Mortality Trends for Neglected Tropical Diseases in the State of Sergipe,Brazil,1980-2013

Background:Neglected Tropical Diseases are a set of communicable diseases that affect the population so low socioeconomic status,particularly 1.4 billion people who are living below the poverty *** study has investigated the magnitude and mortality time trends for these diseases in the state of Sergipe,Northeast Region of ***:We conducted an ecological study of time series,based on secondary data derived from the Mortality Information System of the Ministry of *** mortality rates(crude,age-standardized rates and proportional ratio)were calculated from the deaths due to Neglected Tropical Diseases in the state of Sergipe,from 1980 to *** time trends were obtained using the Joinpoint regression ***:Three hundred six thousand and eight hundred seventy-two deaths were certified in the state and Neglected Tropical Diseases were mentioned as the underlying cause in 1,203 certificates(0.39%).Mean number of deaths was 35.38 per year,and crude and age-standardized mortality rates were,respectively:2.16 per 100000 inhabitants(95%CI:1.45-2.87)and 2.87 per 100000 inhabitants(95%CI:1.93-3.82);the proportional mortality ratio was 0.41%(95%CI:0.27-0.54).In that period,Schistosomiasis caused 654 deaths(54.36%),followed by Chagas disease,with 211(17.54%),and by Leishmaniases,with 142(11.80%)*** other diseases totalized 196 deaths(16.30%).There were increasing mortality trends for Neglected Tropical Diseases,Schistosomiasis and Chagas disease in the last 15 years,according to the age-standardized rates,and stability of the mortality trends for ***:The Neglected Tropical Diseases show increasing trends and are a real public health problem in the state of Sergipe,since they are responsible for significant mortality *** following diseases call attention for showing greater number of deaths in the period of study:Schistosomiasis,Chagas disease and *** finally suggest that public managers take appropri

Vestibular and cochlear dysfunction in aging:Two sides of the same coin?

Objective:Nonspecific complaints of hearing loss,vertigo,imbalance,and instability,without a defined etiology,are very prevalent in the elderly population,with a great impact on morbidity and mortality in this age *** objectives of this study were to verify whether there is age-related vestibular dysfunction and to test the association of vestibular dysfunction with presbycusis in the elderly ***:Original retrospective analytical cross-sectional study,carried out with 80 patients who underwent a videonystagmography and complete audiometric evaluation due to nonspecific vestibular complaints,without a specific vestibular disorder *** were selected and divided into two distinct age groups(group A:>60 years;group B:18-50 years)and,in both groups,we analyzed the caloric tests and the pure-tone ***:In the vestibular evaluation,we found that there was a statistically significant difference(P<0.05)between groups in the prevalence of bilateral vestibular weakness(group A:22.5%;group B:5%),and that the increase in age,above 60 years,is negatively correlated with the mean total caloric ***,we obtained a reasonable negative and statistically significant correlation(r=-0.320,P<0.05)between the mean bone conduction thresholds at high frequencies and total caloric responses in group ***:In patients with hearing loss,it is essential to perform a complete vestibular study to diagnose vestibular disorders and,consequently,prevent adverse outcomes that may result from these alterations.

Suicide Attempts in a General Hospital: A Case-Control

Objective: This study aims is to conduct a comparative analysis of the clinical and sociodemographic profiles of individuals referred to Consultation-Liaison Psychiatry (CLP) at a tertiary general hospital, specifically focusing on those seeking assistance due to suicide attempts (TS) versus CLP requests for other reasons (non-TS) between the years 2010 and 2020. Additionally, we aim to assess factors associated with suicide attempts. Method: This retrospective controlled study involves a thorough examination of medical records encompassing 3913 patients who received CLP services at the Hospital de Base (HB) and the Children and Maternity Hospital (HCM) of São José do Rio Preto Medical School Foundation (Funfarme) from June 2010 to June 2020. The patient cohort is categorized into two groups: those with requests for SA (n = 677) and those with requests for other reasons. Results: In the SA group, as opposed to the non SA group, statistically significant differences were observed for gender (predominantly female), employment status (employed), marital status (single), and age (younger). Notably, a higher prevalence of psychiatric antecedents (88.7%, n = 555 vs. 73.6% n = 2018) was found in the SA group, indicating a significant association. The frequency of SA requests remained stable over the years. Among the SA group, mood disorders were the most prevalent (39%, n = 372), followed by disorders related to substance use (21.2%, n = 202), and personality disorders (16%, n = 152). A history of previous suicide attempts emerged as a crucial factor for the recurrence of SA (p-value ≤ 0.001). Exogenous intoxication emerged as the most frequently employed method in attempts (79.6%, n = 539), particularly prevalent among patients diagnosed with mood disorders (41%, n = 314). Clinical Criteria for Airway Assessment: Correlations with Laryngoscopy and Endotracheal Intubation Conditions

Difficult intubation, inadequate ventilation and esophageal intubation are the principal causes of death or brain damage related to airway manipulation. The objective of this cross-sectional study was to correlate a preanesthetic evaluation that may be capable of predicting a difficult intubation with the conditions encountered at laryngoscopy and endotracheal intubation. Eighty-one patients submitted to general anesthesia were evaluated at a preanesthetic consultation according to the modified Mallampati classification, the Wilson score and the American Society of Anesthesiologists (ASA) difficult airway algorithm. Findings were then correlated with the Cormack-Lehane classification and with the number of attempts at endotracheal intubation. No statistically significant correlations were found between the patients’ Mallampati classification and their Cormack-Lehane grade or between the Mallampati classification and the number of attempts required to achieve endotracheal intubation. Laryngoscopy proved difficult in four patients and in all of these cases the Wilson score had been indicative of a possibly difficult airway, highlighting its good predicting sensitivity. However, the specificity of this test was low, since another 24 patients had the same Wilson score but were classified as Cormack-Lehane I/II. Moreover, two patients who had a Wilson score ≥ 4 were also classified as Cormack-Lehane grade I/II. The study concluded that the Wilson score, although seldom used in clinical practice, is a highly sensitive predictor of a difficult airway;its specificity, however, is low.

Stimulation of the P6 Acupuncture Point for Prophylaxis of Nausea and Vomiting in Pregnant Women Submitted to Cesarean Section: A Blinded Clinical Trial

In pregnant women subjected to spinal anesthesia for a Cesarean section, episodes of nausea and vomiting are common both during and following surgery. Acupuncture for the prophylaxis and treatment of these complications has been gaining in popularity due to its low cost, simplicity, absence of side effects and confirmed efficacy. This study investigated the efficacy of stimulating the P6 acupoint in conjunction with the use of dexamethasone as prophylaxis for nausea and vomiting in pregnant women submitted to spinal anesthesia for a Cesarean section. The patients (n = 100) were randomly distributed into two groups. In the first group (n = 50), a site located one centimeter laterally from P6 was stimulated. This is not a true acupuncture point (sham acupuncture). In the second group (n = 50), P6 was stimulated. In both groups, 4 mg of dexamethasone were administered intravenously. A questionnaire was used to obtain information on the occurrence of nausea and vomiting during surgery and in the first 12 hours postpartum. The chi-square test and Fisher’s exact test were used to assess differences between the groups. Age and physical status were similar in both groups. The incidence of nausea during surgery was 32% (n = 16) in the control group and 22% (n = 11) in the P6 group (p > 0.05). In the first 12 hours following surgery, nausea occurred in 16% of the women in the control group (n = 6) and in 4% in the P6 group (n = 4) (p = 0.045). The incidence of vomiting in the control group was 12% (n = 6) during surgery and 10% (n = 5) in the postoperative period compared to 8% (n = 4) and 4% (n = 2), respectively, in the P6 group (p > 0.05). Although these differences were not statistically significant with the exception of the incidence of nausea in the first 12 hours postpartum, a reduction occurred in the incidence of all the outcomes evaluated in the P6 group.
