Purpose： The clinical significance of isolated flee fluid （FF） without solid organ injury on computed to- mography （CT） continues to pose significant dilemma in the management of patients with blunt abdominal trauma （BAT）. Methods： We reviewed the incidence of FF and the clinical outcome amongst patients with blunt abdominal trauma in a metropolitan trauma service in South Africa. We performed a retrospective study of 121 consecutive CT scans over a period of 12 months to determine the incidence of isolated FF and the clinical outcome of patients managed in a large metropolitan trauma service, Results： Of the 121 CTs, FF was identified in 36 patients （30%）. Seven patients （6%） had isolated FF. Of the 29 patients who had free fluid and associated organ injuries, 33 organ injuries were identified. 86% （25/ 29） of all 29 patients had a single organ injury and 14% had multiple organ injuries. There were 26 solid organ injuries and 7 hollow organ injuries. The 33 organs injured were： spleen, 12; liver, 8; kidney, 5; pancreas, 2; small bowel, 4; duodenum, 1. Six （21%） patients required operative management for small bowel perforations in 4 cases and pancreatic tail injury in 2 cases. All 7 patients with isolated FF were initially observed, and 3 （43%） were eventually subjected to operative intervention. They were found to have an intra-peritoneal bladder rupture in 1 case, a non-expanding zone 3 haematoma in I case, and a negative laparotomy in 1 case. Four （5？%） patients were successfully managed without surgical interventions. Conclusions： Isolated FF is uncommon and the clinical significance remains unclear. Provided that reli- able serial physical examination can be performed by experienced surgeons, an initial non-operative approach should be considered.

WORLD HERITAGE AND SPACE TECHNOLOGY The Convention Concerning the Protection of the World Cultural and Natural Heritage(WHC),adopted by United Nations Educational,Scientific and Cultural Organization(UNESCO)on November 16,1972,aims to ensure the identification,protection,conservation,presentation,and transmission to future generations of the world’s cultural and natural *** WHC works toward these goals by emphasizing the Outstanding Universal Value(OUV)of heritage sites and the unique contribution such places can make to conservation and human development agendas.1 As of the end of january 2023,theWHC has been signed by 194 state parties,covering 1,157 sites(including 900 cultural,218 natural,and 39 mixed properties),55 of which are considered to be in *** sites,totaling an area of more than 370 million hectares are designated as World Heritage(WH)sites(https://***/en/list/).WH sites have played a significant role in the sustainable development of society globally and helped effectively maintain and preserve the cultural diversity and global biodiversity of the Earth.

目的关于FDG—PET在结直肠癌肝转移（***）手术治疗中作用的随机对照试验（RCT）结果表明，8％的患者其手术治疗决策发生了改变。该研究拟探讨***患者近期化疗和随后的PET探测转移性病灶之间的关系，并确定回顾性集中阅片是否会影响随机试验的结果。方法研究纳入来自同一医院的120例患者。首次PET读片报告（PET-L）取自医院数据库，再次回顾性读片报告（PET-C）由其他至少1位阅片者独立完成。记录PET显像前未知的、未被列入手术切除计划的肝外病灶（EHD）和肝内转移灶（SALM）。根据术后病理结果和实体肿瘤疗效评估标准（RECIST），全部患者被分为化疗有效组和其他组。结果50例患者PET显像前90d内进行了化疗．其中37例有效。EHD为30／120（25％）。有效组内PET-L（7／37；18．9％），PET—C（7／37；18．9％）和CT（4／37；10．8％）发现EHD的结果无差异（P=0．375）；但在其他组，PET—L（15／83；18．1％）和PET．C（22／83；26．5％）发现的EHD比cT（8／83；9．6％）多（P=0．039和P〈0．001）。50例患者中，被PET—L和PET—C发现EHD和（或）SALM而没有被cT发现的患者数分别为14例（11．7％）和22例（18．3％）。结论近期化疗对PET显像探测结直肠癌转移的影响表明，先前进行的随机对照试验结果可能低估了可进行肝内转移病灶切除的患者数。

Plants evolved their own strategy to form the essential link between the endoplasmic reticulum(ER) and the plasma membrane(PM) through a complex including NET3 C and VAP27(Wang et al 2014,Curr Biol).However,the function of plant ER-PM contact sites has not been fully *** this study,we have identified a function for ER-PM contact sites(EPCS) in plants and that is in the autophagosome biogenesis pathway,in concert with actin cytoskeleton and the Arabidopsis EH ***1 and AtEH2 are components of the endocytic TPLATE complex which is known to be essential for endocytosis in plants(Gadeyne et al.,2014,Cell).Both proteins(from now on termed AtEH/Pan1) share high sequence similarity with the yeast ARP2/3 complex activator,Pan1 *** AtEH/Panl proteins localise to the plasma membrane and autophagosomes,and their autophagosome membrane recruitment requires its N-terminal EH domains which interact with *** induction of autophagy,AtEH/Pan1 proteins recruit TPC and AP-2 subunitsand ARP2/3 proteins onto the autophagosome *** expression of AtEH1/Pan1 boosts autophagosome formation and suggest independent and redundant pathways for actin mediated autophagy in ***,AtEH/Pan1 regulated autophagosomes are found associated with EPCS where AtEH/Panl interacts with *** knock-down expression of either AtEH/Pan1 or VAP27-1 are more susceptible to nutrient depleted conditions,indicating defective *** conclusion,our results suggest that AtEH/Pan1 proteins regulate an autophagosome-dependent degradation pathway which starts at the EPCS through its interaction with VAP27-1 and merges with endocytic pathways.

Total pancreatectomy with islet autotransplantation(TPIAT)is an effective treatment option for non-diabetic patients with intractable chronic *** outcome and potential benefits for pre-diabetic and diabetic patients are less well ***-four patients underwent TPIAT were retrospectively divided into 3 groups according to pre-operative glycemic control:diabetes mellitus(DM)(n=5,15%),pre-DM(n=11,32%)and non-DM(n=18,54%).Pre-operative fasting c-peptide was detectable and similar in all 3 *** yield in the DM group was comparable to pre-DM and non-DM groups(median islet equivalents[IEQ]was 191,800,111,800,and 232,000 IEQ,respectively).Patients received islet mass of over the target level of 2000 IEQ/kg in pre-DM and DM at lower but clinically meaningful rates compared to the non-DM group:45%(5/11)and 60%(3/5)for a combined 50%(8/16)rate,respectively,compared to 83%(15/18)for the non-DM *** 1 year,fasting c-peptide and HbA1c did not differ between DM and pre-DM groups but c-peptide was significantly higher in *** transplantation failed(negative c-peptide)only in 1 ***,all patients experienced pancreatic pain with daily opioid dependence in 60%to 70%.Pancreatic-type pain gradually subsided completely in all groups with no differences in other painful somatic *** patients with measurable pre-operative c-peptide can achieve similar benefit from TPIAT,with comparable outcomes to pre-diabetic and non-diabetic patients including pain relief and the metabolic benefit of transplanted *** surprisingly,endocrine outcomes for diabetic and prediabetics patients are substantially worse than in those with normal pre-operative glucose control.
