Background: Couple High Blood Pressure (HBP)-Diabetes is a morbid association and a public health problem. The aim of this study was to determine the actual epidemiological profile of HBP in type 2 diabetic. Methods: A cross-sectional, prospective, descriptive and analytical study was conducted at Banque d’insuline of Cotonou, Polyclinique Atinkanmey and CHUD-Ouémé-Plateau. The study took place over a period of 06 months from March 01 to August 30, 2014. The study included patients with type 2 diabetes mellitus seen at consultation who agreed to participate in the study. Results: The survey involved 400 individuals. Among them, 34% were male and the sex ratio was 0.48. The mean age was 55.6 ± 10.3 years (range 28 - 87 years). The prevalence of hypertension in type 2 diabetic patients was 70%. The risk factors significantly associated with HBP were age above 55 years (p = 0.000), abdominal obesity (p = 0.036), a diabetes duration above 10 years (p = 0.009). Complications significantly associated with HBP were stroke (p = 0.013) and diabetic foot (p = 0.044). Conclusion: HBP-type 2 Diabetes association is frequent in Benin.

Introduction: Systemic lupus erythematosus (SLE) is a non-organ-specific autoimmune disease with an unknown origin. The unchanged trend of premature mortality in systemic lupus erythematosus shows the critical unmet need for improved and optimized management of systemic lupus erythematosus. Objectives: To analyze the clinical features and the prognostic factors of death in SLE at the CNHU-HKM of Cotonou. Patients and Methods: This was a retrospective cohort study. It was conducted over the period from January 1st 2010 to December 31st 2021. The study population consisted of all patients followed for SLE in the internal medicine, rheumatology, nephrology and dermatology wards at the CNHU-HKM of Cotonou. Results: 88 cases were recorded in 12 years, i.e. an incidence of 7 cases per year. There were 80 women and 8 men with a mean age of 36.4 ± 13.1 years. The clinical picture was dominated by mucocutaneous (86.3%) and osteoarticular (71.5%) disorders. The biological abnormalities observed were anemia (78.8%), lymphopenia (43.1%) and thrombocytopenia (17.7%). 39 patients had renal damage (44.3%). The incidence of death was 17%. Factors associated with death were renal involvement, infectious complications, high initial SLEDAI score, flare, thrombocytopenia and lymphopenia. Conclusion: Mortality related to SLE remains high at the CNHU-HKM of Cotonou. Renal involvement, infectious complications, high initial SLEDAI score, flare, lymphopenia and thrombocytopenia were the factors associated with death. The presence of these factors should lead to an evaluation of the treatment.

Introduction: Systemic autoimmune diseases have been poorly studied in sub-Saharan Africa and their frequency is not well known. This study provided an overview of the main reasons for the use of care and their evolution in the main hospitals in Benin. Methods: This was a multi-centric descriptive cross-sectional study conducted in the internal medicine, rheumatology, dermatology and nephrology departments of nine (09) public and private hospital centers in Benin over a 57-month period, from January 1st, 2017 to September 30th, 2021. It involved patients followed for a systemic autoimmune disease. The data was collected with a digital survey sheet and then processed and analyzed with the R software (version 3.4). Results: Two hundred and three (203) patients were recorded, which represents a hospital frequency of 0.13%. The mean age was 44 years and the sex ratio (M/F) was 0.29. Connective tissue disease accounted for 95.07% of systemic autoimmune diseases which were dominated by rheumatoid arthritis (40.9%) and systemic lupus (37.4%). Ten cases of vasculitis have been reported and dominated by Behçet’s disease (40%). The main reasons for seeking care were asthenia, weight loss and fever. Arthralgia and skin lesions are the main guiding signs. Six deaths (3.1%) were recorded among connective tissue disease and 1 death (10%) among vasculitis. Conclusion: In spite of being rare, systemic autoimmune diseases are a reality in Benin. A general population study would provide a better understanding of clinical characteristics and identify prognostic factors.

The present study aim was to assess the preventive effects of Ananas comosus juice consumption on the risk factors of obesity in female Wistar rats. 108 rats were tested for 90 days. After randomization, they were shared out into six groups including four experimental groups (GTc, GPlp, GEns and GBrS) and two control groups (). The control group consumed 1.20 ml of distilled water, the experimental rats received in oral, 1.20 ml of drinks made from the different parts of Ananas comosus fruit. The measurements of morphometric and biochemical parameters were carried out on Day 0 (D0), Day 30 (D30), Day 60 (D60) and Day 90 (D90). This paper is to determine the contribution of the ambulatory measure of blood pressure (AMBP) to the detection of hypertension in type 2 diabetic black African in Benin. Hypertension can stay unknown in diabetic patients. Patients and Methods: We conducted a cross-sectional, prospective, descriptive and analytical study at “Banqued’ insuline” of Cotonou, Polyclinique Atinkanmey and CHUD-Ouémé-Plateau. The study took place over a period of 6 months from March 01 to August 30 2014. The study included patients with type 1 or 2 diabetes who agreed to participate in the study and who made ambulatory measure of blood pressure (AMBP). Statistical analysis was done by using the software Excel 2013 and SPSS versus 18.0. Results: Sixty six patients were included. Forty one (62.1%) among them were female;sex-ratio was 0.61. The mean of age was 48.9 ± 8.8 years with range from 30 to 68 years. The prevalence of masked hypertension in type 2 diabetics was 37.9% (25/66). Abdominal obesity was the significative factor related to masked high blood pressure (HBP) in the type 2 diabetics (p = 0.005). Among diabetic with masked hypertension, 14 (56%) had “no dipper” profile and 11 (44%) had “dipper” profile. Conclusion: The ambulatory measure of blood pressure (AMBP) may take an important place in the detection of Hypertension in black type 2 diabetic subjects.

Background: Abdominal obesity is often associated with type 2 diabetes, especially in the context of metabolic syndrome. The objective of this study was to determine the prevalence of corpuscular and abdominal obesity in type 2 diabetes in the two major cities in southern Benin. Methods: It was a multicentric, prospective, descriptive and analytical study that had as framework the center for screening and monitoring of diabetes “Banque d’insuline” of Cotonou, the Polyclinic Atinkanmey of Cotonou and the Internal Medicine Department of the Departmental University Hospital of Ouémé-Plateau in Porto-Novo. The study was a six-month period (March-August 2014). Results: We included a total of 400 type 2 diabetics. Women represented 66% of the study population with a sex ratio of 0.52. The mean age of patients was 55.6 ± 10.3 years with extremes of 28 and 87 years. The prevalence of corpuscular obesity (BMI ≥ 30 Kg/m2) was 38.5% in our study population. For the gender, it was 48.5% in women and 19.1% in men with a statistically significant difference (p = 0.0001). The overall prevalence of abdominal obesity in our study population was 87.8%. Almost all of our diabetic women (99.2%) had abdominal obesity versus 65.4% in men (p = 0.0001). All the patients with type 2 diabetes having corpuscular obesity had also abdominal obesity. This abdominal obesity was observed in 80.1% of non-obese diabetic (p = 0.0001). On the contrary, the majority of patients with abdominal obesity (56.12%) had a BMI below 30 kg/m2. Conclusion: This study shows a high prevalence of abdominal obesity (87.8%) in our study population compared with the corpuscular obesity (38.5%). Hence, the importance of measuring waist circumference is more than BMI in diabetic patients.

Mechanical Confining of Sinonasal Anatomic Variants by Computer Tomography in Togo

Objective: Determine the frequency of the sinonasal mechanical anatomic variants by CT Scan. Patients and method: Six-month retrospective study concerning sinus CT scans in the registers of Teaching Hospitals’ Imaging Services of Lomé and Kara (TOgo). It involves patients without distinction of sex, of over 15 years without anomaly that can lead to a modification of the normal anatomical configuration of the paranasal sinuses’ cavities. The analysis has been made by only one radiologist. Results: The frequency of mechanical sinonasal anatomic variants stood at 91.87%. The narrowing of the middle nasal turbinate was the variant mostly observed (55.03%), dominated by the deviation of the nasal septum whose frequency was 25.48%. The narrowing of the infundibulum represented 10.80% of population. They were more represented by the Haller cell which displayed a frequency of 3.60%. The variants venturing to narrow the upper nasal turbinate, represented only by extra turbinates, involved 1.02% of the population. The other variants of mechanical sinonasal anatomic variants represented 25.02%. They were dominated by the maxillary sinus septa (11.2%). Our study showed a feminine predominance which is statistically important for Haller cells (p = 0.037) and the ethmoidbulla hypertrophy (p = 0.0036). Conclusion: The anatomic variants of mechanical sinonasal are very frequent in Togo.

Napoleonaea vogelii, Lecythidaceae family is a tropical evergreen shrub widely distributed in the coastal regions of West African countries including Benin. It is a medicinal plant whose leaves and bark are of great utility in traditional medicine. Despite its importance, it is little used in ethnoveterinary medicine and its pharmacological basis in this field and especially in the treatment of parasitic diseases caused by Haemonchus contortus is very little documented. This review aims to synthesise existing data on the chemical composition, pharmacological and zootechnical usefulness of N. vogelii and to identify the gaps of these works in order to propose research perspectives. google Scholar database was used to gather the majority of useful information available on N. vogelii. The results showed that of the 23 publications included in the present study, 60.87% dealt distinctly with the pharmacological properties of N. vogelii. These properties include anthelmintic, antidiabetic, wound healing, antimicrobial, antibacterial, anti-inflammatory, antioxidant properties and N. vogelii contains many secondary metabolites including alkaloids, tannins, flavonoids, saponins, glycosides which give it its pharmacological properties. The Correspondence Factor Analysis performed for the variables extracts and chemical compounds showed that the presence of a given chemical compound is not dependent on the type of extract. This work has gathered information on N. vogelii and found that the species has powerful therapeutic potentials but very little use in traditional veterinary medicine. Introduction: Pulmonary embolism is a diagnostic and therapeutic emergency that can be life-threatening. Its mortality is largely attributable to severe forms classically defined by clinical and morphological criteria. The aim of this study is to establish the role of two cardiac biomarkers (NT-proBNP and troponin) in assessing the severity of pulmonary embolism. Patients and Methods: We conducted a descriptive and analytical cross-sectional study. Data collection was retrospective over the period from January 1, 2011 to December 31, 2021. All patients hospitalized for pulmonary embolism in two cardiology referral clinics in Cotonou (Atinkanmey Polyclinic and CICA Clinic) were included. Results: The hospital prevalence of pulmonary embolism was 9.08%. The mean age was 52.6 years, with extremes of 18 and 92 years. The sex ratio was 0.73. Pulmonary embolism was severe according to hemodynamic, morphological and sPESI criteria in 12%, 24% and 39% of cases respectively, and mortality was 61.53%. Mean NT-ProBNP and troponin I levels were significantly higher in patients with severe criteria than in those without. NT-proBNP and troponin had good specificity for predicting cardiovascular arrest (99% and 90%), shock (100% and 98%), and hypotension (99% and 96%). NT-proBNP has the best positive predictive values in relation to the occurrence of shock (100%) and right ventricular dilatation (93%). The best correlation coefficient was obtained between right ventricular dilatation and NT-proBNP (0.78). Conclusion: NT-proBNP and troponin I are good biomarkers for predicting the severity of pulmonary embolism and allowing therapeutic adaptation when they are elevated.
