Watson-Jones Anatomical Approach for Open Reduction and Internal Fixation of Proximal Femoral Fractures without Image Intensifier in a Low-Resource Setting

Introduction: Standard procedures for surgical fixation of proximal femoral fractures (PFF) require an image intensifier which in developing countries remains a luxury. We hypothesized that, with a well-codified technique, the Watson Jones approach (WJA) without image intensifier nor traction table, can allow open reduction and internal fixation (ORIF) of PFF using dynamic hip screw (dHS), with satisfactory outcome. Patients and methods: Forty one consecutive patients (mean age 59.5 ± 21.6 years, 61% males) who were followed in a Teaching Hospital for PFF treated by ORIF using the WJA and dHS from January 2016 to december 2020 were reassessed. The outcome measures were the quality of the reduction, the positioning of the implants, the tip-apex distance (TAd), the rate and delay of consolidation, the functional results using Postel Merle d’Aubigné (PMA) score, the rate of surgical site infection (SSI) and the overall mortality. Logistic regression was used to determine factors associated with mechanical failure. Results: The mean follow-up period was 33.8 ± 15.0 months. Fracture reduction was good in 31 (75.6%) cases and acceptable in 8(19.5%) cases. Implant position was fair to good in 37 (90.2%) patients. The mean TAd was 26.1 ± 3.9 mm. Three patients developed SSI. Consolidation was achieved in 38 (92.6%) patients. The functional results were good to excellent in 80.5% of patients. The overall mortality rate was 7.3%. There were an association between mechanical failure and osteoporosis (p = 0.04), fracture reduction (p = 0.003), and TAd (p = 0.025). In multivariate logistic regression, no independent factors were predictive of mechanical failure. Conclusion: This study shows that ORIF using dHS for PFF via the Watson-Jones approach without an image intensifier can give satisfactory anatomical and functional outcomes in low-resource settings. In this era of biodiversity loss and climate change,quantifying the impacts of natural disturbance on forest communities is imperative to improve biodiversity conservation *** and epixylic lichens are effective forest quality bioindicators,as they are generally long-lived organisms supported by continuity of specific forest structures and their associated microclimatic ***,how lichen communities respond to the effects of fluctuating historical disturbances remains *** a dendrochronological approach,this study investigates how natural disturbance dynamics indirectly influence various lichen community metrics in some of Europe's best-preserved primary mixed-beech *** modelling revealed that natural historical disturbance processes have decades-long effects on forest structural attributes,which had both congruent and divergent impacts on lichen community richness and *** species richness indirectly benefited from both historical and recent higher-severity disturbances via increased standing dead tree basal area and canopy openness respectively-likely through the presence of both pioneer and late-successional species associated with these ***-listed species richness showed a dependence on habitat continuity(old trees),and increased with disturbance-related structures(standing dead trees)whilst simultaneously benefiting from periods without severe disturbance events(old trees and reduced deadwood volume).However,if the disturbance occurred over a century in the past,no substantial effect on forest structure was ***,while disturbance-mediated forest structures can promote overall richness,threatened species appear vulnerable to more severe disturbance events-a concern,as disturbances are predicted to intensify with climate ***,the high number of threatened species found reinforce the critical role of primary forest structural attributes for biodiversity ***

Background:Physiological and biochemical processes across tissues of the body are regulated in response to the high demands of intense physical activity in several occupations,such as firefighting,law enforcement,military,and sports.A better understanding of such processes can ultimately help improve human performance and prevent illnesses in the work ***:To study regulatory processes in intense physical activity simulating real-life conditions,we performed a multi-omics analysis of 3 biofluids(blood plasma,urine,and saliva)collected from 11 wildland firefighters before and after a 45 min,intense exercise *** profiles ***-exercise were compared by Student’s t-test followed by pathway analysis and comparison between the different omics ***:Our multi-omics analysis identified and quantified 3835 proteins,730 lipids and 182 metabolites combining the 3 different types of *** blood plasma analysis revealed signatures of tissue damage and acute repair response accompanied by enhanced carbon metabolism to meet energy *** urine analysis showed a strong,concomitant regulation of 6 out of 8 identified proteins from the renin-angiotensin system supporting increased excretion of catabolites,reabsorption of nutrients and maintenance of fluid *** saliva,we observed a decrease in 3 pro-inflammatory cytokines and an increase in 8 antimicrobial peptides.A systematic literature review identified 6 papers that support an altered susceptibility to respiratory ***:This study shows simultaneous regulatory signatures in biofluids indicative of homeostatic maintenance during intense physical activity with possible effects on increased infection susceptibility,suggesting that caution against respiratory diseases could benefit workers on highly physical demanding jobs.

Primary forests are spatially diverse terrestrial ecosystems with unique characteristics,being naturally regenerative and heterogeneous,which supports the stability of their carbon storage through the accumulation of live and dead ***,little is known about the interactions between biomass stocks,tree genus diversity and structure across a temperate montane primary ***,we investigated the relationship between tree structure(variability in basal area and tree size),genus-level diversity(abundance,tree diversity)and biomass stocks in temperate primary mountain forests across Central and Eastern *** used inventory data from726 permanent sample plots from mixed beech and spruce across the Carpathian *** used nonlinear regression to analyse the spatial variability in forest biomass,structure,and genus-level diversity and how they interact with plot-level tree age,disturbances,temperature and *** found that the combined effects of genus and structural indices were important for addressing the variability in biomass across different spatial *** processes in disturbance regimes and uneven tree age support forest hete rogeneity and the accumulation of live and dead biomass through the natural regeneration,growth and decay of the forest *** complexities in basal area index,supporte d by genus-level abundance,positively influence total biomass stocks,which was modulated by tree age and *** forests showed higher tree density and basal area than mixed beech forests,though mixed beech still contributes significantly to biomass across *** heterogeneity was strongly influenced by complexities in forest composition(tree genus diversity,structure).We addressed the importance of primary forests as stable carbon stores,achieved through structure and *** such ecosystems is critical for ensuring the stability of the primary forest,carbon store and biodiversity into the future.

Numerical Models and Methods of Atmospheric Parameters Originating in the Formation of the Earth’s Climatic Cycle

Atmospheric models are physical equations based on the ideal gas law. Applied to the atmosphere, this law yields equations for water, vapor (gas), ice, air, humidity, dryness, fire, and heat, thus defining the model of key atmospheric parameters. The distribution of these parameters across the entire planet Earth is the origin of the formation of the climatic cycle, which is a normal climatic variation. To do this, the Earth is divided into eight (8) parts according to the number of key parameters to be defined in a physical representation of the model. Following this distribution, numerical models calculate the constants for the formation of water, vapor, ice, dryness, thermal energy (fire), heat, air, and humidity. These models vary in complexity depending on the indirect trigonometric direction and simplicity in the sum of neighboring models. Note that the constants obtained from the equations yield 275.156˚K (2.006˚C) for water, 273.1596˚K (0.00963˚C) for vapor, 273.1633˚K (0.0133˚C) for ice, 0.00365 in/s for atmospheric dryness, 1.996 in2/s for humidity, 2.993 in2/s for air, 1 J for thermal energy of fire, and 0.9963 J for heat. In summary, this study aims to define the main parameters and natural phenomena contributing to the modification of planetary climate. .

development and Optimization of Universal Bone Conduction Hearing Spectacles

Background: Non-implantable bone anchored hearing devices (BCHds) are utilized for patients with conductive or mixed hearing loss who are unsuitable for conventional hearing aids or have unresolved middle ear issues. These devices can be surgically implanted or attached using adhesive plates, dental sticks, elastic headbands, or bone conduction spectacles. Optimal fitting of bone conduction spectacles requires appropriate frame selection and contact pressure in the temporal and mastoid areas. The ANSI S3.6 and dIN EN ISO 389-3 standards recommend a contact area of approximately 1.75 cm2 and a maximum force of 5.4 N for effective sound transmission and comfort. Methods: This study aimed to evaluate the technical fit and mechanical stability of universal bone conduction hearing spectacles compared to established systems. A Sen-Pressure 02 thin-film sensor connected to an Arduino Uno R3 board measured contact force in the temporal and mastoid areas. Several BCHds were tested, including the Bruckhoff la belle BC d50/70, Radioear B71 headset, Radioear B71 elastic headband, Cochlear Baha SoundArc M, and Cochlear Baha elastic headband, on a PVC artificial head, with data analyzed using ANOVA and LSd post hoc tests. Results: The la belle BC d50/70 spectacles showed comparable contact force to established BCHds, ensuring adequate sound transmission and comfort. Significant differences were observed between the systems, with the Radioear B71 headset exhibiting the highest forces. The la belle BC d50/70 had similar forces to the Radioear B71 elastic headband. Conclusion: The la belle BC d50/70 universal bone conduction hearing spectacles are a technically equivalent alternative to established BCHds, maintaining pressure below 5.4 N. Future research should explore the impact of different contact forces on performance and comfort, and the integration of force control in modified spectacles. Peanut is a worldwide oilseed crop and the need to assess germplasm in a non-destructive manner is important for seed nutritional *** this study,Near Infrared Spectroscopy(NIRS)was applied to rapidly assess germplasm variability from whole seed of 699 samples,field-collected and assembled in four genetic and environmentbased sets:one set of 300 varieties of a core-collection and three sets of 133 genotypes of an interspecific population,evaluated in three environments in a large spatial scale of two countries,Mbalmayo and Bafia in Cameroon and Nioro in Senegal,under rainfed *** elemental spectra were gathered on six subsets of seeds of each sample,after three rotation scans,with a spectral resolution of 16 cm-1over the spectral range of867 nm to 2530 *** were then processed by principal component analysis(PCA)coupled with Partial least squares-discriminant analysis(PLS-dA).As results,a huge variability was found between varieties and genotypes for all NIR wavelength within and between *** magnitude of genetic variation was particularly observed at 11 relevant wavelengths such as 1723 nm,usually related to oil content and fatty acid *** yielded the most chemical attributes in three significant PCs(i.e.,eigenvalues>10),which together captured 93%of the total variation,revealing genetic and environment structure of varieties and genotypes into four clusters,corresponding to the four samples *** pattern of genetic variability of the interspecific population covers,remarkably half of spectrum of the core-collection,turning out to be the ***,a PLS-dA model was developed and a strong accuracy of 99.6%was achieved for the four sets,aiming to classify each seed sample according to environment *** confusion matrix achieved for the two sets of Bafia and Nioro showed 100%of instances classified correctly with 100%at both sensitivity and specificity,confirming that their seed quality was different f
