risk factors which were identified in the first component were, the use of the car, age, homes with TV use, urban life and feminine population;the indicators from the second and third component have little influence in the impact of the disease. Conclusions: the study shows the usefulness of the model for the analysis and prioritization of the environmental and social determiners of the disease, information that could sustain the design of public guidelines for the prevention and control of the analyzed disease.

Expectations and Perspectives of Users with the Screening Program for Cervical Cancer

objective: Evaluate user satisfaction before and after taking the Pap. Methodology: observational and prospective study. It was carried out in two health centers in San Luis Potosicity, Mexico in 93 users which attended for a Pap test from February to June 2015. The satisfaction was assessed using multidimensional scaling SERVQUAL whit 5 dimensions to evaluate the quality of services in an organization: reliability, responsibility, security, empathy and touchable and materials goods (tangible elements). This scaling was obtained internal consistency by Cronbach’s alpha coefficient with a value of 0.74. For data analysis, nonparametric test Wilcoxon for related samples was used. Results: In all cases, perspective was less than expectancy where the means of the scores before attention (expectation) and after care (perspective) there was a statistically significant difference (p = 0.000). Before receiving care users had an expectation of 192.96 points, after care perspective score dropped to 184.49 points indicating that the care provided was not what they expected. The difference in scores was 8.47 points (p = 0.004). Conclusions: Users of the screening program for cervical cancer were unsatisfied because the attention was not what they expected. In most of the indicators studied, a high percentage of dissatisfaction was obtained.

objective: To evaluate the effectiveness of an educational intervention to improve knowledge and skills in taking Pap smear conducted by health staff implementing the program on Early Detection of Cervical Cancer. Methods: A quasiexperimental, longitudinal and prospective study, held from october 25 2010 to April 11, 2011 at a hospital in San Luis Potosi, Mexico. To select groups applied a non-probabilistic convenience sampling that was composed of 28 people, 15 for the study group and 13 to the control group. The study group received training that was divided into three phases in which assessed the knowledge and skills taking the smear, the control group received conventional training offered by your workplace. Papanicolaou quality was measured before and after the intervention. The reference used international classification system Bethesda 2001. For data analysis we used the Student t test. Results: The study group increased knowledge (t = 8.768, p = 0.000) in the management of official standards, in the anatomy of the lower female genital tract and in the form of testing. Technical skills (t = 8.639, p = 0.000) also increased in the study group after the intervention. The control group showed no significant changes. The quality report of the samples in the study group increased from 60% to 86.7% and in the control group decreased from 92.3% to 84.6%. Conclusions: The training is effective in improving the knowledge and skills in taking Pap. Health staff who received the training improved their performance and showed more interest in providing high quality service to women.

An improved hybrid position/force controller design of a flexible robot manipulator is presented using a sliding observer. The friction between the end effector and the environment is considered and compensated. For systematic reasons the controller is designed taking into consideration the rigid link subsystems and the flexible joints. The proposed control system satisfies the stability of the two subsystems and copes with the uncertainty of robot dynamics. A sliding observer is designed to estimate the time derivative of the torque applied as input to the rigid part of the robot. For the stability of the observer, it is assumed that the uncertainty of the observed system is bounded. A MRAC algorithm is used for the estimation of the friction forces at the contact point between the end effector and the environment. Finally simulation and experimental results are given, to demonstrate the effectiveness of the proposed controller.

For the development of systems related to renewable energy and specifically for thermo-solar energy,it is essential to study and analyze the different thermal phenomena of heat transfer,in search of the best conditions that allow strengthening the *** such as radiation,convection,and conduction are highly studied,from the field of materials to the field of infrastructure,finding day to day optimal solutions that guarantee a great use of the solar resource available on the earth's *** study presented the results of heat transfer analysis on a receiver pipe in a CCP;designed using the theory of branched fractal geometry to improve the heat transfer associated with concentrating solar capture systems.A fractal structure as a branch or arboreal fractal type network,influenced thermal and electrical parameters,as electrical resistance,thermal resistance,and convective heat transfer,so increasing the level branch heat transfer can be enhancement along of the *** use of renewable energies in agriculture benefits the industrial processes that require a continuous power system,due to climatic conditions and geographical location is not a guarantee in food production fields,which reason solar capture systems contribute to food dehydration processes,water heating,refrigeration,and energy production.

Integration of Probabilistic and Multi-Hazard Risk Assessment Within Urban Development Planning and Emergency Preparedness and Response: Application to Manizales,Colombia

The details of a multi-hazard and probabilistic risk assessment, developed for urban planning and emergency response activities in Manizales, Colombia, are presented in this article. This risk assessment effort was developed under the framework of an integral disaster risk management project whose goal was to connect risk reduction activities by using open access and state-of-theart risk models. A probabilistic approach was used for the analysis of seismic, landslide, and volcanic hazards to obtain stochastic event sets suitable for probabilistic loss estimation and to generate risk results in different metrics after aggregating in a rigorous way the losses associated to the different hazards. Detailed and high resolution exposure databases were used for the building stock and infrastructure of the city together with a set of vulnerability functions for each of the perils considered. The urban and territorial ordering plan of the city was updated for socioeconomic development and land use using the hazard and risk inputs and determinants, which cover not only the current urban area but also those adjacent areas where the expansion of Manizales is expected to occur. The emergency response capabilities of the city were improved by taking into account risk scenarios and after updating anautomatic and real-time post-earthquake damage assessment.

Background:Vestibular symptoms on sitting-up are frequent on patients seen by vestibular ***,a benign paroxysmal positional vertigo(BPPV)variant which elicits vestibular symptoms with oculomotor evidence of posterior semicircular canal(P-SCC)cupula stimulation on sitting-up was described and named sitting-up vertigo BPPV.A periampullar restricted P-SCC canalolithiasis was proposed as a causal ***:To describe new mechanisms of action for the sitting-up vertigo BPPV ***:Eighteen patients with sitting-up vertigo BPPV were examined with a pre-established set of positional maneuvers and follow-up until they resolved their symptoms and clinical ***:All patients showed up-beating torsional nystagmus(UBTn)and vestibular symptoms on coming up from either Dix-Hallpike(DHM)or straight head-hanging *** out of 18 patients presented a sustained UBTn with an ipsitorsional component to the tested side on half-Hallpike maneuver(HH).A slower persistent contratorsional down-beating nystagmus was found in eleven out18 patients tested on nose down position(nD).Conclusions:Persistent direction changing positional nystagmus on HH and nD positions indicative of PSCC heavy cupula was found in 11 patients.A sustained UBTn on HH with the absence of findings on nD,which is suggestive of the presence of P-SCC short arm canalolithiasis,was found on 5 *** patients were treated with canalith repositioning maneuvers without success,but they resolved their findings by means of Brandt-daroff *** propose P-SCC heavy cupula and P-SCC short arm canalolithiasis as two new putative mechanisms for the sitting-up vertigo BPPV variant.

A fully probabilistic seismic risk assessment was developed in Manizales, Colombia, considering assets of different types. The first type includes elements that are part of the water and sewage network, and the second type includes public and private buildings. This assessment required the development of a probabilistic seismic hazard analysis that accounts for the dynamic soil response,assembling high resolution exposure databases, and the development of damage models for different types of elements. The economic appraisal of the exposed assets was developed together with specialists of the water utilities company of Manizales and the city administration. The risk assessment was performed using several Comprehensive Approach to Probabilistic Risk Assessment modules as well as the R-System, obtaining results in terms of traditional metrics such as loss exceedance curve, average annual loss, and probable maximum loss. For the case of pipelines, repair rates were also estimated. The results for the water and sewage network were used in activities related to the expansion and maintenance strategies, as well as for the exploration of financial retention and transfer alternatives using insurance schemes based on technical,probabilistic, and prospective damage and loss *** the case of the buildings, the results were used in the update of the technical premium values of the existing collective insurance scheme.
