Psychological, social, and sexual challenges affecting men receiving male infertility treatment: a systematic review and implications for clinical care

In recent years, social research surrounding the consequences of infertility has increasingly focused on the male perspective;however, a gap exists in the understanding of men’s experiences of male infertility treatment. This review aims to synthesize theexisting evidence concerning the psychological, social, and sexual burden of male infertility treatment on men, as well as patientneeds during clinical care. A systematic search identified 12 studies that are diverse in design, setting, and methods. Psychologicalevaluations have found that urological surgery may have a lasting impact on infertility-specific stress, and treatment failure canlead to feelings of depression, grief, and inadequacy. Men tended to have an avoidant coping mechanism throughout fertilitytreatment, and their self-esteem, relationship quality, and sexual functions can be tied to outcomes of treatment. Partner bonds canbe strengthened by mutual support and enhanced communication;couple separation, however, has been noted as a predominantreason for discontinuing male infertility treatment and may be associated with difficult circumstances surrounding severe maleinfertility. Surgical treatments can affect the sexual functioning of infertile men;however, the impact of testicular sperm extractionoutcomes appears to be psychologically driven whereas the improvements after microsurgical varicocelectomy are only evident inhypogonadal men. Clinically, there is a need for better inclusion, communication, education, and resource provision, to addressreported issues of marginalization and uncertainty in men. Routine psychosocial screening in cases of severe male infertility andfollow-up in cases of surgical treatment failure are likely beneficial.

The accuracy of conventional superposition or convolution methods for scatter correction in kV-CBCT is usually compromised by the spatial variation of pencil-beam scatter kernel (PBSK) due to finite size, irregular external contour and heterogeneity of the imaged obj.ct. This study aims to propose an analytical method to quantify the Compton single scatter (CSS) component of the PBSK, which dominates the spatial distribution of total scatter assuming that multiple scatter can be estimated as a constant background and Rayleigh scatter is the secondary source of scatter. The CSS component of PBSK is the line integration of scatter production by incident primary photons along the beam line followed by the post-scattering attenuation as the scattered photons traverse the obj.ct. We propose to separate the obj.ct-specific attenuation term from the line integration and equivalently replace it with an average value such that the line integration of scatter production is obj.ct independent but only beam specific. We derived a quartic function formula as an approximate solution to the spatial distribution of the unattenuated CSS component of PBSK. The “effective scattering center” is introduced to calculate the average attenuation. The proposed analytical framework to calculate the CSS was evaluated using parameter settings of the On-Board Imager kV-CBCT system and was found to be in high agreement with the reference results. The proposed method shows highly increased computational efficiency compared to conventional analytical calculation methods based on point scattering model. It is also potentially useful for correcting the spatial variant PBSK in adaptive superposition method.

A Brief Overview of Amblyaudia

Amblyaudia, a recent subcategory of auditory processing disorder, is characterized by asymmetrical auditory processing of an individual’s ears. Amblyaudia can result in speech comprehension difficulties, reading difficulties, information processing deficits, and inattention. These difficulties can be mistakenly attributed to Attention Deficit Hyperactivity Disorder (ADHD), Specific Learning Disorders (SLD), depression, anxiety disorders, and communication disorders. Unfortunately, traditional hearing tests do not place the two ears in competition and cannot detect asymmetry. Therefore, students who exhibit these difficulties and have normal performance on traditional hearing tests should be also evaluated for amblyaudia with dichotic listening tests. Amblyaudia can be addressed through dichotic listening tasks that strengthen the non-dominant ear, as well as minor adj.stments to the classroom environment. This paper will examine the current literature on amblyaudia and provide a brief overview of the causes, diagnosis, treatments, and prognosis.

We obtained historical data of rainfall in Warri Town for the period 2003-2012 for the purpose of model identification and those of 2013 for forecast validation of the identified model. Model identification was by visual inspection of both the sample ACF and sample PACF to postulate many possible models and then use the model selection criterion of Residual Sum of Square (RSS), Akaike’s Information Criterion (AIC) complemented by the Schwartz’s Bayesian Criterion (SBC), to choose the best model. The chosen model was the Seasonal ARIMA (1, 1, 1) (0, 1, 1) process which met the criterion of model parsimony with RSS value of 81.098,773, AIC value of 281.312,35 and SBC value of 289.330,84. Model adequacy checks showed that the model was appropriate. We used the model to forecast rainfall for 2013 and the result compared very well with the observed empirical data for 2013.

Establishment of pregnancy in pigs involves maintaining progesterone secretion from the corpora lutea in addition to regulating a sensitive interplay between the maternal immune system and attachment of the rapidly expanding trophoblast for nutrient absorption. The peri-implantation period of rapid trophoblastic elongation followed by attachment to the maternal uterine endometrium is critical for establishing a sufficient placental-uterine interface for subsequent nutrient transport for fetal survival to term, but is also marked by the required conceptus release of factors involved with stimulating uterine secretion of histotroph and modulation of the maternal immune system. Many endometrial genes activated by the conceptus secretory factors stimulate a tightly controlled proinflammatory response within the uterus. A number of the cytokines released by the elongating conceptuses stimulate inducible transcription factors such as nuclear factor kappa B （NFKB） potentially regulating the maternal uterine proinflammatory and immune response. This review will establish the current knowledge for the role of conceptus cytokine production and release in early development and establishment of pregnancy in the pig.

Recent data suggest that cystic fibrosis transmembrane conductance regulator(CFTR)gene alterations negatively impact male fertility beyond *** sought to compare gene alterations,sperm retrieval rates,and intracytoplasmic sperm inj.ction(ICSI)outcomes among men with cystic fibrosis(CF)disease and congenital bilateral absence of the vas deferens(CBAVD)*** retrospectively evaluated all men who underwent surgical sperm retrieval at two academic,high-volume andrology centers from 2010 to *** men with documented CFTR alterations and obstructive azoospermia from either CBAVD or CF were *** between groups for CFTR abnormality,sperm retrieval,and ICSI outcomes were statistically ***,39 patients were included with 10 in the CF and 29 in the CBAVD *** sperm retrieval rates were significantly lower in the CF group for sperm concentration(14.8×10^(6)ml^(-1)vs 61.4×10^(6)ml^(−1),P=0.02)and total motile sperm count(2.9 million vs 11.4 million,P=0.01).This difference was only predicted by homozygous delta F508 CFTR mutations(P<0.05).The CF group also demonstrated a significantly higher rate of rescue testicular sperm extraction(70.0%vs 27.6%,P<0.03)and lower fertilization rate with ICSI(32.5%vs 68.9%,P<0.01).In conclusion,those with CF demonstrated lower sperm quality,greater difficulty with sperm retrieval,and worse ICSI outcomes compared with CBAVD-only *** delta F508 CFTR mutations appear to significantly impair spermatogenesis and sperm function.
