Hepatitis C in the pediatric population: Transmission, natural history, treatment and liver transplantation

The number of children affected by the hepatitis C virus(HCV) in the United States is estimated to be between 23000 to 46000. The projected medical cost for children with HCV in the United States is $199-366 million over the next decade. The implementation of routine screening of blood supply has virtually eliminated transmission via transfusion and vertical transmission is now the most common mode of infection in children. Infections acquired during infancy are more likely to spontaneously resolve and fibrosis of the liver tends to increase with age suggesting slow progressive histologic injury. Anti-viral treatment may be warranted in children with persistently elevated liver enzymes or with significant fibrosis on liver biopsy. Current standard of care includes weekly pegylated interferon and ribavirin twice daily. Predictors of high sustained viral response include genotype 2 and 3 and low viral load in children with genotype 1( 90%). Only 34 pediatric patients were transplanted with hepatitis C between January 2008 and April 2013. The majority of pediatric patients were born prior to universal screening of blood products and, as of June 2013, there are only two pediatric patients awaiting liver transplantation for end-stage liver disease secondary to hepatitis C. Pediatric survival rates post-transplant are excellent but graft survivals are noticeably reduced compared to adults(73.73% for pediatric patients at one year compared to 87.69% in adult patients). New safe and effective antiviral therapies for recurrent HCV should help increase graft survival.

目的评价扁桃体、腺样体切除术对伴有其他上气道疾病的儿童阻塞性睡眠呼吸暂停/低通气综合征(OSAHS)的疗效。方法回顾性总结香港大学附属根德夫人儿童医院睡眠中心1999年5月至2003年5月的儿童OSAHS病例资料。依据睡眠监测结果诊断OSAHS,再根据体格检查及鼻咽侧位片结果确定这些OSAHS患儿是否存在扁桃体、腺样体肥大。将有扁桃体、腺样体肥大的OSAHS患儿分成手术治疗组和对照组,两组随访观察12个月。对术后仍有OSAHS表现的病例行睡眠内窥镜检查寻找原因。结果88例儿童中,61例诊断为OSAHS,其中39例伴有扁桃体、腺样体肥大,25例接受扁桃体、腺样体切除术治疗。在接受手术治疗的患儿中,手术后随访时氧减指数及最低血氧饱和度较手术前得到显著改善。治疗组中24%随访时仍有OSAHS的表现,而对照组85%的患儿OSAHS表现仍持续存在(OR=15.4,95%置信区间为2.7～87.5)。手术后仍有OSAHS表现的主要原因是肥胖、骨软骨发育不良、上气道解剖结构异常以及维持气道开放的神经肌肉功能异常。结论扁桃体、腺样体切除术在扁桃体、腺样体肥大同时伴有其它上气道疾病的大部分亚洲OSAHS儿童中是有效的。由于上气道结构或维持气道开放的神经肌肉功能异常,可以导致手术后OSAHS症状的残存。对这些患儿,需要做进一步检查明确病因。

目的探讨以临床病史、体格检查及颈部侧位X线影像学检查筛查儿童阻塞性睡眠呼吸暂停综合征(obstructivesleepapneasyndrome,OSAS)的可行性。方法在门诊收集有打鼾症状的儿童病例,进行问卷调查、体格检查以及颈部侧位X线检查,以多导睡眠监测仪(PSG)的监测结果作为诊断标准,将患儿分成OSAS和非OSAS组,在两组患儿之间进行临床资料的比较。结果51例鼾症儿童中确诊为OSAS32例。两组患儿的年龄、性别比以及体块指数差异无显著性,而临床表现之间差异有显著性(P<0.001)。如果患儿有8条以上的临床症状、体格检查或影像学的异常,则其患有OSAS的可能性增加,敏感性74%,特异性63%。结论以病史、体格检查及颈部侧位X线检查的结果对患儿进行临床评估,以初步筛查OSAS是可行的。但还需要增加样本量,并改进临床资料的收集工作,以提高筛查的敏感性和特异性。

基于世界顶级学术期刊的中国酒店研究概述

本文对1978年．2012年间发表在三大权威酒店期刊《Cornell Hospitality Quarterly》（CHQ）,《International Journal of Hospitality Management》（IJHM）以及《Journal of Hospitality ＆ Tourism Research》（JHTR）上的有关中国酒店研究的80篇文献进行了综述，以分析和展现国际上中国酒店研究的变革和发展。分析发现，中国酒店研究在近5年（2008-2012）的发展尤为迅猛，研究视角、研究方法、数据收集方式及分析方法都日趋多样和科学。在地区贡献上，以亚太地区为主，未来亟需更多跨地区的合作。但是，本文的文献回顾范围仍有待扩充，今后的综述研究可以尝试运用更多的分析标准以及标准之间的交叉制表分析。

目的观察4期神经母细胞瘤联合治疗的早期疗效,探讨提高生存率及改善生活质量的治疗方案。方法对2016年1月至2021年6月在香港大学深圳医院确诊的14例4期神经母细胞瘤患儿的临床资料、治疗及随访情况进行回顾性分析。结果14例患儿中位发病年龄为3岁7.5个月。骨髓检查阳性9例,N-Myc基因扩增4例,神经元特异性烯醇化酶增高13例,尿香草扁桃酸增高7例。病理结果:分化型6例,未分化型1例,混合型1例,分化差型6例。N7方案化疗10例(含2例N7方案+三氧化二砷化疗患儿),Rapid COJEC方案化疗4例。手术13例;自体造血干细胞移植14例;放疗10例。Ch14.18/CHO免疫治疗8例,其中1例因在治疗过程中出现过敏性休克而停止免疫治疗;其余7例完成治疗,期间未观察到严重不良反应。完成免疫治疗患儿中1例复发后进行3次Lu177 Dotatate治疗,目前仍在化疗中。14例患儿中位随访时间为45个月。2年内复发4例,2年内总生存率100%;3年内复发4例,3年内无病存活7例。结论儿童4期神经母细胞瘤建议选择多学科联合方案治疗,使4期神经母细胞瘤患儿获得更好的生存及预后。

Outcome of severely injured trauma patients at a designated trauma centre in the Hong Kong Special Administrative Region

Background The Hong Kong Special Administrative Region （HKSAR） of the People＇s Republic of China （PRC） has seen significant changes in its trauma service over the last ten years including the implementation of a regional trauma system. The author＇s institution is one of the five trauma centres designated in 2003. This article reports our initial clinical experience. Methods A prospective single-centre trauma registry from January 2004 to December 2008 was reviewed. The primary clinical outcome measure was hospital mortality. The Trauma and Injury Severity Score （TRISS） methodology was used for bench-marking with the North America Major Trauma Outcome Study （MTOS） database. Results There were 1451 patients. The majority （83.9%） suffered from blunt injury. The overall mortality rate was 7.8%. Severe injury, defined as the Injury Severity Score 〉15, occurred in 22.5% of patients, and was associated with a mortality rate of 31.6%. A trend of progressive improvement was noted. The M-statistic was 0.99, indicating comparable case-mix with the MTOS. The Z- and W-statistics of each individual year revealed fewer, but not significantly so, number of survivors than expected. Conclusions Trauma centre designation was feasible in the HKSAR and was associated with a gradual improvement in patient care. Trauma system implementation may be considered in regions equipped with the necessary socio-economic and organizational set-up.

Surgical management of zygomatic complex fractures in a major trauma centre

Aim: To analyse the epidemiology, aetiology, and surgical management of zygomatic complex (ZMC) fractures in our major trauma centre, and to compare the number and location of fixation points and surgical access in our patient cohort with the literature. Methods: Retrospective analysis of all operative cases (Open Reduction and Internal Fixation) of zygomatic complex fractures over a one year period (2016). Results: A greater proportion of patients in our cohort (54%) were treated with one-point fixation compared to the literature, with the zygomaticomaxillary (ZM) buttress being the most popular fixation point (90%). ZM buttress and frontozygomatic (FZ) suture were the commonest choices for two-point fixations (70%). Buccal sulcus incision was used for ZM access in all cases. For FZ access, upper blepharoplasty incision was the most common (56%). For infra-orbital margin access, transconjunctival incision was the most common (75%). There was no significant association between number of fixation points and presence of associated injuries, impact of injury, or time to operation. There were no post-operative complications. Conclusion: A greater proportion of patients in our cohort were successfully treated with one point fixation compared to the literature, and fewer patients underwent orbital floor exploration and repair in our cohort compared to the literature. This study highlights the ongoing variation in the surgical management of ZMC fractures.
